Objective: The present study of interphase cytogenetics and conventional karyotyping was undertaken to perform comprehensive analysis of CLL genetics with an approach to refine early prognostication of disease. Material & Methods: Retrospective analysis by fluorescence in situ hybridization (FISH) was carried out on total 671 patients of CLL at diagnosis between 2008 and 2015. Conventional cytogenetics studies were performed in 50 of 671 patients using CPG Oligonucleotide + IL-2 and TPA (12-O-Tetradecanyl Phorbol 13-acetate) for stimulation of lymphocytes cultures. Results: Interphase cytogenetics could detect recurrent abnormalities such as del(13q14), +12, del(17p13), del(11q22), del(6q23) in 71% of cases. The incidence of del(13q) was higher in Rai stage 0, I, II (p = 0.0005);whereas patients with ≥2 aberrations were more common in advance stage III, IV (p = 0.001). Frequency of IgH translocation was 7%. Morphology and immunophenotypic analysis revealed atypical CLL with higher frequency of t(14;19) than t(14;18). Conventional karyotype could detect abnormal karyotype in 97% of cases which displayed targeted FISH abnormalities along with additional non-targeted chromosomal abnormalities. Patients with negative FISH markers showed clonal non-recurrent numerical and structural changes. The complex karyotype was identified in 24% cases which included targeted FISH aberrations as well as non-targeted numerical and structural abnormalities like deletions, and unbalanced translocations. A significant association was observed between complex karyotype and coexistence of ≥2 FISH markers (p = 0.009) and del(11q22) &/or del(17p) (p = 0.03). Conclusion: Our data of interphase FISH with integration of conventional karyotyping revealed genomic complexity that helped identification of biological subclasses with clinical impact at diagnosis. Further, these cytogenetic subclasses along with molecular markers are likely to evolve more refined prognostic groups, which will help design risk-adapted therapies in B-CLL.展开更多

In the same period, workflow management has experienced significant development, but has become more focused on the industry. However, it seems to us that document workflows have not had the same interest for the scientific community. But nowadays, the emergence and supremacy of the Internet in electronic exchanges are leading to a massive dematerialization of documents;which requires a conceptual reconsideration of the organizational framework for the processing of said documents in both public and private administrations. This problem seems open to us and deserves the interest of the scientific community. Indeed, EDM has mainly focused on the storage (referencing) and circulation of documents (traceability). It paid little attention to the overall behavior of the system in processing documents. The purpose of our researches is to model document processing systems. In the previous works, we proposed a general model and its specialization in the case of small documents (any document processed by a single person at a time during its processing life cycle), which represent 70% of documents processed by administrations, according to our study. In this contribution, we extend the model for processing small documents to the case where they are managed in a system comprising document classes organized in subclasses;which is the case for most administrations. We have thus observed that this model is a Markovian <i>ML×K/ML×K/</i>1 queues network. We have analyzed the constraints of this model and deduced certain characteristics and metrics. <span style="white-space:normal;"><i></i></span><i>In fine<span style="white-space:normal;"></span></i>, the ultimate objective of our work is to design a document workflow management system, integrating a component of global behavior prediction.展开更多

In most cases, it occurs in patients with immune deficiency. Its exceptional location at the wrist can maintain doubt with rheumatic causes which are more frequent. IgG4 deficiency found in this patient was the only cause that could be impugned. We report the case of a young woman of 46 years without any evident cause of immunodeficiency and any particular risk factor, managed for septic arthritis of the wrist due to Streptococcus pneumoniae. This case without any apparent risk factors demonstrates that pneumococcal infection in a more distant articulation of the ENT is still possible. Scan has been a very significant contribution in confirming the positive diagnosis that leads to surgical drainage. The approach of the systematic search for the gateway must be done. In the study of the patient’s immune status, the realization of the protein profile could be interesting and then guide the practitioner in the decision to vaccinate the patient against Streptococcus pneumoniae if he was not. We recommend looking always for a cause of immunodeficiency including primary Ig defiencies even among adult.展开更多

We recommend looking always for a cause of immunodeficiency including primary Ig defiencies even among adult.

文摘Inactivated coronavirus disease 2019(COVID-19)vaccines such as CoronaVac and BBIBP-CorV have been widely used in China. However, more investigation is still needed to understand antibodies'duration and effectiveness against severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)variants in the real world. In this study,575 participants who had been vaccinated with two or three doses of the inactivated vaccine were recruited.Serum samples were collected and tested for anti-spike IgG and neutralizing antibodies against SARS-CoV-2(original strain,Dela, and Omicron).Unsurprisingly,a third dose of the vaccine significantly enhanced antibody responses against SARS-CoV-2 and its variants. However, despite a booster dose, the neutralizing antibody levels against Omicron, particularly the BA.5.2 subvariant, remained low. There was no sex bias, but an age bias was observed.Notably, the predominant IgG subclass antibodies were IgG1 and IgG2,with a much lower level of IgG4.After the booster shot, the ratio of IgG4 to IgG1 significantly increased. The observation of IgG1 to the IgG4 class switch after repeated inactivated vaccinations underscores the importance of continuous monitoring of subclass antibody responses.Further clinical investigations are required to understand the implications of this class switch for optimizing immunization strategies.

文摘Accurate high-resolution maps of oil palm plantations underpin effective management of environmental and socio-economic impacts at global, regional, and national levels. However, young industrial and highly irregular small-holder plantations are mostly unmapped and not included in official FAO statistics. This issue is addressed here by discriminating global oil palm plantation in 2020 into four subclasses:Industrial Mature Oil Palm(IMOP);Industrial Young Oil Palm(IYOP);Smallholder Mature Oil Palm(SMOP);and Smallholder Young Oil Palm(SYOP).Data, resolved to 4.77 m,from Planet&NICFI,Sentinel-1/2,were combined with other layers using the image-oriented classification and regression tree(CART)algorithm which performed best in classification tests. Results show that SMOP dominates distributional extent, but it was also the most accurately mapped subclass typically found at 500–1000 m altitude.IMOP had the most extensive altitude range of 500–1300 m,while IYOP and SYOP were found at similar altitudes of 500–800 m and 500–900 m respectively.Recent developments in South East Asia show oil palm plantations expanding into new areas with a slope of 24 degrees. Results provide data to support Sustainable Development Goal by assisting future oil palm-related development planning and monitoring in the world’s major oil palm-growing countries.

文摘In this paper, we establish the Fekete and Szego inequality for a class of holomorphic functions in the unit disk, and then we extend this result to a class of holomorphic mappings on the unit ball in a complex Banach space or on the unit polydisk in Cn.
