Comprehensive effects of traditional Chinese medicine treatment on heart failure and changes in B-type natriuretic peptide levels: A meta-analysis

BACKGROUND Heart failure(HF),a common cardiovascular condition,is characterized by significant morbidity and mortality.While traditional Chinese medicine(TCM)is often used as a complementary approach in HF management,systematic evalua-tions of its impact on clinical outcomes,TCM syndrome scores,and B-type natriuretic peptide(BNP)levels are lacking.This study fills this gap through a comprehensive analysis of randomized controlled trials(RCTs)focusing on TCM for HF treatment.It encompasses an assessment of methodological quality,a meta-analysis,and an evaluation of evidence quality based on established standards.The results offer crucial insights into the potential advantages and constraints of TCM in HF management.RCTs on TCM for HF treatment published since the establishment of the database were searched in four Chinese and English databases,including China National Knowledge Infrastructure,Wanfang,VIP Information Chinese Science and Technology Journal,and PubMed.Methodological quality was assessed for the included studies with the Cochrane risk-of-bias assessment tool,and the meta-analysis and publication bias assessment was performed with the RevMan5.3 software.Finally,the quality of evidence was rated according to the GRADE criteria.RESULTS A total of 1098 RCTs were initially retrieved.After screening,16 RCTs were finally included in our study,which were published between 2020 and 2023.These RCTs involved 1660 HF patients,including 832 in the TCM group[TCM combined with conventional Western medicine(CMW)treatment]and 828 in the CWM group(CWM treatment).The course of treatments varied from 1 wk to 3 months.TCM syndrome differentiation was analyzed in 11 of the included RCTs.In all included RCTs,outcome indicators included comprehensive clinical outcomes,TCM syndrome scores,and BNP levels.The meta-analysis results showed significant differences between the TCM and CWM groups in terms of comprehensive clinical outcomes[risk ratio=-0.54;95%confidence interval(CI)=-0.61,-0.47;P<0.00001],TCM syndrome scores[weighted mean difference(WMD)=-142.07;95%CI=-147.56,-136.57;P<0.00001],and BNP levels(WMD=-142.07;95%CI=-147.56,-136.57;P<0.00001).According to the GRADE criteria,RCTs where"TCM improves clinical comprehensive outcomes"were rated as low-quality evidence,and RCTs where"TCM reduces TCM syndrome scores"or"TCM decreases BNP levels"were rated as medium-quality evidence.CONCLUSION TCM combined with CWM treatment effectively improves comprehensive clinical outcomes and diminishes TCM syndrome scores and BNP levels in HF patients.Given the low and medium quality of the included RCTs,the application of these results should be cautious. 展开更多

震前重力扰动与背景噪声时空变化特征——以玛多M_(S)7.4与漾濞M_(S)6.4地震为例 被引量:2

2021年5月21—22日,云南漾濞、青海玛多先后发生了M_(S)6.4和M_(S)7.4地震。为检测震前是否存在重力扰动与重力仪背景噪声异常信号,文中基于中国连续重力台网中15台重力仪的秒采样数据,成功提取了震前重力扰动信号,并计算了排列熵(PE)... 2021年5月21—22日,云南漾濞、青海玛多先后发生了M_(S)6.4和M_(S)7.4地震。为检测震前是否存在重力扰动与重力仪背景噪声异常信号,文中基于中国连续重力台网中15台重力仪的秒采样数据,成功提取了震前重力扰动信号,并计算了排列熵(PE)及重力仪在地震频段(200~600s)的背景噪声等级(SNM)。结果表明:1)大地震发生前多数台站在2021年5月15—18日存在1组重力扰动信号,其中沿海台站同时还存在其他2组扰动信号,扰动幅度为±(10~100)μGal,扰动频率集中在0.15~0.25Hz。沿海台站的扰动幅度普遍大于内陆台站,震中距与扰动幅度无明显相关性,沿海台站扰动幅度较大及产生其他2组扰动的原因可能与海浪脉动或局部降雨有关;2)不需要对原始重力序列作任何预处理,PE能够快速检测到数据中的突变信号,PE时间序列中持续显著下降的信号对应震前扰动信号,PE瞬时向下的脉冲信号对应地震波引起的颤动信号;3)多个台站的SNM时变曲线显示,震前2个月(2021年3月初)直至主震发生时背景噪声水平显著升高,SNM的空间分布表明,震前1~2个月,位于青藏高原北缘、巴颜喀拉地块附近台站的背景噪声有明显增大的趋势,震后噪声水平显著降低。综上,我们初步推测:震前的重力扰动可能是由发震断裂慢滑移产生的低频颤动信号,震前背景噪声的增大可能与发震地块的活动性增强有关。 展开更多

The progressive aging of populations has resulted in an increased prevalence of chronic pathologies,especially of metabolic,neurodegenerative and movement disorders.In particular,type 2 diabetes(T2D),Alzheimer’s disease(AD)and Parkinson’s disease(PD)are among the most prevalent age-related,multifactorial pathologies that deserve particular attention,given their dramatic impact on patient quality of life,their economic and social burden as well the etiopathogenetic mechanisms,which may overlap in some cases.Indeed,the existence of common triggering factors reflects the contribution of mutual genetic,epigenetic and environmental features in the etiopathogenetic mechanisms underlying T2D and AD/PD.On this subject,this review will summarize the shared(epi)genomic features that characterize these complex pathologies.In particular,genetic variants and gene expression profiles associated with T2D and AD/PD will be discussed as possible contributors to determine the susceptibility and progression to these disorders.Moreover,potential shared epigenetic modifications and factors among T2D,AD and PD will also be illustrated.Overall,this review shows that findings from genomic studies still deserves further research to evaluate and identify genetic factors that directly contribute to the shared etiopathogenesis.Moreover,a common epigenetic background still needs to be investigated and characterized.The evidences discussed in this review underline the importance of integrating largescale(epi)genomic data with additional molecular information and clinical and social background in order to finely dissect the complex etiopathogenic networks that build up the“disease interactome”characterizing T2D,AD and PD. 展开更多

BACKGROUND Hypertrophic scars(HSs)formation is a complication that occurs after wounds heal with secondary intention and sometimes after clean surgical incisions.Many treatments are in vogue now with varying successes.Although the mechanism or mechanisms that cause a HS to form are not clearly understood,one thing that is clear is that once scar tissue matures,any intervention will not be successful.In this paper,we report on a case where a patient who was known to develop HS was treated with a new combination of ingredients(Phyto-chemicals+Silicone JUMI)to suppress HS formation.CASE SUMMARY A 68-year-old female of African descent presented a severe HS post total knee replacement(TKR),which the patient describes as itchy and painful.Due to complications caused by the scar,she was apprehensive about undergoing TKR on her other knee.However,after the TKR of the contralateral side post-removal of skin clips,JUMI anti-scar cream(JASC)was used to suppress excessive scar formation.CONCLUSION JASC appears potent and efficacious at suppressing excessive scar formation.We believe that this warrants further studies on larger patient groups and on different surgical sites. 展开更多
