Effect of respiratory muscle training on trunk control and balance ability in stroke patients

Objective: To investigate the effect of respiratory muscle training combined with conventional rehabilitation therapy on trunk control, balance ability and activities of daily living in stroke patients. Methods: Sixty stroke patients were divided into control group and experimental group by random number table, 30 each, and received conventional rehabilitation therapy. Moreover, the experimental group underwent respiratory muscle training (RMT), for four weeks. In each patient, maximal inspiratory pressure (MIP) was measured for the inspiratory muscle strength. Maximal expiratory pressure (MEP) was measured for the expiratory muscle strength. The trunk control was measured using the Trunk Impairment Scale (TIS). The Berg balance scale (BBS) was used to assess the balance function. The modified Barthel index (MBI) was used to evaluate activities of daily living. Results: There were no significant differences in MIP, MEP, TIS, BBS and MBI between the two groups before treatment. The MIP, MEP, TIS, BBS and MBI functions of the control group and the experimental group were improved after treatment, And the functional improvement of the above indicators in the experimental group after treatment was significantly higher than that of the control group. Conclusion: This study proved that RMT improved respiratory muscle strength, trunk control, balance function, and ADL in stroke patients. 展开更多

目的探讨迷你临床演练评估(mini-clinical evaluation exercise,Mini-CEX)联合多学科综合治疗协作组(multidisciplinary team,MDT)在急诊科住院医师规范化培训中的运用效果。方法选择于医院急诊科进行规范化培训的住院医师,2022年6—8... 目的探讨迷你临床演练评估(mini-clinical evaluation exercise,Mini-CEX)联合多学科综合治疗协作组(multidisciplinary team,MDT)在急诊科住院医师规范化培训中的运用效果。方法选择于医院急诊科进行规范化培训的住院医师,2022年6—8月进行规范化培训者为传统培训组,2022年9—11月进行规范化培训者为Mini-CEX联合MDT组。传统培训组35名,给予常规教学方式教学,Mini-CEX联合MDT组35名,给予Mini-CEX联合MDT教学,教学完成后评估规范化培训住院医师的理论成绩及实际操作技能,教学前、后通过Mini-CEX考核规范化培训住院医师的问诊能力、临床诊断、操作能力、查体能力、治疗方案、沟通能力,给予评判性思维能力量表-中文版(critical thinking disposition inventory—Chinese version,CTDI-CV)、学生自主学习能力评价量表评价,比较2组教学满意度。结果Mini-CEX联合MDT组理论知识、实际操作技能评分较传统培训组高,差异有统计学意义(P<0.001);Mini-CEX联合MDT组问诊能力、临床诊断、操作能力、查体能力、治疗方案、沟通能力评分较传统培训组高,差异有统计学意义(P<0.001);Mini-CEX联合MDT组CTDI-CV评分较传统培训组低,自主学习能力评分、教学满意度较传统培训组高,差异有统计学意义(P<0.001)。Mini-CEX联合MDT组教学满意度(97.14%)较传统培训组高(85.71%),差异有统计学意义(P<0.05)。结论在急诊科住院医师规范化培训中运用Mini-CEX联合MDT教学,可提升规范化培训住院医师理论及实践操作成绩,提升其综合临床能力、评判性思维能力、自学能力及教学满意度。 展开更多

目的 探究思考-结对-分享(think-pair-share,TPS)教学模式在产科住院医师规范化培训(简称“住培”)中的应用效果。方法 选择2022年1月—2022年12月在中国医科大学附属盛京医院产科参加住院医师规范化培训的30名住院医师为研究对象,根据... 目的 探究思考-结对-分享(think-pair-share,TPS)教学模式在产科住院医师规范化培训(简称“住培”)中的应用效果。方法 选择2022年1月—2022年12月在中国医科大学附属盛京医院产科参加住院医师规范化培训的30名住院医师为研究对象,根据不同住培带教方案分为观察组与对照组,各15名。对照组采用传统住培教学模式,观察组采用TPS教学模式进行带教,对比2组住培后技能理论成绩、批判性思维能力和学员满意度。结果 观察组理论成绩[(89.71±4.67)分]和实践考核成绩得分[(90.57±6.77)分]均高于对照组[(70.63±3.75)分、(68.44±4.76)分],差异有统计学意义(P <0.001)。观察组学员批判性思维能力测量表(critical thinking dispostion inventory-Chinese version,CTDI-CV)中思维开放力、求知欲、认知成熟度和自信息等批判性思维项目评分均高于对照组,差异有统计学意义(P <0.001)。观察组学员教学后学习积极性高于对照组,差异有统计学意义(P <0.05)。观察组学员教学后对教学方法、知识掌握、技能掌握和自主学习兴趣等满意度评分均高于对照组,差异有统计学意义(P<0.001)。结论 在产科住院医师规范化培训中应用TPS模式能够提高住培医师理论和技能,增强批判性思维能力且对教学能力接受度更高。 展开更多

击剑是我国的潜优势项目,但近2届奥运会成绩出现下滑,亟须研制科学训练应对策略。通过对WebofScience、PubMed、EBSCOhost、Scopus和IEEEXplore数据库中检索到的31篇实证与定量研究文献的综合分析,并整合了目前国际国内具有较高共识性... 击剑是我国的潜优势项目,但近2届奥运会成绩出现下滑,亟须研制科学训练应对策略。通过对WebofScience、PubMed、EBSCOhost、Scopus和IEEEXplore数据库中检索到的31篇实证与定量研究文献的综合分析,并整合了目前国际国内具有较高共识性的击剑训练理论与方法。研究发现,击剑是一个高度依赖技术、速度、灵敏性、爆发力和无氧耐力的开放性运动项目。其竞技能力特征及训练策略如下:①竞技能力特征:在体能方面,精英级击剑运动员的肌肉含量存在明显的四肢不对称性,有更大的臂展和腿展,主要依赖无氧代谢能力;在技术方面,弓步及其速度、时空随机变向、冲刺(佩剑除外)和距离感、速度感掌握是关键;在战术方面,快速且准确地对特定刺激作出反应以识别并把握最佳进攻时机至关重要,此外,动作的隐蔽性和技术多样性与变化也非常关键;在心理方面,精英运动员展现出更高的抽象思维、创新能力,以及自我适应与调整能力、情绪控制能力。②训练策略:专项力量应结合快速伸缩复合、北欧式和直腿硬拉等训练方法;专项速度强调以速度、灵敏和快速启动(SAQ)训练和迁移训练来加以改善;能量训练应避免低强度的专项有氧训练(提升恢复能力的有氧训练除外),专注于契合专项的高强度间歇训练;技术训练应采用多样化的步法练习,同时提升击剑比赛特定体能,如强化离心训练;战术训练应重点提升关键分的制胜策略能力,以及通过语言、视觉和实践方法提高战术思维能力;心理训练可采用基于生物反馈技术的放松训练、表象训练和逆境应对训练等提升心理控制能力。 展开更多

认知康复训练结合S-E-T康复疗法对脑卒中后认知障碍患者认知功能、神经功能及运动能力的影响

为有针对性地提升运动员竞技能力、规避伤病以应对竞技体育职业化挑战,功能性训练被引入运动训练领域,丰富了竞技运动训练体系,但也由此导致新的理论与实践问题。对功能性训练的概念、内容进行系统梳理与审视发现:功能性训练的内涵与外... 为有针对性地提升运动员竞技能力、规避伤病以应对竞技体育职业化挑战,功能性训练被引入运动训练领域,丰富了竞技运动训练体系,但也由此导致新的理论与实践问题。对功能性训练的概念、内容进行系统梳理与审视发现:功能性训练的内涵与外延分别存在龃龉,体现为核心内涵的淡化、外延的泛化及不当分隔;引入功能性训练后出现理论困境与挑战,体现为功能性训练与传统训练概念对应失准、与经典运动训练理论架构悬殊且存在逻辑交叠以及理论发展中的概念嬗变、实践转向与价值泛化。以上问题表明,当前“功能性-传统”训练概念、理论和应用步入误区,应超越简单的“二元对立”,形成训练理论间多维聚合的发展路径。提出:顺应功能性训练与传统训练的共同理念,构建“协同互补”的运动训练理论体系;综合两者方法体系,设计“多点发力”的内容与方法库;监控训练风险,形成“互联兼合”的负荷管理与功能筛查系统。 展开更多
