Aim: To determine th...Background: The exact incidence of vascular injuries in Nigeria as a country especially south-east zone of Nigeria is unknown on account of under reporting and uncoordinated management of victims. Aim: To determine the pattern and outcome of common civilian vascular injuries managed in a teaching hospital in the south-east zone of Nigeria. Method: This is a retrospective study from January, 2007 to December, 2013. All case records of common civilian vascular injuries presenting at the accident center and those referred to the clinics as well as operation register and data banks of managing surgeons, were retrieved and analysed. Results: The age range of 12 - 75 years was rec-orded for the 26 patients. In this spectrum, age range of 21 - 30 years (19.2%) was the highest while the age range, 61 - 70 years (0%) was the lowest. One female (3.9%) and twenty five males (96.1%) were recorded, giving a female to male ratio of 1:25. In the upper extremity, brachial artery 6 (23.1%) was the most involved vessel while in the lower extremity, the most involved was the femoral artery 9 (34.6%). The patterns of presentation were isolated bleeding 10 (38%), arterivenous fistula 1 (3.5%) and pseudoaneurysm 8 (30%). 23 (88%) had good outcome. Conclusion: The incidence is about 4 cases per year. 23 (88%) had good outcome. The shortcoming of lattending physicians was because they were not conversant with hard and soft signs of vascular injuries with attendant limb loss and death of one of the victims. This short coming can be averted by training and retraining of doctors.展开更多

Because of the rarity of this lesion and its aspecific clinical features, AFX could be eas...Atypical fibroxanthoma(AFX) is a spindle cell neoplasm with low metastatic potential but high tendency to recur after surgery. Because of the rarity of this lesion and its aspecific clinical features, AFX could be easily misdiagnosed and undertreated by many clinicians who encounter them. Dermoscopy represents a valuable tool for easily assessing skin lesions, even though histological examination is required for final diagnosis. We report a case of a cheek lesion with dermoscopic "blue amber pattern", easily recognisable and not observed in others skin tumours, which could represent an additional feature useful in differentiating this tumour from other skin neoplasms.展开更多
AIM:To describe the prevalence and changes in treatment patterns of ranibizumab and photodynamic therapy(PDT) among retinal disease patients who attended the Ophthalmology Clinic in the tertiary care Hospital Selay...AIM:To describe the prevalence and changes in treatment patterns of ranibizumab and photodynamic therapy(PDT) among retinal disease patients who attended the Ophthalmology Clinic in the tertiary care Hospital Selayang from 2010 to 2014.METHODS:Study subjects were recruited retrospectively using the Electronic Medical Record(EMR) database software in Hospital Selayang.Demographic data,medical history,diagnostic procedure,treatments and diagnosis of patients were recorded.RESULTS:The five-year analysis included 821 patients with a mean age of 65.9±11.73 y.Overall,there were a highernumber of males(63.1%) and a higher number of Chinese(47.4%) patients.Among the 821 patients,62.9% received ranibizumab injection followed by 19.2% PDT therapy and 17.9% had ranibizumab combined with PDT therapy.Age-related macular degeneration(AMD) and polypoidal choroidal vasculopathy(PCV) were the most common retinal eye diseases reported,recording prevalence of 25.0% and 45.6%,respectively.The trend in ranibizumab treatment was reported to increase while PDT showed a decrease in trend from year 2010 to 2014.In terms of treatment,following multiple logistic regression,AMD was associated with the subjects being more likely to have received ranibizumab monotherapy(P〈0.001) while PCV was associated with more likely to have received PDT(P〈0.001) and PDT combined with ranibizumab therapy(P〈0.001).CONCLUSION:The tertiary care setting in Malaysia is consistent with management of patients from other countries whereby ranibizumab is the most common treatment given to patients with AMD,while PCV patients most commonly receive PDT and ranibizumab combined with PDT therapy.展开更多
