Throughout the world, 287 cases of Rhupus have been described. We report two new observations of two patients who presented predominantly distal erosive polyarthritis with positive anti-Sm antibodies in one case and SmRNP in the other case. Observations: Case 1: 37 years old patient, with a recent diagnosis of pulmonary tuberculosis. She has since 8 months an inflammatory, bilaterally and symmetrical polyarthralgia without deformation or ankylosing synovitis, associated with malar erythema without other abnormalities. Immunological tests showed: positive Rheumatoid factor at 158 IU/ml, positive Anti-CCP at 550 IU/ml, and positivity of antinuclear at 1/1280 nuclear fluorescence with a strong presence of anti-Sm >8 IU/ml. The diagnosis of rhupus was concluded, without serious visceral involvement. Case 2: A 28-year-old patient, married with 3 children, with bilateral, symmetrical, deforming and chronic polyarthritis affecting large and small joints, which had been evolving for over 5 years without cutaneous abnormality associated. Paraclinical investigations showed: a biological inflammatory syndrome. Immunology was positive, with rheumatoid factors at 78 IU/ml, anti-CCP at 561 IU/ml, antinuclear antibodies at 1/1280 with positive anti-SmRNP and anti-SSA/Ro52, and a positive direct Coombs test. Joint ultrasound revealed tenosynovitis of the extensors and common flexors of the fingers, erosions and synovitis of multiple PPIs. The diagnosis of rhupus was based on the presence of 10 ACR criteria for RA and 8 ACR/EULAR 2019 criteria for SLE. Conclusion: Rheumatoid arthritis is a rare autoimmune disease combining features of both systemic lupus erythematosus and rheumatoid arthritis in the same patient, often sequentially. Despite a growing number of case reports and series, a consensus on the classification of SLE arthritis is still lacking, and diagnostic criteria for rhupus do not exist. These cases of rhupus must be recognized, as the vital and/or functional prognosis may be different from SLE alone or isolated RA.展开更多

One of the important components of the turbine is the connecting arm, which can bring about a significant effect on the pressure distribution along the span of the turbine blade, herein we call it 3D effect. However, so far the effect is rarely reported in the research, moreover, in numerical simulation. In the present study, a 3D numerical model of the turbine with the connecting arm was developed by using FLUENT software compiling the UDF(User Defined Function) command. The simulation results show that the pressure distribution along the span of blade with the connecting arm model is significantly different from those without the connecting arm. To facilitate the validation of numerical model, the laboratory experiment has been carried out by using three different types of NACA aerofoil connecting arm and circle section connecting arm. And results show that the turbine with NACA0012 connecting arm has the best start-up performance which is 0.346 m/s and the peak point of power conversion coefficient is around 0.33. A further study has been performed and a conclusion is drawn that the aerofoil and thickness of connecting arm are the most important factors on the power conversion coefficient of the vertical axis tidal current turbine.展开更多

A large variety of segmentation methods has been developed;among them, multidimensional histogram methods have been investigated but their implementation stays difficult due to the big size of histograms. We present an original method for segmenting n-D (where n is the number of components in image) images or multidimensional images in an unsupervised way using a fuzzy neighbourhood model. It is based on the hierarchical analysis of full n-D compact histograms integrating a fuzzy connected components labelling algorithm that we have realized in this work. Each peak of the histo- gram constitutes a class kernel, as soon as it encloses a number of pixels greater than or equal to a secondary arbitrary threshold knowing that a first threshold was set to define the degree of binary fuzzy similarity be- tween pixels. The use of a lossless compact n-D histogram allows a drastic reduction of the memory space necessary for coding it. As a consequence, the segmentation can be achieved without reducing the colors population of images in the classification step. It is shown that using n-D compact histograms, instead of 1-D and 2-D ones, leads to better segmentation results. Various images were segmented;the evaluation of the quality of segmentation in supervised and unsupervised of segmentation method proposed compare to the classification method k-means gives better results. It thus highlights the relevance of our approach, which can be used for solving many problems of segmentation.展开更多

We discuss how the terrain induced variations, that are unavoidably caused by the obstructions and irregularities in the surroundings of the transmitting and the receiving antennas, have two distinct effects on the network. Firstly, they reduce the amount of links in the network connectivity graph causing it to behave more randomly, while decreasing the coverage and capacity of the network. Secondly, they increase the length of the established paths between the nodes. The presented results show how the terrain oblique influences the layout of the network connectivity graph, in terms of different network metrics, and gives insight to the appropriate level of details needed to describe the terrain in order to obtain results that will be satisfyingly accurate.展开更多
