Effect of Dahuang Zhechong pill combined with antiviral therapy on serum virus replication indexes as well as immunity and inflammation indexes in patients with chronic hepatitis B cirrhosis

Objective:To study the effect of Dahuang Zhechong pill combined with antiviral therapy on serum virus replication indexes as well as immunity and inflammation indexes in patients with chronic hepatitis B cirrhosis.Methods: A total of 104 patients with chronic hepatitis B cirrhosis who were treated in our hospital between May 2013 and April 2016 were selected and randomly divided into combined treatment group and western medicine treatment group who accepted Dahuang Zhechong pill combined with entecavir treatment and entecavir monotherapy respectively. Before and after treatment, the HBV-DNA loads, liver fibrosis index and immune cell levels as well as inflammatory cytokine levels were determined.Results:HBV-DNA loads of both groups of patients at 48 weeks after treatment were significantly lower than those before treatment, and the HBV-DNA loads were not significantly different between the two groups after treatment;serum HA, LN, PC-III, IV-C, IL-15, IL-16 and TGF-β1 levels as well as peripheral blood Treg cell levels of both groups at 24 weeks and 48 weeks after treatment were significantly lower than those before treatment while peripheral blood NK cell levels were higher than those before treatment, and serum HA, LN, PC-III, IV-C, IL-15, IL-16 and TGF-β1 levels as well as peripheral blood Treg cell levels of combined treatment group were significantly lower than those of western medicine treatment group while peripheral blood NK cell levels were higher than those of western medicine treatment group. Conclusion:Dahuang Zhechong pill combined with antiviral treatment of chronic hepatitis B cirrhosis can regulate the immune response and inflammatory reaction without affecting the inhibiting effect of antiviral drugs on viral replication. 展开更多

Objective:To explore the effect and mechanism of Dahuang Zhechong Pill(DHZCP)on liver fibrosis.Methods:Liver fibrosis cell model was induced by transforming growth factor-β(TGF-β)in hepatic stellate cells(HSC-T6).DHZCP medicated serum(DMS)was prepared in rats.HSC-T6 cells were divided into the control(15%normal blank serum culture),TGF-β(15%normal blank serum+5 ng/mL TGF-β),DHZCP(15%DMS+5 ng/mL TGF-β),DHZCP+PDTC[15%DMS+4 mmol/L ammonium pyrrolidine dithiocarbamate(PDTC)+5 ng/mL TGF-β],and PDTC groups(4 mmol/L PDTC+5 ng/mL TGF-β).Cell activity was detected by cell counting kit 8 and levels of tumor necrosis factor-α(TNF-α),interleukin(IL)-1β,IL-6,aspartate aminotransferase(AST)and alanine aminotransferase(ALT)in the cell supernatant were determined by enzymelinked immunosorbnent assay.Western blot was used to measure the expressions of p38 mitogen-activated protein kinase/nuclear factor kappa B/transforming growth factor-β1(p38 MAPK/NF-κB/TGF-β1)pathway related proteins,and the localization and expressions of these proteins were observed by immunofluorescence staining.Results:DHZCP improved the viability of cells damaged by TGF-βand reduced inflammatory cytokines and ALT and AST levels in the supernatant of HSC-T6 cells induced with TGF-β(P<0.05 or P<0.01).Compared with the TGF-βgroup,NF-κB p65 levels in the DHZCP group were decreased(P<0.05).p38 MAPK and NF-κB p65 levels in the DHZCP+PDTC were also reduced(P<O.01).Compared with the TGF-βgroup,the protein expression of Smad2 showed a downward trend in the DHZCP,DHZCP+PDTC,and PDTC groups(all P<0.01),and the decreasing trend of Samd3 was statistically significant only in DHZCP+PDTC group(P<0.01),whereas Smad7 was increased(P<0.05 or P<0.01).Conclusion:DHZCP can inhibit the process of HSC-T6 cell fibrosis by down-regulating the expression of p38 MAPK/NF-κB/TGF-β1 pathway. 展开更多

The infiltration of immune cells into the hepatocellular carcinoma microenvironment is the main reason why hepatocellular carcinoma patients are prone to carcinoma recurrence and the disease are incurable.Notably,the infiltration of Treg cells is the main trigger.Dahuang Zhechong pill(DHZCP)is a traditional Chinese herbal compound successful in the treatment of hepatitis and hepatocellular carcinoma.DHZCP can heal and nourish while slowing the onset of the disease,thereby strengthening the body’s immune function.It can localize tumors and ultimately achieve the goal of eliminating tumors.In this study,an orthotopic liver cancer model of mice was used to explore the mechanism of DHZCP enhancing anti-tumor immunity,which showed more Th1 cells in the peripheral blood and spleen after DHZCP treatment,while more IFN-γwas secreted to activate CD8^(+)T cells and Treg cell production was inhibited,thereby suppressing the growth of HCC.Finally,we also analyzed the potential components of DHZCP from the perspective of modern targets using network pharmacology methods and experimental results. 展开更多

Cancer still has elevated morbidity and mortality,which undoubtedly impacts the life quality of affected individuals.Remarkable advances have been made in cancer therapy,although the toxicities of traditional therapies remain an obvious challenge.Dahuang Zhechong Pill(DHZCP),developed by Zhongjing Zhang in the Synopsis of the Golden Chamber,represents an effective anticancer traditional Chinese medicine(TCM).In this review,it was found that DHZCP is therapeutically utilized in liver,lung,gastric,pancreatic and other cancers in clinic.Pharmacological evidence showed that its anti-tumor mechanisms mainly involve induced cell cycle arrest,apoptosis and autophagy,as well as suppressed tumor cell proliferation,obstructed angiogenesis and metastasis,enhanced immunity,and reversal of multidrug resistance.The present review provides a solid basis for the clinical application of DHZCP and may promote the wide use of TCM in clinical antitumor application. 展开更多

OBJECTIVE:To investigate the effects of a modified Dahuang Zhechong Pill(MDZP) on the angiogene sis of rhesus choroid-retina endothelial(RF/6A cells and its preliminary mechanism.METHODS:A 3-(4,5-dimethylthiazol-2-yl)-5-(3-car boxymethoxyphenyl)-2-(4-sulfophenyl)-2H-tetrazol ium(MTS) method was used to assess the effect o a MDZP on RF/6A cell proliferation induced by vas cular endothelial growth factor(VEGF).Transwell in serts were used to assess the effect of the MDZP on RF/6A cell migration.Matrigel was used to asses the effect of the MDZP on the tube formation of RF 6A cells.Western blotting and quantitative re al-time reverse transcription polymerase chain reac tion(RT-PCR) were used to detect the protein and mRNA expression,respectively,of VEGF and matri metalloproteinase-2(MMP-2) in RF/6A cells treatedwith the MDZP.RESULTS:RF/6A cell proliferation induced by VEGF was inhibited by 0.2 mg/mL MDZP.At 0,12.5,25 and 50 mg/mL MDZP,the number of cells that migrated through Transwell membranes was 73.33± 4.51,61.33±4.04,28.67±6.66 and 17.67±4.16,respectively,and the number of tubes formed in Matrigel was 20.33±0.58,13.33±1.53,11.00±1.00 and 1.33±0.58,respectively.At 100 and 200 mg/mL MDZP,the protein and mRNA expression of VEGF and MMP-2 were inhibited in RF/6A cells.At 400 mg/mL MDZP,the expression of VEGF mRNA and MMP-2 protein were inhibited in RF/6A cells.CONCLUSIONS:MDZP inhibits the angiogenesis of RF/6A cells via the suppression of proliferation,migration and tube formation of RF/6A cells.Inhibition of the protein and mRNA expression of VEGF and MMP-2 in RF/6A cells may be an important mechanism. 展开更多

Objective: To investigate effects of Dahuang Zhechong Pill (大黄庶虫丸, DHZCP) on the cell cycle and the related signal pathways in vascular smooth muscle cells (VSMCs) stimulated by platelet-derived growth factor (PDGF) with the method of serum pharmacology. Methods: DNA synthesis in VSMCs was examined by detecting 5'-bromo-2'-deoxyuridine incorporation with the immunocytochemical method. The cycle of VSMCs was evaluated with flow cytometry. Expressions of cyclin D1, p27, protein kinase C α (PKC α), and phosphorylated extracellular signal regulated kinase 1/2 (ERK1/2) were quantified by Western blot method. Results: DHZCP containing serum significantly inhibited DNA synthesis of PDGF-stimulated VSMCs, arrested the cells in G1 phase, modulated the protein expressions of cyclin D1 and p27, and suppressed the activation of PKC α and ERK1/2. Conclusion: DHZCP containing serum inhibits VSMCs proliferation via modulating the expressions of cell cycle proteins to arrest the cell in G1 phase, which is attributed to, at least in part, suppressing PKC α -ERK1/2 signaling in VSMCs. 展开更多

Objective: To investigate and compare the effects and mechanisms of three functional parts of Dahuang Zhechong Pill (大黄〈庶虫〉虫丸, DHZCP), including drugs with the function of removing blood stasis and promoting blood circulation (FP- I ), drugs with the function of expelling heat and moistening dryness (FP-II), and drugs with the function of nourishing yin and replenishing blood (FP-m) of DHZCP, on platelet-derived growth factor (PDGF)-stimulated vascular smooth muscle cells (VSMCs) proliferation with the method of serum pharmacology. Methods: VSMCs proliferation of rat was assayed by measuring the cell viability with the 3-(4, 5-dimethylthiazol-2yl)-2, 5-diphenyltetrazolium bromide (Ml-I') method. DNA synthesis in VSMCs was examined by detecting 5'-bromo-2'-deoxyuridine incorporation with the immunocytochemical method. Cycle of VSMCs was evaluated with flow cytometry. Expression of cyclin D1, p27, PKC a, and phosphorylated extracollular signal regulated kinase 1/2 (ERK1/2) was quantified by the Western blotting method. Results: The FP- I and FP-m containing serum was capable of inhibiting PDGF-stimulated proliferation and DNA synthesis of VSMCs, arrested VSMCs in G1 phase, downregulated cycUn D1, and upregulated p27 expression (P〈0.01 or P〈0.05). The FP- I and FP-11I containing serum also inhibited the PDGF-induced phosphorylation of tyrosine of ERK1/2 and PKCo~ expression (P〈0.01 or P〈0.05). Conclusions: FP- I and FP-$ of DHZCP are able to inhibit VSMCs proliferation via interrupting PKC a-ERK1/2 signaling, modulating the expression of cell cycle proteins to result in arresting the cells in G1 phase. The inhibitory effect is mainly related to the function of removing blood stasis and promoting blood circulation, slightly to the function of nourishing yin and replenishing blood, but not to the function of expelling heat and moistening dryness. 展开更多

目的研究大黄蛰虫丸加减联合奥拉帕利治疗乳腺癌基因(breast cancergene,BRCA)突变型卵巢癌的疗效及对癌胚抗原(car-cinoembryonic antigen,CEA)、糖类抗原125(carbohydrate antigen 125,CA125)、糖类抗原199(carbohydrate antigen 199,... 目的研究大黄蛰虫丸加减联合奥拉帕利治疗乳腺癌基因(breast cancergene,BRCA)突变型卵巢癌的疗效及对癌胚抗原(car-cinoembryonic antigen,CEA)、糖类抗原125(carbohydrate antigen 125,CA125)、糖类抗原199(carbohydrate antigen 199,CA199)水平的影响。方法纳入2021年2月至2023年2月在河南中医药大学第三附属医院进行手术治疗的98例卵巢癌患者的临床资料,根据治疗方式将患者分为对照组(n=49)和观察组(n=49),对照组采用奥拉帕利进行干预,观察组采用大黄蛰虫丸加减联合奥拉帕利进行干预,观察两组治疗前后临床疗效、肿瘤标志物CEA、CA125、CA199水平和药物不良反应。结果治疗后观察组CEA、CA125、CA199水平低于对照组(P<0.05);观察组治疗期间的恶心、肝肾功能异常、疼痛发生率均低于对照组(P<0.05);治疗前两组中医症候(小腹胀痛、纳差、面色晦暗)评分差异无统计学意义(P>0.05);治疗后,两组小腹胀痛、纳差、面色晦暗评分均降低,且观察组评分低于对照组(P<0.05);观察组西医治疗有效率(83.67%)高于对照组(55.10%)(P<0.05);对照组中医治疗有效率(53.06%)低于观察组(83.67%)(P<0.05)。结论大黄蛰虫丸加减联合奥拉帕利治疗BRCA突变型乳腺癌能够起到提高临床疗效、减少药物不良反应的作用。 展开更多

目的探讨大黄䗪虫丸联合戈舍瑞林治疗子宫内膜异位症的临床效果及对患者生活质量的影响。方法选择在江西省妇幼保健院住院行腹腔镜手术的子宫内膜异位症患者随机分为两组,各60例。对照组予醋酸戈舍瑞林缓释植入剂,研究组在此基础上予大黄... 目的探讨大黄䗪虫丸联合戈舍瑞林治疗子宫内膜异位症的临床效果及对患者生活质量的影响。方法选择在江西省妇幼保健院住院行腹腔镜手术的子宫内膜异位症患者随机分为两组,各60例。对照组予醋酸戈舍瑞林缓释植入剂,研究组在此基础上予大黄䗪虫丸,观察两组性激素水平、视觉模拟量表(VAS)评分、生存质量评价量表(SF-36)评分。结果治疗后,两组性激素水平及VAS评分均降低、SF-36评分均提高,且相对于对照组,研究组各指标改善情况更明显(P<0.05),患者生活质量得到明显改善。结论大黄䗪虫丸联合戈舍瑞林应用于子宫内膜异位症患者,存在相互补充作用,能够明显改善其疼痛症状,提高生活质量,彰显了中西医药结合的综合治疗优势,临床取得了较为满意的效果,值得推广应用。 展开更多

目的 探讨大黄蛰虫丸调节IL-1β/NF-κB/NLRP3信号通路对动脉粥样硬化(AS)大鼠主动脉炎性损伤的影响。方法 将40只大鼠分为正常对照组(NS)、模型组、地奥心血康干预组(阳性组)及大黄蛰虫丸干预组(大黄蛰虫丸组),每组各10只。除正常组外... 目的 探讨大黄蛰虫丸调节IL-1β/NF-κB/NLRP3信号通路对动脉粥样硬化(AS)大鼠主动脉炎性损伤的影响。方法 将40只大鼠分为正常对照组(NS)、模型组、地奥心血康干预组(阳性组)及大黄蛰虫丸干预组(大黄蛰虫丸组),每组各10只。除正常组外,余下3组小鼠均构建AS模型。应用油红染色对主动脉病理切片情况进行观察,采用EILSA法检测血清脂质水平[甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)]、血清炎症指标(IL-6、IL-1β、TNF-α水平);应用流式细胞术对外周血Th17/Treg细胞水平进行检测,并通过RT-qPCR法和Western blot分别检测IL-1β/NF-κB/NLRP3信号通路mRNA和蛋白表达水平,并计算AI指数。结果 除NC组大鼠以外,其他各组大鼠动脉组织中均出现内膜增厚、斑块沉积和不连续、大量炎症细胞浸润等现象;其中模型组动脉组织损伤现象最为显著,阳性组和大黄蛰虫丸组较模型组明显改善,动脉组织结构情况与NC组相近。模型组血清TC[(9.82±1.46)mmol/L]、TG[(9.82±1.46)mmol/L]、LDL-C[(2.93±0.22)mmol/L]、IL-6[(97.65±9.11)ng/L]、TNF-α[(93.21±11.17)ng/L]水平及外周血Th17[(3.56±0.34)%]、Treg细胞[(4.41±0.38)%]水平高于NC组[分别为(3.71±0.57)mmol/L、(6.47±0.92)mmol/L、(0.94±0.15)mmol/L、(16.52±2.76)ng/L、(5.45±0.84)ng/L、(1.83±0.16)%、(7.72±0.73)%](均P<0.05),HDL-C水平[(2.92±0.31)mmol/L]低于NC组[(0.95±0.08)mmol/L](P<0.05);与模型组相比,大黄蜇虫丸组和阳性组血清TC[分别为(3.92±0.72)mmol/L、(3.71±0.65)mmol/L]、TG[分别为(6.71±0.83)mmol/L、(6.83±0.72)mmol/L]、LDL-C[分别为(1.15±0.11)mmol/L、(1.11±0.13)mmol/L]、IL-6[分别为(19.83±2.55)ng/L、(18.51±3.72)ng/L]、TNF-α[分别为(17.08±1.72)ng/L、(15.92±1.56)ng/L]水平及外周血Th17[分别为(2.11±0.25)%、(2.06±0.22)%]、Treg细胞[分别为(7.45±0.76)%、(7.31±0.72)%]水平偏低,HDL-C偏高[分别为(2.18±0.72)mmol/L、(2.23±0.61)mmol/L],趋近于NC组(均P<0.05)。与NC组AI指数(0.35±0.08)比较,模型组、阳性组和大黄蛰虫丸组(分别为9.71±2.04、1.21±0.15、0.83±0.17)均明显升高(均P<0.05),其中阳性组和大黄蛰虫丸组AI指数低于模型组(均P<0.05)。模型组大鼠的IL-6、TNF-α水平和外周血IL-1β、NF-κB、NLRP3 mRNA和信号通路蛋白表达水平均较NC组、阳性组和大黄蛰虫组明显偏高(均P<0.05)。结论 大黄蛰虫丸对动脉粥样硬化大鼠主动脉的炎性损伤可能具有明显的改善作用,可能是通过抑制IL-1β/NF-κB/NLRP3信号通路来实现治疗效果。 展开更多

目的:研究逍遥散合大黄[庶虫]虫丸对经肝动脉化疗栓塞(TACE)术后的原发性肝癌(PLC)患者气滞血瘀型的治疗效果。方法:将80例肝癌气滞血瘀型患者采用随机数字表法分为观察组和对照组各40例。对照组肝癌患者TACE术后予对症治疗,观察组肝癌... 目的:研究逍遥散合大黄[庶虫]虫丸对经肝动脉化疗栓塞(TACE)术后的原发性肝癌(PLC)患者气滞血瘀型的治疗效果。方法:将80例肝癌气滞血瘀型患者采用随机数字表法分为观察组和对照组各40例。对照组肝癌患者TACE术后予对症治疗,观察组肝癌患者在对症治疗的基础上联合逍遥散合大黄[庶虫]虫丸进行治疗。经治疗1个月后复查肝功能、凝血功能、肿瘤指标及上腹部影像学检查的变化情况。结果:观察组血清甲胎蛋白(AFP)、糖类抗原199(CA199)、糖类抗原125(CA125)等肿瘤指标,谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL)、总蛋白(TP)、白蛋白(ALB)等肝功能指标,凝血酶原时间(PT)、凝血酶原活动度(PTA)、国际标准化比值(INR)、纤维蛋白原(FIB)等凝血功能指标低于对照组,差异具有统计学意义(P<0.05),观察组总有效率75.00%,获益率90.00%,对照组总有效率32.50%,获益率57.50%,观察组总有效率高于对照组(P<0.05)。结论:肝癌气滞血瘀型患者在经过TACE术后,在对症治疗的基础上加用中药内服方逍遥散合大黄[庶虫]虫丸具有更好的疗效,逍遥散合大黄[庶虫]虫丸可以加强对肝功能的保护,有效地改善凝血功能,降低肝癌的转移程度。 展开更多

大黄[庶虫]虫丸出自《金匮要略》,是治疗“虚劳干血”类疾病的经典名方,近年来广泛用于多种恶性肿瘤尤其是肝癌的综合治疗中。检索整理近20年文献,从中医组方特色、临床应用、分子机制三方面总结大黄[庶虫]虫丸治疗肝癌的相关研究,发现... 大黄[庶虫]虫丸出自《金匮要略》,是治疗“虚劳干血”类疾病的经典名方,近年来广泛用于多种恶性肿瘤尤其是肝癌的综合治疗中。检索整理近20年文献,从中医组方特色、临床应用、分子机制三方面总结大黄[庶虫]虫丸治疗肝癌的相关研究,发现大黄[庶虫]虫丸在中医组方上具有虫类逐瘀、草本养虚,肝脾同调、两脏兼顾,蜜丸为剂、峻药缓图等特色,配伍严谨,用药精当。在临床上无论是联合经动脉化疗栓塞(transcatheter arterial chemoembolization,TACE)还是联合静脉化疗,均可减轻治疗不良反应,提高临床有效率,延长患者生存时间;此外,对于晚期原发性肝癌患者以及肝转移癌患者,单药应用也可明显改善症状,提高生活质量,延长终末期患者带瘤生存时间,临床疗效确切,且可长期应用,安全性较高。机制研究中发现大黄[庶虫]虫丸具有抗肝癌前病变、保护肝细胞,调节肝功能,抗肝癌细胞转移、抑制肿瘤血管新生,促进血管正常化、调节免疫等相关机制,分子机制明确。通过以上三方面,全面详细阐述了大黄[庶虫]虫丸治疗肝癌的研究现状,并对目前的临床和实验研究提出不足与改进,以期为后续深入研究及抗肿瘤新药的研发提供思路和依据。 展开更多

目的探析大黄[庶虫]虫片联合胃复春胶囊治疗慢性萎缩性胃炎(chronic atrophic gastritis,CAG)伴肠上皮化生(intestinal metaplasia,IM)的临床疗效。方法选取2022年3月至2023年1月浙江中医药大学附属湖州市中医院收治的50例脾虚络瘀型CAG... 目的探析大黄[庶虫]虫片联合胃复春胶囊治疗慢性萎缩性胃炎(chronic atrophic gastritis,CAG)伴肠上皮化生(intestinal metaplasia,IM)的临床疗效。方法选取2022年3月至2023年1月浙江中医药大学附属湖州市中医院收治的50例脾虚络瘀型CAG伴IM患者为研究对象,根据随机数字表法将其分为观察组和对照组,每组各25例。对照组患者予口服瑞巴派特片治疗,观察组患者予大黄[庶虫]虫片联合胃复春胶囊治疗,两组患者均连续用药3个月。比较两组患者的临床疗效、治疗前后中医证候积分、D-二聚体、胃蛋白酶原Ⅰ(pepsinogenⅠ,PG-Ⅰ)、IM积分。结果观察组患者的总有效率显著高于对照组(χ^(2)=11.590,P=0.009);治疗后,两组患者的中医证候积分、D-二聚体水平均显著低于本组治疗前,血清PG-Ⅰ水平均显著高于本组治疗前(P<0.05),观察组患者的中医证候积分显著低于对照组(P<0.05);观察组患者治疗前后的D-二聚体、PG-Ⅰ差值均显著大于对照组(P<0.05)。观察组患者治疗前后的IM积分差值显著优于对照组(P<0.05)。结论大黄[庶虫]虫片联合胃复春胶囊治疗CAG伴IM的临床疗效显著,可有效减轻患者的临床症状,提高血清PG-Ⅰ水平,减轻胃黏膜萎缩与IM程度,具有临床推广使用价值。 展开更多

目的探讨大黄廑虫丸联合抗病毒治疗乙型肝炎(HBV)肝硬化对病人凝血功能紊乱、肝硬化程度及门静脉系统相关参数的影响。方法选择2020年6月至2022年6月于无锡市中医医院接受治疗的100例HBV肝硬化病人,运用随机数字表法将其分为观察组与对... 目的探讨大黄廑虫丸联合抗病毒治疗乙型肝炎(HBV)肝硬化对病人凝血功能紊乱、肝硬化程度及门静脉系统相关参数的影响。方法选择2020年6月至2022年6月于无锡市中医医院接受治疗的100例HBV肝硬化病人,运用随机数字表法将其分为观察组与对照组,各50例。对照组采用常规抗病毒疗法,观察组基于对照组联合大黄廑虫丸,评估两组疗效并进行比较,对比两组凝血功能、HBV-DNA值、肝硬化程度、门静脉系统相关参数变化。结果观察组治疗显效病人占50.00%高于对照组26.00%,差异有统计学意义(P<0.05);治疗前两组病人的血小板计数(PLT)、凝血酶时间(TT)、纤维蛋白原(FIB)、HBVDNA水平比较差异无统计学意义(P>0.05),治疗后PLT[(125.42±24.57)×10^(9)/L比(116.12±21.38)×10^(9)/L]、FIB水平[(2.76±0.56)g/L比(1.80±0.63)g/L]高于对照组,TT[(14.18±4.28)s比(16.61±5.34)s]、HBV-DNA[(2.72±0.43)×10^(5) IU/L比(3.38±0.68)×10^(5) IU/L]小于对照组,差异有统计学意义(P<0.05);两组治疗前后的肝硬化程度和门静脉内径比较差异无统计学意义(P>0.05),治疗后观察组肝脏硬度数值(LSM)(15.32±2.27)kPa比(17.94±2.34)kPa、肝门静脉内径[(12.29±1.98)mm比(13.80±2.15)mm]低于对照组,差异有统计学意义(P<0.05);两组治疗前天冬氨酸氨基转移酶(AST)、丙氨酰氨基转移酶(ALT)水平比较差异无统计学意义(P>0.05),治疗后ALT、AST水平均下降,观察组下降幅度高于对照组,差异有统计学意义(P<0.05)。结论相较于单独抗病毒治疗,联合大黄廑虫丸治疗,疗效较好,改善肝功能,有助于凝血功能的恢复,缩小肝门静脉内径。 展开更多

目的:观察大黄■虫丸治疗非酒精性脂肪性肝炎(NASH)痰瘀互结证的临床疗效与安全性。方法:选取2020年5月11日至2022年1月20日四川大学华西医院收治的NASH患者,均辨证为痰瘀互结证的患者216例作为研究对象,采用区组随机化方法分为对照组... 目的:观察大黄■虫丸治疗非酒精性脂肪性肝炎(NASH)痰瘀互结证的临床疗效与安全性。方法:选取2020年5月11日至2022年1月20日四川大学华西医院收治的NASH患者,均辨证为痰瘀互结证的患者216例作为研究对象,采用区组随机化方法分为对照组和观察组,每组108例,观察组脱失10例,对照组脱失9例。对照组接受健康宣教,根据病情给予降压、降糖、调脂、保肝降酶等标准化基础治疗;观察组在对照组治疗方法基础上,联用大黄■虫丸。治疗前及治疗后(治疗1年)对比2组肝活检缓解率、肝纤维化扫描仪(FibroScan)测定的肝纤维化评级缓解率、中医证候积分、体质量指数及肝功能指标[血清谷丙转氨酶(GPT)、血清总胆红素(TB)、血清谷草转氨酶(GOT)]、血脂指标[总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)]、临床疗效及安全性。结果:观察组患者肝活检缓解率高于对照组(χ^(2)=14.512,P=0.002),肝纤维化评级改善率高于对照组(χ^(2)=15.550,P=0.001)。观察组临床中医证候总有效率高于对照组(χ^(2)=21.438,P<0.05)。治疗后,2组患者体质量指数及肝功能、血脂指标均较治疗前改善(P<0.05);治疗后,2组体质量指数及血脂指标比较差异无统计学意义(P>0.05),观察组肝功能指标(GPT、GOT、TB)低于对照组(P<0.05)。共发生不良事件37例次,均为轻微不良反应,未经处理或对症治疗后缓解,2组不良事件发生率比较差异无统计学意义(P>0.05)。结论:大黄■虫丸可改善NASH痰瘀互结证患者的肝纤维化、中医证候与肝功能,安全性良好。 展开更多

基于HIF-1α/VEGF信号通路探讨大黄䗪虫丸抑制KM小鼠肿瘤血管的机制研究

目的 研究大黄䗪虫丸对昆明(KM)小鼠移植瘤的抑瘤率、体积、质量、缺氧诱导因子(Hypoxia inducible factor-1,HIF-1α)、血管内皮生长因子(Vascular endothelial growth factor, VEGF)、肿瘤血管密度(Microvascular density, MVD)的影响... 目的 研究大黄䗪虫丸对昆明(KM)小鼠移植瘤的抑瘤率、体积、质量、缺氧诱导因子(Hypoxia inducible factor-1,HIF-1α)、血管内皮生长因子(Vascular endothelial growth factor, VEGF)、肿瘤血管密度(Microvascular density, MVD)的影响,探讨大黄䗪虫丸是否是通过HIF-1α/VEGF信号通路达到抑制肿瘤血管生成进而控制肿瘤生长的作用。方法 将H22人肝癌细胞注射到KM小鼠进行小鼠皮下移植瘤造模,造模成功后按照随机数字表法分为空白组、模型组、大黄䗪虫丸低剂量组、大黄䗪虫丸中剂量组、大黄䗪虫丸高剂量组和索拉非尼组,每组各15只,实验干预1次/d,连续干预2周。大黄䗪虫丸低剂量组、中剂量组、高剂量组和索拉非尼组均给予相应药物灌胃,模型组给予生理盐水灌胃。实验干预结束后取瘤体,对各组瘤体长、短径进行测量,计算瘤体的抑瘤率、体积及体质量并记录;采用苏木精-伊红染色法检测各组肿瘤组织形态学的病理变化;实时荧光定量聚合酶链反应法检测HIF-1α和VEGF mRNA转录水平;蛋白印记法检测HIF-1α和VEGF蛋白表达水平;采用免疫组织化法检测肿瘤组织检测反映MVD的表达。结果 体积抑瘤率比较,大黄䗪虫丸低、中、高剂量组的抑瘤率分别为17.11%、21.73%和37.09%,索拉非尼组的抑瘤率为46.75%;HE染色结果显示:与模型组比较,大黄䗪虫丸高剂量组可使肿瘤组织出现大面积的坏死区域;PCR和WB结果显示:与模型组比较,大黄䗪虫丸各治疗组均可不同程度的显著降低肿瘤组织HIF-1α、VEGF mRNA蛋白水平表达(P<0.05,P<0.01);CD31-MVD结果显示:与模型组比较,大黄䗪虫丸高剂量组MVD明显减少(P<0.05)。结论 大黄䗪虫丸可能是通过HIF-1α/VEGF信号通路达到抑制肿瘤血管生成进而控制肿瘤的生长。 展开更多
