Advances in TCM Treatment of Gastric Cancer and Studies on the Apoptosis

The significance of apoptosis in gastric cancer is now widely recognized, and the induction of apoptosis as a new approach to treat gastric cancer has aroused great interest. In recent years, studies on certain TCM drugs for treating gastric cancer and for inducing apoptosis have brought about great attention both at home and abroad. The following is a summary made in this aspect.

TCM Treatment of Rheumatoid Arthritis by Supplementing the Kidney and Invigorating the Blood Circulation

The therapeutic effects in treating the intermediate and late rheumatoid arthritis by supplementing the kidney and invigorating blood circulation were observed. In the 43 cases of the treatment group, No. 2 Qu Feng Shi Ling capsules (祛风湿灵胶囊2号) and Fenbid (芬必得) were prescribed, while in the 39 cases of the control group Lei Gong Teng Tablets (雷公藤总甙片) and Fenbid were given. The results showed that the total effective rate in the treatment group was more satisfactory than that in the control (P<0.05 or P<0.01). With less toxic effects, the former could better improve the local swelling and lower the blood viscosity.

Recent advances of TCM treatment of childhood atopic dermatitis

Atopic dermatitis( AD) is a common and frequently encountered disease, which frequently appears in children. According to statistics, the percentage of AD patients between ages of 6 to 20 in dermatology clinics is 30%~1, and the incidence in school-age children is 2%~3%~2. AD has no corresponding nomenclature in traditional Chinese medicine(TCM). According to the initial time of attack in early childhood, it can be classified as NaiXuan and TaiXuan; according to the location of the disease(elbow fossa and popliteal fossa), it can be classified as Si Wan Feng; according to the specific cases of different characteristics of lesions, it can be classified as ShiChuang, Jin Yinchuang, XueFengChuang. In TCM, the etiology of AD is complicated. It is generally considered that the pathogenesis of AD is spleen deficiency and damp-heat^3. When children suffer from pathogenic wind, pathogenic dampness, pathogenic heat, or insufficient natural endowment, damp-heat is produced and it injures patients' skin. Also latent damp-heat can deplete the blood and succus in the body, dry the skin and lead to AD. In short, AD can be divided into 3 syndrome types in TCM: syndrome of spleen deficiency, syndrome of damp-heat, syndrome of blood deficiency. At present, the clinical study of AD treated by TCM is increasing, but the study of children's treatment is still rare. This article aims to make a summary of clinical studies of children's AD treated with TCM in recent years.

TCM Stage Differentiation Treatment of Diabetic Gangrene——An Observation on Microcirculatory Changes

Microcirculatory changes and therapeutic effects in 39 non-insulin dependent diabetes mellitus (NIDDM) cases were evaluated. Under the same basic treatment in both groups, the treatment group (21 cases) was treated with TCM drugs according to its stages and compared with the control group (18 cases, treated with 654-2). The results showed that the microcirculatory cumulative scores were significantly decreased in both groups (P<0.05), but the decrease in the treatment group was more marked (P<0.05). In the treatment group, 15 cases were cured, 3 markedly effective, 2 effective and 1 ineffective; while in the control group, 13 cases were cured, 1 markedly effective and 4 ineffective. Statistically, there is no significant difference between the 2 groups (P>0.05) in therapeutic effects.

Treatment of Functional Retrograde Ejaculation with Acupuncture and TCM Herbal Drugs

Acupuncture at the Taichong (LR 3), Sanyinjiao (SP 6) and Ciliao (BL 32) points combined with TCM drugs for soothing the liver, replenishing the kidney, freeing the seminal passage, and eliminating the stasis showed effective for functional retrograde ejaculation in 25 cases. The total effective rate of 68.0% was significantly better than imipramine used in the control group (P<0.05).

饶旺福治疗头痛临床经验撷萃

国医名师饶旺福教授认为风邪是导致头痛最重要的诱因,风又有外风和内风之别,“头为诸阳之会”,最易招致风邪,常常夹痰、夹瘀;风邪侵袭头部,久则损伤脑络,气血运行不畅,形成瘀滞。《素问·评热病论篇》有云:“邪之所凑,其气必虚”,故饶老治疗头痛常不忘久病必虚、久病必瘀的特点,善用川芎、防风、羌活、蔓荆子等风药,尤其善用全蝎、土鳖虫等虫类药以扫除入络之顽风痼疾。文章从理论与临床实际出发,总结饶旺福教授中医治疗头痛四十年临床经验,以飨同道。

基于“浊毒-线粒体自噬”探讨慢性肾脏病心脏损害

心脏损害是慢性肾脏病进展过程中常见并发症之一,大量研究表明线粒体自噬能干预慢性肾脏病心脏损害的病程。线粒体自噬是当今医学调控慢性肾脏病心脏损害重要靶点及研究课题。慢性肾脏病患者体内毒素代谢异常,同时心肾两脏富含线粒体,慢性肾脏病后期尿毒症毒素内积,线粒体凋亡及受损线粒体释放出的大量有害物质的过程与中医浊毒内生相似。通过从慢性肾脏病、线粒体自噬与浊毒的关系,慢性肾脏病并发心脏损害的浊毒理论,浊毒、线粒体自噬与心脏损害的联系,浊毒-线粒体自噬-慢性肾脏病并发心脏损害的形成,中医药祛浊毒理论干预线粒体自噬治疗慢性肾脏病心脏损害来论述。

《临症指南医案》及《张聿青医案》治泻用药规律探析

对《临证指南医案》和《张聿青医案》两部医学著作中94例治疗泄泻的医案所用药物用药频次、归经、药味和药性进行统计分析,发现叶天士和张聿青两位医家在治疗泄泻时,常用健脾和胃、利水渗湿、疏肝理脾、温补肾阳等治法,所用药物归经多归脾、胃经,药味多偏辛、甘,药性多偏温、热,以求为中医临床治疗泄泻提供更多的方法和思路。

基于玄府理论探讨中医外治银屑病

玄府理论方面的研究近年来渐成热点。玄府最早见于黄帝内经,刘完素从“微观”角度来认识玄府,为玄府理论奠定基础。经后世医家不断研究发展,玄府理论逐步完善为独立的辨证体系,被广泛用来阐释人体生理、病机。本文尝试从“玄府”理论探讨中医外治银屑病,就“玄府理论”指导银屑病外治法的机制加以浅析,冀期对中医外治银屑病提供理论依据及治疗思路。

雷火灸联合中药封包治疗暴聋患者临床研究

目的探讨雷火灸联合中药封包应用于暴聋患者的临床疗效。方法自2021年10月—2022年10月,根据纳入和排除标准选取于该院就诊的突发性耳聋(暴聋)患者,共计120例。严格依据随机数字表法将患者分入4组,各30例。对照组予以常规治疗,雷火灸组在对照组基础上联合雷火灸治疗,中药封包组在对照组基础上给予中药封包治疗,联合组则在对照组基础上予以中药封包+雷火灸治疗,所有患者持续治疗1周后进行疗效评价,治疗前后分别记录症状评分和生活质量评分。结果联合组总有效率显著高于其他各组,差异有统计学意义(P<0.05)。与治疗前比,各组患者治疗后主证、次证、舌象评分均下降;并且联合组患者治疗后主症、次症、舌象评分均低于其他3组,差异有统计学意义(P<0.05)。与治疗前比,各组患者治疗后简明健康测量量表评分上升;并且联合组简明健康测量量表评分高于其他各组,差异有统计学意义(P<0.05)。各组不良反应相比,差异不具有统计学意义(P>0.05)。结论雷火灸联合中药封包治疗可以提高暴聋患者的临床疗效,改善中医证候及睡眠、生活状态,值得大力推广。

中药单体靶向自噬治疗骨关节炎的作用机制

背景:越来越多的研究表明自噬在治疗骨关节炎方面发挥着重要作用,适度的自噬可以维持骨关节软骨细胞的正常生理功能。近年来研究发现,中药单体可以靶向调控自噬治疗骨关节炎,其成分单一、疗效明确、价格低廉、易于获得等特点在治疗骨关节炎方面具有明显的优势。目的:对中药单体在治疗骨关节炎中靶向调节自噬的作用及研究进展进行综述,以期为骨关节炎甚至其他骨代谢疾病的治疗奠定基础。方法:以“Traditional Chinese Medicine,Chinese Herbal Monomer,autophagy,osteoarthritis”“中药,中药单体,自噬,骨关节炎”为关键词在PubMed、Web of Science、中国知网和万方数据库检索2012年1月至2022年10月的相关文献。制定纳入和排除标准,通过阅读文献标题、摘要及全文内容进行筛选,最终纳入63篇相关文献进行综述。结果与结论:①中药单体通过靶向自噬抑制软骨细胞凋亡、保护软骨细胞外基质、降低炎症反应以及拮抗氧化应激损伤四方面来治疗骨关节炎。②不同中药单体可以通过相同的途径调节自噬,相同中药单体又可以通过不同途径影响自噬。多种单体联合使用以及单体中药多靶点、多通路调节自噬的研究还有待探索。③中药单体调控自噬可以为骨关节炎的治疗提供新思路、新策略。中药单体适度调节自噬,保持自噬通量通畅,可能成为治疗骨关节炎的关键。

中医住培基地住培医生纯中医诊疗能力培养的问题与对策

全国268家中医住院医师规范化培训(以下简称“住培”)基地、协同单位和基层培养基地是培养中医临床医生的摇篮。中医住培自开展以来,取得了丰硕的成果,但同时也暴露出一些问题,尤其是中医住培基地在培养住培医生纯中医诊疗能力方面面临一些问题与挑战,需要制定相应的对策予以解决,以全面提升住培医生的纯中医诊疗能力。

咳嗽变异性哮喘的中医药诊治进展

整理近年来中医药诊治咳嗽变异性哮喘(cough variant asthma,CVA)的相关文献发现,中医药治疗CVA疗效确切、副作用小,可有效缓解临床症状,改善患者肺功能。中医药治疗CVA虽然独具优势,但目前仍未形成统一中医病名和辨治体系,各家多依据临床经验施治,以专方治疗多见,使得CVA在中医诊断、证候判定及遣方用药等方面缺乏规范性;其次,有关中医治疗CVA机制的研究较少;目前开展的临床研究多以小样本临床观察为主,缺乏高质量的循证医学证据,而且在适应症、禁忌症及不良反应方面的研究相对欠缺,未来可进一步规范CVA的中医病名、证候标准,拓宽CVA治疗思路。

从“毒”论治慢性肝病思路探讨

中医“毒邪”理论内涵丰富,“毒”与慢性肝病的发生发展密切相关,从“毒”论治是慢性肝病的重要治法。本文通过归纳古今文献阐释“毒”相关理论,结合临床经验认为“毒”在慢性肝病中具有偏性较强、毒性缠绵、复杂多变等致病特点,提出“毒使病生,毒致病进”观点,探讨从“毒”论治慢性肝病的思路,包括溯清毒源、厘清毒性、甄别毒变、扶正祛毒,以期为慢性肝病的临床治疗提供参考。
