Acupuncture at Waiguan(SJ5) and sham points influences activation of functional brain areas of ischemic stroke patients: a functional magnetic resonance imaging study 被引量:22

Most studies addressing the specificity of meridians and acupuncture points have focused mainly on the different neural effects of acupuncture at different points in healthy individuals. This study examined the effects of acupuncture on brain function in a pathological context. Sixteen patients with ischemic stroke were randomly assigned to true point group (true acupuncture at right Waiguan (SJ5)) and sham point group (sham acupuncture). Results of functional magnetic resonance imaging revealed activation in right parietal lobe (Brodmann areas 7 and 19), the right temporal lobe (Brodmann area 39), the right limbic lobe (Brodmann area 23) and bilateral oc-cipital lobes (Brodmann area 18). Furthermore, inhibition of bilateral frontal lobes (Brodmann area 4, 6, and 45), right parietal lobe (Brodmann areas 1 and 5) and left temporal lobe (Brodmann area 21 ) were observed in the true point group. Activation in the precuneus of right parietal lobe (Brodmann area 7) and inhibition of the left superior frontal gyrus (Brodmann area 10) was observed in the sham group. Compared with sham acupuncture, acupuncture at Waiguan in stroke patients inhibited Brodmann area 5 on the healthy side. Results indicated that the altered specificity of sensation-associated cortex (Brodmann area 5) is possibly associated with a central mechanism of acupuncture at Waiguan for stroke patients. 展开更多

The action of needling in acupoint therapy has to first be regulated and integrated by the brain, and then it affects the target organ and manifests its therapeutic effects, which is dependent on the specificity of the acupoints. The authors put forward the hypothesis of the "acupoint-related brain". Single-photon emission computed tomography was used to explore the activation of brain regions following true needling in true acupoint Waiguan (SJ 5), sham needling in true acupoint Waiguan, true needling in a sham point, and sham needling in a sham point. The relative specificity of Waiguan in normal persons was analyzed by observing changes in regional cerebral blood flow. Compared with the sham needling in true acupoint group and sham needling in the sham point group, acupuncture at Waiguan can activate brain regions controlling movement. Compared with true needling in the sham point group, acupuncture at Waiguan can also activate brain regions controlling movement. The results suggest that the specificity of needling at an acupoint is related to certain activated cerebral functional regions, which are associated with the clinical application of the acupoint. 展开更多

Functional brain imaging studies of the specificity of acupoint stimulation have revealed that needling at particular acupoints can induce activation of corresponding brain areas, examined in physiological and pathological states. The present study examined Waiguan (SJ 5) as the acupoint of interest. We tested five conditions: a true needling at true acupoint group, a sham needling at true acupoint group, a true needling at sham acupoint group, a sham needling at sham acupoint group, and a healthy control group. We examined changes in glucose metabolism in different functional cortical regions to determine the specific brain activation elicited by acupuncture at Waiguan. The results revealed that true and sham acupuncture at the same acupoint activated different brain regions. In addition, applying acupuncture at a genuine acupoint and a neighboring sham acupoint activated different brain regions. These findings suggest that acupuncture at Waiguan causes activation in some brain regions. 展开更多

However, the physiological mechanisms responsible for this phenomenon remain poorly understood. The present study analyzed 12 ischemic stroke patients, who were randomly assigned to two groups: one group underwent sham needling and true needling at the Waiguan (SJ 5) in the healthy upper limb and the other group underwent sham and true needling at a sham point. Functional magnetic resonance imaging results showed no activation points in brain tissues following needling at SJ 5. However, compared with sham needling at SJ 5, true needling at SJ 5 deactivated Broadmann 4, 6, 24, and 32 areas. In addition, compared to needling at the sham point, true needling at SJ 5 deactivated bilateral hypothalamus. Results demonstrated that SJ 5 needling in the healthy upper limb resulted in specific directional brain action, as manifested by deactivation of cerebral areas related to motor (Broadmann 4 and 6), emotion (hypothalamus), and cognition (Broadmann 24, 32). 展开更多

Few studies have examined the effects of different stimuli at a single acupoint using functional magnetic resonance imaging.The present study applied acupuncture at the Neiguan(PC 6),Waiguan(SJ 5),Zhigou(SJ 6) and Yanglingquan(GB 34) acupoints in healthy volunteers.fMRI was used to examine the activation of brain areas in response to different types of acupuncture(cutaneous or routine acupuncture) at each acupoint.There were no significant differences in the distribution of activation in the regions of interest between cutaneous and routine acupuncture at the Neiguan,Waiguan,and Zhigou acupoints,but some differences were observed between the two methods of acupuncture at the Yanglingquan acupoint.There were no significant differences in the intensity of induced activation between cutaneous and routine acupuncture at the Neiguan,Zhigou and Yanglingquan acupoints,but the activation intensity in the right cerebellum induced by routine acupuncture at the Waiguan acupoint was greater than that induced by cutaneous acupuncture.Results confirmed that cutaneous and routine acupuncture at the Neiguan,Waiguan,Zhigou and Yanglingquan acupoints activated different functional brain areas,and caused activation of different intensities in some areas. 展开更多

目的:运用fMRI脑功能成像技术,观察并比较针刺健康志愿者右侧外关穴和外关穴配伍内关穴对不同脑区的激活情况。方法:16例健康志愿者,随机分外关穴组和外关穴配伍内关穴组(每组8例),分别针刺右侧外关穴和外关穴配伍内关穴;针刺同时进行f... 目的:运用fMRI脑功能成像技术,观察并比较针刺健康志愿者右侧外关穴和外关穴配伍内关穴对不同脑区的激活情况。方法:16例健康志愿者,随机分外关穴组和外关穴配伍内关穴组(每组8例),分别针刺右侧外关穴和外关穴配伍内关穴;针刺同时进行fMRI扫描,所有数据采用SAS9.13软件包进行统计分析,统计外关穴组和外关配伍内关穴组在针刺状态下,脑部各感兴趣区(ROIs)的激活点数、激活几率、激活强度,并对所激活的相同脑区进行比较分析。结果:ROIs激活几率统计比较表明,与外关穴配伍内关穴比较,针刺外关穴右侧小脑激活几率较高(P<0.05);ROIs激活点数统计比较表明,针刺外关穴能相对特异地激活右侧小脑(P<0.05),而外关穴配伍内关穴则能相对特异地激活左侧顶叶(P<0.05);ROIs激活强度统计比较表明,针刺外关穴与针刺外关穴配伍内关穴,对各脑区的激活强度没有显著差异。结论:fMRI脑功能成像研究表明,针刺外关穴能够在维持躯体平衡、改善肌张力障碍及调节随意运动方面发挥较为突出的作用。外关穴配伍内关穴这种表里经配穴能够加强对偏身感觉及运动障碍的治疗作用,这为外关穴特异性及表里经配穴规律的研究提供了初步的参考依据。 展开更多

根据腧穴命名原则和针感效应,提出在腕踝上三寸内外侧各有一个三经交会穴的新观点,分别是三阴交、悬钟、会宗、间使。从三阴交、悬钟、会宗、间使四穴的现代解剖学神经分布,以及针刺四穴后针感循行分布,悬钟、会宗、间使与三阴交一样,... 根据腧穴命名原则和针感效应,提出在腕踝上三寸内外侧各有一个三经交会穴的新观点,分别是三阴交、悬钟、会宗、间使。从三阴交、悬钟、会宗、间使四穴的现代解剖学神经分布,以及针刺四穴后针感循行分布,悬钟、会宗、间使与三阴交一样,均符合三经交会穴的特点,虽然不能将现代的神经与经络完全等同,但经络包含神经是无可争辩的。在《针灸甲乙经》和《针灸大成》的记载中,悬钟、会宗、间使三穴的主治与三经交会穴主治范围尚有一定差距,但不能排除是人们对以上三穴认识得还不够清楚。目前,除三阴交穴之外,其余三穴临床研究有限,这是今后努力的方向。 展开更多

The needling sensation of Deqi during acupuncture is a key factor of influencing acupuncture outcome.Recent studies have mainly focused on the brain function effects of Deqi in a physiological state.Functional magnetic resonance imaging(f MRI)on the effects of acupuncture at Waiguan(SJ5)in pathological and physiological states is controversial.In this study,12 patients with ischemic stroke received acupuncture at Waiguan(SJ5)and simultaneously underwent f MRI scanning of the brain,with imaging data of the activated areas obtained.Based on the patient's sensation,imaging data were allocated to either the Deqi group or non-Deqi group.In the Deqi group,the activated/deactivated areas were the left superior temporal gyrus(BA39)/right anterior lobe of the cerebellum and left thalamus.In the non-Deqi group,the activated areas included the medial frontal gyrus of the right frontal lobe(BA11),right limbic lobe(BA30,35),and left frontal lobe(BA47),while the only deactivated area was the right parietal lobe(BA40).Compared with the non-Deqi group,the Deqi group exhibited marked activation of the right anterior lobe of the cerebellum and right limbic lobe(BA30).These findings confirm that the clinical effect of Deqi during acupuncture is based on brain functional changes.Cerebellar activation may be one of the central mechanisms of acupuncture in the treatment of ischemic stroke. 展开更多

目的:观察单侧针刺曲池穴、外关穴的不同针刺状态对健康受试者双侧运动皮层(M1)长时程增强(LTP)样脑可塑性的影响。方法:纳入18名健康受试者,取右侧曲池、外关穴,给予常规普通针刺,进针后行针,得气为度。于针刺前15 min、进针后30 min... 目的:观察单侧针刺曲池穴、外关穴的不同针刺状态对健康受试者双侧运动皮层(M1)长时程增强(LTP)样脑可塑性的影响。方法:纳入18名健康受试者,取右侧曲池、外关穴,给予常规普通针刺,进针后行针,得气为度。于针刺前15 min、进针后30 min及拔针后20 min分别使用经颅磁刺激运动诱发电位(TMSMEP)检测皮层兴奋性,观察单侧针刺对成对关联刺激(PAS)诱导LTP脑可塑性的影响。操作方法:①TMSMEP的诱导:进行TMS刺激点定位,找到以最小的刺激强度产生最大MEP波幅的位置,即为FDI皮层运动点;并使用TMS外固定架固定线圈、激光定位仪监测。②检测静息运动诱发电位阈值(rMT):基于FDI对应的皮层运动点,寻找给予10次TMS刺激能够产生至少5个MEP波幅≥50μV的刺激强度,记录为rMT。③短潜伏期尺神经体感诱发电位的测定:刺激电极置于FDI肌腹,阴极在近端,刺激电流5~10 mA,刺激强度为食指轻微抽动为宜。④PAS-LTP脑可塑性的诱导:使用电刺激加磁刺激进行诱导,共200对刺激。每位受试者需进行8次检测,每次约80 min。2次检测需间隔1周,以避免针刺后效应影响试验结果。结果:①与针刺前比较,留针时针刺对侧M1区MEP波幅比降低,针刺同侧M1区MEP波幅比升高;拔针后,针刺双侧M1区MEP波幅比均升高;差异均有统计学意义(P<0.01)。②PAS诱导后,留针时针刺同侧M1区MEP波幅升高;起针后,针刺双侧M1区MEP波幅均升高;差异均有统计学意义(P<0.01)。结论:单侧单次针刺曲池、外关穴不同针刺状态(留针、拔针后)对双侧LTP脑可塑性的影响不同,可以通过调节皮层兴奋性特异性改变双侧皮层的可塑性。 展开更多

Objective To observe the therapeutic effects of post-stroke hand motor impairment treated with acupuncture at Zhongzhu (中渚 TE 3) and Waiguan (外关TE 5). Methods Sixty cases of post-stroke hand motor impairment were randomly divided into an observation group and a control group, 30 cases in each one. The conventional medication, physical therapy, occupational therapy and other rehabilitation trainings were applied in both groups. In observation group, Zhongzhu (中渚 TE 3) and Waiguan (外关TE 5) were punctured. The assessment was performed with neurologie deficit scoring of the National Institutes of Health Stroke Scale (NIHSS), hand motor function, walking ability and the modified Barthel index scale of the Activities of Daily Living (ADL) in both groups before and after treatment. Results After treatment, the scores of hand function, walking ability, ADL and NIHSS were improved in both groups as compared with those before treatment (all P〈0.01). The improvements of hand function, walking ability and ADL :in observation group were superior to those in control group (all P〈0.05). Conclusion The conventional medication and rehabilitation trainings combined with acupuncture at Zhongzhu (中渚 TE 3) and Waiguan (外关TE 5) ean obviously improve hand motor function, walking ability, the activities of daily living and rehabilitation efficacy in the patients with post-stroke hand motor impairment. 展开更多

目的:运用正电子发射计算机断层成像(PET)技术,基于脑功能区葡萄糖代谢变化,探讨针刺得气的脑功能成像特点。方法:18例健康志愿者,随机分为经穴组、非穴组、对照组,分别施以外关穴针刺、非穴针刺、不针刺,以18F-FDG为显影剂,运用PET进... 目的:运用正电子发射计算机断层成像(PET)技术,基于脑功能区葡萄糖代谢变化,探讨针刺得气的脑功能成像特点。方法:18例健康志愿者,随机分为经穴组、非穴组、对照组,分别施以外关穴针刺、非穴针刺、不针刺,以18F-FDG为显影剂,运用PET进行脑部扫描,获得志愿者脑功能区葡萄糖代谢变化的图像数据,扫描完毕随即运用针刺感觉量表量化志愿者的感觉,依据感觉量化结果将所得图像数据分为酸麻胀重感组、刺痛感组与无感觉组进行比较,数据在Matlab平台上采用SPM 2.0软件包进行处理和分析。结果:(1)临床针刺效应:外关穴组6人,5人有酸麻胀重感,视觉模拟评分(VAS)4.23±1.50;非穴组6人,5人有刺痛感,VAS 5.73±2.40;两组VAS比较差异有统计学意义(P<0.05)。(2)脑功能成像结果按照P<0.001、相连像素大于10个的统计学标准:酸麻胀重感组与无感觉组比较,显著激活BA 7、13、20、22、39、42、45,主要涉及左颞叶、颞上回等;刺痛感组与无感觉组比较,显著激活BA 18、19、22、24、25、32、36、40、45,主要涉及左边缘叶、海马回等。结论:针刺腧穴产生的感觉主要以徐和的酸麻胀重等得气指征为主,针刺非穴产生的感觉以刺痛为主,前者所激活脑区的调控功能与腧穴的主治作用密切相关;得气的关键与腧穴的定位准确度相关;针刺得气是"针刺""腧穴""感觉""腧穴效应"等的综合体现。 展开更多
