Patient-reported Outcomes (PROs): An Approach to Evaluate Treatment Efficacy of Chinese Medicine or Integrative Medicine

Chinese medicine (CM) has been an experience based practice over the past three thousand years. The safety and efficacy of CM has been practised through experience, rather than nowadays’ modern scientific measurements. In this way, the development of CM has not been keeping pace with the advance in science and technology and the progress in orthodox medicine. In order to meet the requirements of modern science and technology, an evidence based approach must be adopted. Reassurance is needed to convince people for safety and efficacy of CM and give confidence to the evidence based demands ——“Tradition is confirmed by Modern Science” (1) .

Effects of surface finish and treatment on the fatigue behaviour of vibrating cylinder block using frequency response approach

This paper presents the effects of surface finish and treatment on the high cycle fatigue behaviour of vibrating cylinder block of a new two-stroke free piston engine at complex variable amplitude loading conditions using frequency response approach. Finite element modelling and frequency response analysis was conducted using finite element analysis software Package MSC.PATRAN/MSC.NASTRAN and fatigue life prediction was carried out using MSC.FATIGUE software. Based on the finite element results, different frequency response approach was applied to predict the cylinder block fatigue life. Results for different load histories and material combinations are also discussed. Results indicated great effects for all surface finish and treatment. It is concluded that polished and cast surface finish conditions give the highest and lowest cylinder block lives, respectively; and that Nitrided treatment leads to longest cylinder block life. The results were used to draw contour plots of fatigue life and damage in the worst or most damaging case.

In this thorough article, the authors meticulously underlinethe advantages of a modified j-shaped incision for the simultaneoustreatment of difficult liver colorectal metastases （CRM） and right-lung CRM, reporting their experience in a cohort of 11 patients.

Isolated dorsal approach for the treatment of neglected volar metacarpophalangeal joint dislocations

Here, we present the clinical and radiological results of three neglected volar metacarpophalangeal dislocations in 2 patients, which were treated with open reductions 10 and 24 mo after the dislocations. There was a mean of a 20°(range 10°-30°) limitation of extension and a 53.3°(range 30°-70°) limitation of flexion preoperatively. Postoperatively, there was no limitation of extension(at 8 and 12 mo) in any of the fingers. In terms of flexion, one finger had full function, one had a 10° and the last one had a 30° limitation of flexion. Two of the fingers presented anesthesia preoperatively, which improved to hypesthesia postoperatively. One finger had hypesthesia, which improved postoperatively. During surgery, a ruptured dorsal capsule was found to have interposed into the joint, making closed reduction impossible. Our experience with these two patients demonstrated that, even in neglected cases, open reduction using an isolated dorsal approach may result in satisfactory clinical andradiological outcomes.

Transileocolic endovascular treatment by a hybrid approach for severe acute portal vein thrombosis with bowel necrosis:Two case reports

BACKGROUND Acute portal vein thrombosis(PVT)with bowel necrosis is a fatal condition with a 50%-75%mortality rate.This report describes the successful endovascular treatment(EVT)of two patients with severe PVT.CASE SUMMARY The first patient was a 22-year-old man who presented with abdominal pain lasting 3 d.The second patient was a 48-year-old man who presented with acute abdominal pain.Following contrast-enhanced computed tomography,both patients were diagnosed with massive PVT extending to the splenic and superior mesenteric veins.Hybrid treatment(simultaneous necrotic bowel resection and EVT)was performed in a hybrid operating room(OR).EVTs,including aspiration thrombectomy,catheter-directed thrombolysis(CDT),and continuous CDT,were performed via the ileocolic vein under laparotomy.The portal veins were patent 4 and 6 mo posttreatment in the 22-year-old and 48-year-old patients,respectively.CONCLUSION Hybrid necrotic bowel resection and transileocolic EVT performed in a hybrid OR is effective and safe.

Are Pharmaceutical Treatments or Psychological Approaches a More Effective Way of Treating Depression?

Objective:To review the related researches on the treatment of depression around the world and to summarize the advantages and disadvantages of antidepressants and psychotherapy,so that to find a more effective treatment plan for depression.Methods:The principles of several common pharmaceutical treatment combined with psychological approach were analyzed,cognitive behavioral therapy,interpersonal psychotherapy and socia1 support therapy included.Results:Some experimental results were illustrated and the advantages of Pharmaceutical treatment combined with psychological approach was revealed.Conclusion:Research indicates when pharmaceutical treatment is available,the combination of drugs and psychotherapy is more effective than sole pharmaceutical treatment.

The Clinical Effect Two Approaches of Microsurgery the Treatment for Pituitary Tumor

Objective:To analyze the effect of two approaches of microsurgery in the treatment of pituitary tumor.Methods:The main body of this study was 69 patients with pituitary tumor who came to the hospital between December 2016 and December 2019.Taking the coin method as the standard,group A underwent nasal-sphenoid sinus approach with 36 cases;group B underwent transcranial approach with 33 cases.The treatment effects were compared.Results:The total effective rate of group A was 94.44%,and that of group B was 72.73%;the therapeutic index of group A was better than that of group B;the complication rate of group A was 8.33%,and that of group B was 30.30%(P<0.05).After treatment,the tumor volume of both groups decreased,and group A was smaller than group B(P<0.05).Conclusion:Nasal-sphenoid sinus approach for patients with pituitary tumors can improve the treatment index,enhance the curative effect,reduce the size of the tumor,and have better safety.

This review mainly compares the major treatment approaches to child phonological disorder,namely,Core Vocabulary Approach for the treatment of inconsistent phonological disorder,Distinctive Features Approach and ABAB-Withdrawal Approach for the treatment of consistent phonological disorder,and Eclectic Approach for real clinical treatment.Different approaches have different efficacy and the application of a certain treatment approach depends on the various conditions of the subjects.

The Author Reply：We thank Dr. Sucameli et al. for the interest toward our article[1] and for the opportunity for further discussing on this issue,providing a case report of single metastastic fore sites in the liverand lung both treated in a minimal access fashion. However, giventhe interest of the authors insight, it appears misleading when related to that discussed in our report which was clearly referred toother patients＇ profiles. Indeed, they described a case with a singleperipheral nodule in the right lung and a single liver metastasesin segment 5. This uncommon situation （less than 2% of patientsaccording to the LiverMetSurvey registry [2]）, is obviouslya more than reasonable indication for a mini-invasive approach.However, our patients received surgery for complex oncological involvementof the liver： as described, this means large and/or multiplelesions, in contact or invading the hepatic veins at caval confluence.For such conditions we have introduced original surgicalapproaches [3,4]： in such conditions we would select a J-shapedthoracophrenolaparotomy for the liver per se [5]. This incision forsuch complex conditions other than allowing the liver clearance in a single operation rather than in staged approach [6], allows justin case the removal of lung nodules. Therefore, we thank again theauthors for their interest to our report, and furthermore we congratulatefor the original management of the shared clinical case.However, the condition recalled by the authors is related to a scenariooncologically and surgically at the opposite side of that discussedin our paper and for that somehow suggesting a comparisonis misleading.

Application of Solution-Focused Approach in Nursing Care of Advanced Schistosomiasis

Objective:To analyze the effect of solution-focused approach on advanced schistosomiasis(AS).Methods:10 cases of patients with AS that were treated with a solution-focused approach were included in our study.The indicators before and after the intervention were measured(knowledge mastery,prevention and treatment compliance,personal behavior,self-care ability,management satisfaction,complication rate).Results:After analyzing pre-and post-intervention indicators,statistical significance was found(P<0.05).The intervention received a satisfaction rate of 80.00%,with a complication rate of 10.00%.Conclusion:Using a solution-focused approach for ankylosing spondylitis(AS)patients can enhance their understanding and attitude toward disease prevention and treatment,improve their behaviors and self-care ability,resulting in high satisfaction and reduced complications.

经口腔前庭入路与经腋窝入路腔镜手术治疗甲状腺癌的效果对比

目的:分析比较经口腔前庭入路与经腋窝入路腔镜手术治疗甲状腺癌的临床效果。方法:回顾性分析2020年6月-2022年12月在笔者医院接受手术治疗的144例甲状腺癌患者临床资料,依据手术方式不同分为A组(n=45,传统开放手术)、B组(n=50,经口腔前庭入路腔镜手术)及C组(n=49,经腋窝入路腔镜手术),比较三组手术相关指标、术后不同时间点疼痛评分、美学效果[温哥华瘢痕量表(Vancouver scar scale,VSS)]及术后并发症发生率。结果:B组手术时间长于A、C两组(P<0.05);B、C两组术中出血量、引流量均少于A组,术中淋巴结清扫数目均多于A组,住院时间短于A组,差异均有统计学意义(P<0.05);B、C两组术后1 d、术后3 d、术后1个月、术后3个月疼痛VAS评分均低于A组,差异有统计学意义(P<0.05);术后3个月B、C两组VSS评分均低于A组,且B组VSS评分较C组更低,差异有统计学意义(P<0.05);三组间术后并发症比较差异无统计学意义(P>0.05)。结论:与传统开放术比较,经口腔前庭入路与经腋窝入路腔镜手术治疗效果均更好,均可有效减轻术后疼痛,同时经口腔前庭入路具有术中淋巴结清扫数目多,美学效果好等优势,经腋窝入路具有手术时间短的优点,临床可依据不同情况选择合适的手术方案。
