Transcriptomic and metabolomic analysis of the effects of Zhenwu decoction on kidney yang deficiency pattern in chronic kidney disease

Objective:To explore the kidney yang deficiency pattern(KYDP)in a chronic kidney disease(CKD)rat model and the mechanisms underlying the effects of Zhenwu decoction(ZWD)by conducting tran-scriptomic and metabolomic analyses.Methods:Adriamycin(ADR)combined with hydrocortisone(HC)was used to induce CKD with KYDP in rats.ADR was injected into the tail vein twice.HC was injected intramuscularly for 8 weeks.ZWD was administered by gavage for 8 weeks.The general condition was observed,24-h urine protein was detected,serum corticosterone,triiodothyronine,thyroxine,TSH,testosterone,cAMP,and cGMP levels were determined,and pathological analysis was conducted.Transcriptomic and metabolomic analyses were conducted to screen differentially expressed genes(DEGs),differentially expressed metabolites(DEMs),and differentially expressed pathways(DEPs).The core DEMs and DEGs were input to Metab-oanalyst 5.0 to identify the pathways affected by ZWD.Results:In the HC group,KYDP symptoms were observed.Compared with control group,the levels of 24-h urine protein,TSH,and cGMP significantly increased(all P<0.01),and corticosterone,triiodothyronine,thyroxine,and cAMP significantly decreased(all P<0.01)in the HC group.After ZWD intervention,the levels of above-mentioned indicators could be reversed to some extent.Pathological analysis in the HC group revealed kidney lesions.DEGs in the ZWD group were mainly associated with pathways such as nucleotide synthesis and endocrine pathways.In the ZWD group,differences in biosynthesis of unsat-urated fatty acids and butanoate metabolism were observed.The following pathways were significantly affected by ZWD:arachidonic acid metabolism,valine,leucine,and isoleucine biosynthesis,linoleic acid metabolism,and alpha-linolenic acid metabolism.Conclusion:ZWD can be used to treat KYDP in CKD through regulating arachidonic acid metabolism,valine,leucine,and isoleucine biosynthesis,linoleic acid metabolism,and alpha-linolenic acid metabolism. 展开更多

BACKGROUND: In the perspective of traditional Chinese medicine, few studies have focused on the compound preparations though there are many investigations. The present study was undertaken to investigate the effect of Zhenwu Tang Granule on chronic pressure-overloaded left ventricular hypertrophy in rats.METHODS: The study was performed at the laboratory of Guangzhou Institute of Respiratory Disease. Male SD rats were divided randomly into 3 groups: sham operation group (n=8), operation group (n=15) and traditional Chinese medicine (TCM) group (n=15).The model of myocardial hypertrophy was made by gradually constricting the abdominal aorta. Sixteen weeks later, cardiac ultrasonography was performed in all groups in order to ascertain post-operational left ventricular (LV) hypertrophy. And Zhenwu Tang Granule was added at a dose of 12 g/kg in the mixed feedstuff for 8 weeks in the TCM group. In the 24th week, weight, structure as well as function of the heart in each group were measured by high-frequency ultrasonography, and Masson's staining was performed on the cardiac muscles. Meanwhile, total collagen volume fraction (CVF-T) and non-coronary vessel collagen volume fraction (CVF-NV) were analyzed.RESULTS: There was an increase in the weight of the heart in the operation group, with the left ventricule dominated (P〈0.05). The heart was enlarged, with diastolic interventricular septal distance (IVSd) and left ventricular posterior wall distance (LVPWd) dominated (P〈0.01).There was a significant decrease in the cardiac function (P〈0.05). The weight (P〈0.01) and volume of the heart decreased in the TCM group compared with the operation group, with IVSd and systolic left ventricular posterior wall dominated (P〈0.01). And the cardiac function was improved (P〈0.05). Significant interstitial and collagen hyperplasia was shown in the operation group based on pathological analysis, and various improvements were proved in the TCM group, i.e. there was a significant decrease in CVF-T and CVF-NV (P〈0.01) compared with the operation group; but no difference (P〉0.05) was found when compared with the pseudo-operation group.CONCLUSION: Zhenwu Tang Granule could reduce the weight and volume of the heart, improve the cardiac function, inhibit hyperplasia of collagen, and reverse myocardial hypertrophy in rats with pressure-overloaded left ventricular hypertrophy. 展开更多

[Objectives]The paper was to investigate the clinical effects of Zhenwu Decoction combined with Tingli Dazao Xiefei Decoction in the treatment of chronic heart failure with edema syndrome due to yang deficiency.[Methods]Eighty patients with chronic heart failure were randomly divided into 2 groups(control group and treatment group),with 40 cases in each group.The patients in the control group were treated with conventional western medicine for 14 d,and the patients in the treatment group were treated with Zhenwu Decoction and Tingli Dazhao Xiefeng Decoction on the basis of the control group for 14 d.The curative effect of traditional Chinese medicine(TCM)syndrome score,left ventricular ejection fraction(LVEF),plasma N-terminal brain natruretic peptide precursor(NT-proBNP)and New York Heart Association(NYHA)functional class were compared between the two groups before and after treatment.Minnesota living with heart failure questionnaire in the principle of Chinese civilization and traditional medicine characteristics(C-MLHF)at admission and 1 month after discharge were compared between the two groups.[Results]After treatment,the total effective rate of TCM syndrome score in the treatment group was 82.50%,and that in the control group was 67.50%.The curative effect of TCM syndrome score in the treatment group was better than that in the control group(P<0.05).After treatment the LVEF in both groups was increased compared with that before treatment,and the NT-proBNP level was decreased compared with that before treatment(P<0.05),and those in the treatment group were better than those in the control group.The total effective rate of cardiac function in the treatment group was 87.50%,and that in the control group was 65.00%.The improvement of cardiac function in the treatment group was better than that in the control group(P<0.05).One month after discharge,C-MLHF scores in both groups were decreased compared with those before treatment,and that in the treatment group was lower than in the control group(P<0.05).[Conclusions]Zhenwu Decoction combined with Tingli Dazao Xiefei Decoction based on traditional western medicine treatment could improve the clinical efficacy,symptoms and quality of life of patients in the treatment of chronic heart failure with edema syndrome due to yang deficiency. 展开更多

Objective: To carry out a systematic evaluation of the efficacy of modified Zhenwu Decoction combined withwestern medicine in the treatment of heart failure. Methods: Literature was retrieved in China KnowledgeResource Integrated Database (CNKI), Chinese scientific journal database (VIP), Wanfang Data Resource System(WANFANG DATA), PubMed and other traditional Chinese medicine related periodicals by retrieval methods ofsubject terms combined with free words and computer retrieval combined with manual retrieval. Literature wasscreened strictly according to the inclusion criteria.The quality of the included literature was evaluated according tothe quality standard of Jadad scale and Cochrane collaboration network bias risk assessment tool. The literaturedata was extracted. RevMan5.3 software was used to analyze data and the curative effect of modified ZhenwuDecoction combined with western medicine was objectively evaluated. Results: The response rate of theexperimental group was better than that of the control group [OR = 3.69,95% IC (2.36,5.79)]. The value of EF inthe experimental group was higher than that in the control group after treatment [MD = 5.85, 95% IC (3.90, 7.79) ].The Lee, score of the experimental group was lower than that of the control group after treatment [MD = -1.37,95% IC (-2.23, -0.52) ]. The value of BNP in the experimental group was less than that in the control group aftertreatment [MD = -114.48, 95%IC (-186.28, -42.68)]. The life quality score in the experimental group was less thanthat of the control group after treatment [MD = -8.44, 95%IC (-11.73, -5.15)]. Conclusion: The curative effect ofmodified Zhenwu Decoction combined with western medicine in the treatment of heart failure is better than thesimple western medicine treatment.So modified Zhenwu Decoction combined with western medicine in thetreatment of heart failure is worthy of clinical promotion. However, the results of this study still need further validation by more high quality randomized double blind controlled clinical trial because the overall quality of theresearch included. 展开更多

Objective:To study the effect of Zhenwu decoction combined with paclitaxel neoadjuvant chemotherapy on the malignant degree of cervical cancer.Methods: Patients with cervical cancer who accepted neoadjuvant chemotherapy in Nanchong Central Hospital and Suining Central Hospital between March 2015 and June 2017 were selected and randomly divided into two groups, the observation group received neoadjuvant chemotherapy combined with Zhenwu decoction therapy, and the control group received neoadjuvant chemotherapy. Before chemotherapy and after 2 courses of chemotherapy, the serum was collected to detect the contents of tumor markers;after chemotherapy, the surgically removed cervical cancer lesion was collected to detect the mRNA expression of proliferation genes, tumor suppressor genes and invasion genes.Results: After 2 course of chemotherapy, CA12-5, bFGF and SCC-Ag levels in serum of both groups were lower than those before treatment, and CA12-5, bFGF and SCC-Ag levels in serum as well as Ki-67, TK-1, SOX-2, CyclinD1, S100A6, N-cadherin, Gal-3, CatL and CatD mRNA expression in cervical cancer lesion of observation group were lower than those of control group whereas p53, p16, FHIT, NF2-1 and LATS1 mRNA expression in cervical cancer lesions were higher than those of control group.Conclusion:Zhenwu decoction combined with paclitaxel neoadjuvant chemotherapy can be more effective than neoadjuvant chemotherapy alone to kill cervical cancer cells and reduce the malignancy of cervical cancer. 展开更多

Intraoperative ovarian cancer tissue was kept to detect the expression of proliferation genes, invasion genes and autophagy genes through fluorescence quantitative PCR method.Results:Proliferation genes AKT, LSD1, SIRT1 and NOB1 mRNA expression in ovarian lesion tissue of zhenwu decoction group were lower than those of control group;invasion gene DACT1 mRNA expression was higher than that of control group whereas MTA1 and GRP78 mRNA expression were lower than those of control group;autophagy gene Beclin1 mRNA expression was higher than that of control group whereas LC3-Ⅱ and Atg5 mRNA expression were lower than those of control group.Conclusion:Preoperative zhenwu decoction combined with neoadjuvant chemotherapy can effectively inhibit the proliferation and invasion activity of tumor cells and regulate their autophagy function in patients with ovarian cancer. 展开更多

目的 考证真武汤的历史沿革和应用规律,为其深入研究提供精准的文献参考。方法 以真武汤为关键词,从《中华医典》检索、整理该方1991年1月以前的古籍。以真武汤、Zhenwu decoction、Zhenwu tang为主题词从中国知网、万方数据服务知识平... 目的 考证真武汤的历史沿革和应用规律,为其深入研究提供精准的文献参考。方法 以真武汤为关键词,从《中华医典》检索、整理该方1991年1月以前的古籍。以真武汤、Zhenwu decoction、Zhenwu tang为主题词从中国知网、万方数据服务知识平台、维普网和Pub Med中获取2002年1月至2022年12月的临床研究文献。考证真武汤的处方来源、药物组成、剂量、炮制、功能主治、病证要素和应用规律。结果 纳入古籍92部,真武汤首载于《伤寒论》,由茯苓、芍药、白术、附子、生姜组成。剂量以茯苓三两、芍药三两、白术二两、附子一枚、生姜三两居多。在炮制方面,芍药以生白芍为主,白术以炒用为主,附子以熟附子为主。在应用规律方面,真武汤主要治疗少阴病和太阳病,涉及的症状有腹痛、小便不利、四肢疼痛、大便自利等,脾肾阳虚、阳虚水泛、心肾阳虚是临床应用真武汤的辨证要点。真武汤的应用涉及心血管、泌尿、消化等多系统,包括心力衰竭、糖尿病肾病、肾病综合征等疾病。结论本研究体现了真武汤的传承性、稳定性、有效性和安全性,为其研发提供了科学、客观的文献依据。 展开更多

真武汤联合血府逐瘀方治疗老年冠心病伴慢性心力衰竭患者的疗效观察

目的探讨真武汤联合血府逐瘀方治疗老年冠心病伴慢性心力衰竭(Chronic heart failure,CHF)患者的临床疗效。方法选取2020年5月—2023年6月期间六安市中医院收治的老年冠心病伴CHF患者106例作为研究对象,依据简单随机数字表法分为对照组... 目的探讨真武汤联合血府逐瘀方治疗老年冠心病伴慢性心力衰竭(Chronic heart failure,CHF)患者的临床疗效。方法选取2020年5月—2023年6月期间六安市中医院收治的老年冠心病伴CHF患者106例作为研究对象,依据简单随机数字表法分为对照组和研究组,每组各53例。对照组采取常规治疗,研究组在对照组基础上采取真武汤合血府逐瘀方,均治疗4周。观察比较两组患者临床疗效、不良反应发生情况,治疗前后中医证候积分、心功能[左心室射血分数(Left ventricular ejection fraction,LVEF)、心输出量(Cardiac output,CO)、左心室舒张末期内径(Left ventricular end-diastolic diameter,LVEDD)、左心室收缩末期内径(Left ventricular end-systolic diameter,LVESD)]、血流动力学[肺动脉平均压(Mean pulmonary artery pressure,MPAP)、左心房平均压(Mean left atrial pressure,MLAP)、平均二尖瓣跨瓣压差(Mean mitral valve intervalve pressure difference,MMPG)]、心力衰竭标记物[血管内皮生长因子(Vascular endothelial growth factor,VEGF)、可溶性生长刺激表达基因2蛋白(Soluble growth stimulation expressed gene2,sST2)、脑钠肽(Brain natriuretic peptide,BNP)]水平。结果治疗后研究组临床总有效率94.34%(50/53)明显高于对照组81.13%(43/53),差异有统计学意义(P<0.05)。治疗后两组患者肢冷、心悸、气短、喘促及肢体浮肿、胸闷、盗汗、乏力分值均较治疗前降低,差异有统计学意义(P<0.05);且研究组中医证候积分明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者LVEF、CO指标均较治疗前升高,LVEDD、LVESD指标均较治疗前降低,差异有统计学意义(P<0.05);且研究组LVEF、CO指标明显高于对照组,LVEDD、LVESD指标明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者血流动力学MPAP、MLAP、MMPG水平均较治疗前降低,差异有统计学意义(P<0.05);且研究组血流动力学MPAP、MLAP、MMPG水平明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者VEGF指标均较治疗前升高,sST2、BNP指标均较治疗前降低,差异有统计学意义(P<0.05);且研究组VEGF指标明显高于对照组,sST2、BNP指标均明显低于对照组,差异有统计学意义(P<0.05)。治疗期间,研究组不良反应发生率7.55%(4/53)与对照组3.77%(2/53)比较,差异无统计学意义(P>0.05)。结论常规基础上采取真武汤合血府逐瘀方治疗老年冠心病合并CHF可改善患者心功能,调节血流动力学状态及心力衰竭标记物水平,缓解临床症状,提升整体治疗效果,且具有安全性。 展开更多

目的观察真武汤加减治疗扩张型心肌病(Dilated cardiomyopathy,DCM)的临床疗效。方法检索2000年1月1日—2023年10月31日期间万方、维普、中国学术期刊全文数据库(CNKI)及PubMed数据库收录的真武汤加减治疗DCM的研究。同质性研究采用固... 目的观察真武汤加减治疗扩张型心肌病(Dilated cardiomyopathy,DCM)的临床疗效。方法检索2000年1月1日—2023年10月31日期间万方、维普、中国学术期刊全文数据库(CNKI)及PubMed数据库收录的真武汤加减治疗DCM的研究。同质性研究采用固定效应模型、异质性研究采用随机效应模型进行荟萃分析。采用Revman 5.3软件评价文献质量,应用Stata15.1软件合并分析数据,并用TSA 0.9软件对治疗有效率进行序贯分析。结果最终纳入12项研究,共666例患者,Meta分析结果显示,真武汤加减联合西药治疗可更有效的提高DCM患者左室射血分数(Left ventricular ejection fraction,LVEF)(SMD=4.27,95%CI:2.07~6.48,P<0.0001);更大程度缩小左室舒张末期内径(Left ventricular end diastolic dimension,LVEDd)(SMD=-7.74,95%CI:-8.37~-7.11,P<0.00001);显著提高治疗有效率(OR=3.62,95%CI:2.16~6.08,P<0.0001)。结论真武汤加减联合西药常规治疗DCM,在提高总有效、改善LVEF、缩小LVEDd方面优于常规西药治疗。 展开更多

目的探究二参真武汤联合美托洛尔治疗左心室射血分数保留型心力衰竭(Heart failure with preserved ejection fraction,HFpEF)患者的疗效及对其心功能和神经内分泌因子的影响。方法选取2021年6月-2023年6月期间就诊于安徽省中西医结合... 目的探究二参真武汤联合美托洛尔治疗左心室射血分数保留型心力衰竭(Heart failure with preserved ejection fraction,HFpEF)患者的疗效及对其心功能和神经内分泌因子的影响。方法选取2021年6月-2023年6月期间就诊于安徽省中西医结合医院心血管内科的HFpEF患者92例,依据简单随机数字表法分为对照组和研究组,每组各46例。两组患者均给予常规治疗,对照组在常规干预基础上口服美托洛尔,研究组在对照组基础上联合二参真武汤,两组患者均治疗4周。观察比较两组患者临床疗效、不良反应发生情况,治疗前后中医证候积分、心功能[左心室射血分数(Left ventricular ejection fractions,LVEF)、左心室收缩末内径(Left ventricular end systolic diameter,LVESD)、左心室舒张末内径(Left ventricular end diastolic diameter,LVEDD)、左心室等容舒张时间(Isovolumic relaxation time,IVRT)]、神经内分泌因子指标[醛固酮(Aldosterone,ALD)、血管紧张素Ⅱ(AngiotensinⅡ,AngⅡ)、去甲肾上腺素(Noradrenaline,NE)]水平、中性粒细胞明胶酶相关脂质运载蛋白(Neutrophil gelatinase-associated lipocalin,NGAL)、生长分化因子-15(Growth differentiation factor-15,GDF-15)水平。结果治疗后研究组治疗总有效率93.48%(43/46)明显高于对照组78.26%(36/46),差异有统计学意义(P<0.05)。治疗后两组患者胸胁作痛、心悸、气短喘息、浮肿与唇甲青紫、面色晦暗、畏寒肢冷分值均较治疗前降低,差异有统计学意义(P<0.05);且研究组中医证候积分明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者LVEF水平均较治疗前升高,LVESD、LVEDD、IVRT水平均较治疗前降低,差异有统计学意义(P<0.05);且研究组LVEF水平明显高于对照组,LVESD、LVEDD、IVRT水平均明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者血清ALD、AngⅡ、NE水平均较治疗前降低,差异有统计学意义(P<0.05);且研究组神经内分泌因子水平明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者血清NGAL、GDF-15水平均较治疗前降低,差异有统计学意义(P<0.05);且研究组血清NGAL、GDF-15水平均明显低于对照组,差异有统计学意义(P<0.05)。治疗期间,研究组不良反应发生率4.35%(2/46)与对照组8.70%(4/46)比较,差异无统计学意义(P>0.05)。结论联合二参真武汤及美托洛尔治疗HFpEF,可缓解患者临床症状,调节ALD、AngⅡ、NE及NGAL、GDF-15水平,改善心功能,提升整体治疗效果,且具有安全性。 展开更多

目的系统评价真武汤加减联合西药常规治疗慢性心力衰竭(Chronic heart failure,CHF)合并利尿剂抵抗(Diuretic resistance,DR)的临床疗效。方法检索自建库起至2023年11月1日中国知网、万方数据库、维普数据库、中国生物医学文献数据库、P... 目的系统评价真武汤加减联合西药常规治疗慢性心力衰竭(Chronic heart failure,CHF)合并利尿剂抵抗(Diuretic resistance,DR)的临床疗效。方法检索自建库起至2023年11月1日中国知网、万方数据库、维普数据库、中国生物医学文献数据库、PubMed、Cochrane Library、Embase、Web of Science、Clinical Trials、中国临床试验注册中心等数据库中真武汤加减治疗CHF合并DR的随机对照试验,采用Review Manager 5.4软件对文献数据进行Meta分析。结果共纳入15篇文献,包括1187例患者。Meta分析结果显示,与单纯西药对照组相比,真武汤加减联合西药常规治疗组患者在24 h总尿量[n=1058,MD=397.30,95%CI(269.44,525.16),P<0.01]、临床总有效率[n=625,RR=1.24,95%CI(1.15,1.34),P<0.01]、左心室射血分数(Left ventricular ejection fractions,LVEF)[n=697,MD=4.32,95%CI(2.51,6.112),P<0.01]、氨基末端脑利钠肽前体(N-terminal pro-B-type natriuretic peptide,NT-proBNP)水平[n=282,MD=734.4,95%CI(828.93,639.87),P<0.01]、外周血IL-6水平[n=170,MD=-5.70,95%CI(-7.23,-4.16),P<0.01]、外周血TNF-α水平[n=170,MD=-12.79,95%CI(-15.53,-10.04),P<0.01]方面均有明显改善,而左室舒张末内径未见明显改善[n=266,MD=-4.50,95%CI(-9.36,0.35),P=0.07]。药物安全性良好,加载中药治疗可显著降低低钾血症发生率[n=177,RR=0.32,95%CI(0.18,0.57),P<0.01]。结论在单纯西药治疗基础上,予真武汤加减可改善CHF合并DR患者24 h尿量、提高临床总有效率、改善患者心功能、降低炎症因子水平,且安全性较好。 展开更多

目的探讨真武汤加减对心肾阳虚型老年窦性心动过缓(SB)的疗效。方法选取2020年2月—2022年2月于医院门诊就诊的老年窦性心动过缓患者80例,其辨证分型属心肾阳虚证。随机分为对照组与治疗组各40例。两组均予西药常规治疗,治疗组在对照组... 目的探讨真武汤加减对心肾阳虚型老年窦性心动过缓(SB)的疗效。方法选取2020年2月—2022年2月于医院门诊就诊的老年窦性心动过缓患者80例,其辨证分型属心肾阳虚证。随机分为对照组与治疗组各40例。两组均予西药常规治疗,治疗组在对照组基础上予真武汤加减方治疗,4周为1个疗程,观察比较两组患者治疗前后24 h动态心电图疗效、中医证候疗效、心率变异性(HRV)指标(SDNN、SDANN、SDNN Index、rMSSD、PNN50)和心功能指标(CI、SV、LVEF、CO)。结果治疗后,治疗组动态心电图总有效率为90.0%高于对照组的70.0%(P<0.05);两组中医证候积分均比治疗前降低,且治疗组较对照组降低更显著(P<0.05);两组心率均有改善,且治疗组改善效果更显著(P<0.05);两组CI、SV、LVEF、CO均提高,且治疗组更显著(P<0.05);两组HRV分析各指标均有所提高,且治疗组更显著(P<0.05)。结论真武汤加减方治疗心肾阳虚型老年窦性心动过缓有较好临床疗效,对患者心率、心率变异性指标、心功能均有改善。 展开更多

目的:探讨参芪真武汤加味治疗原发性肾病综合征效果及对患者肾功能、微炎状态、凝血纤溶功能的影响。方法:选取脾肾阳虚证原发性肾病综合征患者110例,根据治疗方案分为两组,观察组和对照组各55例。对照组给予常规对症治疗,观察组在对照... 目的:探讨参芪真武汤加味治疗原发性肾病综合征效果及对患者肾功能、微炎状态、凝血纤溶功能的影响。方法:选取脾肾阳虚证原发性肾病综合征患者110例,根据治疗方案分为两组,观察组和对照组各55例。对照组给予常规对症治疗,观察组在对照组基础上给予参芪真武汤加味治疗,两组均连续治疗8周。比较两组治疗总有效率及不良反应发生情况,同时观察治疗前后中医证候积分、肾功能[24 h尿蛋白定量(24 hTP)、血白蛋白(ALB)、尿素氮(BUN)、血清肌酐(Cr)]、微炎状态[高敏-C反应蛋白(hs-CRP)、白细胞介素-21(IL-21)、白细胞介素-13(IL-13)]、凝血纤溶功能[纤维蛋白原(FIB)、D-二聚体(D-D)]变化情况。结果:观察组临床总有效率为90.91%,高于对照组的76.36%(P<0.05)。经治疗,观察组浮肿、面色白、畏寒肢冷、腰膝酸软、恶寒喜暖各项中医证候积分较对照组低(均P<0.05);观察组ALB水平较对照组高,24 hTP、BUN、Cr较对照组低(均P<0.05);观察组IL-13水平较对照组高,hs-CRP、IL-21水平较对照组低(均P<0.05)。两组FIB、D-D水平低于治疗前,且观察组低于对照组(均P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:参芪真武汤加味治疗原发性肾病综合征脾肾阳虚证可提高临床效果,降低中医证候积分,促进肾功能和凝血纤溶功能改善,减轻机体微炎症状态,且安全性佳。 展开更多

目的真武汤在治疗慢性心力衰竭(Heart Failure,HF)过程中能够改善心律失常,旨在从p38-MAPK通路调控肥大心肌结构重构和电重构角度探讨真武汤治疗HF相关心律失常的作用机制。方法用AngⅡ诱导H9c2心肌细胞构建心肌肥大模型,实验共分6组:... 目的真武汤在治疗慢性心力衰竭(Heart Failure,HF)过程中能够改善心律失常,旨在从p38-MAPK通路调控肥大心肌结构重构和电重构角度探讨真武汤治疗HF相关心律失常的作用机制。方法用AngⅡ诱导H9c2心肌细胞构建心肌肥大模型,实验共分6组:正常组(N),模型组(M)和真武汤低剂量组(D)、中剂量组(Z)、高剂量组(G)及抑制剂组(S)。正常组(N)用含有胎牛血清的高糖培养基培养,模型组(M)用含有10^(-6) mol/L浓度的血管紧张素Ⅱ(Angio-tensinⅡ,AngⅡ)培养基处理,低(D)、中(Z)、高(G)剂量组分别予2%、4%、8%体积分数的真武汤含药血清及AngⅡ培养基共同处理,抑制剂组(S)用含有p38MAPK抑制剂SB203580的培养基处理。处理48 h后,镜下观察各组心肌细胞的生长情况,通过酶联免疫吸附试验(Enzyme Linked Immunosorbent Assay,ELISA)法测心肌肥大标志物ANP的含量,用β-半乳糖苷酶染色法检测细胞衰老情况,采用Western blot法检测各组心肌细胞缝隙连接蛋白43(Connexin 43,Cx43)、p38丝裂原激活的蛋白激酶(p38 Mitogen-ctivated Protein Kinase,p38-MAPK)、基质金属蛋白酶2(Matrix Metalloproteinase 2,MMP2)、基金金属蛋白酶9(Matrix Metalloproteinase 9,MMP9)、金属蛋白酶组织抑制剂(Tissue Inhibitor of Metalloprotei-nase1,TIMP1)、p53蛋白的表达情况,采用实时荧光定量PCR(Quantitative Real-time PCR,qPCR)定量分析各组心肌细胞ANP、Cx43、p-Cx43、p38-MAPK、MMP2、MMP9、TIMP1的mRNA转录水平,免疫荧光检测Cx43蛋白的表达和分布情况。结果高剂量的真武汤能够减轻肥大心肌细胞的表面积,中低剂量无明显效果;真武汤能够降低心肌肥大标志物ANP及其mRNA的水平,且其效果与剂量呈正相关;真武汤能改善肥大细胞的衰老情况,效果与抑制剂相似;真武汤能够降低肥大心肌细胞Cx43、p38-MAPK、MMP2、MMP9、p53、p-Cx43蛋白的表达水平及Cx43、p38-MAPK、MMP2、MMP9的mRNA转录水平,且能够增加TIMP1的表达水平及其mRNA转录水平,效果与剂量有关;真武汤能够缓解肥大细胞表面Cx43的分布紊乱及减少该蛋白在细胞侧-侧移位分布。结论真武汤可能通过p38信号通路调控心肌肥大细胞的衰老和纤维化,调节细胞外基质的代谢平衡,维持心肌细胞上Cx43数量、磷酸化水平和分布定位,改善缝隙连接重构,实现对HF心肌结构重构和电重构的统一调节,达到治疗心力衰竭及相关心律失常的目的。 展开更多
