BACKGROUND Gastrointestinal stromal tumors(GISTs)are rare tumors of the gastrointestinal tract accounting for less than 1%of all gut tumors.GISTs occurring in the rectum are extremely rare,and these usually present at an advanced stage compared with other sites.CASE SUMMARY A 60-year-old male who presented with features of sensations of rectal tenesmus was referred to our department with a mass in the lower rectum that was detected during a routine checkup.Colonoscopy,transrectal ultrasound,perianal magnetic resonance imaging and ultrasonic contrast were used to diagnose the rectum GIST,and then the patient underwent complete transanal resection using the ultrasonic scalpel.The patient was discharged ten days after the operation and was defined as low risk.Therefore,he had no need to receive subsequent adjuvant therapies,and he had not suffered any anal dysfunction or had any evidence of recurrence at follow up.CONCLUSION Surgical resection with histologically negative margins is the standard curative treatment for rectal GISTs.Appropriate surgical techniques based on the location,size,and resectability of the tumor should attract great attention from clinicians. 展开更多

Design and Performance Study of an Ultrasonic Bone Scalpel with Composite Horn Structure

The ultrasonic bone scalpel was often used in all kinds of clinical orthopedic surgery as a new type of medical instrument.Based on the working theory of the ultrasonic bone scalpel and combined with the design method of a quarter wavelength transducer and horn,this study proposed an optimization design method for the ultrasonic bone scalpel with a composite horn structure.The structure of the ultrasonic bone scalpel was simplified,and its length was shortened.Finite element analysis,modal analysis,and harmonic response analysis were used to optimize the design of the ultrasonic bone scalpel.The performance of the ultrasonic bone scalpel was explored through impedance analysis,amplitude measurement,temperature measurement,and cutting tests,and the feasibility of the design was verified through torque analysis.The results showed that when the torque force was 4.0 N,the resonance frequency of the ultrasonic bone scalpel was 30,540 Hz,and the maximum amplitude of the ultrasonic bone scalpel could reach 62μm.After spray cooling,the maximum surface temperature of the rear end cover was reduced from 51.8 to 36.3℃,the maximum surface temperature of the composite horn was reduced from 71 to 45.3℃,and the maximum surface temperature of the cutter head was reduced from 101 to 46.1℃,demonstrating a good cooling effect.Under 89 Voltage,the optimal cutting force for cutting chicken leg bones was 0.1 N,the optimal cutting force for cutting pork ribs was 0.3 N,and the cutting process showed tissue selectivity. 展开更多

Objective The aim of this study was to investigate the potential adverse effects of electrosurgical smoke on medical staff performing malignant tumor surgery.Methods This study was divided into two parts:in vitro and in vivo experiments.The human thyroid cancer cell line,ARO,was cultured and passaged.The tumor cells were burned with an ultrasonic scalpel,and the surgical smoke was absorbed by a transwell membrane.The captured particles were diluted in 3 mL of culture medium,and cell survival was assessed under a microscope.DNA was extracted from the cells for genotyping.BALB/c mice were used to construct thyroid cancer xenograft models.The tumor tissues were dissected on day 14 using an ultrasonic scalpel.The smoke from the electrosurgical procedure was collected on a transwell membrane.The membrane was washed in 2 mL of rinsing solution,and the solution was then injected into the right armpit of 10 mice.After sacrifice,the tumor tissues were removed and stained with hematoxylin and eosin(HE).Results Viable ARO cells could be seen on the first day after culturing cell fragments from surgical smoke,and vigorous cell proliferation could be seen on the 17th day of incubation.The genotype of the cells cultured in the presence of smoke particles was identical to the genotype of the original cells.Tumor growth was observed in four out of 10 mice injected with the smoke particle rinse.HE staining showed a significantly increased number of nuclei in the tumor tissue,which was consistent with the general morphological characteristics of malignant tumors.Conclusion Viable tumor cells were detected in surgical smoke generated by ultrasonic scalpel dissection,and these cells had growth activity.Thus,it is necessary to protect patients and medical staff from electrosurgical smoke. 展开更多
