输液泵持续胰岛素输注治疗初发糖尿病辅以WinGlucofacts Professional软件疗效观察

目的在WinGlucofacts Professional糖尿病管理软件配合下,比较输液泵持续皮下输注胰岛素(CVⅡ)与胰岛素泵持续皮下输注胰岛素(CSⅡ)及四次皮下胰岛素注射(MSⅡ)治疗初发糖尿病的疗效及安全性。方法选择60例初发2型糖尿病患者,随机分为3... 目的在WinGlucofacts Professional糖尿病管理软件配合下,比较输液泵持续皮下输注胰岛素(CVⅡ)与胰岛素泵持续皮下输注胰岛素(CSⅡ)及四次皮下胰岛素注射(MSⅡ)治疗初发糖尿病的疗效及安全性。方法选择60例初发2型糖尿病患者,随机分为3组,各20例。CVⅡ组患者采用输液泵持续静脉输注超短效胰岛素,餐前追加大剂量治疗;CSⅡ组患者采用胰岛素泵持续皮下输注超短效胰岛素,餐前追加大剂量治疗;MSⅡ组患者采用四次皮下注射胰岛素治疗。应用WinGlucofacts Professional软件管理3组患者的血糖。比较3组患者治疗后的血糖控制情况、胰岛素使用量及安全性。结果3组患者治疗后空腹血糖、餐后2h血糖水平间差异无统计学意义(P>0.05),均得到较好的控制。CVⅡ组、CSⅡ组血糖达标时间(5.5±0.7)d和(6.0±0.8)d、胰岛素总量(0.68±0.16)U.kg-1.d-1和(0.63±0.23)U.kg-1.d-1、低血糖发生频率(0.50±0.46)次/人和(0.55±0.51)次/人及空腹C肽(2.18±0.58)μg/L和(2.23±0.67)μg/L、餐后2hC肽水平(8.02±0.79)μg/L和(7.86±0.68)μg/L与MSⅡ组分别为(10.3±0.4)d、(0.89±0.25)U.kg-1.d-1、(0.85±0.37)次/人、(1.56±0.94)μg/L和(6.01±0.92)μg/L比较,差异均有统计学意义(P<0.05),CVⅡ组血糖达标时间与CSⅡ组比较,差异亦有统计学意义(P<0.05)。结论CVⅡ与CSⅡ一样能模拟胰岛素的生理分泌模式,较MSⅡ能更快、更安全、更有效地控制高血糖,并能使胰岛β细胞功能得到更好的恢复。 展开更多

Intention to Leave and Associated Factors among Health Professionals in Jimma Zone Public Health Centers, Southwest Ethiopia

Background: Inadequate human resources are a major constraint to improving global health. The health sector is characterized by a high turnover coupled with internal as well as external brain drain but there is little information on intention to leave among health professionals in public health centers of Jimma Zone, Oromia Regional State. The aim of this study is to assess intention to leave and associated factors among health professionals in public health centers of Jimma Zone, southwest Ethiopia. Methods: A cross-sectional quantitative and qualitative study was conducted on seven randomly selected woredas (districts) which have 53 public health centers. All health professionals in sampled district public health centers were included (n = 505). Factor analysis was employed for all Likert scale instruments to extract factor(s) representing each of the scales using SPSS version 16.0. The qualitative data was analyzed by thematic analysis methods. Ethical approval was obtained from Jimma University. Results: Four hundred fifty five (90.1%) health professionals participated in the study;out of this, 290 (63.7%) had intention to leave. Among variables, job satisfaction (Beta = -0.298, (95% CI, -0.568 to -0.029), working environment (Beta = -0.612, (95% CI, -0.955 to -0.270), and organizational management (Beta = -0.552, (95% CI, 0.289 to 0.815) had statistically significant association with intention to leave among health professionals in public health centers of Jimma Zone. Conclusions: The overall intention to leave among health professionals was high. Level of job satisfaction, working environment, work pressure, and organizational management had statistically significant association with intention to leave. Thus, responsible bodies should aggressively work on the concerns identified, like, improvements in salary, promotion in terms of training/educational opportunity, improving working environment, and transfer of health professionals, and improvement of the leadership skills of managers. 展开更多

Professional learning communities(PLCs)have seen a rapid spread all around the world over the past 30 years.Since then it has been proved that they can improve teaching quality and thereby the achievement of students,and so PLCs have attracted increasing attention especially from education systems.In China,PLCs have been developed vigorously within schools since the implementation of the‘New Curriculum Reform’in 2000.However,the west has paid scant attention to and were not familiar with them since most research in this field in China was published in Chinese.Furthermore,PLCs are influenced by contextual and cultural factors,and therefore PLCs in Chinese education systems are different from those in Anglo-American settings due to long-standing cultural differences,which require consideration.Therefore,this research will investigate the PLCs in China that may be of interest.Many recent studies have shown that,although PLCs have made significant progress in Chinese urban middle schools through the enthusiastic efforts of educators and scholars,there are still several barriers.The primary purpose of this research is to review the development of PLCs and additionally to analyse the challenges faced by the principals. 展开更多
