Low risk of postoperative ulnar nerve affection in surgically treated distal humeral fractures when the nerve is released in situ

BACKGROUND Adult distal humeral fractures(DHF)comprise 2%-5%of all fractures and 30%of all elbow fractures.Treatment of DHF may be technically demanding due to fracture complexity and proximity of neurovascular structures.Open reduction and internal fixation(ORIF)are often the treatment of choice,but arthroplasty is considered in case of severe comminution or in elderly patients with poor bone quality.Ulnar nerve affection following surgical treatment of distal humerus fractures is a well-recognized complication.AIM To report the risk of ulnar nerve affection after surgery for acute DHFs.METHODS We retrospectively identified 239 consecutive adult patients with acute DHFs who underwent surgery with ORIF,elbow hemiarthroplasty(EHA)or total elbow arthroplasty(TEA)between January 2011 and December 2019.In all cases,the ulnar nerve was released in situ without anterior transposition.We used our institutional database to review patients’medical records for demographics,fracture morphology,type of surgery and ulnar nerve affection immediately;records were reviewed after surgery and at 2 wk and 12 wk of routine clinical outpatient follow-up.Twenty-nine percent patients were excluded due to pre-or postoperative conditions.Final follow-up examination was a telephone interview in which ulnar nerve affection was reported according to the McGowen Classification Score.A total of 210 patients were eligible for interview,but 13 patients declined participation and 17 patients failed to respond.Thus,180 patients were included.RESULTS Mean age at surgery was 64 years(range 18-88 years);121(67.3%)patients were women;59(32.7%)were men.According to the AO/OTA classification system,we recorded 47 patients with type A3,55 patients with type B and 78 patients with type C fractures.According to the McGowen Classification Score,mild ulnar nerve affection was reported in nine patients;severe affection,in two.A total of 69 patients were treated with ORIF of whom three had mild temporary ulnar nerve affection and one had severe ulnar nerve affection.In all,111 patients were treated with arthroplasty(67 EHA,44 TEA)of whom seven had mild ulnar nerve affection and one had severe persistent ulnar nerve affection.No further treatment was provided.CONCLUSION The risk of ulnar nerve affection after surgical treatment for acute DHF is low when the ulnar nerve is released in situ without nerve transposition,independently of the treatment provided.

Urological Affections after Laparoscopic Hernia Repair in Long-Term Follow up

Objectives: Chronic pain is a severe complication of mesh-based inguinal hernia repair. Pain upon ejaculation, testicular touch sensitivity and dysuria are apparent. Regarding the large amount of patients undergoing laparoscopic hernia repair, the problem seems quite evident. In this prospective, clinical, randomized, double-blind study we intended to investigate the biocompatibility of three different meshes and their influence on urological affections after operative procedure. Methods: 180 male patients with primary inguinal hernia undergoing TAPP were randomized for using a heavyweight (108 g/m2), double-filament PP mesh (Prolene, 10 9 15 cm, group A, n = 60), a multifilament, heavyweight variant (116 g/m2) of PP mesh (Serapren, 10 9 15 cm, group B, n = 60), or a composite mesh (polyglactin and PP) (Vypro II, 10 9 15 cm, group C, n = 60). We compared in terms of complications (seromas, recurrence rate), urological affections and life quality (SF-36 Health Survey). The follow-up period was 60 months. Results: Convalescence in group A was slower than in groups B and C: mean-term values of the visual scales for pain development were significantly (p th week postoperatively. There were no significant differences between groups B and C. Beyond the 12th post-interventional week the differences diminished. Conclusions: Independent which kind of mesh was implanted still 5% of patients suffered from urological affections 60 month later.

Correlation between Depth of Myometrial Invasion and Degree of Lymph Node Affection in Cases of Endometrial Cancer

Introduction: Endometrial cancer is the fourth most frequent cancer in females. Many factors can affect prognosis of this type of cancer, these mainly are the degree of myometrial invasion by the tumour, pelvic and paraaortic lymph node spread as well as the tumour histological type (endometrioid vs non-endometrioid type). transvaginal ultrasound (TVS) is a highly accurate and easy method for preoperative evaluation of myometrial invasion. Aim of the Work: The aim of this work is to assess if there is relation between the depth of myometrial invasion by the tumor and the rate of lymph node involvement in cases of endometrial cancer. Results: It was found that there was a significant relation between lymph node affection and the depth of myometrial invasion, all the positive lymph node affections cases had myomterial invasion > 50.0%. Conclusion: The incidence of pelvic lymph node affection is very high in cases where the myometrium is deeply infiltrated with the tumor. Assessment of myometrial invasion preoperatively by TVU and microscopically by pathological examination of the myometrium after hysterectomy provides an accurate estimation of the rate of pelvic lymph node affection and hence necessitates lymphadenectomy procedures in cases where myometrium is deeply infiltrated by the tumor and omitted in cases where it is tumor free.

CLINICAL ANALYSIS ON TREATMENT OF LUMBAR VERTEBRAL RETROGRADE AFFECTION WITH ACUPUNCTURE,TDP-IRRADIATION AND CUPPING

In the present paper, 70 cases of retrograde affection of the lumbar vertebrae are randomly and evenly divided into treatment group and control group. Shenshu (BL 23), Qihaishu (BL 24), Dachangshu (BL 25), Weizhong (BL 40), Jiaji (EX B 2), etc. are used. In treatment group, acupuncture, TDP irradiation and cupping are performed, and in control group, only acupuncture is given. After 30 sessions (3 therapeutic courses) of treatment, in treatment and control groups, the total effective rates are 91.43% and 71.43% respectively. The therapeutic effect of comprehensive treatment is significantly superior to that of simple acupuncture therapy (P<0.05).

The Life Course of a Long-Lasting Affection Medium:A Fieldwork Based on the Communication Practice of the Rural Loudspeakers in North China

The loudspeakers played a crucial role in rural community during the COVID-19 pandemic,and had returned to the public with the help of short video platforms.However,with the continued popularity of digital media,is the revival of the loudspeakers which belong to traditional media just a flash in the pan?What the historic process of return has the loudspeakers undergone?Based on these questions,the paper applies Bourdieu’s sense of practice theory,focuses on“habit”throughout,adopts the research methods of online ethnography and fieldwork,and conducts participant observation and targeted in-depth interviews in a village in North China for about nine months.The study found that:(1)There is the lag of habit in the state’s handling of social relations in villages.Based on rational habit,village cadres used the loudspeakers to manifest their physical presence and restructure their authority;the villagers,operating under perceptual habit,rationalized the presence of the loudspeakers through using the key words such as elders,nostalgia,and homesickness;(2)the loudspeakers was maintained under the habit of village cadres and ordinary villagers,and the outbreak of the epidemic made the loudspeakers as an habit truly“appearing”;(3)the young village cadres,on the other hand,acquired this habit through imitation,transposed the loudspeakers into the short video platform,and carried out different forms of experimentation and innovation,thus surpassing the previous forms of loudspeakers practice.The return of the loudspeakers is the result of the game of different habits.As a long-lasting affection medium,the loudspeakers,on the one hand,has become an important media scene and memory field indispensable to rural community,and on the other hand,the people living on this land have a deep attachment to the loudspeakers,which is indirectly transformed into the bearer of homesickness.This brings new inspiration for us to rethink the relationship between the traditional media and the new media,the iteration of cadres and the change of rural community.

领导建言寻求对员工主动行为的作用机制研究

基于情感事件理论和调节焦点理论相结合的视角,通过一项多阶段、多来源的问卷调查研究(研究1)与一项实验研究(研究2),探讨了领导建言寻求这一管理行为如何以及何时影响员工的主动行为。研究结果发现:领导建言寻求通过增强员工的积极情感体验并减少消极情感来驱动员工实施主动行为;员工调节焦点调节了领导建言寻求通过积极情感间接影响员工主动行为的中介效应。即相比于低调节焦点,当员工调节焦点较高时,领导建言寻求对员工积极情感的正向作用及其对主动行为的间接作用都会更强,且员工调节焦点同样调节领导建言寻求通过消极情感间接影响员工主动行为的中介效应;相比于高调节焦点,当员工调节焦点较低时,领导建言寻求对员工消极情感的负向作用及其对主动行为的间接作用都会更弱。

随着信息技术快速发展,混合式教学模式逐渐成为多数高校的主流教学模式。如何评价学生的学习成效,分析影响学生成绩的因素,是该教学模式的重点研究方向。本文以新乡医学院医学免疫学课程混合式教学为例,介绍混合式教学的实施过程和评价体系,分析影响学生成绩和学习成效的主要因素,着重探讨形成性评价和“翻转课堂”对学生学科成绩的影响。

教师情感劳动概念界定的三个误区

情感劳动是教师劳动的重要属性。教师情感劳动的概念界定需避免三个误区:对霍克希尔德“emotional labor”概念的拿来主义;未区分“情绪劳动”与“情感劳动”;对劳动异化的批判论遮蔽了基于情感本体的生命存在论,劳动(labor)遮蔽了存在(being)。消解上述误区的重要路径是从“厚概念”角度界定并廓清教师情感劳动的内涵。教师的情感劳动是一个情感意义结构,是一个基于情本体的、饱含着教育价值载荷的厚概念,能够将情绪劳动的合理内涵融括在内,用目的论涵括工具论,据之澄明积极自主建构的生命存在论视域,确证情感劳动之于教师专业成长的本体论意义。

基于在线同伴互评的内隐行为规律挖掘及其影响机理研究

学习行为规律能够有效预测学业成绩,是开展个性化教育的关键,但大部分研究成果以探究学生的外显学习行为为主,对学生内隐行为的分析与挖掘较少。同伴互评是表征学习内隐行为的重要方式,文章以其产生的评语和反馈为分析对象,采用定量内容分析、滞后序列分析法和社会网络分析法,从情感、认知和元认知角度分析高、低成绩学生的内隐行为,并引入系统动力学方法剖析基于在线同伴互评的内隐行为影响机理。依据同伴互评中内隐行为所表现出的情感、认知和元认知差异,洞悉学习过程数据背后隐藏的学习成长轨迹,促进学生自主学习,支持教师及时、精准、有效地进行在线学业预警。

考虑时间变化的洪涝灾害损失评估

洪涝灾害是对我国影响最大的自然灾害之一。聚焦灾害损失评估问题,本文构建了考虑时间变化的洪涝灾害损失函数。一方面,表征灾害损失随着致灾因子强度先递增、而后增幅趋缓的静态S型曲线关系;另一方面,通过量级、形状和位置三个参数综合考虑灾害损失随时间的动态变化情况。基于《中国水旱灾害防御公报》提供的受灾人口、直接经济损失数据,对于各省级行政区2006—2021年洪涝灾害损失进行评估分析。结果表明:通过拟合洪涝灾害损失函数,洪涝灾害损失关于致灾因子强度和时间的三维空间散点图可有效地拓展成为连续的三维空间曲面及二维空间热力图。各省级行政区受灾人口整体上呈现出逐年下降的特征,但随致灾因子强度的增加而趋于增加。各省级行政区直接经济损失则表现出明显的差异,随时间呈现出大幅下降、缓慢下降和略有增加等不同特征。考虑时间变化的洪涝灾害损失函数是变化环境下灾害评估的有效工具,可为区域洪涝灾情评估提供借鉴和参考。

达芬奇机器人手术运营效率影响因素分析

目的:分析某三级甲等医院达芬奇机器人手术运营效率的影响因素,为提高机器人手术效率提供依据。方法:采集某院2018年1月—2020年12月开展的达芬奇机器人手术运行数据,分析首台手术开始时间、接台时间、手术时间占进出手术室总时间的比例、机器人专用手术间利用率等指标。结果:共采集到达芬奇机器人手术运营数据1843例,首台手术平均开始时间为09:25,准点率为21.7%,平均接台时间为(71±21)min,平均手术时间为(175±77)min,平均机器人手术时间为(123±59)min,机器人专用手术间日均手术2.9台/d,临时停台率为18.9%,夜间手术占手术总量21.9%。结论:达芬奇机器人手术的运营效率受多因素影响,提高首台手术准点开台率、缩短接台时间、降低临时停台率对提高机器人手术效率和专用手术间利用率至关重要。

粉丝情感劳动的组织化运作——基于A明星全国后援会的剖析

从商业经济到社会公益,粉丝社群呈现的超强行动力和影响力引发社会和学界的关注。既有研究往往将粉丝社区、社群、组织等概念混为一谈,从个体意见领袖或社群外部的视角解读粉丝的情感劳动,一定程度上忽视了组织视角的情感劳动过程。因此,基于对A明星全国粉丝后援会的参与式观察和深度访谈,研究发现,在媒介、政策、文化等变迁中,后援会管理层与成员通过文化资本、情感等因素的交互架构生成了“饭圈”的组织化运作。一方面,管理层为促使粉丝情感劳动组织化,发展出圈层化、规范化、制度化三种整合策略;另一方面,成员通过自我进化、关系建构、反叛三种策略表现出对后援会的迎合或抵抗。深入挖掘粉丝组织内部的运转与劳动过程,不仅可以深入理解数字时代青年亚文化社群的组织实践,为“饭圈”乱象治理提供新的思路,也对虚拟组织的运转和维系具有重要借鉴意义。

微量元素对超大线能量EH36船板热影响区粗晶区组织和性能的影响

通过焊接热模拟研究了在超大线能量下焊接时Al元素、Mg元素和Ti元素含量对EH36高强船板钢热影响区粗晶区组织、性能的影响规律,采用Thermo-Calc热力学计算与SEM,EDS测试相结合的方法揭示了Al元素、Mg元素和Ti元素含量与母材中氧化物类型、尺寸、数量及粗晶区相变的关系.结果表明,Al_(2)O_(3)无法诱导针状铁素体相变,当Al元素质量分数低于0.005%时,钢中可形成Mg元素、Ti元素或其复合氧化物,可促进粗晶区针状铁素体相变.Mg元素和Ti元素联合添加时,当Mg元素质量分数由0.0042%降低为0.0013%,氧化物类型由MgO转变为Mg_(2)TiO_(4),经统计20个视场内的氧化物数量由408个提高到503个,平均直径由1.37μm减小到1.10μm,显著提高了非均匀形核的比表面积,抑制了晶界铁素体的形成,使t8/5=300 s时粗晶区热模拟试样-20℃冲击吸收能量由43 J提升到127 J.

情动理论与电影的纠缠

魔都印象:基于文本分析的上海城市旅游情感形象研究

情感形象是旅游目的地重要形象之一。以外地游客的上海游记为语料,采用扎根理论研究法、内容分析法分析外地游客对上海旅游目的地的情感态度。结果表明:①游客对上海旅游目的地的情感态度总体上积极正面,体现在旅游吸引物、休闲娱乐、设施服务、环境氛围和管理服务5个维度上;②游客对上海旅游目的地的积极评价多集中在“繁华、现代、时尚、独特”高频词上,说明世界著名旅游城市形象已基本建成;③但是景区商业化、高物价和人多排队等因素带给部分游客负面情绪。针对此,本文提出了夯实景区文化内涵、加快全域旅游建设、加大宣传力度、加大精细化服务管理等提升城市旅游形象的建议。
