BACKGROUND Cirrhosis is a significant risk factor for the development of hepatocellular carcinoma(HCC).Variability in HCC risk among patients with cirrhosis is notable,particularly when considering the diverse etiolog...BACKGROUND Cirrhosis is a significant risk factor for the development of hepatocellular carcinoma(HCC).Variability in HCC risk among patients with cirrhosis is notable,particularly when considering the diverse etiologies of cirrhosis.AIM To identify specific risk factors contributing to HCC development in patients with cirrhosis.METHODS This retrospective study analyzed data from cirrhotic patients at Beijing Youan Hospital from January 1,2012 to September 30,2022 with at least 6 mo of followup.Patient demographics,medical histories,etiologies,and clinical characteristics were examined.Cox regression analysis was used to analyze correlations of the above parameters with hepatocarcinogenesis,while competing risk regression was used to estimate their adjusted hazard ratios accounting for death.The cumulative incidence was plotted over time.RESULTS Overall,5417 patients with cirrhosis(median age:54 years;65.8%males)were analyzed.Hepatitis B virus(HBV)was the most common etiology(23.3%),with 25%(n=1352)developing HCC over a 2.9-year follow-up period.Patients with multiple etiologies had the HCC highest incidence(30.3%),followed by those with HBV-related cirrhosis(29.5%).Significant risk factors included male sex,advanced age,hepatitis C virus(HCV)infection,elevated blood ammonia,and low platelet count.Men had a higher 5-year HCC risk than women(37.0%vs 31.5%).HBV,HCV,and HBV/HCV co-infected patients had 5-year risks of HCC of 45.8%,42.9%,and 48.1%,respectively,compared to 29.5%in nonviral hepatitis cases,highlighting the significant HCC risk from viral hepatitis,especially HBV,and underscores the importance of monitoring these high-risk groups.CONCLUSION In conclusion,HBV-related cirrhosis strongly correlates with HCC,with male sex,older age,viral hepatitis,elevated blood ammonia,and lower albumin and platelet levels increasing the risk of HCC.展开更多

Mitigation strategies h...One of the down sides of crude oil exploration and exploitation in the developing nations is its impacts on the environment. A major manifestation of poor crude oil management is oil-spillages. Mitigation strategies have been too expensive, but a cheaper recent way of managing crude-spills is by developing a severity risk analysis matrix ranking (SRAMR). The spatial data-sets deployed in this study were acquired from the USGS, Google Earth Pro, and NOSDRA. A buffer zone of 100 - 400 meters was created to characterize the LULC characteristics of the area. Also, this was to help develop a risk sensitivity characteristic. The study found that the vegetal cover was the environmental resource at high risk to crude-spills in the area, while other land-uses were at low risk of crude-spill. It is hoped that the finding from this study informs policy development and planning for crude oil spill incidents.展开更多
This paper proposes a risk analysis framework for substation structures based on reliability methods.Even though several risk assessment approaches have been developed for buildings,detailed risk analysis procedures f...This paper proposes a risk analysis framework for substation structures based on reliability methods.Even though several risk assessment approaches have been developed for buildings,detailed risk analysis procedures for infrastructure components have been lacking in prior studies.The proposed framework is showcased by its application to a system of interconnected structures at a power substation in Tehran.Finite element models of structures are developed and validated in accordance with previous experiments.The uncertainties in the material,mass,and geometric properties of structures are described by random variables that are input to the finite element model.An artificial ground motion model is employed to comprehensively consider uncertainty in ground motion.Monte Carlo sampling is subsequently conducted on the library of probabilistic models.The analysis resulted in the loss distribution in the life cycle of structures.Additionally,the loss associated with six earthquake scenarios having specific magnitudes and return periods is computed.The application provides insight into the most vulnerable equipment in the considered system.Furthermore,introduced risk measures can guide stakeholders to make risk-based decisions to optimize design or prioritize a retrofit of infrastructure components under conditions of uncertainty.展开更多

Although information security and cyber security are sometimes used synonymously, this study contends that they are not the same. The concept of cyber security is explored, which goes beyond protecting information resources to include a wider variety of assets, including people [1]. Protecting information assets is the main goal of traditional information security, with consideration to the human element and how people fit into the security process. On the other hand, cyber security adds a new level of complexity, as people might unintentionally contribute to or become targets of cyberattacks. This aspect presents moral questions since it is becoming more widely accepted that society has a duty to protect weaker members of society, including children [1]. The study emphasizes how important cyber security is on a larger scale, with many countries creating plans and laws to counteract cyberattacks. Nevertheless, a lot of these sources frequently neglect to define the differences or the relationship between information security and cyber security [1]. The paper focus on differentiating between cybersecurity and information security on a larger scale. In this study, 30 sediment samples were collected from the lower reaches of the Shichuanhe River in Xi’an, Shaanxi Province, China, to test the distribution of heavy metal elements in this area and for an analysis of...In this study, 30 sediment samples were collected from the lower reaches of the Shichuanhe River in Xi’an, Shaanxi Province, China, to test the distribution of heavy metal elements in this area and for an analysis of the pollution levels of this area, hope to provide guidance on agricultural production activities in this region. The results show that the heavy metal elements in this area are mainly concentrated at the Qinghe River and Shichuanhe River confluences. Furthermore, the element contents are higher than that of the background levels of the continental crust(UCC) and close to the background levels of the soil from Shaanxi Province;the two most enriched elements are Cd and As, with contents of 0.79 and 22.7 mg·kg-1, respectively, and their contents are 3.8 and 1.72 times higher than that of the background values. Herein, the heavy metal pollution assessment methods applied indicated that Cd and As are the two most abundant pollutant elements in the area’s soils. As has a peak geo-accumulation index value of 3, and the pollution level is high, while Cd exhibits high potential ecological risks due to its high toxicity(potential risk index of 143) and an active fraction of more than 64%.In addition, a principal component analysis and hierarchical cluster analysis study showed that there are two sources of the heavy metals in this area. The Zn, As, Ni, Cu, Pb,and Cr are mainly from natural sources, and the Cd likely comes from a discharge of untreated agricultural wastewater in the region. The Cd which poses a high potential risk and mainly results from human activities, needs to be further monitored.展开更多
Background:Breast cancer and endometriosis are two significant diseases that threaten the health of female worldwide.The relationship between them is still in a state of controversy.In present study,we identified the ...Background:Breast cancer and endometriosis are two significant diseases that threaten the health of female worldwide.The relationship between them is still in a state of controversy.In present study,we identified the risk of breast cancer in patients with endometriosis through a meta-analysis.Methods:A systematic search of the PubMed,Embase and Cochrane Library,with the terms for endometriosis,breast and cancer,tumor,neoplasm,or carcinoma combined.Studies on related risk estimates of endometriosis and breast cancer were included in the meta-analysis.Furthermore,a series of analytical methods such as sensitivity analysis,the Cochran Q test,Begg’s and Egger’s test were used to analyze the information of collected studies.Results:Finally,a total of 8 studies were included.After statistical analysis based on STATA software,the combined odds ratio of the results was 1.13(95%CI:1.09–1.17).And the results of subgroup analysis showed that the odds ratio values of different outcome indexes were 1.06(95%CI:1.02–1.09,P<0.05),the effective effect estimates were presented in SIR(P=0.006,I^(2)=80.3%).Conclusion:In present study,we identified that patients with endometriosis have an increased risk of breast cancer.It is worth considering that our research was a summary analysis of epidemiological data.More in-depth studies are needed to determine the pathophysiology of this relationship.展开更多

In order to study the impacts of promoting new energy vehicles on public transportation pollution mitigation and residents’ health benefits, this paper adopts the LEAP model to build some scenarios that fulfill different development needs to quantitatively analyze the ownership of new energy buses, the reduction of pollutants and the losses of residents’ health welfare. It is concluded that promoting new energy buses comprehensively can significantly reduce the emissions of atmospheric pollutants and the economic losses of residents’ health, but cannot fully realize the targets of greenhouse gas reduction under Life Cycle Analysis.展开更多

Our study aimed to investigate the risk factors and prevention of post-ERCP cholangitis.Methods: We retrospectively analyzed 4234 cases undergone ERCP in the Affiliated Drum Tower Hospital of Nanjing University Medical School from January 2008 to December 2013. Patient-related factors and procedure-related factors were analyzed to find the risk factors of post-ERCP cholangitis. The time point of post-ERCP cholangitis was also analyzed. Univariate and multivariate analyses were performed to define the independent risk factors of post-ERCP cholangitis.Results: The success rate of ERCP was 96.8%(4099/4234). The overall complication rate was 9.4%(399/4234). Post-ERCP cholangitis occurred in 102 cases(2.4%, 102/4234). The most dangerous time of post-ERCP cholangitis was from 24 h–48 h after ERCP(45.1%, 46/102). Univariate analysis revealed that age, hypertension, diabetes, previous ERCP history, biliary stent insertion, pancreatography, endoscopic sphincterotomy, balloon dilation and hilar obstruction were risk factors of post-ERCP cholangitis(P < 0.05). Multivariate analysis indicated that age, previous ERCP history and hilar obstruction were independent risk factors(P < 0.05). While endoscopic stone extraction was the potential protective factor.Conclusions: Many risk factors are involved in post-ERCP cholangitis. Among them, old age, previous ERCP history and hilar obstruction were independently related to this post-ERCP complication.展开更多

The simulated results indicate that the flood control limiting level can be raised by 0.40 m under the condition that the reservoir overtopping risk is controlled within a mean variance of 5×10-6. As a result, the reservoir storage will increase to 16 million m3 and electrical energy generation and other functions of the reservoir will also increase greatly.展开更多

Beginning in the 1940s with the construction of the Anchieta highway, the city experienced an influx of migrants drawn by burgeoning industries, leading to unplanned urban growth and the emergence of vulnerable communities like Vila Socó. This article examines the interconnected factors—such as demographic shifts, inadequate planning, and regulatory oversight—that culminated in the devastating fire of 1984, claiming numerous lives and highlighting systemic failures. Utilizing the Haddon Matrix, this study dissects the Vila Socó incident, emphasizing the roles of human error, infrastructure integrity, and socio-economic disparities in disaster causation. By contextualizing the tragedy within Cubatão’s industrial trajectory, it underscores the urgent need for comprehensive risk assessment and proactive mitigation strategies in rapidly developing regions globally. Beyond its immediate focus, this work offers broader insights into the dynamics of industrial disasters and their socio-economic implications. As pipelines continue to play a vital role in global energy infrastructure, the lessons drawn from Vila Socó’s tragedy resonate deeply, emphasizing the imperative of robust safety protocols and accountable governance to prevent similar catastrophes in the future.
文摘One of the down sides of crude oil exploration and exploitation in the developing nations is its impacts on the environment. A major manifestation of poor crude oil management is oil-spillages. Mitigation strategies have been too expensive, but a cheaper recent way of managing crude-spills is by developing a severity risk analysis matrix ranking (SRAMR). The spatial data-sets deployed in this study were acquired from the USGS, Google Earth Pro, and NOSDRA. A buffer zone of 100 - 400 meters was created to characterize the LULC characteristics of the area. Also, this was to help develop a risk sensitivity characteristic. The study found that the vegetal cover was the environmental resource at high risk to crude-spills in the area, while other land-uses were at low risk of crude-spill. It is hoped that the finding from this study informs policy development and planning for crude oil spill incidents.
文摘This paper proposes a risk analysis framework for substation structures based on reliability methods.Even though several risk assessment approaches have been developed for buildings,detailed risk analysis procedures for infrastructure components have been lacking in prior studies.The proposed framework is showcased by its application to a system of interconnected structures at a power substation in Tehran.Finite element models of structures are developed and validated in accordance with previous experiments.The uncertainties in the material,mass,and geometric properties of structures are described by random variables that are input to the finite element model.An artificial ground motion model is employed to comprehensively consider uncertainty in ground motion.Monte Carlo sampling is subsequently conducted on the library of probabilistic models.The analysis resulted in the loss distribution in the life cycle of structures.Additionally,the loss associated with six earthquake scenarios having specific magnitudes and return periods is computed.The application provides insight into the most vulnerable equipment in the considered system.Furthermore,introduced risk measures can guide stakeholders to make risk-based decisions to optimize design or prioritize a retrofit of infrastructure components under conditions of uncertainty.
文摘BACKGROUND Some studies have directed towards an association between diabetes mellitus(DM)and prostate cancer(PCa);however,this specific relationship remains inconclusive.In recent years,Mendelian randomization(MR)has become a widely used analytical method for inferring epidemiological causes.AIM To investigated the potential relationship between DM and PCa using MR.METHODS We downloaded relevant data on"diabetes"and"PCa"from the IEU OpenGWAS project database,performed three different methods to conduct MR,and carried out sensitivity analysis for verification.RESULTS The results indicated that DM was an independent risk factor for PCa.The odds ratio(OR)values obtained using the inverse variance weighted method in this study were as follows:OR=1.018(95%confidence interval:1.004-1.032),P=0.014.CONCLUSION We found that DM could increase the incidence rate of PCa.
文摘Objective:To analyze the existing risks in breast milk management at the neonatal department and provide corresponding countermeasures.Methods:22 risk events were identified in 7 risk links in the process of bottle-feeding of breast milk.Hazard Vulnerability Analysis based on the Kaiser model was applied to investigate and evaluate the risk events.Results:High-risk events include breast milk quality inspection,hand hygiene during collection,disinfection of collectors,cold chain management,hand hygiene during the reception,breast milk closed-loop management,and post-collection disposal.Root cause analysis of high-risk events was conducted and breast milk management strategies outside the hospital and within the neonatal department were proposed.Conclusion:Hazard Vulnerability Analysis based on the Kaiser model can identify and assess neonatal breast milk management risks effectively,which helps improve the management of neonatal breast milk.It is conducive to the safe development and promotion of bottle feeding of breast milk for neonates,ensuring the quality of medical services and the safety of children.
文摘This paper examines how cybersecurity is developing and how it relates to more conventional information security. Although information security and cyber security are sometimes used synonymously, this study contends that they are not the same. The concept of cyber security is explored, which goes beyond protecting information resources to include a wider variety of assets, including people [1]. Protecting information assets is the main goal of traditional information security, with consideration to the human element and how people fit into the security process. On the other hand, cyber security adds a new level of complexity, as people might unintentionally contribute to or become targets of cyberattacks. This aspect presents moral questions since it is becoming more widely accepted that society has a duty to protect weaker members of society, including children [1]. The study emphasizes how important cyber security is on a larger scale, with many countries creating plans and laws to counteract cyberattacks. Nevertheless, a lot of these sources frequently neglect to define the differences or the relationship between information security and cyber security [1]. The paper focus on differentiating between cybersecurity and information security on a larger scale. The study also highlights other areas of cybersecurity which includes defending people, social norms, and vital infrastructure from threats that arise from online in addition to information and technology protection. It contends that ethical issues and the human factor are becoming more and more important in protecting assets in the digital age, and that cyber security is a paradigm shift in this regard [1].

文摘In this study, 30 sediment samples were collected from the lower reaches of the Shichuanhe River in Xi’an, Shaanxi Province, China, to test the distribution of heavy metal elements in this area and for an analysis of the pollution levels of this area, hope to provide guidance on agricultural production activities in this region. The results show that the heavy metal elements in this area are mainly concentrated at the Qinghe River and Shichuanhe River confluences. Furthermore, the element contents are higher than that of the background levels of the continental crust(UCC) and close to the background levels of the soil from Shaanxi Province;the two most enriched elements are Cd and As, with contents of 0.79 and 22.7 mg·kg-1, respectively, and their contents are 3.8 and 1.72 times higher than that of the background values. Herein, the heavy metal pollution assessment methods applied indicated that Cd and As are the two most abundant pollutant elements in the area’s soils. As has a peak geo-accumulation index value of 3, and the pollution level is high, while Cd exhibits high potential ecological risks due to its high toxicity(potential risk index of 143) and an active fraction of more than 64%.In addition, a principal component analysis and hierarchical cluster analysis study showed that there are two sources of the heavy metals in this area. The Zn, As, Ni, Cu, Pb,and Cr are mainly from natural sources, and the Cd likely comes from a discharge of untreated agricultural wastewater in the region. The Cd which poses a high potential risk and mainly results from human activities, needs to be further monitored.
文摘Background:Breast cancer and endometriosis are two significant diseases that threaten the health of female worldwide.The relationship between them is still in a state of controversy.In present study,we identified the risk of breast cancer in patients with endometriosis through a meta-analysis.Methods:A systematic search of the PubMed,Embase and Cochrane Library,with the terms for endometriosis,breast and cancer,tumor,neoplasm,or carcinoma combined.Studies on related risk estimates of endometriosis and breast cancer were included in the meta-analysis.Furthermore,a series of analytical methods such as sensitivity analysis,the Cochran Q test,Begg’s and Egger’s test were used to analyze the information of collected studies.Results:Finally,a total of 8 studies were included.After statistical analysis based on STATA software,the combined odds ratio of the results was 1.13(95%CI:1.09–1.17).And the results of subgroup analysis showed that the odds ratio values of different outcome indexes were 1.06(95%CI:1.02–1.09,P<0.05),the effective effect estimates were presented in SIR(P=0.006,I^(2)=80.3%).Conclusion:In present study,we identified that patients with endometriosis have an increased risk of breast cancer.It is worth considering that our research was a summary analysis of epidemiological data.More in-depth studies are needed to determine the pathophysiology of this relationship.
文摘With the continuous development of urban public transportation, the harmful GHG emissions and pollutants generated by itself and the consequent issues such as the losses of residents’ health, economic value and residents’ welfare have become the focus of social attention. In order to study the impacts of promoting new energy vehicles on public transportation pollution mitigation and residents’ health benefits, this paper adopts the LEAP model to build some scenarios that fulfill different development needs to quantitatively analyze the ownership of new energy buses, the reduction of pollutants and the losses of residents’ health welfare. It is concluded that promoting new energy buses comprehensively can significantly reduce the emissions of atmospheric pollutants and the economic losses of residents’ health, but cannot fully realize the targets of greenhouse gas reduction under Life Cycle Analysis.

Results and Conclusion\ A risk analysis model named the cusp model is presented. Various states regarding the safety of the system such as the accident state, no accident state and miss state can be represented at will on the cusp model.

文摘Based on scenario analysis method, assuming different economic develop- ment policies, and AHP method, the ecological indicators and risk prediction models were established. The trends of ecological risk in the northwest region were ana- lyzed. The results showed that: in the northwest, the potential ecological risks of the population economy, water resources, and land resources and natural disasters still existed. However, the environment and respond to ecological risks were decreasing. Therefore, a regional balanced development measures had been taken to curb the western trend of ecological deterioration. So that socio-economic development, re- sources, environment, and economy were expected to a virtuous cycle, and the eco- logical risks would be reduced. This study provides the basis for land and water use planning and environmental protection of government decision-making and management.

文摘The socio-economic attribute of geo-hazard made us distinguish it from the traditional engineering geology study. It will get more social benefit from the analysis of the geo-hazard in the socio-economic attribute. The hazard and the vulnerability of the element controls the risk level of the regional geo-hazard. The risk analysis supported by GIS in geo-hazard study is one of the most important directions. Based on the author’s studies in recent years, a risk analysis system of regional geo-hazard (RiskAnly) has been developed on the basis of software MAPGIS. The paper introduces the train of system design, the structure and the workflow of RiskAnly. As a case study, the paper also deals with the risk zonation of the regional landslide hazard of China.

文摘Background: Endoscopic retrograde cholangiopancreatography(ERCP) may have complications. Our study aimed to investigate the risk factors and prevention of post-ERCP cholangitis.Methods: We retrospectively analyzed 4234 cases undergone ERCP in the Affiliated Drum Tower Hospital of Nanjing University Medical School from January 2008 to December 2013. Patient-related factors and procedure-related factors were analyzed to find the risk factors of post-ERCP cholangitis. The time point of post-ERCP cholangitis was also analyzed. Univariate and multivariate analyses were performed to define the independent risk factors of post-ERCP cholangitis.Results: The success rate of ERCP was 96.8%(4099/4234). The overall complication rate was 9.4%(399/4234). Post-ERCP cholangitis occurred in 102 cases(2.4%, 102/4234). The most dangerous time of post-ERCP cholangitis was from 24 h–48 h after ERCP(45.1%, 46/102). Univariate analysis revealed that age, hypertension, diabetes, previous ERCP history, biliary stent insertion, pancreatography, endoscopic sphincterotomy, balloon dilation and hilar obstruction were risk factors of post-ERCP cholangitis(P < 0.05). Multivariate analysis indicated that age, previous ERCP history and hilar obstruction were independent risk factors(P < 0.05). While endoscopic stone extraction was the potential protective factor.Conclusions: Many risk factors are involved in post-ERCP cholangitis. Among them, old age, previous ERCP history and hilar obstruction were independently related to this post-ERCP complication.

文摘In this paper, a model of overtopping risk under the joint effects of floods and wind waves, which is based on risk analysis theory and takes into account the uncertainties of floods, wind waves, reservoir capacity and discharge capacity of the spillway, is proposed and applied to the Chengbihe Reservoir in Baise City in Guangxi Zhuang Autonomous Region. The simulated results indicate that the flood control limiting level can be raised by 0.40 m under the condition that the reservoir overtopping risk is controlled within a mean variance of 5×10-6. As a result, the reservoir storage will increase to 16 million m3 and electrical energy generation and other functions of the reservoir will also increase greatly.
