Thirty-two subjects were enrolled in the study with control (n = 17) and DS types II and III (n = 15) subjects. Samples were collected from unstimulated whole saliva, serum and swabs from denture surfaces. Salivary levels of inflammatory mediators and CRP were measured by multiplex. Samples from denture and mucosal surfaces were analyzed by DNA-DNA checkerboard. Saliva from DS subjects showed increase in IL-8 (p = 0.04) and IL-1β (p = 0.04) with trend for increase in IL-1β, TNFα and IL-6 levels. C. albicans higher counts in DS saliva (p = 0.03) showed association with elevated levels of IL-8 (p = 0.03) and IL-1α (p = 0.01). CRP levels were not different among groups (p = 0.74). DNA-DNA checkerboard analyses indicated typical periodontal pathogens below the detection threshold of 104 organisms on both denture and inflamed mucosal surfaces. The data suggest that DS is associated with elevation of salivary IL1 and IL-8 together with increased C. albicans. There was no evidence of systemic inflammation as measured by serum C-reactive protein levels.展开更多

Methods: Materials included gingiva-coloured composite (Fusion Universal G1), acrylic (Imicryl), and subdivided Procryla group. Subgroups comprised 15 and 30-minute heat polymerized (Pro15, Pro30), and 1 wt% (Pro1Z) and 3 wt% (Pro3Z) zirconium added groups. Immersed in beverages for 1, 7, and 14 days, pH and microhardness were assessed. SEM examined random samples. Statistical analysis used repeated measures ANOVA, and post hoc tests (p Results: The gingiva-coloured composites displayed noteworthy time-associated microhardness changes (p 0.05). Despite variable pH levels in beverages, no substantial group interaction effects were observed (p > 0.05). Initial microhardness rankings shifted after a 14-day immersion. However, the strength characteristics of PMMA, such as impact strength and fatigue strength, are poor, and fracturing of PMMA dentures is a common problem in prosthodontic practice. Reinforcing PMMA with various materials, such as carbon fibers, glass fibers (fiberglass), and ultrahigh modulus polyethylene fibers, has been suggested to strengthen the denture-base material. A common problem encountered when packing the resin on these specimens is fiber slippage beyond the denture edges. The present study proposes an alternative method of incorporating fiber meshes into complete dentures, whereby a thin filament of self-polymerizing resin at the perimeter of the fiber mesh is produced, giving a clear and stable shape to the mesh that fits the upper jaw cast. During placement of the shaped mesh on the cast, a positive-negative relationship is created between the mesh and cast, which immobilizes the mesh during the incorporation process.展开更多

Objective: To observe the curative effect of Chinese medicine prescription (Yueshi, Zhongbai, Qingdai, Phelloderma phelloderma and glycyrrhiza) on children with herpetic stomatitis. Methods: 90 patients were divided into 2 groups by random number table method. 45 patients in the observation group were treated with traditional Chinese medicine, and 45 patients in the control group were treated with iodine glycerin. The total effective rate after treatment was compared between the two groups. Results: The total effective rate of the treatment group was 95.55%;the total effective rate of the control group was 71.11%;the total effective rate of the two groups after treatment was compared;the treatment group was significantly better than the control group;the difference was significant (P 0.05). Conclusion: The external use of Chinese medicine in the treatment of pediatric herpetic stomatitis has achieved good results, improving the condition of children, and significantly improving the clinical efficacy.展开更多

The purpose of this study was to document the improvement observed in two cases of severe dementia after denture placement. The subjects, two women in their 70s with severe dementia, were diagnosed with Alzheimer’s disease by the medical doctors in charge of their cases. In the first case, the subject’s symptoms included severe disorientation, impaired communication, and a tendency to wander. Two weeks after a lower complete denture placement, she was able to greet others. Five weeks later, she was able to communicate and precisely read an analog clock. Her condition continued to improve for at least 3 months. In the second case, the subject’s mouth was always open. She was bedridden in a vegetative state and hardly moved, talked, or laughed;it was impossible to communicate with her. Only a few seconds after an upper complete denture placement, she started to talk, albeit unintelligibly. Two weeks after a lower complete denture placement, she could communicate normally, laugh, and walk without help. Although the underlying mechanism has not yet been clarified, the author hypothesizes that positive signals from the oral area are transferred to the brain as positive stimulation via the trigeminal nerve, the largest cranial nerve.展开更多

Results: XPS showed the peak corresponding to C-O getting higher as the treatment proceeded, however at 120 seconds, the peak did not increase any longer and partly crossed with the peak at the duration of 60 seconds. IR spectra showed the wave corresponding to C-H was reduced as O2-plasma treatment proceeded, and then changed little, Wetting angle initially decreased dramatically, however, as the reaction proceeded, wetting angle increased slightly. Conclusion:Equilibrium was reached for introducing oxygen-containing groups and changing of C-H. As the treatment proceeded, wetting angle increased slightly.展开更多

Background data: Recurrent aphthous stomatitis (RAS) is one of the most common and painful ulcers in the oral cavity. Low Level Laser Therapy (LLLT) has been used successfully for reducing pain and healing time in comparison with corticosteroids and placebo in some studies. But these treatments are done in the clinics and it seems that there is no report of the laser treatment as a home care device up to this study. Methods: This was a prospective, randomized trial pilot study. Thirty patients with RAS were divided into three similar groups. The group one (n = 10) was treated with a diode laser, 660 nm, 40 mw, continuous wave, 1.2 J, 6 J/cm2, for five consecutive days (twice per day, 30 seconds each session). The second group (n = 10) was treated with a topical triamcinolone (adcortyl in orabase) ointment four times per day and the third group (n = 10) was treated with placebo (red light pen) the same way as Group 1. All the patients were evaluated with a Visual Analogue Scale (VAS) before and after treatment on the consecutive days. The data was analysed by one way ANOVA and PostHoc tests. Results: The results show that the laser pen and triamcinolon in orabase ointment have similar results and both have better results than placebo in the control of the recurrent aphthous stomatitis pain (p value: 0.001). Conclusion: In this clinical pilot study the laser pen as a home care device seems to be useful for the treatment of RAS.展开更多

Acrylic denture base resins (auto-polymerizing, heat-polymerizing, injected heat-polymerizing resins) were examined after a tungsten carbide bur, and after chairside polishing using 3 polishing kits and pumice. The specimens were subjected to mechanical brushing using a wear tester to simulate 30 000 strokes of brushing. The surface roughness of the acrylic denture base resin specimens was measured using a contact pro-filometer. After the test, the random polished acrylic resins were evaluated by scanning electron mi-croscopy (SEM) and atomic force microscopy (AFM). Acrylic denture base resins polished using the 3 types of polishing kits had a smoother surface than those finished with the tungsten carbide bur (p〈0.05). The surface of the resin polished by a TC cutter exceeded the Ra of 0.2 μm (p〈0.05). The auto-polymerizing resin showed a significantly higher surface roughness than the heat-polymerizing resin and injected heat-polymerizing resin (p〉0.05). In the case of polishing step wise, there was almost no change in surface roughness after brushing (p〉0.05).展开更多

It occurs commonly in developing na-tions with poor living conditions. Objective: To present a case of severe Necrotising Stomatitis in a previously undiagnosed HIV Seropositive patient. Patient and method: This case report describes severe, rapidly spreading necrotising stomatitis in a 35 years old lady, the condition which led to her being diagnosed with HIV. The treatment modalities, challenges of man-agement and the differential diagnosis were discussed. Result: During the course of her management, pa-tient’s oral condition improved. However, later re-currence was seen due to severe anaemia caused by malnutrition and aggravated by Zidovudine antiret-roviral drug. Conclusions: There is need for interdis-ciplinary interactions between the dentists and the physicians managing HIV patients, to allow effective management and afford patient the best treatment.展开更多

Review at the ENT department where a diagnostic rigid oesophagoscopy was done under general anaesthesia showed a denture impacted to the wall of the oesophagus at 17 cm from the incisor. Attempts at retrieval were unsuccessful as the denture was deeply embedded in the oesophageal wall <em>and bled easily on attempted removal</em>. He subsequently underwent a 2-stage surgical management approach by the cardiothoracic surgical team. Patient first underwent nutritional rehabilitation for a month through a created feeding gastrostomy tube via a limited midline laparotomy. He then had a open cervical oesophagotomy and removal of the denture. Currently doing well six months after surgery and tolerating normal oral diet.展开更多

The aim was to report an oral rehabilitation of a patient with severe aesthetic and functional impairment, with Angle’s class III malocclusion. The patient sought the extension project of the School of Dentistry of the Federal University of Minas Gerais with aesthetic complaints. After clinical and radiographic evaluation, the proposed planning was immediate complete denture maxilla and immediate implant-retained overdenture in mandible. During clinical evaluation it was observed that the patient was Angle</span><span>’</span><span>s III. The rehabilitation was performed as proposed and at the end of the treatment, it was possible to revert the class III occlusal pattern to Angle’s I. The excellent maxillomandibular relationship was achieved, which ensured optimal facial aesthetics and functional condition. It was concluded that the immediate complete dentures and immediate implant-retained overdenture might be beneficial for restoring the aesthetics and function of patients with Angle’s class III malocclusion.</span>展开更多

In order to test its machining capacity, some typical microstcucture parts, such as straight channel, hemispherical surface, and molars coronal, were selected for high speed milling. On completi...In the present study, an aero pneumatic fatigue testing machine for complete dentures was designed, fabricated, and tested for the evaluation of the fatigue life of reinforced complete upper denture (CUD). On completion and testing, it was observed that the machine has the potential of generating reliable number of cyclic data. The machine’s performance was evaluated using test specimens of identical CUDs that were machined in conformity with standard procedures. The fatigue machine compressed the lower dental arch over the upper denture-specimen in centric occlusion, in the same way that the two masticatory muscles pull the lower jaw over the upper jaw during chewing. The incorporation of glass fibres into the CUD using a sandwich technique quadruples the lifespan of the denture (<em>P</em> = 0.004). The low standard deviation, along with the low coefficient of variation (CV) of the group of unreinforced dentures shows the repeatability of the results and the reliability of the machine. The high standard deviation and coefficient of variation of reinforced dentures was expected, since a high variation of results is usually recorded in fibre reinforcement cases. This research confirmed the view that the crack during denture fracture initiates in the anterior palatal area and propagates to the posterior.展开更多

There was no effective curative therapy for this disease. Isotretinoin has been recently used in the treatment of Behcet’s disease. Objectives: To evaluate the efficacy and safety of isotretinoin in treating RAS and the long term remission of RAS. Patients and Methods: This single-blind controlled therapeutic study conducted in Department of Dermatology-Baghdad Teaching Hospital during February 2011-January 2012. Thirty patients with typical RAS were included in this work. Detailed history and full examination were done for all patients. They were given isotretinoin 20 mg orally once daily for three months to be seen on Day 14 firstly and then monthly to be assessed using the oral clinical manifestation index (OCMI). After isotretinoin was stopped three months later, patients were given placebo therapy for another 3 months. Results: The results of 30 treated patients were as follows: 17 (56.67%) males and 13 (43.33%) females with male to female ratio was 1.3:1. Their ages ranged from 12 - 60 (35.33 ± 12.06) years. The OCMI before isotretinoin therapy ranged from 7 - 17 (13.13 ± 2.55), while after therapy the mean started to decline to a lower level within the first 14 days (P = 0.103), and continued to decline significantly until the end of the first month of therapy (P = 0.023). Then the OCMI declined very significantly until the end of fourth month of therapy (P < 0.001). After that the mean started to increase until the end of the 5 months (with placebo) but it remained statistically significant compared with the baseline of mean of OCMI before treatment (P = 0.046). Then it continued to increase to become not significant at the end of 6 months of therapy (P = 0.107). Conclusion: Isotretinoin is an effective therapeutic and prophylactic promising remedy in treatment of RAS.展开更多

Their main adverse events are nausea, vomiting, constipation, psychological symptoms, and respirato...Opioids are widely used as analgesics for oral mucositis in allogeneic hematopoietic cell transplantation (allo-HCT). Their main adverse events are nausea, vomiting, constipation, psychological symptoms, and respiratory depression. In our institute, continuous intravenous morphine was generally used until 2007, followed by intravenous fentanyl as the first-line agent because of its potential fewer adverse events. We retrospectively analyzed 99 patients who underwent allo-HCT in the University of Tsukuba Hospital from 2004 to 2009. Out of 99 patients, 64 were treated with opioids (morphine, 32 and fentanyl, 32). The attending physicians were in charge of providing stable pain control. Median age, sex, stem cell source, preparative regimen, and GVHD prophylaxis were similar in the two groups. There were no significant differences in psychological symptoms, drowsiness, nausea, and vomiting in both groups. Defecation ratio (the days having a bowel movement/the days taking opioids) was 63% and 94% in the morphine and fentanyl group, respectively (P < 0.0001). The percentage of patients who needed to use purgative drugs was 25% and 6% in the morphine and fentanyl group, respectively (P = 0.04). It is suggested that fentanyl has less adverse effects on gastrointestinal movement and is safer than morphine when used for oral mucositis in allo-HCT.展开更多

Accordingly, its therapy whether topical or systemic can induce clearance, but the relapse rate is high. Objective: To use 100% topical pumpkin seed oil in RAS as it has many actions as anti-inflammatory and antioxidant effects. Patients and Methods: This single, blind, clinical, therapeutic trial was conducted in the Department of Dermatology-Baghdad Teaching Hospital, Baghdad, Iraq, during February 2015-August 2016. Twenty-five patients with RAS were included in this work. After full history and clinical examination, the clinical diagnosis was established. Oral clinical manifestation index score (OCMI) was carried out before, during and after stopping therapy to assess the different parameters of this score. 100% Topical pumpkin seed oil was given two times daily for 3 months during which short term assessment for each patient was done by using OCMI before, after 4 days and after 8 days of therapy in addition to assessing the mean size of largest diameter of ulcers in each visit to evaluate the therapeutic efficacy of the therapy, while long term assessment of the OCMI was done every month for 3 months after starting of the therapy to evaluate the prophylactic effect of the therapy. After 3 months, pumpkin seed oil was stopped and assessment of OCMI was done monthly for another 3 months to evaluate the remission action of pumpkin seed oil. Results: Twenty-five patients were included in this study: 15 (60%) males and 10 (40%) females. While their ages ranged from 9 - 60 (27.48 ± 11.97) years. The mean of OCMI before pumpkin seed oil therapy was ranged from 9 - 15 (12.96 ± 1.42), while after the therapy the mean started to decline to lower level within 4 days of therapy and was statistically significant (p < 0.0001) and continued to decline significantly till the end of third month of therapy (p < 0.0001). The percent reduction was 79.30% after 4 days of therapy and 94.38% after 8 days of therapy. After stopping therapy, the mean of OCMI started to increase, but it is remained statistically highly significant (p < 0.0001) at the end of third months after stopping therapy when it is compared with baseline mean of OCMI. Conclusion: Pumpkin seed oil had an effective therapeutic and prophylactic action against RAS, in addition, it induced remission for at least 3 months after stopping therapy. No local or systemic side effects were observed during the course of therapy.展开更多

The research proves the maximum efficacy of hydroxyzine hydrochloride, aminodihydrophthalasindione sodium and Eplan used in multiple treatments of recurrent aphthous stomatitis in patients with urogenital infection as evidenced by a marked reduction in the coefficient of local immunity factors balance, which is observed as early as on day 7 of the therapy provided. Moreover, the findings show the growth of immunological parameters of oral fluid (lysozyme and SIgA), the normalization of the coefficient of local immunity factors balance in the treatment of recurrent aphthous stomatitis in women with urogenital infection, the improvement being observed 3 months after the treatment suggesting a favorable local immunity of the oral cavity.展开更多

Materials and Methods: Partially edentulous patients presenting for removable denture treatment at the Prosthodontics Service in the Hospital Affiliated to Kinshasa University (Democratic Republic of Congo) were assessed for enrolment in this study. After applying exclusion criteria, 378 patients were included in the study, and randomly assigned into 2 groups. Oral health related quality of life (OHIP-23) and mastication time (MaT), number of chewing cycles (MaC), mastication frequency (MaF), and the sizes of the peanut fragments (FraS) were compared in both groups. Statistical significance was set at p < 0.05. Results: The average MaC, MaT, MaF, and FraS were 119 (± SD 53.70), 86.75 (±SD 35.35), 1.38 (±SD 0.25), and 3.3 (±SD 3.25) for the denture group and 77.9 (±SD 23.9), 60.2 (±SD 17.91), 1.29 (±SD 0.15), and 1.5 (±SD 0.7) for the non-denture group, respectively. Conclusion: The overall oral health related quality of life was best in the non-denture than denture.展开更多

The aim of this study was to establish a possible correla-tion between removable partial denture in acrylic resin use after 5 years and dental diseases. Materials and Methods: Partially edentulous patients presenting for removable denture treatment at the Prosthodontics service of the Affiliated Hospital of Kinshasa University, Democratic Republic of Congo were assessed in this study. Patients were randomly assigned into 2 groups: Denture and non-denture group. Caries and periodontal diseases were compared between both groups. Statistical significance was set at p < 0.05. Results: The average age in the denture and non-denture-group was 53.15 (±SD 22.05) and 31.59 (±SD 11.98) years. Out of 160 teeth were decayed, both 110 teeth (68.75%) in the denture-group and 50 teeth (31.25%) in the non-denture group. The DMFT index calculated at start time of the study was 0.2 for both groups, after five years was 7.1 for the denture-group and 2.6 for the non-denture-group. The plaque index in the denture-group had an index of 1 and that of non-denture-group was 0.5. Conclusion: The relationship has been found between patients’ wearers a RPD and dental diseases.展开更多
文摘We aimed to evaluate the microbial and inflammatory characteristics associated with Denture Stomatitis (DS) analyzing: l) Levels of salivary cytokines and cultivable C. albicans;2) DNA-DNA checkerboard on biofilm associated with mucosal tissue-bearing denture surfaces, 3) Serum C-reactive protein (CRP) levels. Thirty-two subjects were enrolled in the study with control (n = 17) and DS types II and III (n = 15) subjects. Samples were collected from unstimulated whole saliva, serum and swabs from denture surfaces. Salivary levels of inflammatory mediators and CRP were measured by multiplex. Samples from denture and mucosal surfaces were analyzed by DNA-DNA checkerboard. Saliva from DS subjects showed increase in IL-8 (p = 0.04) and IL-1β (p = 0.04) with trend for increase in IL-1β, TNFα and IL-6 levels. C. albicans higher counts in DS saliva (p = 0.03) showed association with elevated levels of IL-8 (p = 0.03) and IL-1α (p = 0.01). CRP levels were not different among groups (p = 0.74). DNA-DNA checkerboard analyses indicated typical periodontal pathogens below the detection threshold of 104 organisms on both denture and inflamed mucosal surfaces. The data suggest that DS is associated with elevation of salivary IL1 and IL-8 together with increased C. albicans. There was no evidence of systemic inflammation as measured by serum C-reactive protein levels.
文摘Purpose: The study investigated the impact of dietary habits, specifically soda, milk kefir, water kefir, almond milk, and distilled water (control) consumption, on the microhardness of gingiva-coloured composite and acrylic denture bases. Methods: Materials included gingiva-coloured composite (Fusion Universal G1), acrylic (Imicryl), and subdivided Procryla group. Subgroups comprised 15 and 30-minute heat polymerized (Pro15, Pro30), and 1 wt% (Pro1Z) and 3 wt% (Pro3Z) zirconium added groups. Immersed in beverages for 1, 7, and 14 days, pH and microhardness were assessed. SEM examined random samples. Statistical analysis used repeated measures ANOVA, and post hoc tests (p Results: The gingiva-coloured composites displayed noteworthy time-associated microhardness changes (p 0.05). Despite variable pH levels in beverages, no substantial group interaction effects were observed (p > 0.05). Initial microhardness rankings shifted after a 14-day immersion. Conclusions: Gingiva-coloured composite exhibited the highest microhardness pre- and post-immersion, followed by Procryla30 and Imicryl groups. .
文摘The most commonly used material for constructing complete dentures is polymethyl methacrylate (PMMA). However, the strength characteristics of PMMA, such as impact strength and fatigue strength, are poor, and fracturing of PMMA dentures is a common problem in prosthodontic practice. Reinforcing PMMA with various materials, such as carbon fibers, glass fibers (fiberglass), and ultrahigh modulus polyethylene fibers, has been suggested to strengthen the denture-base material. A common problem encountered when packing the resin on these specimens is fiber slippage beyond the denture edges. The present study proposes an alternative method of incorporating fiber meshes into complete dentures, whereby a thin filament of self-polymerizing resin at the perimeter of the fiber mesh is produced, giving a clear and stable shape to the mesh that fits the upper jaw cast. During placement of the shaped mesh on the cast, a positive-negative relationship is created between the mesh and cast, which immobilizes the mesh during the incorporation process.
文摘Background: Pediatric herpetic stomatitis is a common pediatric disease, the course of the disease is about 1 - 2 weeks, and it usually occurs in 1 - 3 years old children, due to infection of herpes simplex virus I and onset. Objective: To observe the curative effect of Chinese medicine prescription (Yueshi, Zhongbai, Qingdai, Phelloderma phelloderma and glycyrrhiza) on children with herpetic stomatitis. Methods: 90 patients were divided into 2 groups by random number table method. 45 patients in the observation group were treated with traditional Chinese medicine, and 45 patients in the control group were treated with iodine glycerin. The total effective rate after treatment was compared between the two groups. Results: The total effective rate of the treatment group was 95.55%;the total effective rate of the control group was 71.11%;the total effective rate of the two groups after treatment was compared;the treatment group was significantly better than the control group;the difference was significant (P 0.05). Conclusion: The external use of Chinese medicine in the treatment of pediatric herpetic stomatitis has achieved good results, improving the condition of children, and significantly improving the clinical efficacy.
文摘Severe dementia, including Alzheimer’s disease, is intractable and progressive. The purpose of this study was to document the improvement observed in two cases of severe dementia after denture placement. The subjects, two women in their 70s with severe dementia, were diagnosed with Alzheimer’s disease by the medical doctors in charge of their cases. In the first case, the subject’s symptoms included severe disorientation, impaired communication, and a tendency to wander. Two weeks after a lower complete denture placement, she was able to greet others. Five weeks later, she was able to communicate and precisely read an analog clock. Her condition continued to improve for at least 3 months. In the second case, the subject’s mouth was always open. She was bedridden in a vegetative state and hardly moved, talked, or laughed;it was impossible to communicate with her. Only a few seconds after an upper complete denture placement, she started to talk, albeit unintelligibly. Two weeks after a lower complete denture placement, she could communicate normally, laugh, and walk without help. Although the underlying mechanism has not yet been clarified, the author hypothesizes that positive signals from the oral area are transferred to the brain as positive stimulation via the trigeminal nerve, the largest cranial nerve.

文摘Objective:To study the relationship between plasma treatment time acrylic resin denture material in the size of 2 mm × 10 mm × 10 mm. and efficacy. Methods:Test specimens were prepared from an Plasma treatment was carried out on the surface of Polymethyl methacrylate(PMMA) at different time. XPS studies, IR spectra studies and measurement of wetting angle were performed. Results: XPS showed the peak corresponding to C-O getting higher as the treatment proceeded, however at 120 seconds, the peak did not increase any longer and partly crossed with the peak at the duration of 60 seconds. IR spectra showed the wave corresponding to C-H was reduced as O2-plasma treatment proceeded, and then changed little, Wetting angle initially decreased dramatically, however, as the reaction proceeded, wetting angle increased slightly. Conclusion:Equilibrium was reached for introducing oxygen-containing groups and changing of C-H. As the treatment proceeded, wetting angle increased slightly.
文摘Objective: The aim of this survey is to evaluate the effect of a new developed laser pen as a home care device for patients’ pain reduction with recurrent aphthous stomatitis. Background data: Recurrent aphthous stomatitis (RAS) is one of the most common and painful ulcers in the oral cavity. Low Level Laser Therapy (LLLT) has been used successfully for reducing pain and healing time in comparison with corticosteroids and placebo in some studies. But these treatments are done in the clinics and it seems that there is no report of the laser treatment as a home care device up to this study. Methods: This was a prospective, randomized trial pilot study. Thirty patients with RAS were divided into three similar groups. The group one (n = 10) was treated with a diode laser, 660 nm, 40 mw, continuous wave, 1.2 J, 6 J/cm2, for five consecutive days (twice per day, 30 seconds each session). The second group (n = 10) was treated with a topical triamcinolone (adcortyl in orabase) ointment four times per day and the third group (n = 10) was treated with placebo (red light pen) the same way as Group 1. All the patients were evaluated with a Visual Analogue Scale (VAS) before and after treatment on the consecutive days. The data was analysed by one way ANOVA and PostHoc tests. Results: The results show that the laser pen and triamcinolon in orabase ointment have similar results and both have better results than placebo in the control of the recurrent aphthous stomatitis pain (p value: 0.001). Conclusion: In this clinical pilot study the laser pen as a home care device seems to be useful for the treatment of RAS.
文摘The effects of 3 chairside polishing kits and mechanical brushing on the surface roughness of 3 different acrylic denture base resins were compared. Acrylic denture base resins (auto-polymerizing, heat-polymerizing, injected heat-polymerizing resins) were examined after a tungsten carbide bur, and after chairside polishing using 3 polishing kits and pumice. The specimens were subjected to mechanical brushing using a wear tester to simulate 30 000 strokes of brushing. The surface roughness of the acrylic denture base resin specimens was measured using a contact pro-filometer. After the test, the random polished acrylic resins were evaluated by scanning electron mi-croscopy (SEM) and atomic force microscopy (AFM). Acrylic denture base resins polished using the 3 types of polishing kits had a smoother surface than those finished with the tungsten carbide bur (p〈0.05). The surface of the resin polished by a TC cutter exceeded the Ra of 0.2 μm (p〈0.05). The auto-polymerizing resin showed a significantly higher surface roughness than the heat-polymerizing resin and injected heat-polymerizing resin (p〉0.05). In the case of polishing step wise, there was almost no change in surface roughness after brushing (p〉0.05).
文摘Swine vesicular stomatitis is an acute, febrile, highly contagious zoonotic diseases caused by vesicular stomatitis virus (VSV). Disease in affected pigs was typically characterized by vesicular lesions on the mouth, snout, oral mucosa, coronet and interdigital skin. The disease is endemic in the Americas, Europe and Africa and other places. As the foreign trade of animals and animal products increase, the risk of swine vesicular stomatitis spreading into our country also will be increased. Therefore, the accurate and timely diagnosis and control of swine vesicular stomatitis are critical and necessary.
文摘Background: Necrotising Stomatitis is an uncommon oral infection associated with diminished systemic resistance (immunocompromised state) including HIV infection. Significant predisposing factors include poor oral hygiene, unusual life stress, recent illness (e.g. Measles), malnutrition, smoking and even inade- quate sleep. It occurs commonly in developing na-tions with poor living conditions. Objective: To present a case of severe Necrotising Stomatitis in a previously undiagnosed HIV Seropositive patient. Patient and method: This case report describes severe, rapidly spreading necrotising stomatitis in a 35 years old lady, the condition which led to her being diagnosed with HIV. The treatment modalities, challenges of man-agement and the differential diagnosis were discussed. Result: During the course of her management, pa-tient’s oral condition improved. However, later re-currence was seen due to severe anaemia caused by malnutrition and aggravated by Zidovudine antiret-roviral drug. Conclusions: There is need for interdis-ciplinary interactions between the dentists and the physicians managing HIV patients, to allow effective management and afford patient the best treatment.
文摘We present the case of a 46-year old man with a two-year history of losing his denture presenting with cachexia and a two-month history of dysphagia to both liquids and solids associated with odynophagia. Review at the ENT department where a diagnostic rigid oesophagoscopy was done under general anaesthesia showed a denture impacted to the wall of the oesophagus at 17 cm from the incisor. Attempts at retrieval were unsuccessful as the denture was deeply embedded in the oesophageal wall <em>and bled easily on attempted removal</em>. He subsequently underwent a 2-stage surgical management approach by the cardiothoracic surgical team. Patient first underwent nutritional rehabilitation for a month through a created feeding gastrostomy tube via a limited midline laparotomy. He then had a open cervical oesophagotomy and removal of the denture. Currently doing well six months after surgery and tolerating normal oral diet.
文摘With the increased life expectancy, complete dentures have been used in the treatment of edentulism<span> </span><span>progressively. It restores stomatognathic system functions, providing improvements in chewing, aesthetics, and phonetics. The aim was to report an oral rehabilitation of a patient with severe aesthetic and functional impairment, with Angle’s class III malocclusion. The patient sought the extension project of the School of Dentistry of the Federal University of Minas Gerais with aesthetic complaints. After clinical and radiographic evaluation, the proposed planning was immediate complete denture maxilla and immediate implant-retained overdenture in mandible. During clinical evaluation it was observed that the patient was Angle</span><span>’</span><span>s III. The rehabilitation was performed as proposed and at the end of the treatment, it was possible to revert the class III occlusal pattern to Angle’s I. The excellent maxillomandibular relationship was achieved, which ensured optimal facial aesthetics and functional condition. It was concluded that the immediate complete dentures and immediate implant-retained overdenture might be beneficial for restoring the aesthetics and function of patients with Angle’s class III malocclusion.</span>

文摘The research and application on small denture machining equipment are great breakthrough for modern dental restoration technology. In this paper, a small denture machining equipment made of two spindles with four-axis was designed based on machining characteristics and functional analysis. Position accuracy and re-position accuracy were measured by accuracy instrument. In order to test its machining capacity, some typical microstcucture parts, such as straight channel, hemispherical surface, and molars coronal, were selected for high speed milling. It was obtained that the denture machining equipment met the machining requirements with high quality and efficiency, according to the acquisition and analysis of form and position errors, surface roughness, and 3-D profile.
文摘In the present study, an aero pneumatic fatigue testing machine for complete dentures was designed, fabricated, and tested for the evaluation of the fatigue life of reinforced complete upper denture (CUD). On completion and testing, it was observed that the machine has the potential of generating reliable number of cyclic data. The machine’s performance was evaluated using test specimens of identical CUDs that were machined in conformity with standard procedures. The fatigue machine compressed the lower dental arch over the upper denture-specimen in centric occlusion, in the same way that the two masticatory muscles pull the lower jaw over the upper jaw during chewing. The incorporation of glass fibres into the CUD using a sandwich technique quadruples the lifespan of the denture (<em>P</em> = 0.004). The low standard deviation, along with the low coefficient of variation (CV) of the group of unreinforced dentures shows the repeatability of the results and the reliability of the machine. The high standard deviation and coefficient of variation of reinforced dentures was expected, since a high variation of results is usually recorded in fibre reinforcement cases. This research confirmed the view that the crack during denture fracture initiates in the anterior palatal area and propagates to the posterior.
文摘Background: Recurrent aphthous ulcer (RAS) is a common oral disease where its etiopathogenesis is not well elucidated. There was no effective curative therapy for this disease. Isotretinoin has been recently used in the treatment of Behcet’s disease. Objectives: To evaluate the efficacy and safety of isotretinoin in treating RAS and the long term remission of RAS. Patients and Methods: This single-blind controlled therapeutic study conducted in Department of Dermatology-Baghdad Teaching Hospital during February 2011-January 2012. Thirty patients with typical RAS were included in this work. Detailed history and full examination were done for all patients. They were given isotretinoin 20 mg orally once daily for three months to be seen on Day 14 firstly and then monthly to be assessed using the oral clinical manifestation index (OCMI). After isotretinoin was stopped three months later, patients were given placebo therapy for another 3 months. Results: The results of 30 treated patients were as follows: 17 (56.67%) males and 13 (43.33%) females with male to female ratio was 1.3:1. Their ages ranged from 12 - 60 (35.33 ± 12.06) years. The OCMI before isotretinoin therapy ranged from 7 - 17 (13.13 ± 2.55), while after therapy the mean started to decline to a lower level within the first 14 days (P = 0.103), and continued to decline significantly until the end of the first month of therapy (P = 0.023). Then the OCMI declined very significantly until the end of fourth month of therapy (P < 0.001). After that the mean started to increase until the end of the 5 months (with placebo) but it remained statistically significant compared with the baseline of mean of OCMI before treatment (P = 0.046). Then it continued to increase to become not significant at the end of 6 months of therapy (P = 0.107). Conclusion: Isotretinoin is an effective therapeutic and prophylactic promising remedy in treatment of RAS.
文摘Opioids are widely used as analgesics for oral mucositis in allogeneic hematopoietic cell transplantation (allo-HCT). Their main adverse events are nausea, vomiting, constipation, psychological symptoms, and respiratory depression. In our institute, continuous intravenous morphine was generally used until 2007, followed by intravenous fentanyl as the first-line agent because of its potential fewer adverse events. We retrospectively analyzed 99 patients who underwent allo-HCT in the University of Tsukuba Hospital from 2004 to 2009. Out of 99 patients, 64 were treated with opioids (morphine, 32 and fentanyl, 32). The attending physicians were in charge of providing stable pain control. Median age, sex, stem cell source, preparative regimen, and GVHD prophylaxis were similar in the two groups. There were no significant differences in psychological symptoms, drowsiness, nausea, and vomiting in both groups. Defecation ratio (the days having a bowel movement/the days taking opioids) was 63% and 94% in the morphine and fentanyl group, respectively (P < 0.0001). The percentage of patients who needed to use purgative drugs was 25% and 6% in the morphine and fentanyl group, respectively (P = 0.04). It is suggested that fentanyl has less adverse effects on gastrointestinal movement and is safer than morphine when used for oral mucositis in allo-HCT.
文摘Background: Recurrent aphthous stomatitis (RAS) is a major oral health problem, where its etiopathogenesis is not well understood. Accordingly, its therapy whether topical or systemic can induce clearance, but the relapse rate is high. Objective: To use 100% topical pumpkin seed oil in RAS as it has many actions as anti-inflammatory and antioxidant effects. Patients and Methods: This single, blind, clinical, therapeutic trial was conducted in the Department of Dermatology-Baghdad Teaching Hospital, Baghdad, Iraq, during February 2015-August 2016. Twenty-five patients with RAS were included in this work. After full history and clinical examination, the clinical diagnosis was established. Oral clinical manifestation index score (OCMI) was carried out before, during and after stopping therapy to assess the different parameters of this score. 100% Topical pumpkin seed oil was given two times daily for 3 months during which short term assessment for each patient was done by using OCMI before, after 4 days and after 8 days of therapy in addition to assessing the mean size of largest diameter of ulcers in each visit to evaluate the therapeutic efficacy of the therapy, while long term assessment of the OCMI was done every month for 3 months after starting of the therapy to evaluate the prophylactic effect of the therapy. After 3 months, pumpkin seed oil was stopped and assessment of OCMI was done monthly for another 3 months to evaluate the remission action of pumpkin seed oil. Results: Twenty-five patients were included in this study: 15 (60%) males and 10 (40%) females. While their ages ranged from 9 - 60 (27.48 ± 11.97) years. The mean of OCMI before pumpkin seed oil therapy was ranged from 9 - 15 (12.96 ± 1.42), while after the therapy the mean started to decline to lower level within 4 days of therapy and was statistically significant (p < 0.0001) and continued to decline significantly till the end of third month of therapy (p < 0.0001). The percent reduction was 79.30% after 4 days of therapy and 94.38% after 8 days of therapy. After stopping therapy, the mean of OCMI started to increase, but it is remained statistically highly significant (p < 0.0001) at the end of third months after stopping therapy when it is compared with baseline mean of OCMI. Conclusion: Pumpkin seed oil had an effective therapeutic and prophylactic action against RAS, in addition, it induced remission for at least 3 months after stopping therapy. No local or systemic side effects were observed during the course of therapy.
文摘This paper studied the dynamics of local immunity factors of the oral cavity in women with recurrent aphthous stomatitis against the background of urogenital infection. The research proves the maximum efficacy of hydroxyzine hydrochloride, aminodihydrophthalasindione sodium and Eplan used in multiple treatments of recurrent aphthous stomatitis in patients with urogenital infection as evidenced by a marked reduction in the coefficient of local immunity factors balance, which is observed as early as on day 7 of the therapy provided. Moreover, the findings show the growth of immunological parameters of oral fluid (lysozyme and SIgA), the normalization of the coefficient of local immunity factors balance in the treatment of recurrent aphthous stomatitis in women with urogenital infection, the improvement being observed 3 months after the treatment suggesting a favorable local immunity of the oral cavity.
文摘The aim of this study was to evaluate whether partial removable denture use indeed leads to improved oral health related quality of life and masticatory function. Materials and Methods: Partially edentulous patients presenting for removable denture treatment at the Prosthodontics Service in the Hospital Affiliated to Kinshasa University (Democratic Republic of Congo) were assessed for enrolment in this study. After applying exclusion criteria, 378 patients were included in the study, and randomly assigned into 2 groups. Oral health related quality of life (OHIP-23) and mastication time (MaT), number of chewing cycles (MaC), mastication frequency (MaF), and the sizes of the peanut fragments (FraS) were compared in both groups. Statistical significance was set at p < 0.05. Results: The average MaC, MaT, MaF, and FraS were 119 (± SD 53.70), 86.75 (±SD 35.35), 1.38 (±SD 0.25), and 3.3 (±SD 3.25) for the denture group and 77.9 (±SD 23.9), 60.2 (±SD 17.91), 1.29 (±SD 0.15), and 1.5 (±SD 0.7) for the non-denture group, respectively. Conclusion: The overall oral health related quality of life was best in the non-denture than denture.
文摘Objective: The goal of the removable partial denture is to restore impaired esthetics and masticatory function by replacing missing teeth. The aim of this study was to establish a possible correla-tion between removable partial denture in acrylic resin use after 5 years and dental diseases. Materials and Methods: Partially edentulous patients presenting for removable denture treatment at the Prosthodontics service of the Affiliated Hospital of Kinshasa University, Democratic Republic of Congo were assessed in this study. Patients were randomly assigned into 2 groups: Denture and non-denture group. Caries and periodontal diseases were compared between both groups. Statistical significance was set at p < 0.05. Results: The average age in the denture and non-denture-group was 53.15 (±SD 22.05) and 31.59 (±SD 11.98) years. Out of 160 teeth were decayed, both 110 teeth (68.75%) in the denture-group and 50 teeth (31.25%) in the non-denture group. The DMFT index calculated at start time of the study was 0.2 for both groups, after five years was 7.1 for the denture-group and 2.6 for the non-denture-group. The plaque index in the denture-group had an index of 1 and that of non-denture-group was 0.5. Conclusion: The relationship has been found between patients’ wearers a RPD and dental diseases.
