Low risk of postoperative ulnar nerve affection in surgically treated distal humeral fractures when the nerve is released in situ

BACKGROUND Adult distal humeral fractures(DHF)comprise 2%-5%of all fractures and 30%of all elbow fractures.Treatment of DHF may be technically demanding due to fracture complexity and proximity of neurovascular structures.Open reduction and internal fixation(ORIF)are often the treatment of choice,but arthroplasty is considered in case of severe comminution or in elderly patients with poor bone quality.Ulnar nerve affection following surgical treatment of distal humerus fractures is a well-recognized complication.AIM To report the risk of ulnar nerve affection after surgery for acute DHFs.METHODS We retrospectively identified 239 consecutive adult patients with acute DHFs who underwent surgery with ORIF,elbow hemiarthroplasty(EHA)or total elbow arthroplasty(TEA)between January 2011 and December 2019.In all cases,the ulnar nerve was released in situ without anterior transposition.We used our institutional database to review patients’medical records for demographics,fracture morphology,type of surgery and ulnar nerve affection immediately;records were reviewed after surgery and at 2 wk and 12 wk of routine clinical outpatient follow-up.Twenty-nine percent patients were excluded due to pre-or postoperative conditions.Final follow-up examination was a telephone interview in which ulnar nerve affection was reported according to the McGowen Classification Score.A total of 210 patients were eligible for interview,but 13 patients declined participation and 17 patients failed to respond.Thus,180 patients were included.RESULTS Mean age at surgery was 64 years(range 18-88 years);121(67.3%)patients were women;59(32.7%)were men.According to the AO/OTA classification system,we recorded 47 patients with type A3,55 patients with type B and 78 patients with type C fractures.According to the McGowen Classification Score,mild ulnar nerve affection was reported in nine patients;severe affection,in two.A total of 69 patients were treated with ORIF of whom three had mild temporary ulnar nerve affection and one had severe ulnar nerve affection.In all,111 patients were treated with arthroplasty(67 EHA,44 TEA)of whom seven had mild ulnar nerve affection and one had severe persistent ulnar nerve affection.No further treatment was provided.CONCLUSION The risk of ulnar nerve affection after surgical treatment for acute DHF is low when the ulnar nerve is released in situ without nerve transposition,independently of the treatment provided.

The Use of Triancinolone for the Treatment of Keloid Scars

Scars, when in good evolution, result in a smooth, thin and discreet tissue. Keloid scars, however, are a type of abnormal and exacerbated repair response to tissue injury, whether in surgical interventions or in various injuries, which present in a prominent and gross way. In this context, there is an excess of collagen deposition in the tissue repair process, which can lead to the formation of keloids. The diagnosis of the condition presented is made by the medical professional or by the patient himself after the surgical intervention or skin injury. Under this analysis, protruding, rough and bad-looking scars are identified. In addition, we highlight the existence of keloids similar to large tumors, described as Jorge Lobo disease. The treatment encompasses massages, compressions, corticosteroids, chemotherapy, collagenase and cryotherapy. At first, we used corticosteroid-based massages, and then we started using compressive dressings until we started intrakeloid infiltrations with injectable triamcinolone. Triamcinolone 10 mg injectable—1/10—in 0.9% saline, with syringe and fixed needle 0.3 mm × 8 mm, intralesional infiltrate, in this context, proved to be effective for its treatment when applied sequentially and linearly. In cases where the medication was applied, there was an improvement after 21 days of application and a definitive improvement 2 months after the injury. In comparison, on the other hand, patients who were not subjected to the application of the medication may improve after 4 months of the injury or worsen compared to the initial case. We have come to the conclusion that this procedure may be one of the chosen ones for the treatment of keloid scars, being one of the most recommended for cases of keloid already installed.

Qualitative Algebra and Graph Theory Methods for Dynamic Trend Analysis of Continuous System

Qualitative algebraic equations are the basis of qualitative simulation,which are used to express the dynamic behavior of steady-state continuous processes.When the values and operation of qualitative variables are redefined,qualitative algebraic equations can be transformed into signed direct graphs,which are frequently used to predict the trend of dynamic changes.However,it is difficult to use traditional qualitative algebra methods based on artificial trial and error to solve a complex problem for dynamic trends.An important aspect of modern qualitative algebra is to model and characterize complex systems with the corresponding computer-aided automatic reasoning.In this study,a qualitative affection equation based on multiple conditions is proposed,which enables the signed di-rect graphs to describe complex systems better and improves the fault diagnosis resolution.The application to an industrial case shows that the method performs well.

Right Ventricular Dysfunction in Patients with Acute STEMI, Radionuclide-Based Assessment

Background: An acute myocardial infarction, particularly one that is large and transmural, can produce alterations in the topography of both the infarcted and noninfarcted regions of the ventricle. Cardiac remodeling after acute myocardial infarction (AMI) is characterized by molecular and cellular mechanisms involving both left and right ventricles. Objective: To study the right ventricular affection in patients with acute myocardial infarction of the left ventricle and treated with primary PCI. Methods: The study was done in the critical care department, Cairo University (in which 2,500 primary percutaneous coronary interventions (PCIs) are done annually). Forty patients with acute ST-elevation myocardial infarction (STEMI) were subjected to primary (PCI), we excluded all patients with RV infarction and patients with pulmonary hypertension. RV function was assessed by first pass radionuclide angiocardiography within 48 hours of admission. Results: Regarding the incidence of right ventricular dysfunction in patients with acute left ventricular STEMI who treated with 1ry PCI, there were 21 patients (52.5%) with RV EF% patients (47.5%) with RV time to peak filling rate (TPFR) > 180 msec. There was a significant relationship between right ventricular dysfunction and duration of ICU stay, impairment of LV systolic function, failure of complete resolution of ST segment elevation, failure of early peaking of cardiac enzymes, occurrence of in-hospital complications and one year mortality. Conclusion: The incidence of right ventricular dysfunction in patients with acute left ventricular STEMI is higher than expected and has a negative impact on their outcome.

Correlation between Depth of Myometrial Invasion and Degree of Lymph Node Affection in Cases of Endometrial Cancer

Introduction: Endometrial cancer is the fourth most frequent cancer in females. Many factors can affect prognosis of this type of cancer, these mainly are the degree of myometrial invasion by the tumour, pelvic and paraaortic lymph node spread as well as the tumour histological type (endometrioid vs non-endometrioid type). transvaginal ultrasound (TVS) is a highly accurate and easy method for preoperative evaluation of myometrial invasion. Aim of the Work: The aim of this work is to assess if there is relation between the depth of myometrial invasion by the tumor and the rate of lymph node involvement in cases of endometrial cancer. Results: It was found that there was a significant relation between lymph node affection and the depth of myometrial invasion, all the positive lymph node affections cases had myomterial invasion > 50.0%. Conclusion: The incidence of pelvic lymph node affection is very high in cases where the myometrium is deeply infiltrated with the tumor. Assessment of myometrial invasion preoperatively by TVU and microscopically by pathological examination of the myometrium after hysterectomy provides an accurate estimation of the rate of pelvic lymph node affection and hence necessitates lymphadenectomy procedures in cases where myometrium is deeply infiltrated by the tumor and omitted in cases where it is tumor free.

CLINICAL ANALYSIS ON TREATMENT OF LUMBAR VERTEBRAL RETROGRADE AFFECTION WITH ACUPUNCTURE,TDP-IRRADIATION AND CUPPING

In the present paper, 70 cases of retrograde affection of the lumbar vertebrae are randomly and evenly divided into treatment group and control group. Shenshu (BL 23), Qihaishu (BL 24), Dachangshu (BL 25), Weizhong (BL 40), Jiaji (EX B 2), etc. are used. In treatment group, acupuncture, TDP irradiation and cupping are performed, and in control group, only acupuncture is given. After 30 sessions (3 therapeutic courses) of treatment, in treatment and control groups, the total effective rates are 91.43% and 71.43% respectively. The therapeutic effect of comprehensive treatment is significantly superior to that of simple acupuncture therapy (P<0.05).

The Life Course of a Long-Lasting Affection Medium:A Fieldwork Based on the Communication Practice of the Rural Loudspeakers in North China

The loudspeakers played a crucial role in rural community during the COVID-19 pandemic,and had returned to the public with the help of short video platforms.However,with the continued popularity of digital media,is the revival of the loudspeakers which belong to traditional media just a flash in the pan?What the historic process of return has the loudspeakers undergone?Based on these questions,the paper applies Bourdieu’s sense of practice theory,focuses on“habit”throughout,adopts the research methods of online ethnography and fieldwork,and conducts participant observation and targeted in-depth interviews in a village in North China for about nine months.The study found that:(1)There is the lag of habit in the state’s handling of social relations in villages.Based on rational habit,village cadres used the loudspeakers to manifest their physical presence and restructure their authority;the villagers,operating under perceptual habit,rationalized the presence of the loudspeakers through using the key words such as elders,nostalgia,and homesickness;(2)the loudspeakers was maintained under the habit of village cadres and ordinary villagers,and the outbreak of the epidemic made the loudspeakers as an habit truly“appearing”;(3)the young village cadres,on the other hand,acquired this habit through imitation,transposed the loudspeakers into the short video platform,and carried out different forms of experimentation and innovation,thus surpassing the previous forms of loudspeakers practice.The return of the loudspeakers is the result of the game of different habits.As a long-lasting affection medium,the loudspeakers,on the one hand,has become an important media scene and memory field indispensable to rural community,and on the other hand,the people living on this land have a deep attachment to the loudspeakers,which is indirectly transformed into the bearer of homesickness.This brings new inspiration for us to rethink the relationship between the traditional media and the new media,the iteration of cadres and the change of rural community.

LMR-CBT: learning modality-fused representations with CB-Transformer for multimodal emotion recognition from unaligned multimodal sequences

Learning modality-fused representations and processing unaligned multimodal sequences are meaningful and challenging in multimodal emotion recognition.Existing approaches use directional pairwise attention or a message hub to fuse language,visual,and audio modalities.However,these fusion methods are often quadratic in complexity with respect to the modal sequence length,bring redundant information and are not efficient.In this paper,we propose an efficient neural network to learn modality-fused representations with CB-Transformer(LMR-CBT)for multimodal emotion recognition from unaligned multi-modal sequences.Specifically,we first perform feature extraction for the three modalities respectively to obtain the local structure of the sequences.Then,we design an innovative asymmetric transformer with cross-modal blocks(CB-Transformer)that enables complementary learning of different modalities,mainly divided into local temporal learning,cross-modal feature fusion and global self-attention representations.In addition,we splice the fused features with the original features to classify the emotions of the sequences.Finally,we conduct word-aligned and unaligned experiments on three challenging datasets,IEMOCAP,CMU-MOSI,and CMU-MOSEI.The experimental results show the superiority and efficiency of our proposed method in both settings.Compared with the mainstream methods,our approach reaches the state-of-the-art with a minimum number of parameters.
