The precise combination of conflicting biological properties through sophisticated structural and functional design to meet all the requirements of anastomotic healing is of great demand but remains challenging.Here,we develop a smart responsive anastomotic staple(Ti–OH-MC)by integrating porous titanium anastomotic staple with multifunctional polytannic acid/tannic acid coating.This design achieves dynamic sequential regulation of antibacterial,anti-inflammatory,and cell proliferation properties.During the inflammatory phase of the anastomotic stoma,our Ti–OH-MC can release tannic acid to provide antibacterial and anti-inflammatory properties,together with immune microenvironment regulation function.At the same time,as the healing progresses,the multifunctional coating gradually falls off to expose the porous structure of the titanium anastomotic staple,which promotes cell adhesion and proliferation during the later proliferative and remodeling phases.As a result,our Ti–OH-MC exceeds the properties of clinically used titanium anastomotic staple,and can effectively promote the healing.The staple’s preparation strategy is simple and biocompatible,promising for industrialisation and clinical application.This work provides an effective anastomotic staple for anastomotic stoma healing and serve as a reference for the functional design and preparation of other types of titanium-based tissue repair materials. 展开更多

Effect of hypercholesterolemia on experimental colonic anastomotic wound healing in rats 被引量:1

AIM: To evaluate the mechanical and biochemical parameters of colonic anastomotic healing in hypercholesterolemic rats. METHODS: Sixty rats were divided into two groups of 30 each according to their dietary regimens. The test group was fed with a high cholesterol-containing diet for two months while the control group had standard diet. These two groups were further divided into three subgroups consisting of ten rats each. After hypercholesterolemia was established, left colon resection and anastomosis were performed in both groups and samples from liver and abdominal aorta were taken to evaluate the systemic effects of hypercholesterolemia. Anastomotic wound healing, blow-out pressures and tissue hydroxyproline levels were evaluated. RESULTS: The test group had a significant weight gain in two months. Microscopic examination of the abdominal aorta revealed no atherosderotic change in none of the groups, but liver tissue specimens showed significant steatosis in the test group. Tissue hydroxyproline levels and anastomotic blow-out pressures were significantly lower in the test group than in the controls. CONCLUSION: Hypercholesterolemia not only increases hydroxyproline levels and blow-out pressures but also worsens anastomotic wound healing. 展开更多

The control group (n = 10) had an end- to-end ... AIM: To investigate the influence of remote ischemic preconditioning (RIPC) on anastomotic integrity. METHODS: Sixty male Wistar rats were randomized to six groups. The control group (n = 10) had an end- to-end ileal anastomosis without RIPC. The preconditioned groups (n = 34) varied in time of ischemia and time of reperfusion. One group received the amino acid L-arginine before constructing the anastomosis (n = 9). On postoperative day 4, the rats were re-laparotomized, and bursting pressure, hydroxyproline concentration, intra-abdominal adhesions, and a histological score concerning the mucosat ischemic injury were collected. The data are given as median (range).RESULTS: On postoperative day 4, median bursting pressure was 124 mmHg (60-146 mmHg) in the control group. The experimental groups did not show a statistically significant difference (P 〉 0.05). Regarding the hydroxyproline concentration, we did not find any significant variation in the experimental groups. We detected significantly less mucosal injury in the RIPC groups. Furthermore, we assessed more extensive intra-abdominal adhesions in the preconditioned groups than in the control group. CONCLUSION: RIPC directly before performing small bowel anastomosis does not affect anastomotic stability in the early period, as seen in ischemic preconditioning. 展开更多

AIM: To investigate whether the simultaneous treatment with human growth hormone (hGH) abolishes the negative effects of everolimus on anastomotic healing.METHODS: Forty-eight male Sprague-Dawley-rats were randomized to three groups of 16 animals each (I: vehicle; II: everolimus 3 mg/kg po; III: everolimus 3 mg/kg po + hGH 2.5 mg/kg sc). Animals were pre-treated with hGH and/or everolimus daily for seven days. Then a standard anastomosis was created in the descending colon and treatment was continued for another seven days. The anastomosis was resected in toto and the bursting pressure was assessed as a mechanical parameter of intestinal healing. Moreover, biochemical (Hydroxyproline, PCNA, MPO, MMP-2 and MMP-9) and histological (cell density, angiogenesis, amount of granulation tissue) parameters of intestinal healing were assessed.RESULTS: Anastomotic bursting pressure was significantly reduced by everolimus and a simultaneous treatment with hGH resulted in considerably higher values (I: 134 &#x000b1; 19 mmHg, II: 85 &#x000b1; 25 mmHg, III: 114 &#x000b1; 25 mmHg; P &#x0003c; 0.05,&#x02005;I&#x02005;vs II; P = 0.09,&#x02005;I&#x02005;vs III and II vs III) Hydroxyproline concentration was significantly increased by hGH compared to everolimus alone (I: 14.9 &#x000b1; 2.5 &#x003bc;g/mg, II: 8.9 &#x000b1; 3.6 &#x003bc;g/mg, III: 11.9 &#x000b1; 2.8 &#x003bc;g/mg; P &#x0003c; 0.05,&#x02005;I&#x02005;vs II/III and II vs III). The number of MPO-positive cells was reduced significantly by hGH compared to everolimus alone (I: 10 &#x000b1; 1 n/mm&#x000b2;, II: 15 &#x000b1; 3 n/mm&#x000b2;, III: 9 &#x000b1; 2 n/mm&#x000b2;; P &#x0003c; 0.05,&#x02005;I&#x02005;vs II and II vs III), while the number of PCNA-positive cells were increased by hGH (I: 28 &#x000b1; 3 /mm&#x000b2;, II: 12 &#x000b1; 3 /mm&#x000b2;, III: 26 &#x000b1; 12 /mm&#x000b2;; P &#x0003c; 0.05,&#x02005;I&#x02005;vs II and II vs III). Corresponding to these biochemical findings, HE-histology revealed significantly increased amount of granulation tissue in hGH-treated animals.CONCLUSION: Inhibition of intestinal wound healing by everolimus is partially neutralized by simultaeous treatment with hGH.
