Association between Serum Uric Acid and the Early Marker of Kidney Function Decline among Chinese Middle-Aged and Older Population:Evidence from the China Health and Retirement Longitudinal Study

Objective To evaluate the association between serum uric acid(SUA)and kidney function decline.Methods Data was obtained from the China Health and Retirement Longitudinal Study on the Chinese middle-aged and older population for analysis.The kidney function decline was defined as an annual estimated glomerular filtration rate(e GFR)decrease by>3 mL/min per 1.73 m^(2).Multivariable logistic regression was applied to determine the association between SUA and kidney function decline.The shape of the association was investigated by restricted cubic splines.Results A total of 7,346 participants were included,of which 1,004 individuals(13.67%)developed kidney function decline during the follow-up of 4 years.A significant dose-response relation was recorded between SUA and the kidney function decline(OR 1.14,95%CI 1.03-1.27),as the risk of kidney function decline increased by 14%per 1 mg/d L increase in SUA.In the subgroup analyses,such a relation was only recorded among women(OR 1.22,95%CI 1.03-1.45),those aged<60 years(OR 1.22,95%CI 1.05-1.42),and those without hypertension and without diabetes(OR 1.22,95%CI 1.06-1.41).Although the dose-response relation was not observed in men,the high level of SUA was related to kidney function decline(OR 1.83,95%CI 1.05-3.17).The restricted cubic spline analysis indicated that SUA>5 mg/dL was associated with a significantly higher risk of kidney function decline.Conclusion The SUA level was associated with kidney function decline.An elevation of SUA should therefore be addressed to prevent possible kidney impairment and dysfunction. 展开更多

Hyperuricemia(HUA)is a vital risk factor for chronic kidney diseases(CKD)and development of functional foods capable of protecting CKD is of importance.This paper aimed to explore the amelioration effects and mechanism of Andrias davidianus bone peptides(ADBP)on HUA-induced kidney damage.In the present study,we generated the standard ADBP which contained high hydrophobic amino acid and low molecular peptide contents.In vitro results found that ADBP protected uric acid(UA)-induced HK-2 cells from damage by modulating urate transporters and antioxidant defense.In vivo results indicated that ADBP effectively ameliorated renal injury in HUA-induced CKD mice,evidenced by a remarkable decrease in serum UA,creatinine and blood urea nitrogen,improving kidney UA excretion,antioxidant defense and histological kidney deterioration.Metabolomic analysis highlighted 14 metabolites that could be selected as potential biomarkers and attributed to the amelioration effects of ADBP on CKD mice kidney dysfunction.Intriguingly,ADBP restored the gut microbiome homeostasis in CKD mice,especially with respect to the elevated helpful microbial abundance,and the decreased harmful bacterial abundance.This study demonstrated that ADBP displayed great nephroprotective effects,and has great promise as a food or functional food ingredient for the prevention and treatment of HUA-induced CKD. 展开更多

Regulating and tonifying spleen and kidney traditional Chinese medicine in the treatment of chronic uric acid nephropathy: A systematic review

Objective:To systematically evaluate the efficacy and safety of traditional Chinese medicine for regulating spleen and kidney.Methods:We developed a search strategy and then retrieved the database including CNKI,Wanfang data knowledge service platform,VIP journals resource integration service platform,PubMed,Embasefor randomized controlled trial of regulating spleen and kidney traditional Chinese medicine compared with conventional western medicine in the treatment of chronic uric acid nephropathy.The search deadline was set to June 30,2020.For the included literature,we applied the cochrane collaboration network risk bias assessment tool to evaluate the methodological quality,and evaluated the level of evidence according to GRADE standards.Quantitative data was analyzed by RevMan5.3 software,and trial sequential analysis method was used to analyze its efficiency.Results:A total of 709 cases in 10 articles were included.Compared with the control group,the related traditional Chinese medicine group improved the effective rate[RR=1.45,95%CI(1.32,1.58)],reduced the level of UA[MD=-36.24,95%CI(-41.03,-31.45)],BUN[SMD=-1.27,95%CI(-1.47,-1.07)]and SCR[MD=-36.33,95%CI(-55.79,-16.87),P=0.0003],the difference between the two groups was statistically significant(P<0.05).There was no evidence that a significant adverse reaction occurred.The results of TSA analysis showed that the Chinese medicine group had definite evidence for improving the efficiency.According to the GRADE evaluation criteria,the efficiency,UA,BUN and SCR outcome indicators were extremely low-quality evidence.Conclusions:Traditional Chinese medicine for regulating spleen and kidney in the treatment of chronic uric acid nephropathy improved efficiency,reduced the level of UA,BUN,SCR.Meanwhile,the therapy was proved to be safe.Nevertheless,the conclusions need further high-quality evidence to support. 展开更多

Consequently, there are many questions about the link between kidney stones formation and level of salivary uric acid and calculus formation on the teeth surfaces. <strong>Objectives:</strong> To evaluate the correlation between the level of salivary uric acid and kidney stones formation and their influence on dental calculus and periodontal status among Saudi patients aged 25 - 70 years. <strong>Materials and Methods:</strong> 120 Saudi male patients were examined (60 of Kidney stones patients and 60 patients of non-kidney stones patients) for clinical evaluation of plaque index (PLI), gingival index (GI), calculus index of oral hygiene (CI) and clinical attachment loss (CAL). Moreover, lab assessment of uric acid level in the collected salivary samples was done. The findings were analyzed using of ANOVA test and Tukey’s test. <strong>Results:</strong> There were statistically significant differences in clinical parameters among kidney stones patients and non-kidney stones patients (p < 0.05), but these differences were highly statistically significant in the correlation between calculus index (CI), plaque index (PLI) and gingival index (GI) among kidney stone patients in group II, moreover, PLI and clinical attachment loss (CAL) among kidney stone patients in group III (p < 0.001). The statistical analyses revealed statistically significant differences in the level of salivary uric acid (mg/dl) in the comparison between kidney stones patients and non-kidney stones patients in group I and group III, whereas there were highly statistically significant in the comparison between kidney stones patients and non-kidney stones patients in group III. <strong>Conclusion:</strong> At the end of this study, we concluded that there was a relationship between dental calculus formation, kidney stones formation, and an increase in the level of salivary uric acid. 展开更多

An Observational Study on the Clinical and Nutritional Factors Determining the Kidney Function at One Year after Transplantation

Objective: The aim of the present study was to investigate the clinical and nutritional factors influencing the renal function of the transplanted kidney during a one-year follow-up period after transplantation. Patients: The present prospective observational study included 52 patients who underwent kidney transplantation at Jichi Medical University Hospital from 2014 to 2016. Results: The serum creatinine (sCr) concentration at one month after transplantation was closely related to the concentration at 12 months. The recipients were divided into two groups based on the sCr concentration at one month after transplantation. Recipients with a sCr concentration greater than or equal to the median were classified into Group H, while those with concentrations that were less than the median were classified into Group L. A stepwise multiple regression analysis demonstrated that the salt intake in male recipients was an independent predictor of the renal function at 12 months (β = 0.663, p β = 0.618, p < 0.01). Moreover, in female recipients, the BMI and serum uric acid values in Group H were significantly higher than those in Group L. The BMI values of the female recipients in Group H were significantly higher than those in Group L at all times during the follow-up period (p < 0.01). Conclusion: The daily salt intake was independently associated with the renal function at one year after kidney transplantation in male recipients. In female recipients, the BMI was independently associated with the sCr concentration during the one-year follow-up period. 展开更多

It has been suggested that exposure to organic solvents may have a role in the impairment of kidney function that may progress to kidney failure. However, this has never been evaluated with an appropriate analytical study of the kidney functions of those people who are chronically exposed to these chemicals. This study was designed to measure the kidney function of car painters in the city of Makkah, Saudi Arabia. Fifty workers were selected at random for this study and compared to thirty male medical students who were taken as a control group. Blood samples were collected for the analysis of kidney function. The levels of blood urea nitrogen (BUN), creatinine, and uric acid were scientifically higher in the tested group compared to the control group. In addition to this, the levels of these parameters were significantly higher in the serum of car painters who worked in this industry for more than ten years compared to painters who worked for less than ten years. Moreover, the number of car painters who were not using protective gloves and masks during working hours were 43 and the number of car painters who visited specialized clinics because of kidney problems were 45 of the 50 tested volunteers. These findings support the hypothesized association of solvent exposure with the development of chronic renal failure. They should prompt clinicians to give greater attention to patients’ occupational exposures. Routine monitoring of kidney functions and the use of protective materials are of greater importance to minimize the occupational diseases caused by organic solvents. 展开更多

目的探讨血尿酸(Serum Uric Acid,SUA)、酸碱平衡紊乱及其交互作用对慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease,COPD)患者免疫功能下降的影响。方法选取2022年10月—2023年10月钦州市第二人民医院收治的223例COPD患者为... 目的探讨血尿酸(Serum Uric Acid,SUA)、酸碱平衡紊乱及其交互作用对慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease,COPD)患者免疫功能下降的影响。方法选取2022年10月—2023年10月钦州市第二人民医院收治的223例COPD患者为研究对象。根据患者是否出现免疫功能下降分为免疫功能下降组(n=67)和免疫功能正常组(n=156)。比较两组的SUA水平、血氧分压(Partial Pressure of Oxygen,PO_(2))、二氧化碳分压(Partial Pressure of Carbon Dioxide,PCO_(2))、碳酸氢根离子和碱剩余(Base Excess,BE),分析SUA和酸碱平衡指标之间的交互作用。结果两组PO_(2)、PCO_(2)、碳酸氢根离子和SUA水平比较,差异有统计学意义(P均<0.05)。PO_(2)、PCO_(2)、碳酸氢根离子、SUA均为COPD患者免疫功能下降的影响因素(OR=2.484、2.554、2.891、5.757,P均<0.05);碳酸氢根离子与SUA存在相加交互作用,但未发现存在相乘交互作用;PO_(2)、PCO_(2)与SUA均不存在相加、相乘交互作用。结论SUA、PO_(2)、PCO_(2)和碳酸氢根离子是COPD患者免疫功能下降的影响因素,并且SUA和HCO3存在相加交互作用,各因素之间无相乘交互作用。 展开更多

目的探究复方木姜叶柯活性茶对高尿酸血症肾病小鼠的尿酸和肾功能的影响,为开发治疗高尿酸血症肾病的药物和功能性食品提供实验依据。方法采用氧嗪酸钾联合腺嘌呤建立高尿酸血症肾病小鼠模型。小鼠随机分为空白组、模型组、阳性组(10 mg... 目的探究复方木姜叶柯活性茶对高尿酸血症肾病小鼠的尿酸和肾功能的影响,为开发治疗高尿酸血症肾病的药物和功能性食品提供实验依据。方法采用氧嗪酸钾联合腺嘌呤建立高尿酸血症肾病小鼠模型。小鼠随机分为空白组、模型组、阳性组(10 mg/(kg·d))、复方茶高、中、低剂量组(10 g/(kg·d))、3.33 g/(kg·d)、1.11 g/(kg·d)。末次给药1 h后,取尿液,采用CBB法测定尿蛋白(UP),脲酶法检测尿液尿素氮(UUN);眼眶取血,采用酶比色法检测尿酸(UA)含量,脲酶法检测血清尿素氮(BUN)含量,酶联免疫法检测白细胞介素6(IL6)、肿瘤坏死因子(TNF-α)含量;取肾组织,荧光定量分析法检测尿酸盐转运蛋白1(URAT1)及葡萄糖转运体9(GLUT9)含量,HE染色观察肾组织病理变化情况。结果与空白组相比,模型组小鼠中UP、UUN、UA、BUN、IL-6、URAT1、GLUT9及TNF-α水平显著增高(P<0.01,P<0.05),肾组织结构正常。与模型组相比,阳性组中UP、UUN、UA、BUN、URAT1、IL-6、TNF-α水平显著降低(P<0.01,P<0.05),肾组织有少量肾小球萎缩变形,偶见肾小管扩张,无炎性细胞浸润。与模型组相比,复方茶高剂量组中UP、UUN、UA、BUN、IL-6、URAT1、TNF-α水平显著降低(P<0.01,P<0.05);复方茶中剂量组中UP、UUN、UA、IL-6、URAT1、GLUT9、TNF-α水平显著降低(P<0.01,P<0.05);复方茶低剂量组中UP、UUN、UA、IL-6、URAT1、BUN、TNF-α、GLUT9水平显著降低(P<0.01,P<0.05)。结论复方木姜叶柯活性茶能降低高尿酸血症肾病小鼠的尿酸,且对肾组织具有一定的保护作用,其机制可能与抑制尿酸重吸收有关,具体机制有待进一步研究。 展开更多

健康体检人群血尿酸与气流阻塞的相关性

目的:探讨中国台湾健康体检人群中血尿酸水平与肺功能指标和气流阻塞之间的相关性。方法:利用中国台湾美兆健康资源中心的体检资料,选取于1996—2016年间进行体检的40岁以上人群进行横断面分析,按照性别分别进行血尿酸水平与肺功能指标... 目的:探讨中国台湾健康体检人群中血尿酸水平与肺功能指标和气流阻塞之间的相关性。方法:利用中国台湾美兆健康资源中心的体检资料,选取于1996—2016年间进行体检的40岁以上人群进行横断面分析,按照性别分别进行血尿酸水平与肺功能指标的相关性分析,以及血尿酸水平对气流阻塞影响的Logistic回归分析。结果:共纳入35465人,平均年龄(45.70±7.30)岁,男性16411人,女性19054人,其中,男性的血尿酸水平高于女性,存在气流阻塞的人群血尿酸水平高于无气流阻塞的人群。女性血尿酸水平与肺功能指标[第一秒用力呼气容积(forced expiratory volume in one second,FEV1)、用力肺活量(force vital capacity,FVC)]之间呈负相关(P<0.05),男性血尿酸水平与肺功能指标FEV1、FVC之间则无显著相关性(P>0.05)。在调整年龄、教育程度、吸烟、饮酒、工作强度、体重指数、是否咳嗽、高血压病史、糖尿病病史等指标,以及白细胞、白蛋白等生化指标后,随着血尿酸浓度的升高,女性更易存在气流阻塞(OR=1.12,95%CI:1.02~1.22,P<0.05)。按照是否患高尿酸血症进行分组,结果显示与血尿酸正常组相比,女性中高尿酸血症组更易存在气流阻塞(OR=1.36,95%CI:1.06~1.75,P<0.05),而男性中血尿酸浓度与气流阻塞患病之间无显著相关性(OR=1.04,95%CI:0.96~1.13,P>0.05),是否患高尿酸血症与气流阻塞的存在无显著相关性(OR=1.12,95%CI:0.89~1.39,P>0.05)。结论:女性健康体检人群中,血尿酸水平与肺功能指标FEV1、FVC之间存在负相关,高尿酸血症与气流阻塞的存在有相关性,在男性中则不明显。未来仍需要前瞻性研究来进一步验证高血尿酸水平是否与气流阻塞的发生相关联。 展开更多

目的探讨超高龄慢性肾脏病病人标化尿酸与肾功能的相关性,为超高龄晚期慢性肾脏病病人降尿酸药物治疗提供参考。方法回顾性收集在滁州市第一人民医院年龄≥90岁的出院病人临床资料,包括基本情况、肾功能指标、用药情况等,分析标化尿酸(... 目的探讨超高龄慢性肾脏病病人标化尿酸与肾功能的相关性,为超高龄晚期慢性肾脏病病人降尿酸药物治疗提供参考。方法回顾性收集在滁州市第一人民医院年龄≥90岁的出院病人临床资料,包括基本情况、肾功能指标、用药情况等,分析标化尿酸(血尿酸/血肌酐比值)和各肾功能指标的相关性。结果共纳入58例超高龄老年慢性肾脏病病人82例次病历资料,病人入院时和出院前的血尿酸和肾功能指标无明显变化,使用降尿酸药物后尿酸和其他肾功能指标差异均无统计学意义(P>0.05)。标化尿酸与血肌酐(r=-0.678)、尿素氮(r=-0.505)、胱抑素C(r=-0.611)呈负相关,与肾小球滤过率呈正相关(r=0.738,P<0.01)。结论超高龄慢性肾脏病病人标化尿酸与肾功能指标相关性强,降尿酸药物使用不能明显改善超高龄慢性肾脏病病人的肾功能,相反标化尿酸在一定范围内升高可能有助于肾功能指标的改善。 展开更多

背景:目前尚无关于血液实验室检查和非创伤性股骨头坏死分期及病程关系的文献,有必要进一步探讨分析以便更好地明确非创伤性股骨头坏死的影响因素。目的:分析血液实验室检查指标与非创伤性股骨头坏死国际骨循环研究会(ARCO)分期病程之... 背景:目前尚无关于血液实验室检查和非创伤性股骨头坏死分期及病程关系的文献,有必要进一步探讨分析以便更好地明确非创伤性股骨头坏死的影响因素。目的:分析血液实验室检查指标与非创伤性股骨头坏死国际骨循环研究会(ARCO)分期病程之间的关系,探讨非创伤性股骨头坏死病程的血液实验室指标影响因素。方法:采用回顾性研究的方法,从中国中医科学院望京医院数据库调取共2103例股骨头坏死患者的病历资料,根据纳入排除标准,最终纳入了1075例非创伤性股骨头坏死患者。收集患者的年龄、性别、体质量指数和血液实验室指标结果,血液实验室指标包含低密度脂蛋白、总胆固醇、三酰甘油、高密度脂蛋白、载脂蛋白β、载脂蛋白α1、尿酸、总蛋白定量、碱性磷酸酶、活化部分凝血活酶时间、凝血酶原时间、凝血酶原时间国际标准化比值、凝血酶原时间活动度、纤维蛋白原定量、凝血酶凝结时间、D-二聚体、总铁结合力、血小板计数。比较不同年龄段、不同ARCO分期患者的相关指标,并应用多元Logistic回归分析探讨股骨头坏死ARCO分期的影响因素。结果与结论:①青年组的总胆固醇、尿酸、凝血酶原时间、凝血酶原时间国际标准化比值、D-二聚体在ARCO分期组间比较差异有显著性意义(P<0.05),青年组ARCOⅡ期患者在总胆固醇水平上高于ARCOⅢ期患者(P<0.05),ARCOⅣ期的尿酸水平高于ARCOⅡ、Ⅲ期患者(P<0.05),ARCOⅡ、Ⅳ期的凝血酶原时间和凝血酶原时间国际标准化比值少于ARCOⅢ期患者(P<0.05),ARCOⅢ、Ⅳ期患者的D-二聚体高于ARCOⅡ患者(P<0.05);②中年组的高密度脂蛋白、凝血酶凝结时间、D-二聚体的ARCO分期组间比较差异有显著性意义(P<0.05),中年组ARCOⅣ期患者在高密度脂蛋白水平上高于ARCOⅡ、Ⅲ期患者(P<0.05),ARCOⅣ期凝血酶凝结时间短于ARCOⅢ期(P<0.05),ARCOⅣ期患者的D-二聚体水平高于ARCOⅡ、Ⅲ期患者(P<0.05);③老年组的尿酸、活化部分凝血活酶时间、D-二聚体、血小板计数组间比较差异有显著性意义(P<0.05),老年组ARCOⅣ期患者的尿酸水平高于ARCOⅡ、Ⅲ期患者(P<0.05),ARCOⅡ期患者的活化凝血酶原时间短于ARCOⅢ期(P<0.05),ARCOⅢ、Ⅳ期患者的D-二聚体水平高于ARCOⅡ期患者(P<0.05),ARCOⅣ期的血小板计数要低于ARCOⅢ期患者(P<0.05);④多元Logistic回归分析显示,总胆固醇和血小板计数是非创伤性股骨头坏死病程的保护性因素,D-二聚体、尿酸、超重、中青年年龄段可能是非创伤性股骨头坏死病程的危险因素;⑤提示不同ARCO分期患者的总胆固醇、高密度脂蛋白、尿酸、凝血酶原时间、凝血酶原时间国际标准化比值、D-二聚体存在统计学差异,总胆固醇、血小板计数可能是非创伤性股骨头坏死病程的保护性因素,D-二聚体、尿酸、超重、中青年年龄段可能是非创伤性股骨头坏死病程的危险因素。 展开更多

不同NYHA分级慢性心力衰竭患者血清cTnI、NT-proBNP、UA水平变化及其与预后转归的关系

目的探讨不同纽约心脏病协会(NYHA)分级慢性心力衰竭(CHF)患者血清肌钙蛋白I(cTnI)、氨基末端B型利钠肽前体(NT-proBNP)、尿酸(UA)水平的变化,并分析其与预后转归的关系。方法收集2020年1月—2022年1月本院收治的95例CHF患者临床资料,采... 目的探讨不同纽约心脏病协会(NYHA)分级慢性心力衰竭(CHF)患者血清肌钙蛋白I(cTnI)、氨基末端B型利钠肽前体(NT-proBNP)、尿酸(UA)水平的变化,并分析其与预后转归的关系。方法收集2020年1月—2022年1月本院收治的95例CHF患者临床资料,采用NYHA分级分为Ⅱ级29例(Ⅱ级组),Ⅲ级38例(Ⅲ级组),Ⅳ级28例(Ⅳ级组),比较3组血清cTnI、NT-proBNP、UA水平,分析CHF患者心功能与血清cTnI、NT-proBNP、UA水平的相关性;评估CHF患者出院后1个月复查时血清cTnI、NT-proBNP、UA水平变化情况,比较血清cTnI、NT-proBNP、UA短期(出院后1个月)升高与未升高者出院12个月心血管终点事件发生情况。结果3组患者性别、年龄、患病史等基线资料比较,差异无统计学意义(P>0.05);Ⅱ级组、Ⅲ级组与Ⅳ级组左心室舒张末期内径(LVEDD)及血清cTnI、NT-proBNP、UA水平依次明显升高,左心室射血分数(LVEF)则依次明显下降,且两两比较差异均有统计学意义(P<0.05)。Pearson相关系数分析显示,CHF患者血清cTnI、NT-proBNP、UA水平均与LVEDD呈显著正相关(P<0.05),均与LVEF呈显著负相关(P<0.05)。出院后1个月,3组患者血清cTnI、NT-proBNP、UA水平均较入院次日显著降低(P<0.05),但cTnI、NT-proBNP、UA升高>20%者分别有4.21%、7.37%、30.53%。95例CHF患者中有72例获得出院后12个月随访,有心血管终点事件31例(43.06%)。血清cTnI、NT-proBNP短期升高与未升高者出院后12个月心血管终点事件发生率比较,差异无统计学意义(P>0.05);血清UA短期升高者出院后12个月心血管终点事件发生率明显高于未升高者(P<0.05),无心血管终点事件发生的生存时间明显低于未升高者(P<0.05)。结论入院时血清cTnI、NT-proBNP、UA水平与CHF患者心功能分级有关,血清UA短期升高者心血管终点事件发生风险更高。 展开更多

目的探究沙库巴曲缬沙坦对老年高血压患者血压控制及丙氨酸氨基转移(ALT)、肌酐(Cr)、血钾及血尿酸水平的影响。方法选取2021年5月至2022年4月温州怡宁老年医院收治的98例老年高血压患者作为研究对象,按照随机抽签法分为对照组与观察组... 目的探究沙库巴曲缬沙坦对老年高血压患者血压控制及丙氨酸氨基转移(ALT)、肌酐(Cr)、血钾及血尿酸水平的影响。方法选取2021年5月至2022年4月温州怡宁老年医院收治的98例老年高血压患者作为研究对象,按照随机抽签法分为对照组与观察组,每组49例。对照组口服缬沙坦治疗,观察组口服沙库巴曲缬沙坦治疗,比较两组临床疗效和用药前后血压、肝肾功能(ALT、Cr)、血钾、血尿酸水平及不良反应发生率。结果观察组治疗总有效率为95.92%,高于对照组的81.63%,差异有统计学意义(P<0.05)。用药3个月后,两组舒张压、收缩压水平均低于用药前,且观察组低于对照组,差异有统计学意义(P<0.05)。用药3个月后,两组ALT、Cr水平均高于用药前,血尿酸水平均低于用药前,观察组血钾水平高于用药前,对照组血钾水平低于用药前,且观察组ALT、血钾水平均高于对照组,Cr、血尿酸水平均低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论沙库巴曲缬沙坦对老年高血压患者的血压控制效果更理想,可改善患者肝、肾功能,降低血尿酸水平,提高血钾水平,不会增加不良反应的发生,用药安全较高,值得临床推广应用。 展开更多

The Effect of Several Commonly Used Antipsychotic Drugs on the Renal Function of Patients with Mental Illness

Objective: To understand the effects of several commonly used antipsychotics on the renal function of patients with mental illness. Method: Collected patients with mental illness who were hospitalized in our hospital from January 2020 to June 2021, and selected as the research subjects patients with psychiatric disorders who were treated with 2 kinds of commonly used antipsychotic drugs;and collected 3 ml of venous blood before treatment and one month after treatment for renal function tests;observed the changes of renal function indexes before and after treatment. Results: In the collected 694 patients with mental illness, before using antipsychotic drugs, the renal function indexes were BUN: 4.42 ± 1.92 mmol/l;Cr: 70.97 ± 16.92 μmol/l;CCr: 88.37 ± 21.07 ml/min;β2-MG: 1.67 ± 0.61 mg/L;UA: 359.90 ± 112.82 μmol/l;CYS-C: 0.92 ± 0.24 mg/L. One month after using antipsychotics, BUN: 3.77 ± 1.37 mmol/l;Cr: 70.46 ± 16.71 μmol/l;CCr: 87.78 ± 20.63 ml/min;β2-MG: 1.75 ± 0.64 mg/L;UA: 332.53 ± 91.48 umol/l;CYS-C: 0.92 ± 0.24 mg/L;the renal function indexes of urea nitrogen, β2 microglobulin, uric acid and other items all changed significantly. The differences before and after treatment were statistically significant, P < 0.01. Conclusion: Several commonly used antipsychotic drugs have a greater impact on the renal function of patients with mental illness. During the treatment, the changes in renal function should be monitored regularly, if severe renal damage is found, the treatment plan or dosage should be adjusted in time to avoid endangering life. 展开更多

目的:糖尿病肾病是糖尿病最严重的并发症之一,也是慢性肾脏病和终末期肾脏病(end-stage renal disease,ESRD)的主要原因。寻找和控制影响肾功能进展的因素至关重要。本研究旨在探讨血尿酸(serum uric acid,SUA)轨迹与2型糖尿病(Type 2 d... 目的:糖尿病肾病是糖尿病最严重的并发症之一,也是慢性肾脏病和终末期肾脏病(end-stage renal disease,ESRD)的主要原因。寻找和控制影响肾功能进展的因素至关重要。本研究旨在探讨血尿酸(serum uric acid,SUA)轨迹与2型糖尿病(Type 2 diabetes mellitus,T2DM)患者肾功能进展的关系。方法:选取2009年1月至2021年12月于中南大学湘雅三医院肾内科和内分泌科就诊且基线估算肾小球滤过率(estimated glomerular filtration rate,eGFR)≥60 mL/(min·1.73 m^(2))的846例T2DM患者作为研究对象。收集多次测量的SUA数据,通过组基轨迹模型(group-based trajectory modeling,GBTM)识别SUA的发展轨迹。根据不同的SUA轨迹,将患者分为低轨迹组(105例)、中轨迹组(396例)、中高轨迹组(278例)及高轨迹组(67例)。采用Cox回归分析SUA轨迹对T2DM患者肾功能进展的影响,并按性别、年龄、病程、体重指数(body mass index,BMI)及糖化血红蛋白(hemoglobin A1c,HbA1c)分层进行亚组分析。结果:中位随访4.8年,随访结束时158例患者出现不同程度的肾功能进展。校正了多种混杂因素的Cox回归分析结果显示:高轨迹组出现eGFR<60 mL/(min·1.73 m^(2))、eGFR下降率≥50%、血肌酐(serum creatinine,Scr)翻倍及复合终点(eGFR下降率≥50%、Scr翻倍或ESRD)的风险较低轨迹组显著增加,HR分别为3.84(95%CI 1.83~8.05)、6.90(95%CI 2.27~20.96)、6.29(95%CI 2.03~19.52)、8.04(95%CI 2.68~24.18)。各轨迹组出现ESRD的风险差异均无统计学意义(均P>0.05)。亚组分析显示:与低轨迹组相比,在男性、女性、年龄<65岁、病程<10年、BMI≥24 kg/m^(2)、HbA1c≥7%亚组中,高轨迹组出现eGFR<60 mL/(min·1.73 m^(2))的风险均增加(均P<0.05)。SUA轨迹与性别、年龄、病程、BMI及HbA1c均无交互作用(均P交互>0.05)。结论:高SUA轨迹增加了T2DM患者肾功能进展的风险,今后需关注SUA的长期纵向变化。 展开更多
