Application of anterior decompression and reconstruction using titanium mesh with locking plates in the management of cervical spondylotic myelopathy 被引量:3

Objective:To observe the clinical effect of anterior decompression and reconstruction using titanium mesh with locking plates in the treatment of cervical spondylotic myelopathy. Methods:One hundred and twenty patients with cervical spondylotic myelopathy were treated by anterior decompression and reconstruction using titanium mesh with locking plates. There were 66 men and 54 women ranges in age from 37 to 72 Years(mean age, 58.3 years). The mean Japanese orthopedic surgery association(JOA) scale was 9.6 points before operation. Patients were followed up clinically and radiographically. Results:Having stood surgery well, the operation time ranged between 60-100 min and bleeding during operation ranged between 20-200 ml. There were no case of postoperative infection, recurrent laryngeal nerve palsy, or esophageal or tracheal laceration or rupture. The average follow-up period was 14.3 months(range, 12 to 24 months) in 96 who were followed up. At the last follow-up visit the mean JOA scale had improved to 14.4 points, reflecting an improvement of 4.8 points. The results were considered to be excellent in 87 patients, good in 25, fair in 6, and poor in 2. No hardware-related complications or adjacent segment degenerative changes were encountered during the follow-up periods. Stable bone union was observed in all cases and the average time required for fusion was 5.7 months. Conclusion:Titanium mesh filled with autologous bone graft can avoid the complications associated with harvesting bone from the iliac crest donor site. When combined with cervical anterior locking plate, it can obtain satisfatory clinical results for the treatment of cervical spondylotic myelopathy. 展开更多

The catalytic performance of two oxides coated anodes (OCSs) meshes and one OCA plate was investigated in a zinc electrowinning electrolyte at 38 ℃. Their electrochemical behaviors were compared with that of a conventional Pb-0.7%Ag alloy anode. Electrochemical measurements such as cyclic voltammetric, galvanostatic, potentiodynamic, open-circuit potential (OCP) and in situ electrochemical noise measurements were considered. After 2 h of OCP test, the linear polarization shows that the corrosion current density of the Ti/(IrO2-Ta2O5) mesh electrode is the lowest (3.37μA/cm^2) among the three OCAs and shows excellent performance. Additionally, after 24 h of galvanostatic polarization at 50 mA/cm^2and 38 ℃, the Ti/MnO2mesh anode has the highest potential (1.799 V), followed by the Ti/(IrO2-Ta2O5) plate (1.775 V) and Ti/(IrO2-Ta2O5) mesh (1.705 V) anodes. After 24 h of galvanostatic polarization followed by 16 h of decay, the linear polarization method confirms the sequence obtained after 2 h of OCP test, and the Ti/(IrO2-Ta2O5) mesh attains the lowest corrosion current density. The Ti/(IrO2-Ta2O5) mesh anode also shows better performance after 24 h of galvanostatic polarization with the overpotential lower than that of the conventional Pb-Ag anode by about 245 mV. 展开更多

Guided bone regeneration(GBR)is an effective and simple method for bone augmentation,which is often used to reconstruct the alveolar ridge when the bone defect occurs in the implant area.Titanium mesh has expanded the indications of GBR technology due to its excellent mechanical properties and biocompatibility,so that the GBR technology can be used to repair alveolar ridges with larger bone defects,and can obtain excellent and stable bone augmentation results.Currently,GBR with titanium mesh has various clinical applications,including different clinical procedures.Bone graft materials,titanium mesh covering methods,and titanium mesh fixing methods are also optional.Moreover,the research of GBR with titanium mesh has led to multifarious progresses in digitalization and material modification.This article reviews the properties of titanium mesh and the difference of titanium mesh with other barrier membranes;the current clinical application of titanium mesh in bone augmentation;common complications and management and prevention methods in the application of titanium mesh;and research progress of titanium mesh in digitization and material modification.Hoping to provide a reference for further improvement of titanium mesh in clinical application and related research of titanium mesh. 展开更多

Guided bone regeneration(GBR)uses resorbable and non-resorbable membranes as biological barriers.This study compared the differences in hard tissue stability between GBR using evidence-based digital titanium mesh and resorbable collagen membranes during implant placement.A total of 40 patients(65 implant sites)were enrolled and divided into two groups:resorbable membrane and digital titanium mesh groups.The alveolar bone was analyzed at two-and three-dimensional levels using conebeam computed tomography and by reconstructing and superimposing the hard tissues at four time points:preoperatively,postoperatively,before second-stage surgery,and 1 year after loading.The use of digital titanium mesh showed less alveolar bone resorption in vertical and horizontal directions two-dimensionally before the second-stage surgery and 1 year after loading.Regarding volumetric stability,the percentage of resorption after 6 months of healing with resorbable membrane coverage reached 37.5%.However,it was only 23.4%with titanium mesh.Although postoperative bone volume was greater at all labial sites with resorbable membrane than with digital titanium mesh,after substantial bone resorption within 1 year of loading,the labial bone thickness at the upper part of implants was thinner with resorbable membrane than with digital titanium mesh.Furthermore,digital titanium meshes made according to ideal bone arch contour reduced soft tissue irritation,and the exposure rate was only 10%.Therefore,although both resorbable membrane and digital titanium mesh in GBR were able to successfully reconstruct the bone defect,digital titanium meshes were better at maintaining the hard tissue volume in the osteogenic space. 展开更多

Methods From January 2007 to January 2011, 14 patients with sternal tumors were admitted into Peking Union Medical Hospital.... Objective To evaluate the use of titanium mesh reconstruction after sternal tumor resection. Methods From January 2007 to January 2011, 14 patients with sternal tumors were admitted into Peking Union Medical Hospital. The clinical characteristics, surgical resection, and technique of reconstruction were reviewed. Results Of the 14 patients, 3 had a metastatic sternal tumor, the primary sites of which were as follows: hepatic carcinoma in one case (metastasis 19 years after operation), breast carcinoma in another case (metastasis 5 years after operation), and renal carcinoma in the other case (found simultaneously). Two patients showed local involvement of the sternum: 1 had thymic carcinoma, and the other had myofibrosarcoma. The remaining 9 patients had primary tumors: 4 were osteochondroma, 3 chondrosarcoma, 1 eosinophilic granuloma, 1 non-Hodgekin's lymphoma. En bloc resection of the sternal tumor was performed in all the 14 patients. The defect was repaired with the titanium mesh adjusted to the shape of the defect and fixed with the stainless steel wire. Eleven patients were followed up for a period from 2 months to 4 years, during which no translocation or broken of the titanium mesh was observed. Conclusions Radical en bloc excision remains the treatment of choice for sternal tumors. Sternum defect reconstruction using titanium mesh as a rigid replacement proves appropriate and effective. 展开更多

In this study,the interface characteristics of a direct hot rolling titanium-clad steel plate were analyzed, and the mechanism of interface cracking was explored. The detrimental effect from the formation of TiFe ,TiC,and a Si-enriched layer on the bonding strength was clarified, and an industrial-scaled titanium-clad steel plate with shear strength over 200 MPa was produced with a carefully set schedule accordingly. It was found that hot rolling titanium-clad steel plates had a flat interface without obvious cracks. In the rolling process,both Ti and Fe atoms interdiflhsed,but Fe difthsed much faster than Ti. The Fe-diffused area consisted of three regions. After a high temperature heat treatment, the diffusion depth of Fe and Ti elements increased significantly and evident Si segregation and TiFe layers were identified. Thermal cracking initiated in the Si segregation layer and then propagated along the TiFe layer and Fe-diffused layer on the titanium side. 展开更多

The low cycle fatigue behavior of zirconium−titanium−steel composite plate under symmetrical and asymmetric stress control was studied.The effects of mean stress and stress amplitude on cyclic deformation,ratcheting effect and damage mechanism were discussed in detail.The results show that under symmetric stress control,the forward ratcheting deformation is observed.Under asymmetric stress control,the ratcheting strain increases rapidly with mean stress and stress amplitude increasing.Under high stress amplitude,the influence of mean stress is more significant.In addition,by studying the variation of strain energy density,it is found that the stress amplitude mainly promotes the fatigue damage,while the mean stress leads to the ratcheting damage.In addition,fractographic observation shows that the crack initiates in the brittle metal compound at the interface,and the steel has higher resistance to crack propagation.Finally,the accuracy of life prediction model considering ratcheting effect is discussed in detail,and a high-precision life prediction model directly based on mean stress and stress amplitude is proposed. 展开更多

It can restore normal anat... <strong>Rationale:</strong> The three-dimensional (3D) computer-made titanium mesh is widely used in the skull repair for those patients receiving decompression of the bone flap. It can restore normal anatomy to a greater extent and make a better appearance. <strong>Case Presentation: </strong>We reported two cases of patients admitted to our hospital who have experienced high scalp tension after skull repair. At first, these two patients underwent decompression of the bone flap, and the physical examination results showed a defect of skull. No neurological symptoms and signs were found. The 3D computed tomography (CT) reconstruction of skull was performed, and then the skull repair with 3D titanium mesh was conducted. But because of high scalp tension, they underwent a second operation, during which we re-trimmed and reduce the arc of the titanium mesh. The scalp incision of both patients healed well and no titanium mesh was exposed. Both patients have a good prognosis. <strong>Lessons:</strong> We highlight that the high tension of scalp due to overstretching after 3D titanium mesh repair for skull defect should be paid much attention to. Trimming and reducing the arc of titanium mesh is an effective treatment for this situation. 展开更多

The classification of spatial characteristics and discharge modes of dielectric barrier discharge(DBD)are gaining increasing attention in industrial applications,especially in the field of surface treatment of materials.In this work,gray level histogram(GLH)and Fourier energy spectrum based on the digital image processing tech no logy are applied to investigate the spatial structure and discharge mode of mesh-plate DBD.The coefficient of variation(CV)is calculated to describe the uniformity of the discharge.The results show that the discharge mode of mesh-plate DBD changes from periodic discharge to filamentary discharge when the applied voltage increases from 11-15 kV.Moreover,a more regular spatial structure is obtained under lower applied voltages during the discharge process.It is also found that the apertures of mesh electrodes which are below 1 mm have smaller values of CV compared to plate electrodes,indicating more uniform discharge.Finally,polypropylene is treated by mesh-plate DBD for surface modification.The hydrophilicity is significantly improved as the water contact angle decreased by 64°,and the dyeing depth is also enhanced. 展开更多

Objective To compare the clinical effects of treating cervical prolapse of intervertebral disc by using Solis cage, titanium cage and autogenous iliac crest graft (AICG) combined with titanium plate. Methods 64 cases of cervical prolapse of intervertebral disc were analyzed retrospectively. All the patients were followed up for about one year. Group A is composed of 20 patients (30 intervertebral spaces) treated with microdiscectomy and Solis cage fusion; group B is composed of 21 patients(22 intervertebral spaces) treated with titanium cage and group C is involved of 23 patients(28 intervertebral spaces) which were dealt with AICG combined with titanium plate. The differences in the total X ray exposure time, time working on iliac bone, incidence of complications, fusion rate, incidence of JOA score recovery rate more than 50% and rehabilitation time of the three groups are analyzed. Results All indexes from group A and B were more satisfactory than those from group C (P<0.05). The statistic analysis results of the incidence of complications of cervical part, rehabilitation time in group A are 5.0±1.8, 5.1±1.2; and that is 14.3±2.6, 7.5 ±1.6 and 26.1±6.2, 8.6±2.3 in group B and group C respectively. There is significant difference between group A and group B on the incidence of complications of cervical part and rehabilitation time (P<0.05). Results of the incidence of JOA score recovery rate >50% in group A(95.8±3.2) was more satisfactory than that in group B(93.6±2.9), while there was no statistical difference between group A and group B (P>0.05). Conclusion The Solis cage is better in treating the cervical prolapse of intervertebral disc than other ways. Also, it makes the operation easier, rehabilitation time shorter, and it causes fewer complications. 展开更多

Because they are flexible and pliable, titanium plates are proposed to reconstruct the chest wall, even though such chest wall reconstruction has been performed in only a few cases worldwide. We present a case of a 49-year-old man with a chondrosarcoma arising from the left 1st rib, invading the manubrium, clavicle, 2nd and 3rd ribs, and the anterior segment of the left upper lobe. After wide resection, the chest wall was reconstructed using titanium rib plates and Marlex mesh- the Bovine pericardium sandwich type. The patient tolerated the pain well, and fourteen months after surgery, the chest wall was well preserved function mobility and improved pulmonary function test. 展开更多

Computer-assisted surgery is a promising tool aiming to improve the safety and precision of such surgery. A computer-assisted surgical navigation approach for reconstruction of mandibular defects using a patient-specific titanium mesh tray and particulate cancellous bone and marrow (PCBM) harvested from bilateral anterior ilia is proposed. This case report involves a large multicystic ameloblastoma affecting the right mandible of a 31-year-old male patient. Following detailed clinical examination, radiological interpretation, and histopathological diagnosis, computer-assisted surgical simulation with a virtual 3-dimensional (3-D) model was designed using surgical planning software based on the pre-operative computed tomography data. Long-span segmental resection of the mandible was planned, and the defect was analyzed for reconstruction using a patient-specific reconstruction titanium mesh tray mediated with computer-aided design and manufacturing (CAD/CAM) techniques. During the actual surgery, the ultrasonic bone cutting instrument in the surgeon’s hand was connected to the navigation system to touch an anatomical position on the patient. Therefore, osteotomies were performed finely and smoothly according to the navigation images of the cutting bone line by sequentially moving the instrument. Finally, a CAD/CAM-mediated titanium mesh tray condensed by PCBM was adapted to the remaining mandibular fragments. Six months postoperatively, the patient had a good mandibular configuration and facial contour. Integration of different technologies, such as software planning and 3-D surgical simulation, combined with intraoperative navigation and CAD/CAM techniques, provides safe and precise mandibular reconstruction surgery. 展开更多

In this study,the characteristics of heat transfer on an unsteady magnetohydrodynamic(MHD)Casson nanofluid over an exponentially accelerated vertical porous plate with rotating effects were investigated.The flow was driven by the combined effects of the magnetic field,heat radiation,heat source/sink and chemical reaction.Copper oxide(CuO)and titanium oxide(TiO2)are acknowledged as nanoparticle materials.The nondimensional governing equations were subjected to the Laplace transformation technique to derive closed-form solutions.Graphical representations are provided to analyze how changes in physical parameters,such as the magnetic field,heat radiation,heat source/sink and chemical reaction,affect the velocity,temperature and concentration profiles.The computed values of skin friction,heat and mass transfer rates at the surface were tabulated for various sets of input parameters.It is perceived that there is a drop in temperature due to the rise in the heat source/sink and the Prandtl number.It should be noted that a boost in the thermal radiation parameter prompts an increase in temperature.An increase in the Prandtl number,heat source/sink parameter,time and a decrease in the thermal radiation parameter result in an increase in theNusselt number.The computed values of the skin friction,heat andmass transfer rates at the surface were tabulated for various values of the flow parameters.The present results were compared with those of previously published studies andwere found to be in excellent agreement.This research has practical applications in areas such as drug delivery,thermal medicine and cancer treatment. 展开更多

背景:下颌骨骨折在坚强内固定后的愈合受多种因素影响,包括接骨板的材料、骨折部位以及患者骨密度等,然而,目前对于不同骨质下下颌骨骨折固定稳定性关系的研究相对较少,并且缺乏科学的依据。目的:利用有限元法分析生物可吸收板和微型钛... 背景:下颌骨骨折在坚强内固定后的愈合受多种因素影响,包括接骨板的材料、骨折部位以及患者骨密度等,然而,目前对于不同骨质下下颌骨骨折固定稳定性关系的研究相对较少,并且缺乏科学的依据。目的:利用有限元法分析生物可吸收板和微型钛板固定不同骨质条件下颌骨骨折的稳定性。方法:根据ZARB和LEKHOLM提出的骨质分类方法,分别建立Ⅰ-Ⅳ类下颌骨骨折三维有限元模型,每类骨质条件下分别模拟下颌骨正中、体部和下颌角3个部位的骨折模型,采用生物可吸收板(或微型钛板)对上述骨折进行内固定,模拟健侧咬合状态,利用有限元分析骨折段的相对位移与内固定物应力分布情况。结果与结论:①随着骨质等级的增加,内固定物的最大应力值基本呈逐渐增加趋势,微型钛板组、生物可吸收板组下颌体部Ⅳ类骨质下的内固定物最大应力值最高,分别为382.74,96.11 MPa;在相同骨质条件下,钛板组各部位骨折模型的内固定物最大应力值均高于生物可吸收板组。②对于Ⅲ和Ⅳ类下颌骨正中部骨折,微型钛板组、可吸收板固定组骨折断端位移较大,超过了骨愈合极限值(大于150μm);对于Ⅳ类骨质下颌骨体部骨折,生物可吸收板组骨折断端位移超过了愈合极限值,微型钛板组骨折断端位移接近愈合极限值;在相同骨质条件与骨折部位下,微型钛板组骨折断端位移要小于生物可吸收板组。③结果显示,两种内固定物的强度和刚度均足以支持Ⅰ-Ⅳ类骨质下颌骨3种部位骨折的骨愈合,并且生物可吸收板的固定稳定性与微型钛板几乎相同,可以提供骨折早期的愈合条件。在治疗下颌骨骨折时应将下颌骨骨质类型纳入考虑因素,下颌骨骨质等级越高,骨折固定的稳定性越差,术后更易发生骨愈合不良等并发症。 展开更多
