AIM:To evaluate the reliability of measurements of corneal changes with accommodation in healthy eyes using a Scheimpflug imaging-based system and how these measurements distribute in the normal population.METHODS:Pro...AIM:To evaluate the reliability of measurements of corneal changes with accommodation in healthy eyes using a Scheimpflug imaging-based system and how these measurements distribute in the normal population.METHODS:Prospective,non-randomized,comparative study including 27 healthy subjects(54 eyes),including emmetropia(13 eyes),myopia(17 eyes),hyperopia(4 eyes)and astigmatism(20 eyes)groups.In all cases,a complete eye examination was performed,including the analysis of corneal changes with different accommodative stimuli(+2.00,0.00 and-3.00 D)using the Pentacam AXL system.The investigation was structured in 2 phases:repeatability analysis and characterization of accommodation-related corneal changes in healthy populations.RESULTS:In the repeatability analysis,the index of height asymmetry(IHA)showed the greatest variability with the three accommodative stimuli,being the results for the rest of parameters acceptable.The group of emmetropes showed significant differences with accommodative changes in the position of maximum keratometry(Kmax;P<0.05),whereas in the astigmatism group,significant changes were not only observed in the position of Kmax,but also in minimum corneal thickness(MCT),corneal spherical aberration,and total and low order aberration root mean square(all P<0.05).Likewise,a significant difference was found in the displacement of the X position of Kmax with+2.00 D and-3.00 D in the myopia group(P=0.033)as well as in changes with+2.00 D and-3.00 D in the magnitude of the position vector of Kmax in the emmetropia group(P<0.05).No significant changes were found between accommodative stimuli in the displacement of coordinates of MCT(P≥0.109).CONCLUSION:The position of Kmax and MCT in healthy corneas can change significantly when presenting different accommodative stimuli using the accommodation mode of the Pentacam system,with different trends in these accommodation-related corneal changes between refractive errors.Likewise,the consistency of the measurements obtained with Scheimpflug has been confirmed.展开更多
A fault accommodation zone is a type of structure that is defined as regulating displacement and strain between faults structure.Increasing numbers of fault accommodation zones are being identified in graben basins,in...A fault accommodation zone is a type of structure that is defined as regulating displacement and strain between faults structure.Increasing numbers of fault accommodation zones are being identified in graben basins,indicating the potential exploration target and petroleum accumulation areas.This study aims to analyze the formation mechanism and development of fault accommodation zones under combined stress by a numerical simulation method considering geomechanical modeling.Using three-dimensional(3-D)seismic interpretation and fractal dimension method,exampled with the Dongxin fault zone,the fault activity and fault combination pattern were conducted to quantitatively characterize the activity difference in fault accommodation zones.Combined with mechanical experiment test,a geomehcanical model was established for fault accommodation zones in a graben basin.Integrating the paleostress numerical simulations and structural physical simulation experiment,the developmental characteristics and genetic mechanism of fault accommodation zones were summarized.Influenced by multi movements and combined stresses,three significant tectonic evolution stages of the Dongxing Fault Zone(DXFZ)were distinguished:During the E_(s)^(3)sedimentary period,the large difference in the stress,strain,and rupture distribution in various faults were significant,and this stage was the key generation period for the prototype of the DXFZ,including the FAZ between large-scale faults.During the E_(s)^(2)sedimentary period,the EW-trending symmetric with opposite dipping normal faults and the NE-SW trending faults with large scale were furtherly developed.The junction area of two secondary normal faults were prone to be ruptured,performing significant period for inheriting and developing characteristics of fault accommodation zones.During the Es1 sedimentary period,the high-order faults in the DXFZ exhibited the obvious fault depressions and strike-slip activity,and the fault accommodation zones were furtherly inherited and developed.This stage was the molded and formative period of the FAZ,the low-order faults,and the depression in the DXFZ.展开更多
AIM:To investigate the biocompatibility and bacterial adhesion properties of light responsive materials(LRM)and analyze the feasibility and biosafety of employing LRM in the preparation of accommodative intraocular le...AIM:To investigate the biocompatibility and bacterial adhesion properties of light responsive materials(LRM)and analyze the feasibility and biosafety of employing LRM in the preparation of accommodative intraocular lenses(AIOLs).METHODS:Employing fundamental experimental research techniques,LRM with human lens epithelial cells(hLECs)and human retinal pigment epithelium cells(ARPE-19 cells)were co-cultured.Commercially available intraocular lenses(IOLs)were used as controls to perform cell counting kit-8(CCK-8),cell staining under varying light intensities,cell adhesion and bacterial adhesion experiments.RESULTS:LRM exhibited a stronger inhibitory effect on the proliferation of ARPE19 cells than commercially available IOLs when co-cultured with the undiluted extract for 96h(P<0.05).Under other culturing conditions,the effects on the proliferation of hLECs and ARPE-19 cells were not significantly different between the two materials.Under the influence of light irradiation at intensities of 200 and 300 mW/cm^(2),LRM demonstrated a markedly higher inhibitory effect on the survival of hLECs compared to commercially available IOLs(P<0.0001).They also showed a stronger suppressive effect on the survival rate of ARPE-19 cells,with significant differences observed at 200 mW/cm^(2)(P<0.001)and extremely significant differences at 300 mW/cm^(2)(P<0.0001).Additionally,compared to commercially available IOLs,LRM had a higher number of cells adhering to their surface(P<0.05),as well as a significantly greater number of adherent bacterium(P<0.0001).CONCLUSION:LRM,characterized by their excellent non-contact tunable deformability and low cytotoxicity to ocular tissues,show considerable potential for use in the fabrication of AIOLs.These materials demonstrate strong cell adhesion;however,during photothermal conversion processes involving shape deformation under various light intensities,the resultant temperature rise may harm surrounding cells.These factors suggest that while the material plays a positive role in reducing the incidence of posterior capsule opacification(PCO),it also poses potential risks for retinal damage.Additionally,the strong bacterial adhesion of these materials indicates an increased risk of endophthalmitis.展开更多

It’s a multi-criteria decision support system designed to...SAGeLogE is the acronym of “Système d’Aide à la Gestion de Logements étudiants” which means student accommodation management support system. It’s a multi-criteria decision support system designed to manage accommodation allocation for university students. Within SAGeLogE we developed the “Minimum of Ranks” (MIRA) aggregation method. MIRA method involves the application of multi-criteria decision-making methods (MCDM) to initially obtain different ranking. Thus, we chose the Weighted Sum Method (WSM), Technique for Order of Preference by Similarity to Ideal Solution (TOPSIS) and Combined Compromise Solution (CoCoSo) methods to be implemented in SAGeLogE. To prioritize the weights of criteria, the Anality Hierarchy Process (AHP) method was used. The interest of using the MIRA method was to obtain a compromised ranking coming from these three multi-criteria decision-making methods (WSM, TOPSIS, CoCoSo). So, we know that the accommodation to be allocated to students is always lower than the number of accommodation requests received by the organization. Thus, SAGeLogE proposed a technique that makes it easier to select students ranked by the multi-criteria decision-making and MIRA methods according to a method of calculation of quota of available accommodations within university residences. Finally, university accommodation managers had a wide choice of student ranking according to the methods implemented in SAGeLogE. The SAGeLogE experiment was carried out on the site of Toamasina University in Madagascar for an academic year with 2026 student accommodation applications received. The experimentation showed the ability of SAGeLogE to achieve the result.展开更多

Patients with liver cirrhosis exhibit several features of gut dysfunction which may contribute to the development of cirrhosis complications as well as have an impact on nutritional status and health-related quality o...Patients with liver cirrhosis exhibit several features of gut dysfunction which may contribute to the development of cirrhosis complications as well as have an impact on nutritional status and health-related quality of life.Gastrointestinal symptoms are common in cirrhosis and their pathophysiology probably involves factors related to liver disease severity,psychological distress,and gut dysfunction(e.g.,increased gastric sensitivity to distension and delayed gut transit).They may lead to reduced food intake and,thus,may contribute to the nutritional status deterioration in cirrhotic patients.Although tense ascites appears to have a negative impact on meal-induced accommodation of the stomach,published data on gastric accommodation in cirrhotics without significant ascites are not unanimous.Gastric emptying and small bowel transit have generally been shown to be prolonged.This may be related to disturbances in postprandial glucose,insulin,and ghrelin levels,which,in turn,appear to be associated to insulin resistance,a common finding in cirrhosis.Furthermore,small bowel manometry disturbances and delayed gut transit may be associated with the development of small bowel bacterial overgrowth.Finally,several studies have reported intestinal barrier dysfunction in patients with cirrhosis(especially those with portal hypertension),which is related to bacterial translocation and permeation of intestinal bacterial products,e.g.,endo-toxin and bacterial DNA,thus potentially being involved in the pathogenesis of complications of liver cirrhosis.展开更多

The gastric accommodation process is a complex phenomenon that describes how the size of the gastric compartment changes in response to a meal. The electronic barostat is considered the gold standard in assessing gastric accommodation. Imaging methods, including MRI, SPECT, and ultrasonography may also be used, particularly in patients who are stress-responsive, e.g. functional dyspepsia patients, as a non-invasive and less stress-inducing method is favourable. METHODS: A prospective, non-randomized, case-control study was performed from September to October 2016. Eighty-three children with myopia were divided into two groups. One group was treated with the rigid contact lens for overnight Ortho-k, and the other was treated with single-vision spectacle lens(SVL). Accommodation function were assessed by accommodative amplitude(AA), accommodative sensitivity(AS), accommodative lag(Lag), negative relative accommodation(NRA), and positive relative accommodation(PRA) before and 1, 3, 6 and 12 mo after treated. RESULTS: Totally 72 myopic children were finished the follow-up: 37 in Ortho-k group and 35 in SVL group. Wearing time had a significant effect on AA, AS, Lag, and NRA of myopic children in two groups(all P〈0.05). Meanwhile, there was an interaction effect between wearing time and wearing types(FAA=5.3, FAS=45.5, FLag=7.0, FNRA=3.7, all P〈0.05). However, the between-group difference of AA(F=0.1), AS(F=3.2), Lag(F=1.1), NRA(F=0.3), and PRA(F=0.1) showed no significance. AA, AS, NRA, Lag and PRA were improved significantly in Ortho-k group within 1 to 6 mo after wearing. CONCLUSION: Wearing Ortho-k can improve the accommodation function of myopic children, which might be one of the mechanisms for myopia control.展开更多

METHODS: Between April 2011 and March 2012, 45 patients with FD and...AIM: To assess the value of ultrasonography (US) in evaluation of proximal gastric accommodation disorder in patients with functional dyspepsia (FD). METHODS: Between April 2011 and March 2012, 45 patients with FD and 27 healthy volunteers were enrolled in this study. Two-dimensional ultrasound (2DUS) and 3-dimensional ultrasound (3DUS) were performed sequentially to measure proximal gastric area (PGA), maximal proximal gastric diameter (MPGD), and proximal gastric volume (PGV). These values were measured separately in the two groups every other 5 min for a duration of 25 min after the beginning of ingestion of a test meal. Air pocket grading was done separately for images of 2DUS and blocks of 3DUS obtained at five scanning time points. RESULTS: Both PGA and PGV of patients were significantly smaller than healthy controls (P = 0.000 and 0.002, respectively). Comparing the two parameters between the groups at each time point, the differences were also statistically significant (P = 0.000-0.013), except at 10 min for the PGV (P = 0.077). However, no overall difference was found between the groups in the MPGD measurements (P = 0.114), though it was statistically significant at a 20-minute examination point (P = 0.026). A total of 360 sets or blocks of images were obtained for both 2DUS and 3DUS. For the images analyzed by 2DUS, none were excluded because of gastric gas, and 50 (13.9%) and 310 (86.1%) sets were determined as air pockets grades 1 and 2, respectively. For the images analyzed by 3DUS, 23 (6.4%) blocks were excluded from the measurement due to presence of a large fundus air pocket (grade 3); fifty (13.9%) and 287 (79.7%) blocks were also graded as 1 and 2, respectively. CONCLUSION: Measurement of both PGA and PGV by 2DUS and 3DUS could be useful for assessment of the proximal gastric accommodation.展开更多

There is overwhelming evidence that functional gastrointestinal disorders(FGIDs) are associated with specific mechanisms that constitute important targets for personalized treatment. There are specific mechanisms in p...There is overwhelming evidence that functional gastrointestinal disorders(FGIDs) are associated with specific mechanisms that constitute important targets for personalized treatment. There are specific mechanisms in patients presenting with functional upper gastrointestinal symptoms(UGI Sx). Among patients with UGI Sx, approximately equal proportions(25%) of patients have delayed gastric emptying(GE), reduced gastric accommodation(GA), both impaired GE and GA,or neither, presumably due to increased gastric or duodenal sensitivity.Treatments targeted to the underlying pathophysiology utilize prokinetics,gastric relaxants, or central neuromodulators. Similarly, specific mechanisms in patients presenting with functional lower gastrointestinal symptoms, especially with diarrhea or constipation, are recognized, including at least 30% of patients with functional constipation pelvic floor dyssynergia and 5% has colonic inertia(with neural or interstitial cells of Cajal loss in myenteric plexus); 25% of patients with diarrhea-predominant irritable bowel syndrome(IBSD) has evidence of bile acid diarrhea; and, depending on ethnicity, a varying proportion of patients has disaccharidase deficiency, and less often sucrose-isomaltase deficiency. Among patients with predominant pain or bloating, the role of fermentable oligosaccharides, disaccharides, monosaccharides and polyols should be considered. Personalization is applied through pharmacogenomics related to drug pharmacokinetics, specifically the role of CYP2 D6, 2 C19 and 3 A4 in the use of drugs for treatment of patients with FGIDs. Single mutations or multiple genetic variants are relatively rare, with limited impact to date on the understanding or treatment of FGIDs. The role of mucosal gene expression in FGIDs, particularly in IBS-D, is the subject of ongoing research. In summary, the time for personalization of FGIDs, based on deep phenotyping, is here;pharmacogenomics is relevant in the use of central neuromodulators. There is still unclear impact of the role of genetics in the management of FGIDs.展开更多

METHODSRandomized controlled trial was conducted to check the effect of itopride on gastric accommodation, gastric emptying, capacity of tolerating nutrient liquid and symptoms of FD. We recruited a total of 31 patients having FD on the basis of ROME III criteria. After randomization, itopride was received by 15 patients while 16 patients received placebo. Gastric accommodation was determined using Gastric Scintigraphy. <sup>13</sup>C labeled octanoic breadth test was performed to assess gastric emptying. Capacity of tolerating nutrient liquid drink was checked using satiety drinking capacity test. The intervention group comprised of 150 mg itopride. Patients in both arms were followed for 4 wk. RESULTSMean age of the recruited participant 33 years (SD = 7.6) and most of the recruited individuals, i.e., 21 (67.7%) were males. We found that there was no effect of itopride on gastric accommodation as measured at different in volumes in the itopride and control group with the empty stomach (P = 0.14), at 20 min (P = 0.38), 30 min (P = 0.30), 40 min (P = 0.43), 50 min (P = 0.50), 60 min (P = 0.81), 90 min (P = 0.25) and 120 min (P = 0.67). Gastric emptying done on a sub sample (n = 11) showed no significant difference (P = 0.58) between itopride and placebo group. There was no significant improvement in the capacity to tolerate liquid in the itopride group as compared to placebo (P = 0.51). Similarly there was no significant improvement of symptoms as assessed through a composite symptom score (P = 0.74). The change in QT interval in itopride group was not significantly different from placebo (0.10). CONCLUSIONOur study found no effect of itopride on gastric accommodation, gastric emptying and maximum tolerated volume in patients with FD.展开更多
AIM:To explore the photopic pupil size behavior in myopic children undergoing overnight orthokeratology(ortho-k)over 1-year period and its effects on the axial elongation.METHODS:A total of 202 Chinese myopic children...AIM:To explore the photopic pupil size behavior in myopic children undergoing overnight orthokeratology(ortho-k)over 1-year period and its effects on the axial elongation.METHODS:A total of 202 Chinese myopic children were enrolled in this prospective clinical trial.Ninetyfive subjects in ortho-k group and eighty-eight subjects in spectacle group completed the 1-year study.Axial length(AL)was measured before enrollment and every 6mo after the start of ortho-k.The photopic pupil diameter(PPD)was determined using the Pentacam AXL and measured in an examination room with lighting of 300-310 Lx.Stepwise multiple linear regression analysis was used to identify variables contribution to axial elongation.RESULTS:Compared with spectacle group,the average 1-year axial elongation was significantly slower in the ortho-k group(0.25±0.27 vs 0.44±0.23 mm,P<0.0001).In ortho-k group,PPDs significantly decreased from 4.21±0.62 mm to 3.94±0.53 mm after 1mo of lens wear(P=0.001,Bonferroni correction)and the change lasts for 3-month visit.No significantly change during the other follow-up visits was found(P>0.05,Bonferroni correction).The 4.81 mm PPD may be a possible cutoff point in the ortho-k group.Subjects with PPD below or equal to 4.81 mm tended to have smaller axial elongation compared to subjects with PPD above 4.81 mm after 1-year period(t=-3.09,P=0.003).In ortho-k group,univariate analyses indicated that those with older age,greater degree of myopia,longer AL,smaller baseline PPD(PPDbaseline)experienced a smaller change in AL.In multivariate analyses,older age,greater AL and smaller PPDbaseline were associated with smaller increases in AL.In spectacle group,PPD tended to be stable(P>0.05,Bonferroni correction)and did not affect axial growth.CONCLUSION:PPDs experience significantly decreases at 1-month and 3-month ortho-k treatment.Children with smaller PPD tend to experience slower axial elongation and may benefit more from ortho-k.展开更多

This paper observes and compares the surgical efficacy of three incisions of different length for phacoemulsification to identify the optimal method for cataract surgery. Ninety patients were enrolled in the present study and divided into three groups. The 1.8-mm group received Bausch & Lomb MI60 foldable intraocular lens (IOL) implantation (n=30), 3.2-mm group received Bausch & Lomb Akreos AO foldable lens implantation (n=30), and 5.5-mm group received Alcon TYPE 05 rigid IOL implantation (n=30). Visual acuity, Oculyzer-based anterior segment analysis, and corneal endothelial cell count before surgery, and 3, 7, 30, and 90d after surgery were recorded and compared. Pseudophakic accommodation three days, one week, one month, and three months after surgery was determined. Intraoperative ultrasound time and ultrasonic energy were recorded. It was finally concluded that for phacoemulsification with the same phaco tip, a 1.8-mm microincision can lead to quicker recovery of visual acuity, more stable astigmatism, and higher pseudophakic accommodation than conventional incision.展开更多

文摘AIM:To evaluate the reliability of measurements of corneal changes with accommodation in healthy eyes using a Scheimpflug imaging-based system and how these measurements distribute in the normal population.METHODS:Prospective,non-randomized,comparative study including 27 healthy subjects(54 eyes),including emmetropia(13 eyes),myopia(17 eyes),hyperopia(4 eyes)and astigmatism(20 eyes)groups.In all cases,a complete eye examination was performed,including the analysis of corneal changes with different accommodative stimuli(+2.00,0.00 and-3.00 D)using the Pentacam AXL system.The investigation was structured in 2 phases:repeatability analysis and characterization of accommodation-related corneal changes in healthy populations.RESULTS:In the repeatability analysis,the index of height asymmetry(IHA)showed the greatest variability with the three accommodative stimuli,being the results for the rest of parameters acceptable.The group of emmetropes showed significant differences with accommodative changes in the position of maximum keratometry(Kmax;P<0.05),whereas in the astigmatism group,significant changes were not only observed in the position of Kmax,but also in minimum corneal thickness(MCT),corneal spherical aberration,and total and low order aberration root mean square(all P<0.05).Likewise,a significant difference was found in the displacement of the X position of Kmax with+2.00 D and-3.00 D in the myopia group(P=0.033)as well as in changes with+2.00 D and-3.00 D in the magnitude of the position vector of Kmax in the emmetropia group(P<0.05).No significant changes were found between accommodative stimuli in the displacement of coordinates of MCT(P≥0.109).CONCLUSION:The position of Kmax and MCT in healthy corneas can change significantly when presenting different accommodative stimuli using the accommodation mode of the Pentacam system,with different trends in these accommodation-related corneal changes between refractive errors.Likewise,the consistency of the measurements obtained with Scheimpflug has been confirmed.

文摘A fault accommodation zone is a type of structure that is defined as regulating displacement and strain between faults structure.Increasing numbers of fault accommodation zones are being identified in graben basins,indicating the potential exploration target and petroleum accumulation areas.This study aims to analyze the formation mechanism and development of fault accommodation zones under combined stress by a numerical simulation method considering geomechanical modeling.Using three-dimensional(3-D)seismic interpretation and fractal dimension method,exampled with the Dongxin fault zone,the fault activity and fault combination pattern were conducted to quantitatively characterize the activity difference in fault accommodation zones.Combined with mechanical experiment test,a geomehcanical model was established for fault accommodation zones in a graben basin.Integrating the paleostress numerical simulations and structural physical simulation experiment,the developmental characteristics and genetic mechanism of fault accommodation zones were summarized.Influenced by multi movements and combined stresses,three significant tectonic evolution stages of the Dongxing Fault Zone(DXFZ)were distinguished:During the E_(s)^(3)sedimentary period,the large difference in the stress,strain,and rupture distribution in various faults were significant,and this stage was the key generation period for the prototype of the DXFZ,including the FAZ between large-scale faults.During the E_(s)^(2)sedimentary period,the EW-trending symmetric with opposite dipping normal faults and the NE-SW trending faults with large scale were furtherly developed.The junction area of two secondary normal faults were prone to be ruptured,performing significant period for inheriting and developing characteristics of fault accommodation zones.During the Es1 sedimentary period,the high-order faults in the DXFZ exhibited the obvious fault depressions and strike-slip activity,and the fault accommodation zones were furtherly inherited and developed.This stage was the molded and formative period of the FAZ,the low-order faults,and the depression in the DXFZ.

文摘AIM:To investigate the biocompatibility and bacterial adhesion properties of light responsive materials(LRM)and analyze the feasibility and biosafety of employing LRM in the preparation of accommodative intraocular lenses(AIOLs).METHODS:Employing fundamental experimental research techniques,LRM with human lens epithelial cells(hLECs)and human retinal pigment epithelium cells(ARPE-19 cells)were co-cultured.Commercially available intraocular lenses(IOLs)were used as controls to perform cell counting kit-8(CCK-8),cell staining under varying light intensities,cell adhesion and bacterial adhesion experiments.RESULTS:LRM exhibited a stronger inhibitory effect on the proliferation of ARPE19 cells than commercially available IOLs when co-cultured with the undiluted extract for 96h(P<0.05).Under other culturing conditions,the effects on the proliferation of hLECs and ARPE-19 cells were not significantly different between the two materials.Under the influence of light irradiation at intensities of 200 and 300 mW/cm^(2),LRM demonstrated a markedly higher inhibitory effect on the survival of hLECs compared to commercially available IOLs(P<0.0001).They also showed a stronger suppressive effect on the survival rate of ARPE-19 cells,with significant differences observed at 200 mW/cm^(2)(P<0.001)and extremely significant differences at 300 mW/cm^(2)(P<0.0001).Additionally,compared to commercially available IOLs,LRM had a higher number of cells adhering to their surface(P<0.05),as well as a significantly greater number of adherent bacterium(P<0.0001).CONCLUSION:LRM,characterized by their excellent non-contact tunable deformability and low cytotoxicity to ocular tissues,show considerable potential for use in the fabrication of AIOLs.These materials demonstrate strong cell adhesion;however,during photothermal conversion processes involving shape deformation under various light intensities,the resultant temperature rise may harm surrounding cells.These factors suggest that while the material plays a positive role in reducing the incidence of posterior capsule opacification(PCO),it also poses potential risks for retinal damage.Additionally,the strong bacterial adhesion of these materials indicates an increased risk of endophthalmitis.
文摘The results of scientific studies of human social facts in the field of health show that the management of a patient should involve the patient’s entourage,whatever the status or size of the health establishment.In healthcare establishments in the Congo,the following are recognised as being responsible for medical care:specialist doctors,doctors,midwives,nurses and care assistants.The patient’s family and close friends are responsible for looking after the patient and financing care.The hospital infrastructure does not provide any space for the patient warden who accompany the patient during reception and hospitalisation.This makes Congolese hospitals inefficient for patient care.How can we integrate the function of the Sick guard and the assistance of the family,in order to reduce the mortality rate and repair the harm caused to patients requiring the presence of relatives during their stay in hospital,which is considered to be a dangerous place?This article examines the functional principles for configuring the space that patient warden would occupy in the patient care system.On the basis of a documentary analysis of sociological and architectural studies of existing facilities,this article proposes a typical accommodation model with the spaces needed to ensure the well-being and effectiveness of the patient warden with the patient.These are rooms with minimum space for 2 to 4 individual beds,equipped with toilets and showers.The accommodation has a dining area,kitchen and laundry facilities.In the future,this accommodation will become part of the hospital estate and may be occupied by orderlies and patient warden recruited by the hospital administration.
文摘SAGeLogE is the acronym of “Système d’Aide à la Gestion de Logements étudiants” which means student accommodation management support system. It’s a multi-criteria decision support system designed to manage accommodation allocation for university students. Within SAGeLogE we developed the “Minimum of Ranks” (MIRA) aggregation method. MIRA method involves the application of multi-criteria decision-making methods (MCDM) to initially obtain different ranking. Thus, we chose the Weighted Sum Method (WSM), Technique for Order of Preference by Similarity to Ideal Solution (TOPSIS) and Combined Compromise Solution (CoCoSo) methods to be implemented in SAGeLogE. To prioritize the weights of criteria, the Anality Hierarchy Process (AHP) method was used. The interest of using the MIRA method was to obtain a compromised ranking coming from these three multi-criteria decision-making methods (WSM, TOPSIS, CoCoSo). So, we know that the accommodation to be allocated to students is always lower than the number of accommodation requests received by the organization. Thus, SAGeLogE proposed a technique that makes it easier to select students ranked by the multi-criteria decision-making and MIRA methods according to a method of calculation of quota of available accommodations within university residences. Finally, university accommodation managers had a wide choice of student ranking according to the methods implemented in SAGeLogE. The SAGeLogE experiment was carried out on the site of Toamasina University in Madagascar for an academic year with 2026 student accommodation applications received. The experimentation showed the ability of SAGeLogE to achieve the result.

文摘Patients with liver cirrhosis exhibit several features of gut dysfunction which may contribute to the development of cirrhosis complications as well as have an impact on nutritional status and health-related quality of life.Gastrointestinal symptoms are common in cirrhosis and their pathophysiology probably involves factors related to liver disease severity,psychological distress,and gut dysfunction(e.g.,increased gastric sensitivity to distension and delayed gut transit).They may lead to reduced food intake and,thus,may contribute to the nutritional status deterioration in cirrhotic patients.Although tense ascites appears to have a negative impact on meal-induced accommodation of the stomach,published data on gastric accommodation in cirrhotics without significant ascites are not unanimous.Gastric emptying and small bowel transit have generally been shown to be prolonged.This may be related to disturbances in postprandial glucose,insulin,and ghrelin levels,which,in turn,appear to be associated to insulin resistance,a common finding in cirrhosis.Furthermore,small bowel manometry disturbances and delayed gut transit may be associated with the development of small bowel bacterial overgrowth.Finally,several studies have reported intestinal barrier dysfunction in patients with cirrhosis(especially those with portal hypertension),which is related to bacterial translocation and permeation of intestinal bacterial products,e.g.,endo-toxin and bacterial DNA,thus potentially being involved in the pathogenesis of complications of liver cirrhosis.
文摘Gastric accommodation is important for the understanding of the pathophysiology in functional dyspepsia and is also relevant for symptom generation in other disorders. The term gastric accommodation has at least three different meanings: The accommodation process, the accommodation reflex, and the accommodation response. The gastric accommodation process is a complex phenomenon that describes how the size of the gastric compartment changes in response to a meal. The electronic barostat is considered the gold standard in assessing gastric accommodation. Imaging methods, including MRI, SPECT, and ultrasonography may also be used, particularly in patients who are stress-responsive, e.g. functional dyspepsia patients, as a non-invasive and less stress-inducing method is favourable. Ultrasonography satisfies these criteria as it does not by itself distort the physiological response in stress-responsive individuals.

文摘AIM: To study effects of orthokeratology(Ortho-k) on accommodation function in myopic children. METHODS: A prospective, non-randomized, case-control study was performed from September to October 2016. Eighty-three children with myopia were divided into two groups. One group was treated with the rigid contact lens for overnight Ortho-k, and the other was treated with single-vision spectacle lens(SVL). Accommodation function were assessed by accommodative amplitude(AA), accommodative sensitivity(AS), accommodative lag(Lag), negative relative accommodation(NRA), and positive relative accommodation(PRA) before and 1, 3, 6 and 12 mo after treated. RESULTS: Totally 72 myopic children were finished the follow-up: 37 in Ortho-k group and 35 in SVL group. Wearing time had a significant effect on AA, AS, Lag, and NRA of myopic children in two groups(all P〈0.05). Meanwhile, there was an interaction effect between wearing time and wearing types(FAA=5.3, FAS=45.5, FLag=7.0, FNRA=3.7, all P〈0.05). However, the between-group difference of AA(F=0.1), AS(F=3.2), Lag(F=1.1), NRA(F=0.3), and PRA(F=0.1) showed no significance. AA, AS, NRA, Lag and PRA were improved significantly in Ortho-k group within 1 to 6 mo after wearing. CONCLUSION: Wearing Ortho-k can improve the accommodation function of myopic children, which might be one of the mechanisms for myopia control.

文摘AIM: To assess the value of ultrasonography (US) in evaluation of proximal gastric accommodation disorder in patients with functional dyspepsia (FD). METHODS: Between April 2011 and March 2012, 45 patients with FD and 27 healthy volunteers were enrolled in this study. Two-dimensional ultrasound (2DUS) and 3-dimensional ultrasound (3DUS) were performed sequentially to measure proximal gastric area (PGA), maximal proximal gastric diameter (MPGD), and proximal gastric volume (PGV). These values were measured separately in the two groups every other 5 min for a duration of 25 min after the beginning of ingestion of a test meal. Air pocket grading was done separately for images of 2DUS and blocks of 3DUS obtained at five scanning time points. RESULTS: Both PGA and PGV of patients were significantly smaller than healthy controls (P = 0.000 and 0.002, respectively). Comparing the two parameters between the groups at each time point, the differences were also statistically significant (P = 0.000-0.013), except at 10 min for the PGV (P = 0.077). However, no overall difference was found between the groups in the MPGD measurements (P = 0.114), though it was statistically significant at a 20-minute examination point (P = 0.026). A total of 360 sets or blocks of images were obtained for both 2DUS and 3DUS. For the images analyzed by 2DUS, none were excluded because of gastric gas, and 50 (13.9%) and 310 (86.1%) sets were determined as air pockets grades 1 and 2, respectively. For the images analyzed by 3DUS, 23 (6.4%) blocks were excluded from the measurement due to presence of a large fundus air pocket (grade 3); fifty (13.9%) and 287 (79.7%) blocks were also graded as 1 and 2, respectively. CONCLUSION: Measurement of both PGA and PGV by 2DUS and 3DUS could be useful for assessment of the proximal gastric accommodation.

文摘With the ever-increased installed capacity of renewable energy generation units in a power system,the so-called shared energy storage(SES),a novel business model under the umbrella of the shared economy principle,has the potential to play an essential role in the accommodation of renewable energy generation.However,unified evaluation standards and methods,which can help decision-makers analyze the performance of the SES market,are still not available.In this paper,an evaluation index system of the SES market is designed based on the trading rules of China’s Qinghai province and the structure-conduct-performance(SCP)analytical model.Moreover,the definition and characteristics of the indices,which can show the performance of the SES market from different perspectives,are given.Furthermore,the ideal cases are presented as the evaluation benchmark based on the development expectation of the SES market,and the analytic hierarchy process(AHP)and the technique for order preference by similarity to an ideal solution(TOPSIS)are applied to evaluate the SES market comprehensively.Finally,a case study based on actual data of the SES trading pilot project in Qinghai shows that the evaluation index system can reflect the operation status,existing problems and influencing factors of the SES market.
文摘There is overwhelming evidence that functional gastrointestinal disorders(FGIDs) are associated with specific mechanisms that constitute important targets for personalized treatment. There are specific mechanisms in patients presenting with functional upper gastrointestinal symptoms(UGI Sx). Among patients with UGI Sx, approximately equal proportions(25%) of patients have delayed gastric emptying(GE), reduced gastric accommodation(GA), both impaired GE and GA,or neither, presumably due to increased gastric or duodenal sensitivity.Treatments targeted to the underlying pathophysiology utilize prokinetics,gastric relaxants, or central neuromodulators. Similarly, specific mechanisms in patients presenting with functional lower gastrointestinal symptoms, especially with diarrhea or constipation, are recognized, including at least 30% of patients with functional constipation pelvic floor dyssynergia and 5% has colonic inertia(with neural or interstitial cells of Cajal loss in myenteric plexus); 25% of patients with diarrhea-predominant irritable bowel syndrome(IBSD) has evidence of bile acid diarrhea; and, depending on ethnicity, a varying proportion of patients has disaccharidase deficiency, and less often sucrose-isomaltase deficiency. Among patients with predominant pain or bloating, the role of fermentable oligosaccharides, disaccharides, monosaccharides and polyols should be considered. Personalization is applied through pharmacogenomics related to drug pharmacokinetics, specifically the role of CYP2 D6, 2 C19 and 3 A4 in the use of drugs for treatment of patients with FGIDs. Single mutations or multiple genetic variants are relatively rare, with limited impact to date on the understanding or treatment of FGIDs. The role of mucosal gene expression in FGIDs, particularly in IBS-D, is the subject of ongoing research. In summary, the time for personalization of FGIDs, based on deep phenotyping, is here;pharmacogenomics is relevant in the use of central neuromodulators. There is still unclear impact of the role of genetics in the management of FGIDs.

文摘AIMTo study the effect of itopride on gastric accommodation, gastric emptying and drinking capacity in functional dyspepsia (FD). METHODSRandomized controlled trial was conducted to check the effect of itopride on gastric accommodation, gastric emptying, capacity of tolerating nutrient liquid and symptoms of FD. We recruited a total of 31 patients having FD on the basis of ROME III criteria. After randomization, itopride was received by 15 patients while 16 patients received placebo. Gastric accommodation was determined using Gastric Scintigraphy. <sup>13</sup>C labeled octanoic breadth test was performed to assess gastric emptying. Capacity of tolerating nutrient liquid drink was checked using satiety drinking capacity test. The intervention group comprised of 150 mg itopride. Patients in both arms were followed for 4 wk. RESULTSMean age of the recruited participant 33 years (SD = 7.6) and most of the recruited individuals, i.e., 21 (67.7%) were males. We found that there was no effect of itopride on gastric accommodation as measured at different in volumes in the itopride and control group with the empty stomach (P = 0.14), at 20 min (P = 0.38), 30 min (P = 0.30), 40 min (P = 0.43), 50 min (P = 0.50), 60 min (P = 0.81), 90 min (P = 0.25) and 120 min (P = 0.67). Gastric emptying done on a sub sample (n = 11) showed no significant difference (P = 0.58) between itopride and placebo group. There was no significant improvement in the capacity to tolerate liquid in the itopride group as compared to placebo (P = 0.51). Similarly there was no significant improvement of symptoms as assessed through a composite symptom score (P = 0.74). The change in QT interval in itopride group was not significantly different from placebo (0.10). CONCLUSIONOur study found no effect of itopride on gastric accommodation, gastric emptying and maximum tolerated volume in patients with FD.

文摘AIM:To explore the photopic pupil size behavior in myopic children undergoing overnight orthokeratology(ortho-k)over 1-year period and its effects on the axial elongation.METHODS:A total of 202 Chinese myopic children were enrolled in this prospective clinical trial.Ninetyfive subjects in ortho-k group and eighty-eight subjects in spectacle group completed the 1-year study.Axial length(AL)was measured before enrollment and every 6mo after the start of ortho-k.The photopic pupil diameter(PPD)was determined using the Pentacam AXL and measured in an examination room with lighting of 300-310 Lx.Stepwise multiple linear regression analysis was used to identify variables contribution to axial elongation.RESULTS:Compared with spectacle group,the average 1-year axial elongation was significantly slower in the ortho-k group(0.25±0.27 vs 0.44±0.23 mm,P<0.0001).In ortho-k group,PPDs significantly decreased from 4.21±0.62 mm to 3.94±0.53 mm after 1mo of lens wear(P=0.001,Bonferroni correction)and the change lasts for 3-month visit.No significantly change during the other follow-up visits was found(P>0.05,Bonferroni correction).The 4.81 mm PPD may be a possible cutoff point in the ortho-k group.Subjects with PPD below or equal to 4.81 mm tended to have smaller axial elongation compared to subjects with PPD above 4.81 mm after 1-year period(t=-3.09,P=0.003).In ortho-k group,univariate analyses indicated that those with older age,greater degree of myopia,longer AL,smaller baseline PPD(PPDbaseline)experienced a smaller change in AL.In multivariate analyses,older age,greater AL and smaller PPDbaseline were associated with smaller increases in AL.In spectacle group,PPD tended to be stable(P>0.05,Bonferroni correction)and did not affect axial growth.CONCLUSION:PPDs experience significantly decreases at 1-month and 3-month ortho-k treatment.Children with smaller PPD tend to experience slower axial elongation and may benefit more from ortho-k.
文摘Phacoemulsification is a commonly used surgical method in cataract surgery. This paper observes and compares the surgical efficacy of three incisions of different length for phacoemulsification to identify the optimal method for cataract surgery. Ninety patients were enrolled in the present study and divided into three groups. The 1.8-mm group received Bausch & Lomb MI60 foldable intraocular lens (IOL) implantation (n=30), 3.2-mm group received Bausch & Lomb Akreos AO foldable lens implantation (n=30), and 5.5-mm group received Alcon TYPE 05 rigid IOL implantation (n=30). Visual acuity, Oculyzer-based anterior segment analysis, and corneal endothelial cell count before surgery, and 3, 7, 30, and 90d after surgery were recorded and compared. Pseudophakic accommodation three days, one week, one month, and three months after surgery was determined. Intraoperative ultrasound time and ultrasonic energy were recorded. It was finally concluded that for phacoemulsification with the same phaco tip, a 1.8-mm microincision can lead to quicker recovery of visual acuity, more stable astigmatism, and higher pseudophakic accommodation than conventional incision.
