AIM: Blood platelets (plt) and monocytes are the cells that play a crucial role in the pathogenesis of liver damage and liver cirrhosis (LC). In this paper, the analysis of mutual relationship between platelets and monocytes activation in LC was conducted.METHODS: Immunofluorescent flow cytometry was usedto measure the percentage of activated platelet populations(CD62P, CD63), the percentage of plt-monocyte aggregates (pma) (CD41/CD45), and activated monocytes (CD11b, CD14, CD16) in the blood of 20 volunteers and 40 patientswith LC. Platelet activation markers: sP-selectin, platelet factor 4 (PF4), beta-thromboglobulin (βTG) and monocyte chemotactic peptide-1 (MCP-1) were measured and compared in different stages of LC.RESULTS: Platelet activation with the increase in bothβTG serum concentration and elevation of plt population(CD62P and CD63 as well as MIF CD62P and CD63) is elevated as LC develops and thrombocytopenia rises. There is a positive correlation between medial intensityof fluorescence (MIF) CD62P and MIF CD63 in LC. We did not show any relationship between monocyte activation and pma level. SP-selectin concentration correlates positively with plt count and pma, and negatively with stage of plt activation and MIF CD62P and MIF CD63. There was no correlation between MCP-1 concentration andpit, monocyte activation as well as pma level in LC. CD16 monocytes and MIF CD16 populations are significantlyhigher in the end stage of LC. A positive correlation occurs between the value of CD11b monocyte population andMIF CD14 and MIF CD16 on monocytes in LC.CONCLUSION: Platelet and monocyte activation plays an important role in LC. Platelet activation stage does not influence monocyte activation and production of plt aggregates with monocytes in LC. With LC development, thrombocytopenia may be the result of plt consumption in platelet-monocyte aggregates. 展开更多

BACKGROUND For compensated advanced chronic liver disease(cACLD)patients,the first decompensation represents a dramatically worsening prognostic event.Based on the first decompensation event(DE),the transition to decompensated advanced chronic liver disease(dACLD)can occur through two modalities referred to as acute decompensation(AD)and non-AD(NAD),respectively.Clinically Significant Portal Hypertension(CSPH)is considered the strongest predictor of decompensation in these patients.However,due to its invasiveness and costs,CSPH is almost never evaluated in clinical practice.Therefore,recognizing noninvasively predicting tools still have more appeal across healthcare systems.The red cell distribution width to platelet ratio(RPR)has been reported to be an indicator of hepatic fibrosis in Metabolic Dysfunction-Associated Steatotic Liver Disease(MASLD).However,its predictive role for the decompensation has never been explored.AIM In this observational study,we investigated the clinical usage of RPR in predicting DEs in MASLD-related cACLD patients.METHODS Fourty controls and 150 MASLD-cACLD patients were consecutively enrolled and followed up(FUP)semiannually for 3 years.At baseline,biochemical,clinical,and Liver Stiffness Measurement(LSM),Child-Pugh(CP),Model for End-Stage Liver Disease(MELD),aspartate aminotransferase/platelet count ratio index(APRI),Fibrosis-4(FIB-4),Albumin-Bilirubin(ALBI),ALBI-FIB-4,and RPR were collected.During FUP,DEs(timing and modaities)were recorded.CSPH was assessed at the baseline and on DE occurrence according to the available Clinical Practice Guidelines.RESULTS Of 150 MASLD-related cACLD patients,43(28.6%)progressed to dACLD at a median time of 28.9 months(29 NAD and 14 AD).Baseline RPR values were significantly higher in cACLD in comparison to controls,as well as MELD,CP,APRI,FIB-4,ALBI,ALBI-FIB-4,and LSM in dACLD-progressing compared to cACLD individuals[all P<0.0001,except for FIB-4(P:0.007)and ALBI(P:0.011)].Receiving operator curve analysis revealed RPR>0.472 and>0.894 as the best cut-offs in the prediction respectively of 3-year first DE,as well as its superiority compared to the other non-invasive tools examined.RPR(P:0.02)and the presence of baseline-CSPH(P:0.04)were significantly and independently associated with the DE.Patients presenting baseline-CSPH and RPR>0.472 showed higher risk of decompensation(P:0.0023).CONCLUSION Altogether these findings suggest the RPR as a valid and potentially applicable non-invasive tool in the prediction of timing and modalities of decompensation in MASLD-related cACLD patients. 展开更多

The oxidized LDL (50-300 μg/ml) induced the disc-sphere transformation of platelets, the formation of pseudopodia, centralization of granules and degranulation. Gallic acid is one of the components of Chinese herbal drug Radix paeoniae used for promoting blood circulation to remove blood stasis. This paper studied the effects of gallic acid and its esters (e.g. ethyl, propyl, isobutyl and butyl gallate) on model and human blood platelet membranes by FTIR which was used for monitoring the physical state of the acyl chain, interfacial and head group region of the membrane lipid bilayer. From the experimental results it can be seen that the gallic acid and its esters have the modifying function on the pure and cholesterol-containing DPPC model membranes, and have the quantity-effective and structural-effective relationships. In addition, it is discovered that these esters have the modifying effect on the structure of human blood platelet membrane and can reverse the effect of ADP. That the effect of the esters of gallic acid counteracts the effect of cholesterol and ADP on human blood platelet perhaps provides a new explanation of the mechanism of Chinese herbal 展开更多

Objective: To observe effects of Dan Wei Powder (胆胃散 Powder for treating the gall bladder and stomach) Tea Bag (DWSTB) on the aggregation rate of blood platelet in vivo and in vitro. Methods: Increase of the platelet aggregation rate in the rat in vivo was induced by carrageenin, and increase of the rabbit platelet aggregation rate in vitro was induced by adenosine diphosphate (ADP) and collagen, respectively. The effects of DWSTB on the platelet aggregation rate were investigated in vivo and in vitro, respectively. Results: The maximum in vivo platelet aggregation rate in the rat was significantly decreased after administration of 2.0 and 4.0 g·kg-1 DWSTB (P<0.05.P<0.01). The maximum rabbit platelet aggregation rate induced by ADP and collagen in vitro were suppressed significantly by 2.0-16.0 mg·mL-1 and 2.0-8.0 mg·mL-1 DWSTB, respectively (P<0.05.P<0.01). And the effect of DWSTB on platelet aggregation was raised with increase ofits dose. Conclusion: Dan Wei Powder Tea Bag can restrain the aggregation of platelet in vivo and in vitro. 展开更多

In this study, a point source mathematical model is proposed to describe the diffusion of adenosine di-phosphate (ADP) from either damaged red blood cell (RBC) or activated platelet. The convective diffusion equation is reduced to describe the suggested problem. The final differential equation is solved using Laplace transforms and ADP concentration profiles around the source are obtained. Thrombi of 5 to 20 μm<sup>3</sup> containing platelets and a range of red blood cells (RBCs) (0%, 25%, 50%, 75%, 100%) concentrations are used to apply the model. Reported ADP concentrations in the literature are used and its dynamic release from the point source is calculated. Results suggest that RBC chemical contribution to platelet aggregation in the bulk is much less than that of platelet (almost) negligible. However, the physical effect of RBCs is dominant in the bulk through augmentation of released ADP and platelets diffusivities. Moreover, the chemical contribution reported in previous studies is suggested to be as a result of interaction of RBC with the surface under the influence of shear stresses in the boundary region. 展开更多

Methods: Spinal cord injury at C6 segment was induced with Allen’s ’method in cats. PAF and PAF receptor antagonist BN52021 were administered by arachnoid space and intravenous injection respectively, and their effects on PAF levels, blood spinal cord barrier and cervical cord edema in the injuried zone and adjacent cervical cord tissue following cervical cord injury were investigated. Results: PAF levels, Evens content and water content in the injuried and adjacent cervical cord tissues significantly increased following trauma. PAF levels, Evens content and water content were evidently elevated with PAF by arachnoid space injection. PAF receptor antagonist BN52021 could inhibit the increase in PAF levels and reduce Evens and water content in the cervical cord tissue following trauma. Conclusion: PAF is an important contributing factor causing post-traumatic damage to the blood spinal cord barrier, while PAF receptor antagonist can effectively relieve post-traumatic damage to the blood spinal cord barrier. 展开更多

目的 探讨全血4℃冷藏条件下保存制备去白细胞混合浓缩血小板的可行性,为成分制备提供理论依据。方法 将采集的400 mL ACD-B抗凝全血随机分两组,分别进行4℃和室温保存,在保存后6 h内分离制备出白膜层,于22℃静置过夜保存,次日将白膜层... 目的 探讨全血4℃冷藏条件下保存制备去白细胞混合浓缩血小板的可行性,为成分制备提供理论依据。方法 将采集的400 mL ACD-B抗凝全血随机分两组,分别进行4℃和室温保存,在保存后6 h内分离制备出白膜层,于22℃静置过夜保存,次日将白膜层汇集制备去白细胞混合浓缩血小板,于制备后d1、d3、d5、d7取样,进行血细胞计数及相关参数检测,测定pH、葡萄糖、乳酸含量反映代谢情况,检测血栓弹力图、血小板聚集率及PAC-1、CD62P反映血小板体外功能及活化情况,比较两组血小板差异。结果 随着保存时间的延长,两组去白细胞混合浓缩血小板计数逐渐下降,血小板分布宽度(PDW)、平均血小板体积(MPV)、大型血小板比例(P-LCR)均逐渐上升,差异无统计学意义(P>0.05)。两组去白细胞混合浓缩血小板pH、葡萄糖含量均逐渐降低,乳酸含量逐渐增高,无显著性差异(P>0.05)。血栓弹力图结果显示,反应血小板功能的MA值保存期内无显著性变化,且两组间无显著性差异(P>0.05)。血小板聚集率随着保存时间的延长而逐渐降低,两组无统计学差异。血小板PAC-1及CD62P表达率随着保存时间的延长而逐渐升高,两组无统计学差异(P>0.05)。结论 全血4℃冷藏保存与室温保存6 h内制备的去白细胞混合浓缩血小板在体外计数、功能、活化方面无统计学差异,4℃冷藏6 h内的全血可考虑作为制备去白细胞混合浓缩血小板的起始血液。 展开更多

中华跌打丸活血化瘀作用的实验研究

目的:观察中华跌打丸对大鼠急性血瘀模型的体外血栓形成、血液黏度、血小板聚集率和对小鼠微循环障碍模型耳郭微循环的影响,评价中华跌打丸活血化瘀的作用。方法:通过注射肾上腺素加冰水刺激的方法建立大鼠急性血瘀模型,测定体外血栓形... 目的:观察中华跌打丸对大鼠急性血瘀模型的体外血栓形成、血液黏度、血小板聚集率和对小鼠微循环障碍模型耳郭微循环的影响,评价中华跌打丸活血化瘀的作用。方法:通过注射肾上腺素加冰水刺激的方法建立大鼠急性血瘀模型,测定体外血栓形成和血液黏度;按照Born氏比浊法加入诱导剂诱导聚集测定大鼠血小板聚集率;尾静脉注射15%高分子右旋糖酐造成微循环障碍,观察小鼠耳郭各时段微动脉和微静脉管血液流速和血管管径的变化。结果:中华跌打丸可降低血瘀模型大鼠的血液黏度,在切变率150 S^(-1)、60 S^(-1)及5 S^(-1)下的全血黏度均显著降低(P<0.05),明显缩短血瘀模型大鼠血栓长度,减轻血栓湿重和干重(P<0.05,P<0.01);中华跌打丸具有抑制血小板聚集的作用,各剂量均能明显降低ADP诱导的大鼠血小板聚集率(P<0.05,P<0.01);中华跌打丸对高分子右旋糖酐所致小鼠耳郭微循环障碍有明显改善作用,于造模10 min、20 min、30 min时增加血液流速(P<0.05,P<0.01),扩大微动脉、微静脉管径(P<0.05,P<0.01),减轻红细胞聚集(P<0.05,P<0.01)。结论:中华跌打丸可明显改善血液黏度,抑制血小板聚集,改善微循环障碍,有较好的活血化瘀作用。 展开更多

延长氨甲环酸使用时间有助于减少老年股骨转子间骨折围术期隐性失血

背景:股骨转子间骨折好发于老年人,手术后会出现大量的隐性失血,减少隐性失血可以降低并发症和住院时间。目的:评估延长氨甲环酸的使用时间对股骨近端防旋髓内钉治疗转子间骨折围术期隐性失血的影响。方法:选择2022年1月至2023年5月自... 背景:股骨转子间骨折好发于老年人,手术后会出现大量的隐性失血,减少隐性失血可以降低并发症和住院时间。目的:评估延长氨甲环酸的使用时间对股骨近端防旋髓内钉治疗转子间骨折围术期隐性失血的影响。方法:选择2022年1月至2023年5月自贡市第四人民医院经急诊收治入院的老年股骨转子间骨折患者共62例,均在牵引床上闭合牵引复位后置入股骨近端防旋髓内钉治疗。根据氨甲环酸的使用时间分为2组,对照组38例在切开皮肤前15-30 min给予氨甲环酸1 g静脉滴注,3 h后追加1 g;试验组24例在对照组的基础上,术后第1天再追加氨甲环酸1 g静脉滴注每12 h一次。术前、术后当天、术后第1,3,5天均复查血常规,统计血红蛋白量和红细胞压积,理论总失血量采用Gross方程计算,同时记录两组患者并发症发生情况。结果与结论:①通过统计学分析,两组患者术中显性出血量相差较小,差异无显著性意义(P>0.05);②试验组围术期血红蛋白下降量、总失血量、隐性失血量低于对照组,差异有显著性意义(P<0.05);③试验组术后第3天的血红蛋白值、术后第1,3天的红细胞压积值高于对照组,差异有显著性意义(P<0.05);④两组术后血红蛋白和血小板计数呈下降趋势,第3天的血红蛋白值为最低值,术后第1天的血小板值为最低值,然后开始回升;⑤两组患者术后并发症发生率相比差异无显著性意义(P>0.05);⑥结果表明,术后延迟氨甲环酸的使用时间,可能有助于减少股骨近端防旋髓内钉治疗股骨转子间骨折的隐性失血量,并不会增加并发症风险。 展开更多

目的:观察益气固本方联合穴位贴敷治疗脾气亏虚型原发免疫性血小板减少性紫癜(ITP)的疗效。方法:选取100例ITP患者,按随机数字表法分为观察组及对照组各50例,分别剔除2例,最终2组各48例。对照组接受重组人血小板生成素联合地塞米松治疗... 目的:观察益气固本方联合穴位贴敷治疗脾气亏虚型原发免疫性血小板减少性紫癜(ITP)的疗效。方法:选取100例ITP患者,按随机数字表法分为观察组及对照组各50例,分别剔除2例,最终2组各48例。对照组接受重组人血小板生成素联合地塞米松治疗,观察组在对照组基础上加用益气固本方联合穴位贴敷治疗。2组连续治疗2周。比较2组短中期疗效、中医证候疗效,比较2组治疗前后中医症状积分及血小板计数(BPC)水平的变化。结果:2组短期、中期总有效率比较,差异无统计学意义(P>0.05)。治疗后,观察组中医证候总有效率为95.83%,对照组为72.92%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组慢性出血、体倦乏力、神疲懒言、食欲不振、食后腹胀中医症状积分均较治疗前下降(P<0.05),观察组上述5项中医症状积分均低于对照组(P<0.05)。随着治疗时间的延长,2组BPC水平均较治疗前升高,治疗后第14天、第30天、第60天、第90天,观察组BPC水平均高于对照组(P<0.05)。结论:益气固本方联合穴位贴敷能改善脾气亏虚型ITP患者中医证候,稳定血小板。 展开更多

目的:探讨血小板与白细胞比值(platelet and white blood cells ratio, PWR)、血小板与淋巴细胞比值(platelet and lymphocyte ratio, PLR)、中性粒细胞与淋巴细胞比值(neutrophil to lymphocyte ratio, NLR)以及尿白细胞计数(urinary l... 目的:探讨血小板与白细胞比值(platelet and white blood cells ratio, PWR)、血小板与淋巴细胞比值(platelet and lymphocyte ratio, PLR)、中性粒细胞与淋巴细胞比值(neutrophil to lymphocyte ratio, NLR)以及尿白细胞计数(urinary leucocyte count, ULEU)与接受PD-1抑制剂治疗晚期尿路上皮癌(urothelial carcinoma, UC)患者预后的关系。方法:收集78例接受PD-1抑制剂治疗晚期UC患者的临床资料,利用Kaplan-Meier法计算生存率和Log-rank检验比较不同组间的生存差异,同时采用COX回归分析预后影响因素。结果:低PLR组与高PLR组的1年生存率分别为71.98%和47.63%,差异有统计学意义(P<0.05);低NLR组与高NLR组的1年生存率分别为71.36%和48.15%,差异有统计学意义(P<0.05);PWR高低两组和ULEU状态预测UC患者生存率无显著性差异(P>0.05)。单因素回归分析结果显示,N分期、M分期、TNM分期、远处转移个数、PLR、NLR是影响晚期UC患者生存情况的危险因素(P<0.05)。多因素回归分析显示,PLR是影响接受PD-1抑制剂治疗的UC患者预后的独立因素。结论:PLR、NLR为影响接受PD-1抑制剂治疗晚期UC患者预后的独立因素,PLR高值组和NLR高值组患者的预后较PLR低值组和NLR低值组的患者差。 展开更多

目的:分析急性缺血性脑卒中取栓患者的血栓成分及调节性T细胞表达与临床结局的关系。方法:连续纳入中国医科大学绍兴医院神经内科2021年6月至2022年10月接受颅内动脉取栓治疗的急性缺血性脑卒中患者。收集患者临床相关资料,对取栓术中... 目的:分析急性缺血性脑卒中取栓患者的血栓成分及调节性T细胞表达与临床结局的关系。方法:连续纳入中国医科大学绍兴医院神经内科2021年6月至2022年10月接受颅内动脉取栓治疗的急性缺血性脑卒中患者。收集患者临床相关资料,对取栓术中改良脑梗死溶栓(mTICI)分级和发病3个月改良Rankin量表(mRS)评分等临床结局进行评价。所有血栓标本均进行苏木精-伊红染色、免疫组织化学染色,血栓红细胞、纤维蛋白/血小板以及调节性T细胞行半定量分析。结果:共纳入具有完整血栓数据的44例患者,其中红细胞型15例、混合型11例、纤维蛋白/血小板型18例,三组间ORG 10172急性脑卒中治疗分类试验(TOAST)病因分型差异具有统计学意义(P<0.01),而其余一般临床资料、手术相关资料无明显差异(均P>0.05)。按TOAST病因分型分为大动脉粥样硬化型28例和心源性栓塞型16例。大动脉粥样硬化型患者血栓的红细胞比例明显高于心源性栓塞型[分别为58.0%(44.2%,72.5%)和24.5%(12.7%,48.0%),P<0.01],而心源性栓塞型患者血栓的纤维蛋白/血小板比例明显高于大动脉粥样硬化型[分别为73.0%(49.2%,84.5%)和40.0%(25.2%,54.5%),P<0.01]。44例患者中,神经功能结局良好者19例,神经功能结局不良者25例,对患者相关资料进行单因素二元logistic回归分析,发现年龄、手术时间、CD4^(+)CD25^(+)T细胞数与患者的神经功能结局相关(均P<0.05),而其余临床相关资料及手术相关资料与患者神经功能结局无关(均P>0.05)。采用多因素二元logistic回归分析,纳入年龄、手术时间、CD4^(+)CD25^(+)T细胞数等变量,发现血栓CD4^(+)CD25^(+)T细胞数是患者神经功能结局的独立影响因素(OR=1.369,95%CI:1.101~1.701,P<0.01)。结论:血栓中红细胞、纤维蛋白/血小板成分与患者功能结局无明显相关性,但调节性T细胞表达增高与缺血性脑卒中患者神经功能结局良好相关。 展开更多

目的探究临床上重症脓毒血症的治疗中,采用连续性血液净化的效果和价值。方法选取2018年6月至2021年6月南宁市第一人民医院收治的100例重症脓毒血症患者作为研究对象,采用随机数字表法分为对照组(50例)和试验组(50例),对照组采用常规治... 目的探究临床上重症脓毒血症的治疗中,采用连续性血液净化的效果和价值。方法选取2018年6月至2021年6月南宁市第一人民医院收治的100例重症脓毒血症患者作为研究对象,采用随机数字表法分为对照组(50例)和试验组(50例),对照组采用常规治疗方法,试验组采用连续性血液净化治疗方法。比较两组患者的不良反应总发生率、临床疗效、血小板含量、肾功能指标以及炎症因子水平。结果试验组的不良反应总发生率低于对照组,且临床疗效高于对照组,血小板含量低于对照组,差异有统计学意义(P<0.05);治疗前两组的肌酐、尿素氮、C反应蛋白(CRP)、降钙素原(PCT)、肿瘤坏死因子-α(TNF-α)水平比较,差异无统计学意义(P>0.05),治疗后,试验组的肌酐、尿素氮、CRP、PCT、TNF-α低于对照组,差异有统计学意义(P<0.05)。结论对重症脓毒血症患者进行连续性血液净化治疗的效果显著,能够改善患者的肾功能指标以及炎症因子水平。 展开更多

目的探讨血细胞计数及相关参数是否可以预测流感合并下呼吸道感染患者的预后。方法回顾性研究2013年至2023年首都医科大学附属北京朝阳医院经急诊医学科、发热门诊或呼吸科收治入院或留观的流感合并下呼吸道感染患者,以28天死亡为研究终... 目的探讨血细胞计数及相关参数是否可以预测流感合并下呼吸道感染患者的预后。方法回顾性研究2013年至2023年首都医科大学附属北京朝阳医院经急诊医学科、发热门诊或呼吸科收治入院或留观的流感合并下呼吸道感染患者,以28天死亡为研究终点,分为生存组和死亡组,比较分析两组间血细胞计数及相关参数的差异,回归分析死亡的独立预测因素,建模并对模型进行评价。结果此项研究最终纳入417例患者,28天生存组361例(86.6%),死亡组56例(13.4%)。死亡组淋巴细胞计数和血小板计数显著低于生存组,红细胞分布宽度、血小板分布宽度、血小板平均体积及大血小板比率显著高于生存组(P<0.05)。多变量Logistic回归分析筛选独立相关变量,当淋巴细胞计数≤0.68×10^(9)/L,或血小板计数≤147×10^(9)/L,或红细胞分布宽度>12.7%,或血小板分布宽度>13.9 fL时,患者28天死亡风险增加,四个指标建立预测模型的AUC为0.734(95%CI 0.664~0.804),敏感度58.9%,特异度80.4%,阳性预测值32.0%,阴性预测值92.7%,阳性似然比3.01,阴性似然比0.51。模型的一致性指数为0.735,拟合优度Hosmer-Lemeshow检验显示拟合效果较好(P=0.406)。结论淋巴细胞计数、血小板计数、红细胞分布宽度、血小板分布宽度可作为早期预测流感合并下呼吸道感染患者28天死亡风险的指标,以这些指标构建的列线图模型可计算临床实测值所对应的死亡风险概率。 展开更多
