Effect of two surgical approaches on the lung function and prognosis of patients with combined esophagogastric cancer

BACKGROUND Adenocarcinoma of the esophagogastric junction has a center of origin within 5 cm of the esophagogastric junction.Surgical resection remains the main treatment.A transthoracic approach is recommended for Siewert I adenocarcinoma of the esophagogastric junction and a transabdominal approach is recommended for Siewert III adenocarcinoma of the esophagogastric junction.However,there is a need to determine the optimal surgical approach for Siewert II adenocarcinoma of the esophagogastric junction to improve lung function and the prognosis of patients.AIM To investigate and compare the surgical effects,postoperative changes in pulmonary function,and prognoses of two approaches to treating combined esophagogastric cancer.METHODS One hundred and thirty-eight patients with combined esophagogastric cancer treated by general and thoracic surgeries in our hospital were selected.They were divided into group A comprising 70 patients(transabdominal approach)and group B comprising 68 patients(transthoracic approach)based on the surgical approach.The indexes related to surgical trauma,number of removed lymph nodes,indexes of lung function before and after surgery,survival rate,and survival duration of the two groups were compared 3 years after surgery.RESULTS The duration of surgery,length of hospital stay,and postoperative drainage duration of the patients in group A were shorter than those of the patients in group B,and the volume of blood loss caused by surgery was lower for group A than for group B(P<0.05).At the one-month postoperative review,the first second,maximum ventilation volume,forceful lung volume,and lung volume values were higher for group A than for group B(P<0.05).Preoperatively,the QLQ-OES18 scale scores of the patients in group A were higher than those in group B on re-evaluation at 3 mo postoperatively(P<0.05).The surgical complication rate of the patients in group A was 10.00%,which was lower than that of patients in group B,which was 23.53%(P<0.05).CONCLUSION Transabdominal and transthoracic surgical approaches are comparable in treating combined esophagogastric cancer;however,the former results in lesser surgical trauma,milder changes in pulmonary function,and fewer complications.

Analysis of the Energy and Environmental Sustainability of a Built Space System: The Case of Patte d’Oie University Campus in Ouagadougou

The aim of this study was to carry out a dynamic simulation of the energy and environmental performance of a built space system, with a view to assessing its energy and environmental class. The use of a simulation and modeling tool, supported by various methodological references, formed the basis of our approach. Adopting a systemic perspective, we described the structural and functional aspects of the systems making up built spaces, as well as the associated energy flows. Our approach was also based on a typology, taking into account typical days, structural and functional configurations at different scales and angles of observation. The analysis tool we developed in Java was applied to the built space system of the Patte d’Oie university campus in Ouagadougou. Annual electricity consumption was measured at 124387.34 kWh, closely aligned with the average annual electricity bill (125224.31 kWh), with a maximum relative deviation of 1%, followed by a carbon emission balance of 58337.66 kg eq CO2 per year. This validation confirmed the effectiveness of our tool. In addition, following the analysis of electricity consumption using our tool, the university campus was classified in energy class B and environmental class C. These results will be based on the emission factors of the energy mix of the West African Economic and Monetary Union (WAEMU) territory, with particular emphasis on Burkina Faso.

Modelling Foot and Mouth Disease in the Context of Active Immigrants

This study employs mathematical modeling to analyze the impact of active immigrants on Foot and Mouth Disease (FMD) transmission dynamics. We calculate the reproduction number (R0) using the next-generation matrix approach. Applying the Routh-Hurwitz Criterion, we establish that the Disease-Free Equilibrium (DFE) point achieves local asymptotic stability when R0 α1 and α2) are closely associated with reduced susceptibility in animal populations, underscoring the link between immigrants and susceptibility. Furthermore, our findings emphasize the interplay of disease introduction with population response and adaptation, particularly involving incoming infectious immigrants. Swift interventions are vital due to the limited potential for disease establishment and rapid susceptibility decline. This study offers crucial insights into the complexities of FMD transmission with active immigrants, informing effective disease management strategies.

Isogeometric nonlocal strain gradient quasi-three-dimensional plate model for thermal postbuckling of porous functionally graded microplates with central cutout with different shapes

This study presents the size-dependent nonlinear thermal postbuckling characteristics of a porous functionally graded material(PFGM) microplate with a central cutout with various shapes using isogeometric numerical technique incorporating nonuniform rational B-splines. To construct the proposed non-classical plate model, the nonlocal strain gradient continuum elasticity is adopted on the basis of a hybrid quasithree-dimensional(3D) plate theory under through-thickness deformation conditions by only four variables. By taking a refined power-law function into account in conjunction with the Touloukian scheme, the temperature-porosity-dependent material properties are extracted. With the aid of the assembled isogeometric-based finite element formulations,nonlocal strain gradient thermal postbuckling curves are acquired for various boundary conditions as well as geometrical and material parameters. It is portrayed that for both size dependency types, by going deeper in the thermal postbuckling domain, gaps among equilibrium curves associated with various small scale parameter values get lower, which indicates that the pronounce of size effects reduces by going deeper in the thermal postbuckling regime. Moreover, we observe that the central cutout effect on the temperature rise associated with the thermal postbuckling behavior in the presence of the effect of strain gradient size and absence of nonlocality is stronger compared with the case including nonlocality in absence of the strain gradient small scale effect.

Razumikhin-Type Theorems on General Decay Stability of Impulsive Stochastic Functional Differential Systems with Markovian Switching

In this paper, the Razumikhin approach is applied to the study of both p-th moment and almost sure stability on a general decay for a class of impulsive stochastic functional differential systems with Markovian switching. Based on the Lyapunov-Razumikhin methods, some sufficient conditions are derived to check the stability of impulsive stochastic functional differential systems with Markovian switching. One numerical example is provided to demonstrate the effectiveness of the results.

A Symbolic-Numerical Approach for theSynthesis of Planar Function Generators

Based on the geometrical relation between the relative pole and the rotationangles of links in the four-bar linkage, a third order algebraic equation withsingle variable for the closed form solution of planar function generators hasbeen derived by using computer symbolic manipulating technique. The newlydeveloped solution method eliminates the requirement of solving system ofnoulinear equations. All solutions of the problem can be obtained directly andeasily without the need of initial values. A numerical example is givendemonstratiap the efficiency and the advantages of this method.

Optimization of the Impeller Geometry for an Automotive Torque Converter Using Response Surface Methodology and Desirability Function

Response surface methodology (RSM) based on desirability function approach (DFA) is applied to obtain an optimal design of the impeller geometry for an automotive torque converter. The relative importance of six design parameters including impeller blade number, blade thickness, bias angle, scroll angle, inlet angle and exit angle is investigated using orthogonal design approach. The impeller inlet angle, exit angle and bias angle are found to exert the greatest influence on the overall performance of a torque converter, with two flow area factors being considered, namely 17% and 20%. Then, RSM together with central composite design (CCD) method is used to in-depth evaluate the interaction effect of the three key parameters on converter performance. The results demonstrate that impeller exit angle has the strongest impact on peak efficiency, with larger angles yielding the most favorable results. The stall torque ratio maximization is attainable with the increase of impeller bias angle and inlet angle together with smaller exit angle. In the end, an optimized design for the impeller geometry is obtained with stall torque ratio and peak efficiency increased by 1.62% and 1.1%, respectively. The new optimization method can be used as a reference for performance enhancement in the design process of impeller geometry for an automotive torque converter.

A Unified Approach to the Thermodynamics and Quantum Scaling Functions of One-Dimensional Strongly Attractive SU(w) Fermi Gases

We present a unified derivation of the pressure equation of states, thermodynamics and scaling functions for the one-dimensional （1D） strongly attractive Fermi gases with SU（w） symmetry. These physical quantities provide a rigorous understanding on a universality class of quantum criticality characterized by the critical exponents z = 2 and correlation length exponent v= 1/2. Such a universality class of quantum criticality can occur when the Fermi sea of one branch of charge bound states starts to fill or becomes gapped at zero temperature. The quantum critical cone can be determined by the double peaks in specific heat, which serve to mark two crossover temperatures fanning out from the critical point. Our method opens to further study on quantum phases and phase transitions in strongly interacting fermions with large SU （ w） and non-SU （ w ） symmetries in one dimension.

Vertical impedance functions of pile groups under low-to-high loading amplitudes:numerical simulations and experimental validation

Piles in a group experience additional displacements in soil due to pile-to-pile interactions apart from those resulting from the external loading.The effect of these interactions determined assuming soil as an elastic and/or viscoelastic material on pile head impedance functions of the pile group is studied by relating the group stiffness to the static stiffness of a single pile.However,the prevailing elastic solutions may misestimate the resulting pile group response due to the lack of consideration for either soil(material)and/or soil-pile interface nonlinearities.It is well established that soil behaves nonlinearly under moderate-to-high loading amplitudes,and besides,the soil-pile interface nonlinearity can exist even at small loading amplitudes.This study addresses the effects of these nonlinearities on the vertical impedance functions of a 3×3-pile group using numerical methods by direct analyses and superposition using pile-to-pile interaction factors.The numerical results are validated using scaled model tests under 1 g conditions.The results highlight the overestimation of pile-to-pile interactions in the pile group when assuming elastic soil conditions.The cases either by direct analyses or superposition approach involving soil and soil-pile interface nonlinearities agree well with the experimental pile group responses under close-to-elastic and nonlinear conditions.

Approach to Riemann Hypothesis by Combined Commensurable Step Function Approximation with Bonnet Method

To the Riemann hypothesis, we investigate first the approximation by step-wise Omega functions Ω(u) with commensurable step lengths u0 concerning their zeros in corresponding Xi functions Ξ(z). They are periodically on the y-axis with period proportional to inverse step length u0. It is found that they possess additional zeros off the imaginary y-axis and additionally on this axis and vanish in the limiting case u0 → 0 in complex infinity. There remain then only the “genuine” zeros for Xi functions to continuous Omega functions which we call “analytic zeros” and which lie on the imaginary axis. After a short repetition of the Second mean-value (or Bonnet) approach to the problem and the derivation of operational identities for Trigonometric functions we give in Section 8 a proof for the position of these genuine “analytic” zeros on the imaginary axis by construction of a contradiction for the case off the imaginary axis. In Section 10, we show by a few examples that monotonically decreasing of the Omega functions is only a sufficient condition for the mentioned property of the positions of zeros on the imaginary axis but not a necessary one.
