Various surgical proc...Chronic Subdural Hematoma (CSDH) is one of the most common types of intracranial hemorrhage, and the prognosis is good if treated properly. The standard treatment for CSDH is surgical evacuation. Various surgical procedures are used such as burr holes evacuation, twist-drill craniostomy, and craniotomy. Our aim is to evaluate the feasibility and safety of twist dill as a first-choice treatment in evacuation of CSDH in comorbid patients. Methods: This study is a prospective analysis of CSDH evacuation using two twist drill craniostomy holes and irrigation conducted on 21 patients with different comorbidities in Assuit University Hospital between May 2017 and May 2018. Results: The overall postoperative clinical improvement was 95.2%. The operative time was less than 10 minutes in 71.4% of the patients. 4 patients showed residual collection in the post-operative CT (19%). Pneumocephaly was detected in 2 patients (9.5%). Postoperative seizures occurred in 2 patients (9.5%), and subarachnoid hemorrhage was detected in one patient (4.7%). Conclusion: This approach is a new modification of twist drill craniostomy which can decrease the postoperative residual and recurrence rates and also still a minimally invasive and lifesaving technique in patients with different comorbidities.展开更多

The manufacturing process of the drill is introduced, and drilling experiments are carried out by using of the drill developed. As a result, not only it can drill holes with a high efficiency, but also a good quality of hole inlet and outlet can be obtained for such materials as glasses, marble, granite, Al 2O 3, etc. The prospect in practical application of the technique developed is also discussed.展开更多

However, there has not been a systema...Many researches on drilling force and temperature have been done with the aim to reduce the labour intensiveness of surgery, avoid unnecessary damage and improve drilling quality. However, there has not been a systematic study of mid- and high-speed drilling under dry and physiological conditions(injection of saline). Furthermore, there is no consensus on optimal drilling parameters. To study these parameters under dry and physiological drilling conditions, pig humerus bones are drilled with medical twist drills operated using a wide range of drilling speeds and feed rates. Drilling force and temperature are measured using a YDZ-II01W dynamometer and a NEC TVS-500EX thermal infrared imager, respectively, to evaluate internal bone damage. To evaluate drilling quality, bone debris and hole morphology are observed by SEM(scanning electron microscopy). Changes in drilling force and temperature give similar results during drilling such that the value of each parameter peaks just before the drill penetrates through the osteon of the compact bone into the trabeculae of the spongy bone. Drilling temperatures under physiological conditions are much lower than those observed under dry conditions, while a larger drilling force occurs under physiological conditions than dry conditions. Drilling speed and feed rate have a significant influence on drilling force, temperature, bone debris and hole morphology. The investigation of the effect of drilling force and temperature on internal bone damage reveals that a drilling speed of 4500 r/min and a feed rate of 50 mm/min are recommended for bone drilling under physiological conditions. Drilling quality peaks under these optimal parameter conditions. This paper proposes the optimal drilling parameters under mid- and high-speed surgical drilling, considering internal bone damage and drilling quality, which can be looked as a reference for surgeons performing orthopedic operations.展开更多

An accurate drill flute profile design is generated for two-flute conical twist drills using analytical equations to generate a drill flute profile design needed for the production of twist drills with straight lips. The required grinding wheel profile for a flute production was expressed in digitized form as well as in terms of two curve-fitted circular arcs. The drill flute profile geometry can never be precisely generated when required grinding wheel profile is represented by two circular arcs and the generated flute profile is just a very good approximation of the design flute profile. A CAD (computer aided design) software has been developed using MATLAB to determine the required grinding wheel profile for generating a given drill flute profile design.展开更多
