Lung cancer is the most common and fatal malignant disease worldwide and has the highest mortality rate among tumor-related causes of death.Early diagnosis and precision medicine can significantly improve the survival...Lung cancer is the most common and fatal malignant disease worldwide and has the highest mortality rate among tumor-related causes of death.Early diagnosis and precision medicine can significantly improve the survival rate and prognosis of lung cancer patients.At present,the clinical diagnosis of lung cancer is challenging due to a lack of effective non-invasive detection methods and biomarkers,and treatment is primarily hindered by drug resistance and high tumor heterogeneity.Liquid biopsy is a method for detecting circulating biomarkers in the blood and other body fluids containing genetic information from primary tumor tissues.Bronchoalveolar lavage fluid(BALF)is a potential liquid biopsy medium that is rich in a variety of bioactive substances and cell components.BALF contains information on the key characteristics of tumors,including the tumor subtype,gene mutation type,and tumor environment,thus BALF may be used as a diagnostic supplement to lung biopsy.In this review,the current research on BALF in the diagnosis,treatment,and prognosis of lung cancer is summarized.The advantages and disadvantages of different components of BALF,including cells,cell-free DNA,extracellular vesicles,and micro RNA are introduced.In particular,the great potential of extracellular vesicles in precision diagnosis and detection of drug-resistant for lung cancer is highlighted.In addition,the performance of liquid biopsies with different body fluid sources in lung cancer detection are compared to facilitate more selective studies involving BALF,thereby promoting the application of BALF for precision medicine in lung cancer patients in the future.展开更多
Background:For patients with lung cancer,timely identification of new lung lesions as infectious or non-infectious,and accurate identification of pathogens is very important in improving OS of patients.As a new auxiliar...Background:For patients with lung cancer,timely identification of new lung lesions as infectious or non-infectious,and accurate identification of pathogens is very important in improving OS of patients.As a new auxiliary examination,metagenomic next-generation sequencing(mNGS)is believed to be more accurate in diagnosing infectious diseases in patients without underlying diseases,compared with conventional microbial tests(CMTs).We designed this study tofind out whether mNGS has better performance in distinguishing infectious and non-infectious diseases in lung cancer patients using bronchoalveolar lavagefluid(BALF).Materials and Methods:This study was a real-world retrospective review based on electronic medical records of lung cancer patients with bronchoalveolar lavage(BAL)and BALF commercial mNGS testing as part of clinical care from 1 April 2019 through 30 April 2022 at The First Affiliated Hospital of Sun Yat-sen University.164 patients were included in this study.Patients were categorized into the pulmonary non-infectious disease(PNID)group(n=64)and the pulmonary infectious disease(PID)group(n=100)groups based onfinal diagnoses.Results:BALF mNGS increased the sensitivity rate by 60%compared to CMTs(81%vs.21%,p<0.05),whereas there was no significant difference in specificity(75%vs.98.4%,p>0.1).Among the patients with PID,bacteria were the most common cause of infection.Fungal infections occurred in 32%of patients,and Pneumocystis Yersini was most common.Patients with Tyrosine kinase inhibitors(TKIs)therapy possess longer overall survival(OS)than other anti-cancer agents,the difference between TKIs and immuno-checkpoint inhibitors(ICIs)was insignificant(median OS TKIs vs.ICIs vs.Anti-angiogenic vs.Chemo vs.Radiotherapy=76 vs.84 vs.61 vs.58 vs.60).Conclusions:our study indicates that BALF mNGS can add value by improving overall sensitivity in lung cancer patients with potential pulmonary infection,and was outstanding in identifying Pneumocystis infection.It could be able to help physicians adjust the follow-up treatment to avoid the abuse of antibiotics.展开更多

Methods: A total of 68 patients with COPD complicated by severe pneumonia who were treated in the hospital between November 2015 and April 2017 were collected, retrospectively analyzed and then divided into the group A (n=35) who received noninvasive positive pressure ventilation and the group B (n=33) who received lung lavage via fiber bronchoscope combined with non-invasive positive pressure ventilation. The differences in arterial blood gas and serum index levels were compared between the two groups before and after treatment. Results: Before treatment, there was no statistically significant difference in arterial blood gas index levels as well as serum contents of inflammatory mediators, stress hormones and myocardial enzyme spectrum indexes between the two groups. After treatment, arterial blood gas indexes PH and PaO2 levels of group B were higher than those of group A;serum inflammatory mediators HMGB1, PCT and hs-CRP contents were lower than those of group A;serum stress hormones Cor, AngⅠ and AngⅡcontents were lower than those of group A;serum myocardial enzyme spectrum indexesα-HBDH and cTn-Ⅰ contents were lower than those of group A. Conclusion: Lung lavage via fiber bronchoscope combined with non-invasive positive pressure ventilation can effectively optimize the arterial blood gas levels, reduce systemic inflammatory stress state and protect the myocardial function of patients with COPD complicated by severe pneumonia.展开更多

The aim of this study is to clarify the clinicopathological character...Background: The pleural lavage cytology (PLC) for non-small cell lung cancer (NSCLC) patients has been reported as a significant prognostic factor. The aim of this study is to clarify the clinicopathological characteristics of 5-year survivors of patients with positive PLC. Methods: Among 401 resected NSCLC patients, 30 (7.48%) patients with positive PLC were reviewed retrospectively. Results: Only 7 of 30 patients (23.3%) survived more than 5-year. There were no differences in age, gender, histology, pT status and pN status between 5-year survivors and non-survivors. On the other hand, the serum carcinoembryonic antigen (CEA) level was significantly higher in non-survivors. Among these 5-year survivors, 4 of 7 patients died of NSCLC recurrences more than 5 years after surgery. Remaining 3 patients survived without cancer recurrences. Interestingly these 3 survivors had not received any adjuvant therapies after surgery. Conclusion: None of the 5-year survivor with positive PLC had high serum CEA level. Furthermore 5-year survival did not equal to cure in NSCLC patients with positive PLC.展开更多
Objective:To analyze the detection results of pathogen nucleic acids for bronchoalveolar lavage fluid(BALF)of lung infection infants from Uighur nationality and Han nationality.Methods:A retrospective analysis was per...Objective:To analyze the detection results of pathogen nucleic acids for bronchoalveolar lavage fluid(BALF)of lung infection infants from Uighur nationality and Han nationality.Methods:A retrospective analysis was performed on the 318 infants with lung infection who were admitted to the hospital from April 2018 to April 2019.According to their nationality,they were divided into Uighur nationality group(190 cases)and Han nationality group(128 cases).The BALF specimens were collected to test pathogen nucleic acid.The distribution and positive rates of[respiratory syncytial virus(RSV),adenovirus(ADV),influenza virus A(IFA),influenza virus B(IFB),parainfluenza virus type 1(PIV I),parainfluenza virus type 2(PIV II),parainfluenza virus type 3(PIV III)],bacteria(Streptococcus pneumoniae,Haemophilus influenzae,staphylococcus aureus,Pseudomonas aeruginosa,klebsiella pneumoniae),Mycoplasma pneumoniae(MP)and Chlamydia pneumoniae(CP)in both groups were observed and compared.Results:The virus detection for RSV,ADV and PIV III were on the top three in BALF from the children in both groups.The total positive rate of virus examination in Uighur nationality group was higher than that in Han nationality group(P<0.05).BALF in both groups was mainly on Streptococcus pneumoniae.The total positive rate of bacteria,MP and detection rate of chlamydia were higher in Uighur nationality group were higher than those in Han nationality group(P<0.05).Conclusion:The pathogen nucleic acid examination for bronchoalveolar lavage fluid in infants with lung viral infection is in the majority,mainly on RSV virus infection.The positive rates of virus,bacteria,MP and CP of children in Uighur nationality are high than those in Han nationality.展开更多
Objective:To evaluate the efficacy of transbronchoscopic large volume lung lavage in the treatment of pneumoconiosis complicated with chronic obstructive pulmonary disease(COPD).Methods:The clinical data of 80 patient...Objective:To evaluate the efficacy of transbronchoscopic large volume lung lavage in the treatment of pneumoconiosis complicated with chronic obstructive pulmonary disease(COPD).Methods:The clinical data of 80 patients with pneumoconiosis complicated with COPD admitted to our hospital from June 2017 to January 2019 were retrospectively analyzed.40 patients in the control group were treated with conventional drugs and 40 patients in the observation group treated with conventional drugs plus transbronchoscopic large-volume lung lavage.Dyspnea score and healthy quality of life were compared between the two groups.Results:the scores of dyspnea in the observation group were significantly lower than those in the control group at 12,24 and 48 weeks after treatment,and the(SGRQ)scores of George’s respiratory problems questionnaire in the observation group were significantly lower than those in the control group at 12,24 and 48 weeks after treatment.Conclusion:Thetransbronchoscopic large volume of lung lavage has a significant effect on the treatment of pneumoconiosis patients with COPD,which can effectively reduce the degree of dyspnea and improve the quality of life.展开更多

The majority of bronchial cancer is diagnosed from small biopsy specimens and the diagnosis often based on cytological methods. Aims: In this study, we reevaluated cytologic specimens in order to determine the diagnostic reliability of pulmonary cytopathologic techniques performed in our department. Material and methods: In our center bronchial lavage/bronchoalveolar lavage (BL/BAL) specimens are obtained both before and after forceps biopsy (FB) and subsequently processed. Retrospective data from a period of 60 months were retrieved from the institutional files. Sensitivity, specificity, as well as accuracy of cytological tumor typing were determined using histopathology of FB as gold standard. Also, the diagnostic yield of BL/BAL before and after FB was determined. Results: 678 cases were retrieved from the institutional files. The sensitivity and specificity of cytology were 83.0% and 83.4%, respectively. By FB in 3.9% of cytologically diagnosed non-small cell lung carcinomas (NSCLC) a histological assignment to a NSCLC entity was not possible. Conclusions: Cytology is a reliable diagnostic tool in the diagnosis of lung malignancies. High diagnostic accuracy is achieved by a combination of BL/BAL before and after FB. The diagnostic yield of BL/BAL after FB was significantly higher than BL/BAL before FB. Subsequent tumor typing of cytologically diagnosed NSCLC was feasible in more than 95% of cases.展开更多

The ani...Acute pulmonary injury triggered by sodium chromate (Na 2CrO 4) and it′s recovery duration were evaluated by analysing the biochemical responses in bronchoalveolar lavage fluids(BALF) in Sprague Dawley rats. The animals of five groups that received single intratracheal instillations of Na 2CrO 4 suspension at doses of 0.008, 0.04, 0.20, 0.98 and 4.88 mg·kg -1 body wt respectively were killed at d 2 after treatment in the dose response experiment. The animals of seven groups that received the instillations of 0.98 mg·kg -1 Na 2CrO 4 were sacrificed at d 2, 4, 7, 10, 14, 21 and 28 separately after exposure in the time course experiment. The biochemical indicators examined in the BALF included N acetyl β D glucosaminidase (NAG), gamma glutamyl transferase (GGT), alkaline phosphatase (ALP), lactate dehydrogenase (LDH), angiotensin converting enzyme (ACE), α 1 antitripsin(A1AT), albumin (Alb) and lysozyme(LYS). Based on the present findings, it was found that no effect dose and threshold dose of Na 2CrO 4 were 0.008 and 0.04 mg·kg -1 respectively and that the majority of biochemical parameters in the BALF showed the dose dependent and significant increases at dosed of ≥0.20 mg·kg -1 . And at the severe intoxication dose ≥0.98 mg·kg -1 all parameter values could significantly be increased, which lasted about 1 week and the recovery duration needed at least 4 weeks. The present study indicates that ≥0.98 mg·kg -1 Na 2CrO 4 can result in acute lung injury including pulmonary cyto biomembrane damage, diffuse alveolitis and edema, and that NAG, LYS, ALP, Alb and ACE are probably the valuable and early sensitive indicators for evaluating the injury after exposure to lower dose Na 2CrO 4 (<0.98 mg·kg -1 ).展开更多

The results showed that the positive rates or both hEGF (58.1%) and β2-MG (62. 2%) in BALF of the affected side of lung cancer were higher than those of either the healthy side of lung cancer or benign lung disease (P<0.05), and also higher than that in serum.The level of BALF-hEGF was highest in squamous cell carcinoma and adenocarcinoma, and it was higher in stages Ⅲ, Ⅳ than in stages Ⅰ, Ⅱ. The combinative detection or hEGF and β2-MG in BALF and serum will he valuable to early diagnosis of lung cancer.展开更多

Current study is designed to analyse the radioprotective e...Radiation induced reactive oxygen/nitrogen species (ROS/RNS) are reported to cause lung injuries such as pneumonitis and fibrosis which may be fatal at times. Current study is designed to analyse the radioprotective efficacy of P. hexandrum active principles (G-002M) on lungs of mice exposed to high dose of gamma irradiation (7 Gy). Cellular profiles and inflammatory cell infiltrates of irradiated bronchoalveolar lavage fluid (BALF) have shown correlations with lung pathology. Cell counts were determined in BALF of control, 7 Gy radiation exposed and radiation with G-002M pretreated mice. ROS/Nitric Oxide (NO) production was measured by 2,7?dichlorodihydrofluorescein diacetate (DCF-DA) and diaminofluorescein diacetate (DAF-2DA) through microscopy and flow cytometry respectively. Immunostaining of inducible nitric oxide synthase (iNOS) in BALF cells and lung sections was also observed microscopically. iNOS ex- pression was observed in lungs by western blotting. BALF was also processed to estimate total protein, LDH, and phospholipids content. Catalase, reduced Glutathione (GSH), Glutathione reductase (GR) and lipid peroxidation were estimated in lung tissues. Pre-administration of G-002M significantly decreased radiation mediated neutrophils count in BALF of irradiated mice. ROS generation, iNOS expression, total protein, LDH and phospholipids were found less affected in G-002M pretreated group in comparison to radiation alone group. Radiation exposure to mice was found apparently leading to parenchymal fibrosis, an architectural distortion of the lung tissue with edema, infiltration of inflammatory blood cells with increased immunolabeling of iNOS. G-002M pretreatment significantly countered radiation mediated increased lipid peroxidation and decreased GR, catalase and GSH in mice. Current study demonstrates possible role of P. hexandrum (G-002M) in minimizing lung damage induced by radiation mediated ROS/RNS generation.展开更多

Methods: Twenty four cases of patients with rheumatic heart disease were en...Objective: To explore the role of polymorphonuclear neutrophils (PMN) activation in lung injury following cardiopulmonary bypass (CPB). Methods: Twenty four cases of patients with rheumatic heart disease were enrolled in this study to detect the situation of PMN activation and the degree of lung injury. Bronchoalveolar lavage fluid (BALF) and blood samples were collected just after anaesthesia and 4 h postoperatively. Related clinic data were recorded. Results: This cohort included 4 male and 20 female with age ranging from 29 to 69 years old, and body mass from 37 to 73 kg. Mean cardiopulmonary bypass time was (106.46±33.58) rain, mean cross clamp time was (77.58_+28.02) rain, and mean mechanical ventilation time was (24.17±30.90) h. Postoperative PaO2/FiO2 decreased significantly than that during preoperation (P=0.000). The postoperative WBC counts in peripheral blood and in BALF were both increased significantly than those during preoperation. And the postoperative rates of PMN in BALF were also increased significantly. Postoperative neutriphil elastase (NE) and myeloperoxidase (MPO) released from inflammatory ceils were increased significantly (P=0.000) both in plasma and BALF. The postoperative yon Willebrand factor (vWF) concentration in plasma was also increased significantly (P=0.000). The postoperative concentration of sLPI in plasma was decreased, whereas it was increased significantly in BALF. Furthermore, there was a negative correlation between NE and PaO2/FiO2 both in BALF and in plasma. A negative correlation was present between vWF in plasma and PaO2/FiO2. A positive correlation was found between sLPI in plasma and PaO2/FiO2. Conclusion: PMN is recruited and activated after CPB and released large quantity of proteases, and lead to lung injury, sLPI is an important protective factor against inflammatory injury.展开更多

Objective:This study aimed to examine the therapeutic effects of Bai-Ri-Ke syrup(BRK)on mice with acute lung injury(ALI).Methods:Fifty male C57BL/6 mice were equally divided into the control group,model group,dexameth...Objective:This study aimed to examine the therapeutic effects of Bai-Ri-Ke syrup(BRK)on mice with acute lung injury(ALI).Methods:Fifty male C57BL/6 mice were equally divided into the control group,model group,dexamethasone(DXM)group,Bai-Ri-Ke syrup(BRK)low-group,and BRK-high group,with six mice per group.An intratracheal injection of 5 mg/kg POLY(I:C)was used to construct an ALI mouse model.After a successful model construction,the mice in the DXM group were given[0.2 mg/10 g·d]dexamethasone sodium phosphate injection(1 mL:2 mg)on the following day via intraperitoneal injection.The mice in the BRK-low group were given 0.015 mL APS everyday by gavage,and the mice in the BRK-high group were given 0.030 mL APS everyday by gavage for three days.The wet to dry weight(W/D)ratio of the lungs was observed every day.Bronchoalveolar lavage fluids(BALFs)were collected from the left lungs to measure the BALF protein level and neutrophil count after 72 h of treatment.The IL-6,IL-1β,and TNF-αlevels in BALF were also measured.HE staining was done to observe the histopathological changes in the lungs.Results:The ALI mice in the BRK-high group had significantly increased W/D(P<0.01).ELISA results showed that the DXM group and BRK-high group had significantly decreased BALF protein content(P<0.01),neutrophil count(P<0.01),and IL-6,IL-1β,and TNF-αlevels(P<0.01).Hematoxylin and eosin(H&E)results showed that the DXM group and BRK-high group had alleviated alveolar tissue injury,edema,bleeding,and inflammation.Conclusions:The BRK can decrease the W/D,BALF protein content,neutrophil count,and TNF-α,IL-1β,and IL-6 levels and alleviate the histopathological changes in the lungs of ALI mice.展开更多

文摘Background:For patients with lung cancer,timely identification of new lung lesions as infectious or non-infectious,and accurate identification of pathogens is very important in improving OS of patients.As a new auxiliary examination,metagenomic next-generation sequencing(mNGS)is believed to be more accurate in diagnosing infectious diseases in patients without underlying diseases,compared with conventional microbial tests(CMTs).We designed this study tofind out whether mNGS has better performance in distinguishing infectious and non-infectious diseases in lung cancer patients using bronchoalveolar lavagefluid(BALF).Materials and Methods:This study was a real-world retrospective review based on electronic medical records of lung cancer patients with bronchoalveolar lavage(BAL)and BALF commercial mNGS testing as part of clinical care from 1 April 2019 through 30 April 2022 at The First Affiliated Hospital of Sun Yat-sen University.164 patients were included in this study.Patients were categorized into the pulmonary non-infectious disease(PNID)group(n=64)and the pulmonary infectious disease(PID)group(n=100)groups based onfinal diagnoses.Results:BALF mNGS increased the sensitivity rate by 60%compared to CMTs(81%vs.21%,p<0.05),whereas there was no significant difference in specificity(75%vs.98.4%,p>0.1).Among the patients with PID,bacteria were the most common cause of infection.Fungal infections occurred in 32%of patients,and Pneumocystis Yersini was most common.Patients with Tyrosine kinase inhibitors(TKIs)therapy possess longer overall survival(OS)than other anti-cancer agents,the difference between TKIs and immuno-checkpoint inhibitors(ICIs)was insignificant(median OS TKIs vs.ICIs vs.Anti-angiogenic vs.Chemo vs.Radiotherapy=76 vs.84 vs.61 vs.58 vs.60).Conclusions:our study indicates that BALF mNGS can add value by improving overall sensitivity in lung cancer patients with potential pulmonary infection,and was outstanding in identifying Pneumocystis infection.It could be able to help physicians adjust the follow-up treatment to avoid the abuse of antibiotics.
文摘Objective:To study the improving effect of sputum aspiration combined with bronchoalveolar lavage by fiber bronchoscope on the condition and inflammation in lung cancer patients with postoperative pulmonary infection.Methods:A total of 78 patients with lung cancer who received surgical treatment in our hospital between August 2013 and January 2015 and were with postoperative infection were selected as the research subjects and randomly divided into two groups, control group received mechanical ventilation therapy, observation group received bronchoalveolar lavage by bronchoscope combined with mechanical ventilation, and the inflammatory factor levels, RAAS system activity and respiratory mechanics indexes of two groups were determined after treatment.Results:3 d after treatment, serum hs-CRP, TNF-α, IL-8, PCT, renin, angiotensin II and aldosterone levels of observation group were lower than those of control group, and PIP, Raw and WOB values were lower than those of control group while Cdyn value was higher than that of control group.Conclusion:For lung cancer patients with postoperative pulmonary infection, bronchoalveolar lavage by bronchoscope combined with mechanical ventilation can optimize patients' condition, relieve systemic inflammatory response and improve respiratory function, and it has positive clinical significance.
文摘Objective: To discuss the effect of lung lavage via fiber bronchoscope combined with non-invasive positive pressure ventilation on the blood gas results and systemic state of patients with COPD complicated by severe pneumonia. Methods: A total of 68 patients with COPD complicated by severe pneumonia who were treated in the hospital between November 2015 and April 2017 were collected, retrospectively analyzed and then divided into the group A (n=35) who received noninvasive positive pressure ventilation and the group B (n=33) who received lung lavage via fiber bronchoscope combined with non-invasive positive pressure ventilation. The differences in arterial blood gas and serum index levels were compared between the two groups before and after treatment. Results: Before treatment, there was no statistically significant difference in arterial blood gas index levels as well as serum contents of inflammatory mediators, stress hormones and myocardial enzyme spectrum indexes between the two groups. After treatment, arterial blood gas indexes PH and PaO2 levels of group B were higher than those of group A;serum inflammatory mediators HMGB1, PCT and hs-CRP contents were lower than those of group A;serum stress hormones Cor, AngⅠ and AngⅡcontents were lower than those of group A;serum myocardial enzyme spectrum indexesα-HBDH and cTn-Ⅰ contents were lower than those of group A. Conclusion: Lung lavage via fiber bronchoscope combined with non-invasive positive pressure ventilation can effectively optimize the arterial blood gas levels, reduce systemic inflammatory stress state and protect the myocardial function of patients with COPD complicated by severe pneumonia.
文摘Background: The pleural lavage cytology (PLC) for non-small cell lung cancer (NSCLC) patients has been reported as a significant prognostic factor. The aim of this study is to clarify the clinicopathological characteristics of 5-year survivors of patients with positive PLC. Methods: Among 401 resected NSCLC patients, 30 (7.48%) patients with positive PLC were reviewed retrospectively. Results: Only 7 of 30 patients (23.3%) survived more than 5-year. There were no differences in age, gender, histology, pT status and pN status between 5-year survivors and non-survivors. On the other hand, the serum carcinoembryonic antigen (CEA) level was significantly higher in non-survivors. Among these 5-year survivors, 4 of 7 patients died of NSCLC recurrences more than 5 years after surgery. Remaining 3 patients survived without cancer recurrences. Interestingly these 3 survivors had not received any adjuvant therapies after surgery. Conclusion: None of the 5-year survivor with positive PLC had high serum CEA level. Furthermore 5-year survival did not equal to cure in NSCLC patients with positive PLC.
文摘Objective:To analyze the detection results of pathogen nucleic acids for bronchoalveolar lavage fluid(BALF)of lung infection infants from Uighur nationality and Han nationality.Methods:A retrospective analysis was performed on the 318 infants with lung infection who were admitted to the hospital from April 2018 to April 2019.According to their nationality,they were divided into Uighur nationality group(190 cases)and Han nationality group(128 cases).The BALF specimens were collected to test pathogen nucleic acid.The distribution and positive rates of[respiratory syncytial virus(RSV),adenovirus(ADV),influenza virus A(IFA),influenza virus B(IFB),parainfluenza virus type 1(PIV I),parainfluenza virus type 2(PIV II),parainfluenza virus type 3(PIV III)],bacteria(Streptococcus pneumoniae,Haemophilus influenzae,staphylococcus aureus,Pseudomonas aeruginosa,klebsiella pneumoniae),Mycoplasma pneumoniae(MP)and Chlamydia pneumoniae(CP)in both groups were observed and compared.Results:The virus detection for RSV,ADV and PIV III were on the top three in BALF from the children in both groups.The total positive rate of virus examination in Uighur nationality group was higher than that in Han nationality group(P<0.05).BALF in both groups was mainly on Streptococcus pneumoniae.The total positive rate of bacteria,MP and detection rate of chlamydia were higher in Uighur nationality group were higher than those in Han nationality group(P<0.05).Conclusion:The pathogen nucleic acid examination for bronchoalveolar lavage fluid in infants with lung viral infection is in the majority,mainly on RSV virus infection.The positive rates of virus,bacteria,MP and CP of children in Uighur nationality are high than those in Han nationality.

文摘Objective:To evaluate the efficacy of transbronchoscopic large volume lung lavage in the treatment of pneumoconiosis complicated with chronic obstructive pulmonary disease(COPD).Methods:The clinical data of 80 patients with pneumoconiosis complicated with COPD admitted to our hospital from June 2017 to January 2019 were retrospectively analyzed.40 patients in the control group were treated with conventional drugs and 40 patients in the observation group treated with conventional drugs plus transbronchoscopic large-volume lung lavage.Dyspnea score and healthy quality of life were compared between the two groups.Results:the scores of dyspnea in the observation group were significantly lower than those in the control group at 12,24 and 48 weeks after treatment,and the(SGRQ)scores of George’s respiratory problems questionnaire in the observation group were significantly lower than those in the control group at 12,24 and 48 weeks after treatment.Conclusion:Thetransbronchoscopic large volume of lung lavage has a significant effect on the treatment of pneumoconiosis patients with COPD,which can effectively reduce the degree of dyspnea and improve the quality of life.
文摘Background: Due to new therapeutic options in thoracic oncology, the pathological diagnosis of bronchial carcinoma has become more challenging. The majority of bronchial cancer is diagnosed from small biopsy specimens and the diagnosis often based on cytological methods. Aims: In this study, we reevaluated cytologic specimens in order to determine the diagnostic reliability of pulmonary cytopathologic techniques performed in our department. Material and methods: In our center bronchial lavage/bronchoalveolar lavage (BL/BAL) specimens are obtained both before and after forceps biopsy (FB) and subsequently processed. Retrospective data from a period of 60 months were retrieved from the institutional files. Sensitivity, specificity, as well as accuracy of cytological tumor typing were determined using histopathology of FB as gold standard. Also, the diagnostic yield of BL/BAL before and after FB was determined. Results: 678 cases were retrieved from the institutional files. The sensitivity and specificity of cytology were 83.0% and 83.4%, respectively. By FB in 3.9% of cytologically diagnosed non-small cell lung carcinomas (NSCLC) a histological assignment to a NSCLC entity was not possible. Conclusions: Cytology is a reliable diagnostic tool in the diagnosis of lung malignancies. High diagnostic accuracy is achieved by a combination of BL/BAL before and after FB. The diagnostic yield of BL/BAL after FB was significantly higher than BL/BAL before FB. Subsequent tumor typing of cytologically diagnosed NSCLC was feasible in more than 95% of cases.

文摘Acute pulmonary injury triggered by sodium chromate (Na 2CrO 4) and it′s recovery duration were evaluated by analysing the biochemical responses in bronchoalveolar lavage fluids(BALF) in Sprague Dawley rats. The animals of five groups that received single intratracheal instillations of Na 2CrO 4 suspension at doses of 0.008, 0.04, 0.20, 0.98 and 4.88 mg·kg -1 body wt respectively were killed at d 2 after treatment in the dose response experiment. The animals of seven groups that received the instillations of 0.98 mg·kg -1 Na 2CrO 4 were sacrificed at d 2, 4, 7, 10, 14, 21 and 28 separately after exposure in the time course experiment. The biochemical indicators examined in the BALF included N acetyl β D glucosaminidase (NAG), gamma glutamyl transferase (GGT), alkaline phosphatase (ALP), lactate dehydrogenase (LDH), angiotensin converting enzyme (ACE), α 1 antitripsin(A1AT), albumin (Alb) and lysozyme(LYS). Based on the present findings, it was found that no effect dose and threshold dose of Na 2CrO 4 were 0.008 and 0.04 mg·kg -1 respectively and that the majority of biochemical parameters in the BALF showed the dose dependent and significant increases at dosed of ≥0.20 mg·kg -1 . And at the severe intoxication dose ≥0.98 mg·kg -1 all parameter values could significantly be increased, which lasted about 1 week and the recovery duration needed at least 4 weeks. The present study indicates that ≥0.98 mg·kg -1 Na 2CrO 4 can result in acute lung injury including pulmonary cyto biomembrane damage, diffuse alveolitis and edema, and that NAG, LYS, ALP, Alb and ACE are probably the valuable and early sensitive indicators for evaluating the injury after exposure to lower dose Na 2CrO 4 (<0.98 mg·kg -1 ).
文摘Human epidermal growth factor (hEGF) and β2-Microglohulin(β2-MG) in bronchoalveolar lavage fluid (BALF) and serum were detected in 74 patients with lung cancer and 76 patients with benign lung disease. The results showed that the positive rates or both hEGF (58.1%) and β2-MG (62. 2%) in BALF of the affected side of lung cancer were higher than those of either the healthy side of lung cancer or benign lung disease (P<0.05), and also higher than that in serum.The level of BALF-hEGF was highest in squamous cell carcinoma and adenocarcinoma, and it was higher in stages Ⅲ, Ⅳ than in stages Ⅰ, Ⅱ. The combinative detection or hEGF and β2-MG in BALF and serum will he valuable to early diagnosis of lung cancer.
文摘Radiation induced reactive oxygen/nitrogen species (ROS/RNS) are reported to cause lung injuries such as pneumonitis and fibrosis which may be fatal at times. Current study is designed to analyse the radioprotective efficacy of P. hexandrum active principles (G-002M) on lungs of mice exposed to high dose of gamma irradiation (7 Gy). Cellular profiles and inflammatory cell infiltrates of irradiated bronchoalveolar lavage fluid (BALF) have shown correlations with lung pathology. Cell counts were determined in BALF of control, 7 Gy radiation exposed and radiation with G-002M pretreated mice. ROS/Nitric Oxide (NO) production was measured by 2,7?dichlorodihydrofluorescein diacetate (DCF-DA) and diaminofluorescein diacetate (DAF-2DA) through microscopy and flow cytometry respectively. Immunostaining of inducible nitric oxide synthase (iNOS) in BALF cells and lung sections was also observed microscopically. iNOS ex- pression was observed in lungs by western blotting. BALF was also processed to estimate total protein, LDH, and phospholipids content. Catalase, reduced Glutathione (GSH), Glutathione reductase (GR) and lipid peroxidation were estimated in lung tissues. Pre-administration of G-002M significantly decreased radiation mediated neutrophils count in BALF of irradiated mice. ROS generation, iNOS expression, total protein, LDH and phospholipids were found less affected in G-002M pretreated group in comparison to radiation alone group. Radiation exposure to mice was found apparently leading to parenchymal fibrosis, an architectural distortion of the lung tissue with edema, infiltration of inflammatory blood cells with increased immunolabeling of iNOS. G-002M pretreatment significantly countered radiation mediated increased lipid peroxidation and decreased GR, catalase and GSH in mice. Current study demonstrates possible role of P. hexandrum (G-002M) in minimizing lung damage induced by radiation mediated ROS/RNS generation.

文摘Objective: To explore the role of polymorphonuclear neutrophils (PMN) activation in lung injury following cardiopulmonary bypass (CPB). Methods: Twenty four cases of patients with rheumatic heart disease were enrolled in this study to detect the situation of PMN activation and the degree of lung injury. Bronchoalveolar lavage fluid (BALF) and blood samples were collected just after anaesthesia and 4 h postoperatively. Related clinic data were recorded. Results: This cohort included 4 male and 20 female with age ranging from 29 to 69 years old, and body mass from 37 to 73 kg. Mean cardiopulmonary bypass time was (106.46±33.58) rain, mean cross clamp time was (77.58_+28.02) rain, and mean mechanical ventilation time was (24.17±30.90) h. Postoperative PaO2/FiO2 decreased significantly than that during preoperation (P=0.000). The postoperative WBC counts in peripheral blood and in BALF were both increased significantly than those during preoperation. And the postoperative rates of PMN in BALF were also increased significantly. Postoperative neutriphil elastase (NE) and myeloperoxidase (MPO) released from inflammatory ceils were increased significantly (P=0.000) both in plasma and BALF. The postoperative yon Willebrand factor (vWF) concentration in plasma was also increased significantly (P=0.000). The postoperative concentration of sLPI in plasma was decreased, whereas it was increased significantly in BALF. Furthermore, there was a negative correlation between NE and PaO2/FiO2 both in BALF and in plasma. A negative correlation was present between vWF in plasma and PaO2/FiO2. A positive correlation was found between sLPI in plasma and PaO2/FiO2. Conclusion: PMN is recruited and activated after CPB and released large quantity of proteases, and lead to lung injury, sLPI is an important protective factor against inflammatory injury.

文摘Objective:This study aimed to examine the therapeutic effects of Bai-Ri-Ke syrup(BRK)on mice with acute lung injury(ALI).Methods:Fifty male C57BL/6 mice were equally divided into the control group,model group,dexamethasone(DXM)group,Bai-Ri-Ke syrup(BRK)low-group,and BRK-high group,with six mice per group.An intratracheal injection of 5 mg/kg POLY(I:C)was used to construct an ALI mouse model.After a successful model construction,the mice in the DXM group were given[0.2 mg/10 g·d]dexamethasone sodium phosphate injection(1 mL:2 mg)on the following day via intraperitoneal injection.The mice in the BRK-low group were given 0.015 mL APS everyday by gavage,and the mice in the BRK-high group were given 0.030 mL APS everyday by gavage for three days.The wet to dry weight(W/D)ratio of the lungs was observed every day.Bronchoalveolar lavage fluids(BALFs)were collected from the left lungs to measure the BALF protein level and neutrophil count after 72 h of treatment.The IL-6,IL-1β,and TNF-αlevels in BALF were also measured.HE staining was done to observe the histopathological changes in the lungs.Results:The ALI mice in the BRK-high group had significantly increased W/D(P<0.01).ELISA results showed that the DXM group and BRK-high group had significantly decreased BALF protein content(P<0.01),neutrophil count(P<0.01),and IL-6,IL-1β,and TNF-αlevels(P<0.01).Hematoxylin and eosin(H&E)results showed that the DXM group and BRK-high group had alleviated alveolar tissue injury,edema,bleeding,and inflammation.Conclusions:The BRK can decrease the W/D,BALF protein content,neutrophil count,and TNF-α,IL-1β,and IL-6 levels and alleviate the histopathological changes in the lungs of ALI mice.
