Integration of traditional,complementary,and alternative medicine with modern biomedicine:the scientization,evidence,and challenges for integration of traditional Chinese medicine

Traditional medicine has garnered significant global recognition,with an estimated 80%of the global population using it.Therefore,it is essential to fully understand the integration of traditional medicines into current healthcare systems.This review aims to provide a comprehensive overview of the standard process to modernize traditional medicine scientifically in the context of modern biomedicine,further termed here as“scientization”.Specifically,we aim to summarize the advancements made in understanding the efficacy,effectiveness,and underlying mechanisms of herbal medicine.We also examined the transition from experience-to evidence-based medicine during acupuncture.Furthermore,we explore the development of universal safety and quality control standards.Finally,we discuss international trade and export markets for Chinese medical products.The development and integration of traditional medicine have allowed it to further improve human health,resulting in a more comprehensive health solution for the global population.

Comprehensive Rehabilitation Therapy of Traditional Chinese Medicine Combined with Modern Rehabilitation Training Improves the Spasticity and Motor Function of Hemiplegia after Stroke

Objective:To analyze the impact of comprehensive rehabilitation therapy of traditional Chinese medicine(TCM)(based on modern rehabilitation training)on the spasticity and motor function in stroke patients with hemiplegia.Methods:Seventy-nine stroke and hemiplegia patients admitted to the hospital from June 2021 to June 2023 were selected and randomly divided into a control group(39 cases)using modern rehabilitation training,and an observation group combined with comprehensive TCM rehabilitation therapy(40 cases),over 1 month.The clinical index data of the two groups were compared.Results:There were differences in the clinical index data between the two groups.The total effective rate after 2 treatment in the observation group(92.50%)was higher than that of the control group(74.36%)(χ^(2)=4.727,P<0.05).All central sensitization inventory(CSI)and stroke quality of life(PRO)scores in both groups were lower after treatment,with the observation group having lower scores as compared to the control group(P<0.05).The scores of FMA(upper limbs,lower limbs),Barthel index scores,and Functional Ambulation Categories(FAC)scores of both groups increased after treatment,with the observation group having higher scores as compared to the control group(P<0.05).Conclusion:Comprehensive TCM rehabilitation therapy had a significant therapeutic effect on patients with hemiplegia after stroke.It improved the patient’s spasticity,limb movement,and walking function.Their daily living abilities and quality of life were also enhanced.

Our Insights into Modern Traditional Chinese Medicine

This article will present the result of 35 years of studies andresearch initiated by two teachers:Professor Leung KokYuen(China)and Professor Truong Thin(Vietnam).This article will briefly introduce the systems of traditional Chinese medicine,such as Jing(精),Xue(血),Qi(气),Shen(神)and Jin Ye(津液)compared with various systems in modern medicine.

Adolescence dysfunctional uterine bleeding:perspectives from the combination of traditional Chinese medicine and modern medicine

Because their endocrine system is not yet developmentally mature,adolescent girls are prone to dysfunctional uterine bleeding caused by insufficiency of the hypothalamic-pituitary-gonadal axis.The view of modern medicine is mainly aimed at the secretory function of sex hormones,such as estrogen and progesterone.However,it causes distress to patients because of the susceptibility of their treatment to endocrine system disorders.Traditional Chinese medicine considers the damage to the function of the Conception Vessel and Thoroughfare Vessel as the key to the disease.In this way,we guide the use of traditional Chinese medicine and acupuncture and achieve good results.The pathogenesis and management of functional uterine bleeding in adolescence are discussed from two medical perspectives in this article.

East to west:research progress in traditional Chinese medicine for antiaging strategies

Delaying the onset of aging is considered an important tool for dealing with the global crisis of the aging population.Accordingly,several antiaging studies have been conducted using strategies of modern medicine and traditional Chinese medicine(TCM).In this review,we summarize the current state of antiaging research related to both modern medicine and TCM and provide suggestions for further research.Various theories related to the aging process have been proposed,including the free radical theory,mitochondrial DNA damage theory,telomere theory,cross-linking theory,and intestinal flora theory,and a number of antiaging chemical treatments have been developed using modern medicine.These theories and achievements have resulted in major advancements in antiaging research.TCM is based on practical experience and has a long history with unique advantages in antiaging research.Some antiaging practices such as moderate diet consumption,acupuncture,Tai Chi(a traditional Chinese martial art that can relax the nerves to slow down the aging process),and Wuqin Xi(a traditional Chinese exercise to strengthen the body)have been adopted to maintain health.Several TCMs and prescriptions,particularly those related to medicine food homology,have been used in antiaging treatment,and their antiaging properties have been demonstrated to be effective based on pharmacological experiments and clinical applications.

Development status and trend of traditional Chinese medicine database

Following the trend of information technology,the development of traditional Chinese medicine(TCM)databases has led to great changes in terms of data.For example,the storage and dissemination medium of data has achieved a shift from paper books to the internet,and the content has expanded from basic information to comprehensive information such as targets and molecular structures of modern medicine.In recent years,the amount of information contained in the TCM databases has grown at an unparalleled rate.However,there are still challenges correlated with the database construction,including insufficient data volume,inconsistent construction standards,and a low level of platformization.Additionally,the prevalence of applications in the field of TCM like network pharmacology,bioinformatics,and artificial intelligence requires a large volume of high-quality data.Generally speaking,the advancements in life science and artificial intelligence technology have outpaced the development of TCM databases.Therefore,this paper compiled the current status of TCM databases,discussed the benefits and drawbacks of various databases,and concluded that the development trend should be comprehensive,platform-based,and tightly integrated with modern life science technology and artificial intelligence technology,so as to provide assistance to the modernization research of TCM.

Thoughts on the treatment of cancer pain under the guidance of traditional Chinese medicine

中医诊治肿瘤是当今中医防治重大疾病的一个特色。癌性疼痛是癌症患者尤其是中晚期癌症患者的主要临床症状之一,严重影响患者生存质量。目前对于癌痛的药物治疗主要采用三阶梯止痛原则,但长期使用止痛药物容易引起的便秘、恶心呕吐等副作用,故受患者耐受性的限制,而使疼痛没有得到有效的控制。癌痛发生的机制极其复杂,全面了解癌痛中医的发病和证型,是进行有效缓解癌痛症状的重要基础。中药治疗癌痛具有不成瘾性、耐药性及毒副作用小等独特优势。文章从癌痛病因病机特点出发,探讨不同分类中药治疗使用规律,对恶性肿瘤引起癌痛的基本病机的再认识和治疗具有重要指导作用。

A Review of Study of Yellow Water Disease in Tibetan Medicine Theory

Based on the Tibetan medical theory,the relevant information and diagnosis and treatment ideas of yellow water disease are discussed,and Mongolian medicine also takes its own basic medical theory as the starting point to discuss and explain,while traditional Chinese medicine has made less theoretical description of this disease,but there are also some understandings and treatment guidelines.This paper mainly discusses the cognitive aspects of this disease,starting from the essence,analyzes the relationship between this disease and traditional Chinese medicine diseases as well as modern medicine,and makes a theoretical description for a better understanding of the yellow water disease.

Tacit Knowledge Mining:The Key Traditional Chinese Medical Inheritance

Traditional Chinese medicine(TCM)is a treasure of traditional Chinese culture and a gift to the world.TCM tacit knowledge refers to the knowledge and experiences formed in the process of learning and practice of TCM.The objective of this study is to discuss the importance of TCM tacit knowledge in the inheritance and education of TCM.As the essence of the TCM,TCM tacit knowledge has the characteristics of massive,complicated,relativistic,highly individualized,constantly innovative,the dependence of cultural background and the regional environment,as well as difficult to explicate.It exists in every aspect of the TCM theory and the process of dialectical treatment.Besides the traditional master‑apprentice,family‑based,school‑based,and inheritance and education methods,together with the inheritance based on the books,images,and network platforms,in the process of TCM modernization,a variety of modern theoretical models and computing techniques have also been used in the mining of the TCM tacit knowledge.In this study,we introduced the usage of SECI model,complexity adaptive system,latent variable model,and some of the data mining technologies in the TCM tacit knowledge mining.An accurate and efficient inheritance of TCM tacit knowledge is the key to maintain the vitality and innovative development of TCM.Under the reasonable application and combination of the traditional education methods,modern mining methods,and further the artificial intelligence,the explicit and inheritance of TCM tacit knowledge will get tremendous development,and it could extremely improve the efficiency and accuracy of the TCM inheritance and the TCM modernization.

CLINICAL OBSERVATION ON TREATMENT OF 80 CASES OF PROLAPSE OF LUMBAR INTERVERTEBRAL DISC BY COMBINATION OF ACUPUNCTURE AND MEDICINE

cases of prolapse of lumbar intervetebral disc were randomly divided into treatment and control groups. 80 cases in treatment group were treated with combination of acupuncture, oral administration of Chinese medicinal herbs, traction, point injection and intravenous drip of energy dehydration mixture; 70 cases in control group were managed by combination of acupuncture, oral administration of herbal medicines, traction, acupoint injection (without energy dehydration mixture). The results showed that the curative rate and the total effective rate of the treatment group were 91.25% and 98.75% separately, while those of the control group 62.86% and 92.86% respectively. Comparison of the curative rate between the 2 groups showed a remarkable significance (P<0.01). The observation demonstrated that combined treatment of prolapse of lumbar intervertebral disc with traditional Chinese and Western medicine is an effective approach.

国家重点研发计划中医药现代化研究文献计量分析

目的:分析国家重点研发计划中医药现代化研究重点专项(以下简称专项)的研究进展。方法:基于中国知网(CNKI)、中国学术期刊数据库(万方数据)、中文科技期刊数据库(VIP)、中国生物医学文献数据库(CBM)和Web of Science核心合集数据库(WoSCC),检索2017—2023年专项资助发表的论文,利用文献计量学和CiteSpace软件对论文作者、机构、研究热点等进行分析。结果:共检索到中文文献2701篇,涉及核心作者108位、形成2个较大的研究团队,中国中医科学院发文量最多,研究热点包括用药规律、中医证候等中医药临床评价和机制研究,主要研究疾病包括心血管病、恶性肿瘤等。英文文献732篇,其中中国科学院发文量最多,药物效用机制、药物代谢动力学等药理学研究是研究热点。结论:在专项支持下,中医药基础研究和基于循证医学评价的临床研究不断深入,在中医药防治重大疾病研究方面取得了重要进展。未来应加强科技成果转化,深化人工智能技术在中医药现代化研究中的应用。

中医正骨学古今教学特点刍议

中医正骨学是一门技术性学科,在古今都占有着重要的医疗地位。文章针对中医正骨学的古代和现代教学特点进行讨论,并根据发展的实际情况分析教学模式的变化特点,以便更好地保持我国中医药治疗的特色和优势,使中医正骨学保持学科特色、发挥学科优势,充分利用现代科学技术,走在科技的前线,与现代高新技术相结合,促进其教学发展。

Chemometrics and modernization of traditional Chinese medicine

Development of chromatographic fingerprinting and its related chemometric methods in the research of quality control of traditional Chinese medicines(TCMs) are discussed. The quality control methods for guarantying the authentication and stability of products and semi-products of TCMs are firstly assessed. The technique based on chromatographic fingerprinting is essentially a kind of high-through put and integral tools to explore the complexity of herbal medicines. In order to further control the comprehensive quality of TCMs,confirmation and identification of their important chemical components are necessary. Some new strategies are proposed to trace the chemical changes of chromatographic fingerprints both in product processing and/or after their administration by modern chromatographic techniques and chemometrics. Combined with systems biology and bioinformatics,it seems possible for one to reveal the working mechanism of TCMs and to further control their intrinsic quality comprehensively.

中医史学研究的审视思考

中医医学史研究的历史理论方面评价,代表成果较为不足。从医学史学理论研究问题导向视角下,史学史的建构先行对于中医学科发展规律揭示与从史学维度认知完善有现实意义,开展具有中华文化特色底蕴的史学研究与贯彻中国文化精神个性的史学史研究评价,有利于对医史研究范式、医学史研究定位等方面探索较为成熟的路径。

Efficacy of the combination of modern medicine and traditional Chinese medicine in pulmonary fibrosis arising as a sequelae in convalescent COVID-19 patients: a randomized multicenter trial

Background:The coronavirus disease 2019(COVID-19)caused by severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)has led to a significant number of mortalities worldwide.COVID-19 poses a serious threat to human life.The clinical manifestations of COVID-19 are diverse and severe and 20%of infected patients are reported to be in a critical condition.A loss in lung function and pulmonary fibrosis are the main manifestations of patients with the severe form of the disease.The lung function is affected,even after recovery,thereby greatly affecting the psychology and well-being of patients,and significantly reducing their quality of life.

近代中医药期刊视角下的带下病研究——以《杏林医学月报》(1929—1937)为例

以近代中医药期刊《杏林医学月报》为研究主体,旁参《苏州国医杂志》《针灸杂志》《医界春秋》等,通过比较分析,探讨新文化运动时代背景下,中医学界内部受西学东渐思想影响后,带下病的病因病机、治法方药及预防方法等变化,勾勒、还原其历史样貌,概括、分析其特征和意义,客观分析时代背景对医学理论的具体影响,呈现近代医学知识、医学观念及医疗技术的嬗变,以期对中医近代疾病史的研究、医案和临证经验的整理研究有所裨益。

基于古今医案云平台探讨视神经炎的中医证型及用药规律

目的 基于古今医案云平台研究视神经炎(ON)的中医证型及用药规律。方法纳入自建库以来至2022年12月31日收录于古今医案云平台、中国知网、万方医学网、维普网及中国生物医学文献服务系统中使用中医药治疗ON的医案及文献。将中医证型、处方组成及治法录入Excel软件并建立数据库,对中药名称规范化处理后,运用古今医案云平台(V2.3.5)的数据挖掘模块对中医证型、中药四气、五味、高频中药使用频次、关联规则等进行分析。结果 (1)一般情况:共纳入医案15个和文献110篇,包含68种证型,“肝郁气滞证”出现频次最高(28次,12.02%);233个中药复方,涉及中药211味,累计频次为2,579次,四气以寒性最多(49味,23.22%);五味以甘味药最多(76味,36.02%);归经以肝经药物最多(73味,34.60%)。(2)高频中药使用频次:用药频次≥30次的中药共21味,排在前5位的分别是当归(165次,70.82%)、茯苓(143次,61.37%)、柴胡(127次,54.51%)、白芍(118次,50.64%)、白术(110次,47.21%)。(3)高频中药关联规则:共得到8类核心药对,排在前5位的分别是“白芍-当归”“柴胡-当归”“白术-当归”“白术-茯苓”“白芍-茯苓”。(4)高频中药聚类分析:聚类分析得到4个药物聚类,功效分别为清热凉血活血、疏肝健脾益气、养阴清心凉血、补肾益气利水。(5)复杂网络分析:提取出的核心处方为丹栀逍遥散加减,涵盖10味中药。结论 ON的中医辨证与“肝”密切相关,中药治疗ON以调肝为主,分别采用疏肝、清肝、养肝之法;且兼顾心脾,补益气血;同时辅以子类药,具有脏窍同治的特点。

从中西医不同视角探讨情志与肝脏的相关性

近年来,随着生物-心理-社会医学模式的发展,精神情志因素与肝脏的关系越来越被重视。中医学传统理论认为,肝主疏泄,主调畅情志,与情志关系密切。流行病学研究表明,各类肝脏疾病均伴有不同程度的精神障碍,同时精神情志异常可能促进肝病的发生,影响其预后。肝脏与情志在病理上存在共同的发病机制,涉及神经、内分泌、免疫系统功能失调。本综述基于中医基础理论并结合现代医学研究,分析了情志与肝之间的相关性,同时从神经递质、炎症因子、脑源性神经营养因子(Brain derived neurotrophic factor,BDNF)、可溶性环氧化物水解酶(Soluble epoxide hydrolase,sEH)、肠道微生态、下丘脑-垂体-肾上腺(HPA)轴、下丘脑-垂体-甲状腺(HPT)轴7个方面总结了肝脏疾病并发情志疾病的潜在机制,为今后的理论研究与临床治疗提供一定的参考价值。
