Effects of high-quality neurosurgical nursing care on improving clinical nursing quality

BACKGROUND With continuous advancements in medical technology,neurosurgical nursing is constantly developing and improving to provide higher-quality nursing services.AIM To explore the effects of different types of high-quality nursing care on clinical nursing quality and patient satisfaction in neurosurgical nursing.METHODS Eighty patients who received neurosurgical treatment in the Affiliated Hospital of Southwest Medical University from June to December 2020 were selected as study participants and categorised into study and control groups.The study group comprised 40 patients who received 4 different types of high-quality nursing care,whereas the control group comprised 40 patients who received conventional nursing care.After a specific period,nursing satisfaction levels and adverse event and complication rates were compared between the two groups.RESULTS Satisfaction with high-quality care was higher than that with conventional care,and high-quality health services and regional services showed the highest satisfaction levels,with an average score of 12 on the Glasgow scale.The satisfaction levels of the study and control groups were 75%and 57%,respectively,with a statistically significant difference(t=7.314,P<0.05).During the nursing period,the adverse event and complication rates were the highest in patients with level III pathology grade and those who underwent neurosurgery(40.02%and 85.93%,respectively),and the difference was statistically significant.CONCLUSION In neurosurgical nursing,employing appropriate high-quality nursing methods can effectively reduce adverse event and complication rates in patients,thereby improving the quality of nursing care and increasing clinical nursing value. 展开更多

Objective:To enhance the overall nursing effectiveness in the rheumatology and immunology department,explore specific strategies to improve nursing quality using a high-quality care model,and develop a comprehensive nursing system.Methods:A total of 120 hospitalized patients with rheumatoid arthritis from 2022 to January 2024 were selected.These patients were randomly divided into two groups:an observation group(60 patients)and a control group(60 patients).The control group received conventional nursing care,which included creating a good care environment,providing drug guidance,and offering psychological support.In the observation group,a high-quality nursing model was implemented with a focus on meeting the specific needs of the patients.The two groups were compared in terms of complication rates,treatment compliance,nurse-patient disputes,patient satisfaction with nursing care,sleep quality(PSQI score),and pain levels(VAS score).Results:The incidence of complications in the observation group was 3.33%,significantly lower than that of the control group,with a statistically significant difference(P<0.05).Nursing compliance in the observation group was 100%,and patient satisfaction was 96.67%,both higher than in the control group,with a statistically significant difference(P<0.05).Additionally,the VAS pain score in the observation group decreased to 2.12±1.89,and the PSQI sleep quality score decreased to 6.89±0.33,both lower than in the control group,with statistically significant differences(P<0.05).Conclusion:Focusing on the needs of patients through a high-quality nursing model provides targeted care that improves overall nursing effectiveness for patients with rheumatoid arthritis.This approach enhances patient satisfaction with nursing care,increases treatment compliance,reduces the incidence of complications and nurse-patient disputes,improves sleep quality,and alleviates pain.This model holds significant value for broader applications. 展开更多

Objective:Many strategic efforts were made to address nurses'shortage in controlling Coronavirus disease 2019(COVID-19),including recruiting student health volunteers.This review aimed to explore the contribution and involvement of nursing student volunteers during COVID-19.Methods:A systematic review was registered in PROSPERO International CRD42021283069.Electronic databases for ar ticle search included:SCOPUS,EBSCO MEDLINE,Pub Med,Pro Quest,Springer,Sage Pub,and hand searching.The critical appraisal study quality using The Joanna Briggs Institutes.Data extraction and synthesis used Population,Intervention,Comparison,Outcome,and Design(PICO and D)framework with thematic analysis.Results:Twelve studies were enrolled from 2545 retrieved,with 4 synthesized themes:(1)determinant of the involvement of nursing student volunteers during COVID-19,which includes 3 subthemes:motivations,perception,and barriers,(2)expectations and actual condition of nursing students as volunteers,(3)the need for nursing student volunteers,and(4)the impact and level of student volunteers'contribution.All student volunteers contribute to providing direct or indirect services to patients.Conclusions:This review provides an overview of nursing student volunteers'needs useful in developing competency frameworks and the role of culturally sensitive volunteers in education and training.In the future,student volunteers could choose the appropriate volunteer program and location according to their competence. 展开更多

Effectiveness of flipped classrooms in nursing education: Systematic review and meta-analysis 被引量:2

Background: The flipped classroom has generated considerable interest in nursing education in the last few years, especially in higher education in China. However, research to date has been insufficient to confirm the effectiveness of the flipped classroom approach. Since most of the Chinese studies are published in Chinese journals, they may not be easily accessible to international researchers. Therefore,this meta-analysis aimed to review the effectiveness of a flipped classroom in nursing education both in English and Chinese databases.Objective: The aim of the present study was to identify the robust available evidence about the effectiveness of flipped classrooms in nursing education through a systematic review and meta-analysis.Methods: A systematic search of English databases, including PubMed, EMBASE, Science Director,CINAHL and Google Scholar, and Chinese databases, including Chinese National Knowledge Infrastructure(CNKI), WanFang Data, VIP Information and Chinese Biomedical Literature(CMB), were conducted to identify peer-reviewed studies that met the inclusion criteria. Two reviewers independently performed study identification with Endnote X7 software and used Excel to extract data. The risk of bias in the included studies was assessed with Cochrane Collaboration's risk of bias tool, and a meta-analysis was conducted using RevMan 5.3 software following the guidelines proposed by PRISMA reporting standards with the protocol register No. CRD42016041826.Results: A total of 29 studies were included in systematic review. There was a significant postintervention improvement in academic performance both in knowledge with a pooled random-effects standardized mean difference of 1.13 and skills with a pooled random-effects standardized mean difference(SMD) of 1.68. Students' self-learning abilities were also improved with a pooled random-effects SMD of 1.51 compared with traditional lecture. In the subgroup analysis, we found that the effect sizes had high fidelity in terms of nursing degrees and research settings. There was a high rating of flipped classroom pedagogy from teaching evaluations, study satisfaction, study attitude and improvement in critical thinking and problem-solving skills.Conclusions: These results indicated that a flipped classroom might help nursing students improve in knowledge, skills, attitudes, self-learning, study satisfaction, critical thinking and problem-solving skills.We recommend adopting this approach for nursing education reform. There is a call for robust empirical research and unification of appraisal standards for further support of the effectiveness of the flipped classroom. Furthermore, a meta-regression analysis is also recommended to explore the sources of heterogeneity in included studies. 展开更多

Objective:To evaluate the effectiveness of integrated nursing interventions for fatigue in patients with advanced cancer.Methods:Medline,Pubmed,Embase,CINAHL,Web of Science,and the Cochrane Library were searched systematically till June 2017.A systematic review was conducted to collect randomized controlled trials(RCTs)reporting on the effect of nurse-driven interventions to improve fatigue in patients with advanced cancer.Quality assessment was conducted using the Cochrane Collaboration’s risk of bias tool.Results:Six RCTs involving 736 adult participants were included.The fatigue intensity was improved significantly by nursing interventions.The analyzed results revealed significant improvements in the intervention group:less than 3 months(standard mean difference[SMD]=?0.33,95%confidence interval[CI][?0.48,?0.19],P<0.01)and more than or equal to 3 months(SMD=?0.40,95%CI[?0.57,?0.24],P<0.01).Four studies with a moderate risk of bias were judged,and the remaining studies were at high risk of bias.Conclusions:The results indicate that integrated nursing interventions may relieve fatigue in patients with advanced cancer.However,due to the high risk of bias in most of the included studies and the diversity of interventions,the results and implementation process should be carefully monitored. 展开更多

Background:The prevalence of frailty among nursing home residents is high.However,in China,prevalence of frailty keeps unknown in this population.The purpose of this meta-analysis is to assess the prevalence of frailty and prefrailty and find out the factors related to the prevalence of frailty among nursing home residents in China.Methods:We searched both English(PubMed,Cochrane Library,Embase,Web of Science and Scopus)and Chinese(CNKI,WanFang,SinoMed,and VIP)databases from their inception date until 15 September 2020.Two reviewers independently screened literature,extracted data,and evaluated the risk of bias included studies.Stata12.0 software was used to conduct meta-analysis.Results:A total of 16 studies were included in this review,involving 7,777 nursing home residents.The prevalence of frailty ranged from 29.2% to 66.6% in the included studies.The pooled prevalence of frailty and prefrailty were 49%(95%CI:43-55%)and 42%(95%CI:34–50%),respectively.The assessment tool(P<0.001),gender(P<0.001),self-rated health(P<0.001)and type of institution(P<0.001)were significantly associated with the prevalence of frailty among nursing home residents in China.Conclusions:The prevalence of frailty among nursing home residents in China was relatively high.Considering the adverse health outcomes of frailty,nursing home administrators should screen frailty regularly,identify the risk factors of frailty,take actions accordingly and improve the frailty status for older adults. 展开更多

Background: Nursing practice has to contribute to evidence pointing out why there is a need for more nurse-designed randomized control trials (RCTs) focusing on evidence-based practice (EBP). How far this EBP has progressed in different health aspects is usually established by systematic reviews of RCTs. Nurse-led RCTs exist but no study has addressed the essentials of nursing care. Aim: The aim was therefore to determine the essentials of nurses’ interventions by means of nurse-led RCTs in somatic care focusing on the stated context, goals, content, strategies as well as the nurse’s role related to effectiveness. Methods: A systematic review was realized according to Cochrane review assumptions to identify, appraise and synthesize all empirical evidence meeting pre-specified eligibility criteria. The PRISMA statement guided the data extraction process (n = 55) from PubMed and CINAHL. Results: Of the RCTs in somatic care, 71% showed a positive effectiveness of nurse-led interventions, of which the nurse had a significant role with regard to being the main responsible in 67% of the studies. Also, 47% of the RCTs presented a theoretical standpoint related to the nurse-led interventions and most prominent were international evidence-based guidelines. Goals were found to have either a patient-centered or a professional-centered ambition. Strategies were based on patient-directed initiatives, nurse-patient-directed initiatives or nurse-directed initiatives, while contents were built upon either a patient-nurse interaction or a nursing management plan. Conclusions: This review underlines the necessity of a holistic view of a person, as nurse-led RCTs comprising a patient-centered ambition, patient-directed initiative and patient-nurse interaction plan showed beneficial nursing care effectiveness, particularly if theory-based. In a nurse-led RCT, a basic theoretical perspective is advantageous as well as to elucidate the role of the nurse in relation to the estimated effects. 展开更多

Objective:To study the application value of high-quality nursing intervention in patients with cirrhotic ascites.Methods:From February 2018 to February 2020,80 patients with cirrhotic ascites,treated in Yancheng No.1 People’s Hospital,were selected as the subjects for this study.The patients were divided into two groups based on the type of nursing intervention:a routine group and an observation group,with 40 cases in each group.The routine group received routine nursing,and the observation group received high-quality nursing intervention on the basis of routine nursing.The nursing effects and adverse reactions of the two groups were observed and compared.Results:The total effective cases in the observation group were 39 cases,with a total effective rate of 97.5%;the total effective cases in the routine group were 33 cases,with a total effective rate of 82.5%.The total effective rate of the observation group was significantly higher than that of the routine group(P<0.05).In the routine group,there were eight cases of adverse reactions,in which the incidence of adverse reactions was 20.0%,whereas there were only two cases of adverse reactions in the observation group,in which the incidence of adverse reactions was 5.0%.The incidence of adverse reactions in the observation group was significantly lower than that in the routine group(P<0.05).Conclusion:The application of high-quality nursing intervention in patients with cirrhotic ascites can improve the nursing effect and reduce the occurrence of adverse reactions;thus,it should be popularized in clinical settings. 展开更多

Objective:To explore and analyze the effect of high-quality nursing on patients with epidemic hemorrhagic fever with renal syndrome.Methods:Ninety patients with epidemic hemorrhagic fever with renal syndrome treated in Huyi District People’s Hospital from October 2021 to January 2022 were randomly divided into an experimental group and a control group,with 45 cases in each group.The control group received routine nursing and the experimental group received high-quality nursing.The effect of routine nursing and high-quality nursing were compared and analyzed based on the Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),the degree of satisfaction with nursing care,and the patients’quality of life.Results:Before nursing,there was no significant difference in the patients’psychological state between the two groups(P>0.05);after nursing,there was significant difference in the patients’psychological state between the two groups(P<0.05);the degree of satisfaction with nursing care of the experimental group was significantly higher than that of the control group(P<0.05);comparing the quality of life of the two groups,the quality-of-life score of the control group was significantly lower than that of the experimental group(P<0.05).Conclusion:For patients with epidemic hemorrhagic fever with renal syndrome,high-quality nursing can significantly and effectively improve their psychological state,level of satisfaction with nursing care,and quality of life. 展开更多

A systematic review of the effectiveness of clinical nurse specialist interventions in patients with chronic obstructive pulmonary disease(COPD) 被引量:2

Objective: This review aimed to examine the effectiveness of clinical nurse specialist (CNS) interventions in patients with chronic obstructive pulmonary disease (COPD). COPD significantly affects people's health worldwide. With the development in nursing, CNSs are playing increasingly important roles in different departments. However, the studies on the effectiveness of CNSs in COPD are not as well organized as the studies on the effectiveness of CNSs in bronchiectasis and asthma. Therefore, this review aims to find some updated evidence on the CNS interventions for patients with COPD and on whether these interventions are effective.Methods: A narrative analysis of the data was performed for the eligible studies. Four databases were chosen: CINAHL, MEDLINE, British Nursing Index, and Cochrane Library. Other websites such as the National Institute for Health and Clinical Excellence, National Health Service Evidence, Association of Respiratory Nurse Specialists, and National Association of Clinical Nurse Specialist were searched as well. Two reviewers performed study identification independently, and all the retrieved articles were stored using the EndNote X7 software. The risk of bias in the included studies was assessed using the Cochrane Collaboration's risk of bias tool. Results: A total of nine studies were included in this review. There were five current interventions by CNSs for patients with COPD. These interventions were home nursing support, CNS]s supported discharge, multidisciplinary cooperation programs, nurse-led care programs, and self-care management education. The effectiveness of these five interventions was evaluated individually. There is low- to moderate-quality evidence indicating that home nursing support interventions may have a positive effect on mortality and quality of life. No significant difference in quality of life has been found between the CNS-supported discharge intervention and the usual service. The multidisciplinary cooperation program probably had a positive effect on quality ot life in patients with COPD. Both nurse-led care and self-care management education intervention had a positive effect on mortality of patients with COPD.Conclusions: The findings of this review provide updated evidence on the effectiveness of CNS interventions for patients with COPD. Although nine trials were included and five types of interventions were identified, there is still lack of high-quality evidence. 展开更多

Objective: To explore the effect of systematic pre-job training for isolation ward nurses during the Corona Virus Disease 2019 (COVID-19) pandemic. Methods: Establish a pre-job training program for the isolation ward for COVID-19, standardize the content of theoretical and skill training, formulate training SOPs, and conduct training for the nurses using online teaching assessment, video teaching, on-site scenario simulation operation drills, as well as real-time protection guidance and supervision. 60 nurses from non-infectious departments temporarily selected by the hospital were trained;the theoretical knowledge scores, quarantine techniques, and nursing quality of nurses before and after the training were compared, and the effect of the intervention was evaluated. Results: The scores of the COVID-19 protection theory test were 81.17 ± 8.46 after the nurses were trained for 3 days. The pass rates of hand hygiene compliance tests and protective clothing putting-on and taking-off practices were 96.67% and 100%, respectively. There was no significant difference between the scores of the COVID-19 protection theory test for the nurses that were trained for 3 days and the scores for the nurses originally at the quarantine zone (81.59 ± 7.59, P > 0.05). The pass rate of hand hygiene compliance and the pass rate of protective clothing putting-on and taking-off practices were significantly improved compared with those before training (81.67% and 56.67% respectively, P < 0.001). The scores of the COVID-19 protection theory test at 30 days of training were 95.67 ± 5.89, which were significantly higher than those at 3 days of training (P < 0.001). The qualified rate of disinfection and quarantine in the first month for the trained nurses entering the isolation ward was 89.47%;compared with that for the nurses originally in the isolation wards (94.7%), there was no significant difference (P > 0.05). The comprehensive nursing ability scores of bedside nurses in the first month of training were 80.14 ± 5.63, which were lower than those of nurses originally in the isolation wards (86.88 ± 4.53, P Conclusion: Systematic pre-job training for nurses in isolation wards can help improve nurses’ knowledge of the COVID-19, self-protection awareness, and protection skills, and can quickly train nurses who are competent for work in isolation wards. It is an important guarantee of “zero infection” for medical staff, and it can quickly and effectively help medical institutions respond to the COVID-19 pandemic in an emergency. 展开更多

The aim of this study was to identify and evaluate evidence of clinical nurses’ research capacity building in practice. A systematic review of studies of nurses’ research capacity building in practice was performed. The quality of the articles was evaluated and reflected on in accordance with the Quality Assessment and Validity Tool for Correlation Studies. The literature searches identified a total of 4748 abstracts and titles. Eight quantitative studies were included in the evaluation. Three themes emerged from the analysis: Failure to ensure research quality and standards, Developing a research culture and Collaboration and organization of research utilization. The first theme has one sub-theme: Lack of knowledge about how to increase research utilization. The second theme is based on three sub-themes: Ability to identify clinical problems, changing nurses’ attitudes to research and research supervision. Finally, the third theme has one sub-theme: Funding as a success factor. In conclusion, research capacity building requires the development of research competence to generate knowledge that enhances quality and patient safety. Nurse leaders are essential for establishing evidence-based practice and a research culture, thus enhancing nurses’ scientific attitudes and capacity. 展开更多

Healing in VLU is an incredibly complex process, which puzzles even experts. The majority of general nurses do not have this level of expertise, especially those nurses serving in community. Function is the basis of nursing activities. Patients always show different clinical manifestations and selfcare abilities due to various function states, which cannot be reflected completely by the existing nursing practice. How to describe nursing practice standardized in order to demonstrate the effectiveness of interventions and facilitate interdisciplinary communication is another urgent problem. Therefore, the aim of this project is to develop an accurate nursing program based on function in chronic venous leg ulcers, which can both satisfy the needs of patients and promote nursing revolution.Methods: This study will use International Classification of Functioning Disability and Health as a framework to choose suitable functions and to filter function classification standards of chronic venous leg ulcers through evidence-based systematic research.Nursing interventions related to VLU are selected based on Nursing Interventions Classification, adding other nursing activities by methods of evidence-based systematic review and clinical observation. Then, nursing interventions and function status are matched through steering committee. Finally, the Delphi survey method is adopted to make nursing program native and scientific.Conclusions: This study is expected to be very significant and meaningful in using standardized nursing terminology. The aim of this review was to clarify the elements that promote socialization of nursing professionals from the findings of previous studies, with keywords related to socialization among undergraduate nursing students, and obtain indicators for promoting professional socialization in nursing students in the current nursing education. The literature review was conducted using a qualitative analysis of the promotion of professional socialization among nursing students. The results were reported according to the reporting checklist of the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) statement. The authors conducted a literature search using electronic databases in July 2021, such as CINAHL (EBSCOhost), MEDLINE (EBSCOhost) for English articles, and Ichushi for Japanese articles. Included studies were from 1985 to 2021. Previous studies were integrated to current studies. A total of 23 articles in English and 34 articles in Japanese were analyzed. The methods for promoting professional socialization in nursing students include self-learning, online media, extracurricular elements, and clinical practice. Socialization process differed for each school year;the first-year students tend to have ideological answers, while the fourth-year students tend to have practical answers as a result of education. The process needs supports from instructors and mentors. Nursing education promoting professional socialization in nursing students has developed diverse modeling, positive effects for improving social skills, and impact of self-efficacy and self-esteem and has fostered professional commitment and professional identity. However, loss of idealism of professional identity after graduation should be considered with supports from instructors and mentors. To establish nursing professional identity smoothly, nursing instructors supporting skills should be developed by understanding students’ stage of professional socialization. 展开更多

BACKGROUND As a significantly important part of clinical practice,the professional nursing process can be advanced in many ways.Despite the fact that case reports are regarded to be of a lower quality grade in the hierarchy of evidence,one of the principles of evidence-based medicine is that decision-making should be based on a systematic summary of evidence.However,the evidence on the reporting characteristics of case reports in the nursing field is deficient.AIM To use the CARE guidelines to assess reporting quality and factors influencing the quality of case reports in the nursing field.METHODS Nursing science citation indexed(SCI-indexed)journals were identified from the professional website.Each of the identified journals was searched on their website for articles published before December 2017.Twenty-one sub-items on the CARE checklist were recorded as“YES”,“PARTLY”,or“NO”according to information reported by the included studies.The responses were assigned corresponding scores of 1,0.5,and 0,respectively.The overall score was the sum of the 21 sub-items and was defined as“high”(more than 15),“medium”(10.5 to 14.5),and“low”(less than 10).The means,standard deviations,odds ratios(OR),and the associated 95%confidence interval(CI)were determined using Stata 12.0 software.RESULTS Ultimately,184 case reports from 16 SCI-indexed journals were identified,with overall scores ranging from 6.5 to 18(mean=13.6±2.3).Of the included case reports,10.3%were regarded low-quality,52.7%were considered middle-quality,and 37%were regarded high-quality.There were statistical differences in the mean overall scores of the included case reports with funding versus those without funding(14.2±1.7 vs 13.6±2.4,respectively;P=0.4456)and journal impact factor<1.8 versus impact factor≥1.8(13.3±2.3 vs 13.6±2.4,respectively;P=0.4977).Five items from the CARE guidelines,5a(Patient),6(Clinical findings),8c(Diagnostic reasoning),9(Therapeutic intervention),and 11d(The main take-away lessons)were well-reported(Reporting rate more than 90%)in most of the included case reports.However,only three items,2(Keywords,OR=0.42,95%CI:0.19-0.92,P=0.03),4(Introduction,OR=0.35,95%CI:0.15-0.83,P=0.017),and 11b(The relevant medical literature,OR=0.19,95%CI:0.06-0.56,P=0.003)were considered better-reported after the CARE guidelines published in 2013.CONCLUSION The reporting quality of case reports in the nursing field apparently has not improved since the publication of the CARE guidelines. 展开更多

Aim:To determine the effect of foot reflexology intervention on pain and physiological indicators in postoperative patients.Data resources:The Cochrane library,PubMed,EMBASE,MEDLINE,SCOPUS,CINAHL,China National Knowledge Infrastructure(CNKI),VIP,and Wan Fang Data were searched from inception until January2020.Review methods:Studies were screened according to the PICOS principle.The methodological quality was assessed with Cochrane Risk of Bias Tool.Meta-analysis was performed using RevMan 5.3 software.Results:5 randomized clinical trials involving 318 participants were included in the meta-analysis.There were significant differences in pain and heart rate in postoperative patients between the two groups.Conclusion:The foot reflexology intervention decrease the postoperative pain and heart rate for the postoperative patients.It can be considered to be an effective non-pharmaceutical therapy to reduce the postoperative pain and heart rate.The evidence is not enough to prove the effectiveness of reducing postoperative blood pressure,respiratory rate,pulse rate and oxygen saturation using the foot reflexology. 展开更多
