Tumor size discrepancy between endoscopic and pathological evaluations in colorectal endoscopic submucosal dissection

BACKGROUND Tumor size impacts the technical difficulty and histological curability of colorectal endoscopic submucosal dissection(ESD);however,the preoperative evaluation of tumor size is often different from histological assessment.Analyzing influential factors on failure to obtain an accurate tumor size evaluation could help prepare optimal conditions for safer and more reliable ESD.METHODS This was a retrospective study conducted at a single institution.A total of 377 lesions removed by colorectal ESD at our hospital between April 2018 and March 2022 were collected.We first assessed the difference in size with an absolute per-centage of the scaling discrepancy.Subsequently,we compared the clinicopatho-logical characteristics of the correct scaling group(>-33%and<33%)with that of the incorrect scaling group(<-33%or>33%),which was further subdivided into the underscaling group(-33%or less of the discrepancy)and overscaling group(33%or more of the discrepancy),respectively.As secondary outcome measures,parameters on size estimation were compared between the underscaling and correct scaling groups,as well as between the overscaling and correct scaling groups.Finally,multivariate analysis was performed in terms of the following relevant parameters on size estimation:Pathological size,location,and possible influential factors(P<0.1)in the univariate analysis.RESULTS The mean of absolute percentage in the scaling discordance was 21%,and 91 lesions were considered to be incorrectly estimated in size.The incorrect scaling was significantly remarkable in larger lesions(40 mm vs 28 mm;P<0.001)and less experience(P<0.001),and these two factors were influential on the underscaling(75 lesions;P<0.001).Conversely,compared with the correct scaling group,16 lesions in the overscaling group were significantly small(20 mm vs 28 mm;P<0.001),and the small lesion size was influential on the overscaling(P=0.002).CONCLUSION Lesions indicated for colorectal ESD tended to be underestimated in large tumors,but overestimated in small ones.This discrepancy appears worth understanding for optimal procedural preparation. 展开更多

In the experiments, we have found that the scattered signal intensity varied greatly with the opening area of the pulsed valve. Cautionary Remarks When Testing Agreement between Two Raters for Continuous Scale Measurements: A Tutorial in Clinical Epidemiology with Implementation Using R

Background: When continuous scale measurements are available, agreements between two measuring devices are assessed both graphically and analytically. In clinical investigations, Bland and Altman proposed plotting subject-wise differences between raters against subject-wise averages. In order to scientifically assess agreement, Bartko recommended combining the graphical approach with the statistical analytic procedure suggested by Bradley and Blackwood. The advantage of using this approach is that it enables significance testing and sample size estimation. We noted that the direct use of the results of the regression is misleading and we provide a correction in this regard. Methods: Graphical and linear models are used to assess agreements for continuous scale measurements. We demonstrate that software linear regression results should not be readily used and we provided correct analytic procedures. The degrees of freedom of the F-statistics are incorrectly reported, and we propose methods to overcome this problem by introducing the correct analytic form of the F statistic. Methods for sample size estimation using R-functions are also given. Results: We believe that the tutorial and the R-codes are useful tools for testing and estimating agreement between two rating protocols for continuous scale measurements. The interested reader may use the codes and apply them to their available data when the issue of agreement between two raters is the subject of interest. 展开更多

To address the intermittent positioning and drift of personnel positioning RTK in the high-frequency signal interference environment of substations, we propose to use IMU as the positioning compensation module of RTK and adopt the joint RTK/PDR positioning method to solve the positioning results. The heading angle is easily scattered in the pedestrian heading projection (PDR) process and the heading angles calculated from the output data of the gyroscope, accelerometer and magnetometer after denoising are input into the complementary filter for fusion. To improve the accuracy of step estimation in the PDR process, an improved step estimation model is used. For RTK/PDR data fusion, the extended Kalman filter (EKF) method is used, which helps to achieve outdoor full-scene high-accuracy positioning. The final simulation results show that RTK can be effectively compensated by PDR under the interference of high-frequency signals, and the positioning accuracy reaches 0.02 m. 展开更多

Precise assessment of spinal cord cystic lesions is crucial to formulate effective therapeutic strategies,yet histological assessment of the lesion remains the primary method despite numerous studies showing inconsistent results regarding estimation of lesion size via histology.On the other hand,despite numerous advances in micro-computed tomography(micro-CT)imaging and analysis that have allowed precise measurements of lesion size,there is not enough published data on its application to estimate intraspinal lesion size in laboratory animal models.This work attempts to show that micro-CT can be valuable for spinal cord injury research by demonstrating accurate estimation of syrinx size and compares between micro-CT and traditional histological analysis.We used a post-traumatic syringomyelia rat model to compare micro-CT analysis to conventional histological analysis.The study showed that micro-CT can detect lesions within the spinal cord very similar to histology.Importantly,micro-CT appears to provide more accurate estimates of the lesions with more measures(e.g.,surface area),can detect compounds within the cord,and can be done with the tissue of interest(spinal cord)intact.In summary,the experimental work presented here provides one of the first investigations of the use of micro-CT for estimating the size of intraparenchymal cysts and detecting materials within the spinal cord.All animal procedures were approved by the University of Akron Institutional Animal Care and Use Committee(IACUC)(protocol#LRE 16-05-09 approved on May 14,2016). 展开更多

Estimation of fruit size in tree fruit crops is essential for selective robotic harvesting and crop-load estimation.Machine vision systems for fruit detection and localization have been studied widely for robotic harvesting and crop-load estimation.However,only a few studies have been carried out to estimate fruit size in orchards using machine vision systems.This study was carried out to develop a machine vision system consisting of a color CCD camera and a time-of-flight(TOF)light-based 3D camera for estimating apple size in tree canopies.As a measure of fruit size,the major axis(longest axis)was estimated based on(i)the 3D coordinates of pixels on corresponding apple surfaces,and(ii)the 2D size of individual pixels within apple surfaces.In the 3D coordinates-based method,the distance between pairs of pixels within apple regions were calculated using 3D coordinates,and the maximum distance between all pixel pairs within an apple region was estimated to be the major axis.The accuracy of estimating the major axis using 3D coordinates was 69.1%.In the pixel-size-based method,the physical sizes of pixels were estimated using a calibration model developed based on pixel coordinates and the distance to pixels from the camera.The major axis length was then estimated by summing the size of individual pixels along the major axis of the fruit.The accuracy of size estimation increased to 84.8%when the pixel size-based method was used.The results showed the potential for estimating fruit size in outdoor environments using a 3D machine vision system. 展开更多

This paper presents the optical navigation results of the asteroid explorer Hayabusa2 during the final rendezvous approach phase with the asteroid Ryugu.The orbit determination of Hayabusa2 during the cruising phase uses a triangulation-based method that estimates the probe and asteroid orbits using the directions from which they are observed.Conversely,the asteroid size is available as optical information just prior to arrival.The size information allows us to estimate the relative distance between the probe and the asteroid with high accuracy,that is strongly related to the success or failure of the rendezvous.In this study,the relative distance and asteroid size in real space are simultaneously estimated in real time by focusing on the rate of change of the asteroid size observed in sequential images.The real-time estimation results coincided with those of precise analyses performed after arrival. 展开更多

An efficient and accurate exponential wave integrator Fourier pseudospectral (EWI-FP) method is proposed and analyzed for solving the symmetric regularized-long-wave (SRLW) equation, which is used for modeling the weakly nonlinear ion acoustic and space-charge waves. The numerical method here is based on a Gautschi-type exponential wave integrator for temporal approximation and the Fourier pseudospectral method for spatial discretization. The scheme is fully explicit and efficient due to the fast Fourier transform. Numerical analysis of the proposed EWI-FP method is carried out and rigorous error estimates are established without CFL-type condition by means of the mathematical induction. The error bound shows that EWI-FP has second order accuracy in time and spectral accuracy in space. Numerical results are reported to confirm the theoretical studies and indicate that the error bound here is optimal. 展开更多
