Objective:To explore the characteristics of Xiao Chai Hu(XCH)decoction in randomized controlled trials(RCTs)using a bibliometric method.Methods:Seven databases including PubMed,Web of Science,Embase,Cochrane library,China National Knowledge Infrastructure(CNKI),Chinese Scientific Journal Database(VIP)and Wanfang database were retrieved from inception to May 27,2022.In the study,XCH decoction(or modified)used alone or combined with conventional Western medicine as an intervention measure was included.The basic characteristics,funding support,relevant diseases,intervention methods,and adverse events(AEs)were analyzed.Data analysis was performed using SPSS 26.0 software.Results:A total of 813 RCTs were included,published from 1989 to 2022.There was only one Englishlanguage literature with the recent impact factor of 5.374.There were 147 studies were from Chineselanguage core journals,with the highest impact factor of 2.414.Only 6.15%of the literatures mentioned funding support.96.31%of the included literature reported the statistical significance of using XCH decoction.The diseases treated mainly included chronic hepatitis B(9.35%),cough variant asthma(5.66%),dizziness(5.54%),bile reflux gastritis(4.43%),and fever(4.18%).However,the overall research design of the included literature was poor,and large sample size,multicenter RCTs are needed.The incidence of AEs of XCH decoction alone was 8.86%,which was significantly lower than that of conventional Western medicine treatment.The combination of XCH decoction and conventional Western medicine treatment could reduce the incidence of AEs,and no serious adverse event was reported.Conclusion:Although the included studies show that XCH decoction is widely used,and has good efficacy and few AEs.Due to the low quality of the included RCTs,there may be some bias,and its rational use based on the specific conditions is recommended in clinical practice. 展开更多

We have previously shown that the active fractions of Xiao-Xu-Ming decoction attenuate cerebral ischemic injury. However, the global protein profile and signaling conduction pathways regulated by Xiao-Xu-Ming decoction are still unclear. This study established a two-vessel occlusion rat model by bilateral common carotid artery occlusion. Rats were intragastrically administered 50 or 150 mg/kg Xiao-Xu-Ming decoction for 4 consecutive weeks. Learning and memory abilities were measured with Morris water maze. Motor ability was detected with prehensile test. Coordination ability was examined using the inclined screen test. Neuronal plasticity was observed by immunofluorescent staining. Differentially expressed proteins of rat hippocampus were analyzed by label-free quantitative proteomics. Real time-polymerase chain reaction and western blot assay were used to identify the changes in proteins. Results showed that Xiao-Xu-Ming decoction dramatically alleviated learning and memory deficits, and motor and coordination dysfunction, and increased the expression of microtubule-associated protein 2. Xiao-Xu-Ming decoction extract remarkably decreased 13 upregulated proteins and increased 39 downregulated proteins. The regulated proteins were mainly involved in oxidation reduction process, intracellular signaling cascade process, and protein catabolic process. The signaling pathways were mainly involved in ubiquitin mediated proteolysis and the phosphatidylinositol signaling system. Furthermore, there was an interaction among Rab2 a, Ptpn1, Ppm1 e, Cdk18, Gorasp2, Eps15, Capza2, Syngap1 and Mt-nd1. Protein analyses confirmed the changes in expression of MTND1. The current findings provide new insights into the molecular mechanisms of Xiao-Xu-Ming decoction extract's effects on chronic cerebral hypoperfusion. 展开更多

OBJECTIVE Xiao-xu-ming decoction(XXMD),a well-known traditional Chinese herbal prescription,has been widely used to treat stroke.It is recorded in″Bei Ji Qian Jin Yao Fang″written by Si-miao Sun of the Chinese ancient Tang Dynasty.In our previous study,the active fraction of XXMD(XXM)against cerebral ischemia has been prepared by modern separation and purification techniques.This study was to investigate XXM against lipopolysaccaride(LPS)-induced neuroinflammation in mice.METHODS LPS is an endotoxin from the outer membrane of Gram-negative bacteria that activates inflammation.XXM was pre-treated in BALB/C mice followed by injected intraperitoneally with LPS(5 mg·kg-1).The effects of XXM on LPS-induced pro-inflammatory factors and proteins were measured by ELISA,Western blot,and immunofluorescence in vivo.RESULTS Mice treated with XXM showed significantly decreased proinflammatory factors level,including IL-1β(P<0.01),IL-6(P<0.01),TNF-α(P<0.05),and MCP-1(P<0.01).Furthermore,XXM also significantly inhibited the inflammatory pathway proteins expression induced by LPS,including TLR4,MyD 88,and COX-2.CONCLUSION XXM possesses anti-neuroinflammation in mice and might be a promising therapeutic agent for stroke. 展开更多

OBJECTIVE Using bioinformatics methods,to establish Xiao-Xu-Ming decoction(XX.MD) "compound-vasoconstriction G Protein-Coupled Receptors(GPCR) targets" network,and analyze the vasoconstriction regulatory effective components and the potential targets of XXMD.METHODS Ac.cording to the XXMD herb sources,we retrieved the chemical structures from the national scientific da.ta sharing platform for population and health pharmaceutical information center,TCMSP database and the latest research literature.The chemical molecular library was established after class prediction and screening for medicinal and metabolic properties.Five kinds of vasoconstriction GPCR crystal structure including 5-HT receptors(5-HT1 AR,5-HT1 BR),AT1 R,β2-AR,hUTR and ETB were retrieved from Bank Pro.tein Data Bank database or homology modeling using Discovery Studio 4.1 built-in modeling tools.After virtual screening by Libdock molecular docking,the highest rated 50 compounds of each target were col.lected and analyzed.The collected data were further used to construct and analyze the network.RE.SULTS 859 single compound structures information in XXMD were generalized following the screen.ing of obtained 2043 compounds.The complicated compound-vasoconstriction GPCR targets network of XXMD was then constructed and analyzed by molecular docking with the above five kinds of GPCR target receptors.Most of the chemical composition effects were associated with different vasoconstric.tion GPCR targets,while a few effective components can be applied to multiple GPCR targets at the same time,therefore forming synergies.CONCLUSION Vasorelaxant effects of XXMD may not only result from the collaborative interaction between a variety of active ingredients in Chinese medicine and multi.ple targets,but also from the interaction between some effective component and multiple targets. 展开更多

OBJECTIVE Vascular dementia(VD) refers to a progressive decline in memory and cognitive function caused by chronic cerebral ischemia.2-Vessels occlusion(2-VO) has been widely used as a model of VD.Xiao-Xu-Ming decoction,a well-known traditional Chinese medicine prescrip.tion,has been widely used to treat stroke and sequelae of stroke.The present study was to investigate the mechanism of Xiao-Xu-Ming decoction(XXM) against chronic cerebral ischemia injury in rats.METHODS After XXM treatment,rats were performed a memory testing with Morris water maze and motor ability testing using prehensile test and inclined screen test.Neuronal plasticity was observed by immunofluorescent staining with MAP2 antibody.Differentially expressed proteins of rat hippocampus were analyzed by Label-free quantitative proteomics.RESULTS XXM significantly alleviated 2-VOinduced learning and memory deficits,motor ability dysfunction,and neuronal plasticity injury in rats.The mechanism might be involved in up-regulation of 39 proteins and down-regulation of 13 proteins in the hippocampus of rats after XXM treatment vs 2-VO group rats.Gene ontology and pathway analysis showed that the regulated proteins are mainly involved in oxidation reduction process,intracellular signaling cascade process,and protein catabolic process,etc.The signal pathways are mainly involved in ubiquitin mediated proteolysis and phosphatidylinositol signaling system.CONCLUSION Current findings provide new insights into the molecular mechanisms of XXM on chronic cerebral ischemia. 展开更多

Human intestinal bacteria play an important role in the metabolism of herbal medicines, leading to the variations in their pharmacological profile. The present study aimed to investigate the metabolism of Xiao-Cheng-Qi decoction(XCQD) by human intestinal bacteria and to discover active component combination(ACC) contributing to the anti-inflammatory activity of XCQD. The water extract of XCQD was anaerobically incubated with human intestinal bacteria suspensions for 48 h at 37 °C. A liquid chromatography-hybrid quadrupole time-of-flight mass spectrometry(LC-Q-TOF/MS) method was performed for identification of the metabolites. In addition, the anti-inflammatory effects of XCQD and biotransformed XCQD(XCQD-BT) were evaluated in vitro with cytokines in RAW264.7 cells induced by lipopolysaccharide(LPS). A total of 51 compounds were identified in XCQD and XCQD-BT. Among them, 20 metabolites were proven to be transformed by human intestinal bacteria. Significantly, a combination of 14 compounds was identified as ACC from XCQD-BT, which was as effective as XCQD in cell models of inflammation. In conclusion, this study provided an applicable method, based on intestinal bacterial metabolism, for identifying combinatory compounds responsible for a certain pharmacological activity of herbal medicines. 展开更多

目的:观察中医序贯疗法(中药+火针)联合口服异维A酸软胶囊治疗中重度痤疮的临床疗效。方法:选取2022年9月-2022年11月笔者医院中重度痤疮患者78例,随机分为3组,每组26例,治疗组在不同阶段给予中医序贯疗法(自拟消痤汤+火针)联合口服异维... 目的:观察中医序贯疗法(中药+火针)联合口服异维A酸软胶囊治疗中重度痤疮的临床疗效。方法:选取2022年9月-2022年11月笔者医院中重度痤疮患者78例,随机分为3组,每组26例,治疗组在不同阶段给予中医序贯疗法(自拟消痤汤+火针)联合口服异维A酸软胶囊治疗,对照1组给予消痤汤联合口服异维A酸软胶囊治疗,对照2组给予消痤汤+火针治疗。治疗组火针疗法于服用异维A酸软胶囊2周后进行,每2周1次。三组均连续治疗3个月。比较各组临床疗效,对比皮损消退时间、GAGS评分、Skindex-16评分、不良反应及复发率。结果:治疗结束后,治疗组总有效率为96.15%,对照组分别为69.23%、65.38%,治疗组疗效高于对照组(P<0.05)。治疗组皮损消退时间、GAGS评分均低于对照组(P<0.05),治疗组Skindex-16评分高于对照组(P<0.05),各组不良反应差异无统计学意义,治疗组复发率3.85%,低于对照组30.76%和34.61%(P<0.05)。结论:中医序贯疗法联合异维A酸治疗中重度痤疮,可明显提升疗效,降低痤疮复发率,改善患者生活质量,值得临床推广应用。 展开更多

普济消毒饮古代文献考证与分析

搜集有关普济消毒饮的中医古籍文献,采用文献计量学方法对普济消毒饮的历史源流、药物组成、组方原理、剂量炮制、主治病证、用法等进行统计分析。经筛选得到普济消毒饮相关古籍文献235条,涉及中医古籍139部。普济消毒饮源于《东垣试效... 搜集有关普济消毒饮的中医古籍文献,采用文献计量学方法对普济消毒饮的历史源流、药物组成、组方原理、剂量炮制、主治病证、用法等进行统计分析。经筛选得到普济消毒饮相关古籍文献235条,涉及中医古籍139部。普济消毒饮源于《东垣试效方》,由黄芩、黄连、人参、陈皮、玄参、甘草、连翘、牛蒡子、板蓝根、马勃、僵蚕、升麻、柴胡、桔梗14味中药组成,主要功效为清热解毒、疏风散邪,主治为上焦热盛所致的大头瘟等疾病。在涉及药味炮制的76条文献中,以甘草最多,多为生(44/45);其次为僵蚕,多为炒(38/44);黄连、黄芩多为酒炒(28/40、24/37);用法上以煮散剂为主,自明末剂型逐渐转变为汤剂。通过数据挖掘整理、系统分析记载普济消毒饮的中医古籍文献,为古代名方的临床应用和现代制剂研发提供参考。 展开更多

目的探讨方氏头皮针立联合小续命汤治疗脑卒中后遗症的临床价值。方法选取2020年10月至2022年10月瑞金市中医院收治的100例脑卒中后遗症患者作为研究对象,按照随机数字表法分为对照组与观察组,每组50例。对照组施以传统毫针治疗,观察组... 目的探讨方氏头皮针立联合小续命汤治疗脑卒中后遗症的临床价值。方法选取2020年10月至2022年10月瑞金市中医院收治的100例脑卒中后遗症患者作为研究对象,按照随机数字表法分为对照组与观察组,每组50例。对照组施以传统毫针治疗,观察组施以方氏头皮针合小续命汤治疗,比较两组治疗前后中医证候积分、感觉障碍情况(Fugl-Meyer运动功能量表、Lindmark平衡功能量表评分)、日常生活活动能力[Barthel指数(BI)、功能独立性评定量表(FIM)评分]、生存质量[脑卒中生存质量量表(SSQOL)评分]。结果治疗后,两组口舌斜歪、半身不遂、头晕目眩、言语謇涩或不语、便干便秘积分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组Fugl-Meyer、Lindmark评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);治疗后,两组BI、FIM、SSQOL评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论方氏头皮针联合小续命汤治疗脑卒中后遗症,能有效改善患者的临床症状和肢体感觉障碍,有利于提高患者的肢体平衡功能和生存质量。 展开更多

【目的】探讨小柴胡汤合香砂六君子汤对肝郁脾虚型慢性萎缩性胃炎(CAG)患者胃黏膜病理积分改变与胃肠激素的影响。【方法】将156例肝郁脾虚型CAG患者随机分为对照组和观察组,每组各78例。对照组给予常规西医治疗,观察组在对照组的基础... 【目的】探讨小柴胡汤合香砂六君子汤对肝郁脾虚型慢性萎缩性胃炎(CAG)患者胃黏膜病理积分改变与胃肠激素的影响。【方法】将156例肝郁脾虚型CAG患者随机分为对照组和观察组,每组各78例。对照组给予常规西医治疗,观察组在对照组的基础上给予小柴胡汤合香砂六君子汤治疗,疗程为4周。观察2组患者治疗前后中医证候积分(胃脘胀痛、纳差便溏、四肢乏力、嗳气泛酸)、胃黏膜病理积分、胃肠激素[胃动素(MTL)、胃泌素(GAS)]水平的变化情况,比较2组患者的幽门螺旋杆菌(Hp)转阴率,评价2组患者的临床疗效和安全性。【结果】(1)治疗4周后,观察组的总有效率为93.59%(73/78),显著高于对照组的82.05%(64/78),组间比较,差异有统计学意义(P<0.05)。(2)治疗后,2组患者的胃脘胀痛、纳差便溏、四肢乏力、嗳气泛酸等各项中医证候积分均较治疗前明显降低(P<0.05),且观察组的降低幅度均明显优于对照组(P<0.05)。(3)治疗后,2组患者的胃黏膜病理积分均较治疗前明显降低(P<0.05),且观察组的降低幅度明显优于对照组(P<0.01)。(4)治疗4周后,观察组的Hp转阴率为91.03%(71/78),明显高于对照组的75.64%(59/78),组间比较,差异有统计学意义(P<0.05)。(5)治疗后,2组患者的血清GAS水平均较治疗前明显降低(P<0.05),血清MTL水平均较治疗前明显升高(P<0.05);且观察组对血清GAS水平的降低幅度及对血清MTL水平的升高幅度均明显优于对照组(P<0.05)。(6)2组患者用药期间的血、尿、大便常规以及肾、肝功能和心电图等安全性指标均无明显异常,也均未出现头晕、皮疹、胸闷等不良反应,具有较高的安全性。【结论】小柴胡汤合香砂六君子汤对肝郁脾虚型GAS的治疗效果显著,可有效减轻患者临床症状与胃黏膜病理改变,促进Hp转阴,其作用机制可能与调节胃肠激素水平有关。 展开更多

【目的】通过数据挖掘方法总结当代岭南地区名中医治疗糖尿病的组方用药规律。【方法】检索中国知网(CNKI)、万方(Wanfang)和维普(VIP)数据库中当代岭南名中医治疗糖尿病的文献,并查阅当代岭南名中医的经验集、医案集,筛选岭南名中医治... 【目的】通过数据挖掘方法总结当代岭南地区名中医治疗糖尿病的组方用药规律。【方法】检索中国知网(CNKI)、万方(Wanfang)和维普(VIP)数据库中当代岭南名中医治疗糖尿病的文献,并查阅当代岭南名中医的经验集、医案集,筛选岭南名中医治疗糖尿病的相关医案。将医案中治疗糖尿病的中药处方资料录入Excel建立数据库,对中药的性味归经、分类和中医证型等进行频数统计,并采用IBM SPSS Modeler 18.0和SPSS Statistics 26.0进行关联规则分析和聚类分析。【结果】共纳入包括邓铁涛、刘仕昌、熊曼琪、李赛美、朱章志、刘敏、范冠杰等24位当代岭南地区名中医辨治糖尿病的医案资料,筛选得到62篇医案,101首处方,涉及210味中药。出现频次≥15次的常用中药有炙甘草、黄芪、柴胡、葛根、茯苓、白术、党参、黄芩等29味,中药功效分类的类别以补虚药、清热药、祛风湿药为主。证型主要为脾肾两虚证、肝郁脾虚证,并涉及气虚、阴虚、阳虚、寒湿、痰湿、湿热等证素。关联规则分析得到2~4味的核心药物组合28个,聚类分析得到5个新处方组合,主要为小柴胡汤、附子理中汤、玉泉丸合增液汤、左归丸等经典方剂的化裁方。【结论】当代岭南地区名中医治疗糖尿病主要以健脾温肾、疏肝利湿化浊为法,具有较鲜明的地域特色,可为岭南地区辨治糖尿病提供参考。 展开更多

小建中汤加味治疗脾胃虚寒型中枢介导的腹痛综合征患者的效果

目的观察小建中汤加味治疗脾胃虚寒型中枢介导的腹痛综合征(centrally mediated abdominal pain syndrome,CAPS)的临床效果。方法选取宝鸡市陈仓医院2021年9月至2023年8月收治的68例脾胃虚寒型CAPS患者进行随机对照试验。采用随机数字... 目的观察小建中汤加味治疗脾胃虚寒型中枢介导的腹痛综合征(centrally mediated abdominal pain syndrome,CAPS)的临床效果。方法选取宝鸡市陈仓医院2021年9月至2023年8月收治的68例脾胃虚寒型CAPS患者进行随机对照试验。采用随机数字表法将其分为对照组和观察组,各34例。对照组男18例,女6例,年龄(49.97±9.35)岁,病程6个月~8年;观察组男15例,女19例,年龄(51.68±11.83)岁,病程6个月~10年。对照组予以调节肠功能、解痉、益生菌调节肠道菌群等常规临床治疗,并加用多塞平抗焦虑治疗。观察组给予小建中汤加味治疗。比较两组疼痛量表评分、汉密顿焦虑量表(Hamilton Anxiety Scale,HAMA)评分、生活质量、治疗效果及用药不良反应。采用t检验、χ^(2)检验进行统计分析。结果治疗4周后,观察组疼痛量表评分、HAMA评分均低于对照组[(2.47±0.83)分比(4.03±1.00)分、(6.26±2.26)分比(9.21±2.19)分,均P<0.05]。观察组有效率高于对照组[94.12%(32/34)比73.53%(25/34),P=0.021]。观察组不良反应发生率低于对照组[17.65%(6/34)比76.47%(26/34),P<0.05]。观察组生活质量评分高于对照组[(88.94±1.59)分比(79.56±2.05)分,P<0.05]。结论小建中汤加味治疗脾胃虚寒型CAPS患者能有效减轻其症状,改善患者生活质量,总体疗效更好,并且不良反应发生率较低。 展开更多

Xiao-Xu-Ming decoction(XXMD) was a traditional Chinese prescription and first recorded in "Bei Ji Qian Jin Yao Fang".It has been widely used to treat theoplegia and the sequel of theoplegia in China.In the present work,high-performance liquid chromatography coupled with high resolution mass spectrometry(HPLC-HRMS) combined with the mass spectral tree similarity filter technique(MTSF)was used to rapidly discover and identify the compounds of the active fraction of XXMD.A total of 3362 compounds were automatically detected by HPLC-HRMS,and final 68 compounds were identified in the active fraction of XXMD.including 14 templated compounds(reference compounds),50 related compounds fished by MTSF technique,and 4 unrelated compounds identified by manual method.This study successfully applied MTSF technology for the first time to discover and identify the components of Chinese prescription.The results demonstrated that MTSF technique should be useful to the discovery and identification of compounds in Chinese prescription.This study also proved that MTSF can be applied to the targeted phytochemical separation. 展开更多
