<strong>Objective:</strong> The purpose of this work was to determine the epidemiological, clinical and evolutionary aspects of the pulmonary embolism of the young person in the cardiology department of th...<strong>Objective:</strong> The purpose of this work was to determine the epidemiological, clinical and evolutionary aspects of the pulmonary embolism of the young person in the cardiology department of the University Hospital Center (CHU) Point G in Bamako-Mali. <strong>Methodology:</strong> This was an analytical study from January 01, 2018 to December 31, 2018 in the CHU Point G cardiology department, including all patients hospitalized during this period. <strong>Results:</strong> Of 1379 hospitalized patients, 19 patients were concerned by pulmonary embolism of the young person. The prevalence of pulmonary embolism of the young person was 1.37%. The most affected age group was 21 - 30 (47.4%) of patients. The predominance was female (89.47%) with a gender ratio of 0.11 in favour of women. The average age in the series was 29.79 years with extremes of 16 years and 40 years. Factors predisposing to pulmonary embolism were dominated by peri partum, cardiomyopathy and obesity with 47.3%, 31.57% and 21.1%, respectively. The dominant signs were chest pain and dyspnea in 94.7% and 89.5% of cases respectively. Pulmonary embolism was unlikely in 60.52% according to the Geneva and Wells score simplified. At the thoracic angioscanner, the embolism was bilateral in 52.6% of cases and distal in 36.8% of patients;in 10 patients who performed cardiac ultrasound, pulmonary arterial hypertension (70%), dilation of the right ventricle (20%) and left ventricular dilation (40%). Hyper leucocytosis (47.4%), anemia and low prothrombin rate (TP) (22.2%) were the most found biological abnormalities. More than 2/3 (68.5%) our patients had an intermediate mortality risk according to the PESI (Pulmonary Embolism Severity Index) score. The average hospital stay was 10 days. Hospital mortality was 10.5%. <strong>Conclusion:</strong> The pulmonary embolism of the young person is a frequent, serious and multifactorial pathology and the female sex is most affected especially during peri partum periods, hence the need for preventive measures. Clinical signs are not specific and based on the assessment of clinical probability. Pulmonary angioscanner remains the confirmation review in our context.展开更多
BACKGROUND Chronic hepatitis C(CHC)is associated with type 2 diabetes mellitus.Although the pathogenesis remains to be elucidated,a growing evidence has suggested a role of pro-inflammatory immune response.Increased s...BACKGROUND Chronic hepatitis C(CHC)is associated with type 2 diabetes mellitus.Although the pathogenesis remains to be elucidated,a growing evidence has suggested a role of pro-inflammatory immune response.Increased serum concentrations of Interleukin 6(IL-6)have been associated with insulin resistance,type 2 diabetes mellitus as well as advanced forms of liver disease in chronic hepatitis C infection.AIM To investigate the frequency of IL-6-174G/C(rs1800795)single nucleotide polymorphism(SNP)in CHC patients and in healthy subjects of the same ethnicity.Associations between type 2 diabetes mellitus(dependent variable)and demographic,clinical,nutritional,virological and,IL-6 genotyping data were also investigated in CHC patients.METHODS Two hundred and forty-five patients with CHC and 179 healthy control subjects(blood donors)were prospectively included.Type 2 diabetes mellitus was diagnosed according to the criteria of the American Diabetes Association.Clinical,biochemical,histological and radiological methods were used for the diagnosis of the liver disease.IL-6 polymorphism was evaluated by Taqman SNP genotyping assay.The data were analysed by logistic regression models.RESULTS Type 2 diabetes mellitus,blood hypertension and liver cirrhosis were observed in 20.8%(51/245),40.0%(98/245)and 38.4%(94/245)of the patients,respectively.The frequency of the studied IL-6 SNP did not differ between the CHC patients and controls(P=0.81)and all alleles were in Hardy-Weinberg equilibrium(P=0.38).In the multivariate analysis,type 2 diabetes mellitus was inversely associated with GC and CC genotypes of IL-6-174(OR=0.42;95%CI=0.22-0.78;P=0.006)and positively associated with blood hypertension(OR=5.56;95%CI=2.79-11.09;P<0.001).CONCLUSION This study was the first to show that GC and CC genotypes of IL-6-174 SNP are associated with a decreased risk of type 2 diabetes mellitus in patients chronically infected with hepatitis C virus.The identification of potential inflammatory mediators involved in the crosstalk between hepatitis C virus and the axis pancreas-liver remains important issues that deserve further investigations.展开更多

In this paper, I suggest that, even more than the state, capitalist enterprise is the most influential institution of neoliberalism, both for the effect it conveys in terms of management in public institutions (New P...In this paper, I suggest that, even more than the state, capitalist enterprise is the most influential institution of neoliberalism, both for the effect it conveys in terms of management in public institutions (New Public Management) and its influence on the subjectivity of neoliberal individuals. However, one could stress, from the perspective of economic theory of the firm, the neoliberal transformations of the capitalist enterprises itself. Two transformations that are taking place from the 1980s seem particularly important. The first is the firm's transformation in a "nexus of contracts" as claimed by the agency theory of Jensen and Meckling that describe and enable the current financialization of the economy. The second is the trans- formation of organisations in a "nexus of skills" as claimed by the cognitive paradigm of the firm that apply to the company economic reasoning in terms of "human capital". I then examine the effects on neoliberal subjectivation of these two major changes, from the labour point of view. First, the neoliberal subject imagines, describes and manages itself as the subject of investment in "human capital". Second, the disappearance of traditional employment contract corresponds to the emergence and intensification of another form of contract, the "psychological contract", which tends to become a major form of relationship-employed but also a model for relationships between individuals. Finally, lifelong learning is becoming the major form of empowerment of employees, but also a "lifestyle" based on the apprecia6on of one's own human capital and the infinite task of perfecting oneself to match up our "skills portfolio" with the labour market. These transformations of neoliberal subjectivity have costs that can be summarised by the widespread phe- nomena of burn out, but also the birth of a new form of racism, based on "quality" of human capital (skills, abilities, capabilities).展开更多

The response of interest for a specific game is whether the home team won, tied or lost, who the home team was, and who the opponent was. The response is ordinal-valued. A generalized linear model is fit and then, given the remaining fixtures, used to predict the final standings of the season. Other explanatories, such as round number, are considered for inclusion in the model. Simulation is employed to estimate probabilities of interest.展开更多
