The nose radius of the cone is 0.5 mm; the cone half-angle is 5°, and the Mach number of the oncoming flow is 6. The base flow of the blunt cone was obtained by direct numerical simulation. The linear stability theory was applied for the analysis of the first mode and the second mode unstable waves under both isothermal and adiabatic wall condition, and e^N method was used for the prediction of transition location. The N factor was tentatively taken as 10, as no experimentally confirmed value was available. It is found that the wall temperature condition has a great effect on the transition location. For adiabatic wall, transition would take place more rearward than those for isothermal wall. And despite that for high Mach number flows, the maximum amplification rate of the second mode wave is far bigger than the maximum amplification rate of the first mode wave, the transition location of the boundary layer with adiabatic wall is controlled by the growth of first mode unstable waves. The methods employed in this paper are expected to be also applicable to the transition prediction for the three dimensional boundary layers on cones with angle of attack.展开更多

It is shown that second-mode suppression is achieved via entropy layer modulation of the basic state density gradient. A weakening of the density gradient disrupts the acoustic resonance necessary to sustain second-mode growth. These results are consistent with the thermoacoustic interpretation which posits that second-mode instability can be modeled as thermoacoustic resonance of acoustic energy trapped within an acoustic impedance well.Furthermore, the generalized inflection point criterion of Lees and Lin is applied to develop a criterion for the existence of second-mode instability based on the strength of the basic state density gradient.展开更多
In this work,we studied the bluntness effect on the hypersonic boundary-layer transition over a slender cone at Mach 6 with interchangeable tips in a noisy Ludwieg tube tunnel before the so-called“transition reversal...In this work,we studied the bluntness effect on the hypersonic boundary-layer transition over a slender cone at Mach 6 with interchangeable tips in a noisy Ludwieg tube tunnel before the so-called“transition reversal”phenomenon occurs.The evolution of instability waves is characterized using surface flush-mounted pressure sensors deployed along the streamwise direction within unit Reynolds number from 4E+6/m≤Reunit≤10E+6/m,and the bluntness of the cone nose ranges from 0.1 mm to 5 mm.Power spectral density(PSD)of pressure fluctuation indicates that small nose bluntness(ReR≤2000)has little influence on the evolution of instability waves along the hypersonic boundary-layer,whereas with a moderate nose size(2000≤ReR≤5000),the hypersonic boundary layer transition is delayed monotonically as the nose radius increases before the boundary-layer turns into fully laminar without instability waves.The delaying effect can be attributed to the increased entropy-layer swallowing distance with a large tip radius.Instability wave characterization reveals that the second mode instability wave plays a dominant role before the transition reversal happens.The quadratic phase locking of second mode instabilities can be identified by bispectral analysis,and it attenuates as the nose tip radius increases.展开更多
BACKGROUND Traumatic internal carotid artery(ICA)occlusion is a rare complication of skull base fractures,characterized by high mortality and disability rates,and poor prognosis.Therefore,timely discovery and correct ...BACKGROUND Traumatic internal carotid artery(ICA)occlusion is a rare complication of skull base fractures,characterized by high mortality and disability rates,and poor prognosis.Therefore,timely discovery and correct management are crucial for saving the lives of such patients and improving their prognosis.This article retrospectively analyzed the imaging and clinical data of three patients,to explore the imaging characteristics and treatment strategies for carotid artery occlusion,combined with severe skull base fractures.CASE SUMMARY This case included three patients,all male,aged 21,63,and 16 years.They underwent plain film skull computed tomography(CT)examination at the onset of their illnesses,which revealed fractures at the bases of their skulls.Ultimately,these cases were definitively diagnosed through CT angiography(CTA)examinations.The first patient did not receive surgical treatment,only anticoagulation therapy,and recovered smoothly with no residual limb dysfunction(Case 1).The other two patients both developed intracranial hypertension and underwent decompressive craniectomy.One of these patients had high intracranial pressure and significant brain swelling postoperatively,leading the family to choose to take him home(Case 2).The other patient also underwent decompressive craniectomy and recovered well postoperatively with only mild limb motor dysfunction(Case 3).We retrieved literature from PubMed on skull base fractures causing ICA occlusion to determine the imaging characteristics and treatment strategies for this type of disease.CONCLUSION For patients with cranial trauma combined with skull base fractures,it is essential to complete a CTA examination as soon as possible,to screen for blunt cerebrovascular injury.展开更多
To explore the response law of non-lethal large-size kinetic energy projectiles to blunt attack on skin tissue,and to evaluate the skin injury characteristics of the attacked personnel and the use safety of kinetic en...To explore the response law of non-lethal large-size kinetic energy projectiles to blunt attack on skin tissue,and to evaluate the skin injury characteristics of the attacked personnel and the use safety of kinetic energy projectiles.Based on the LS-DYNA simulation software,a three-layer skin simulation model and a Flash-Ball rubber bullet model are established,and the force-time and deformation-time biomechanical corridors of the Flash-Ball rubber bullet impacting human skin tissue are obtained.The corridor curve and the energy transfer and diffusion are analyzed and compared.The safety evaluation of the damage caused by the rubber bullet shooting a human body at different distances is carried out using the empirical formula of the penetration limit.Finally,the safe shooting distance is obtained.The results show that the model used in the simulation has a good correlation with the experimental data,its biomechanical corridor characteristics are different from those of conventional vehicle impact and smallsize projectile response characteristics.The energy transfer and action time of medium and low-speed impact may cause greater damage.The fat layer is the largest energy absorption unit.The minimum safe shooting distance to ensure skin tissue from penetrating damage is 15.8 m,and the limit specific kinetic energy of skin damage is 7.88 J/cm^(2).This study can be extended to the study of biomechanical response law and safety evaluation under the impact of the same type of large kinetic energy projectile,which provides an important theoretical reference for the police to use large kinetic energy projectiles to conduct safe shooting in peacekeeping operations.展开更多
BACKGROUND The spleen is the most commonly injured solid organ in blunt abdominal trauma,and splenic pseudoaneurysm rupture is associated with a high risk of mortality.Nonoperative management has become the standard t...BACKGROUND The spleen is the most commonly injured solid organ in blunt abdominal trauma,and splenic pseudoaneurysm rupture is associated with a high risk of mortality.Nonoperative management has become the standard treatment for hemodynam-ically stable patients with splenic injuries.On the other hand,delayed splenic pseudoaneurysms can develop in any patient,and at present,there are no known risk factors that may reliably predict their occurrence.Furthermore,there is a lack of consensus regarding the most appropriate strategies for monitoring and mana-ging splenic injuries,especially lower-grade(I-III).AIM To determine the predictors of pseudo-aneurysm formation following splenic injury and develop follow-up strategies for early detection of pseudoaneurysms.METHODS We retrospectively analyzed patients who visited the Level I Trauma Center bet-ween January 2013 and December 2022 and were diagnosed with spleen injuries after blunt abdominal trauma.RESULTS Using the American Association for the Surgery of Trauma spleen injury scale,the splenic injuries were categorized into the following order based on severity:Grade I(n=57,17.6%),grade II(n=114,35.3%),grade III(n=89,27.6%),grade IV(n=50,15.5%),and grade V(n=13,4.0%).Of a total of 323 patients,35 underwent splenectomy and 126 underwent angioembolization.19 underwent delayed angioembolization,and 5 under-went both initial and delayed angioem-bolization.In 14 patients who had undergone delayed angioembolization,no extravasation or pseudoaneurysm was observed on the initial computed tomography scan.There are no particular patient-related risk factors for the formation of a delayed splenic pseudoaneurysm,which can occur even in a grade I spleen injury or even 21 days after the injury.The mean detection time for a delayed pseudoaneurysm was 6.26±5.4(1-21,median:6,inter-quartile range:2-9)days.CONCLUSION We recommend regular follow-up computed tomography scans,including an arterial and portal venous phase,at least 1 week and 1 month after injury in any grade of blunt traumatic spleen injury for the timely detection of delayed pseudoaneurysms.展开更多

BACKGROUND Pelvic fractures(PF)with concomitant injuries are on the rise due to an increase of high-energy trauma.Increase of the elderly population with age related comorbidities further complicates the management.Ab...BACKGROUND Pelvic fractures(PF)with concomitant injuries are on the rise due to an increase of high-energy trauma.Increase of the elderly population with age related comorbidities further complicates the management.Abdominal organ injuries are kindred with PF due to the proximity to pelvic bones.Presence of contrast blush(CB)on computed tomography in patients with PF is considered a sign of active bleeding,however,its clinical significance and association with outcomes is debatable.AIM To analyze polytrauma patients with PF with a focus on the geriatric population,co-injuries and the value of contrast blush.METHODS This retrospective cohort study included 558 patients with PF admitted to level 1 trauma center(01/2017-01/2023).Analyzed variables included:Age,sex,mechanism of injury(MOI),injury severity score(ISS),Glasgow coma scale(GCS),abbreviated injury scale(AIS),co-injuries,transfusion requirements,pelvic angiography,embolization,laparotomy,orthopedic pelvic surgery,intensive care unit and hospital lengths of stay,discharge disposition and mortality.The study compared geriatric and non-geriatric patients,patients with and without CB and abdominal co-injuries.Propensity score matching was implemented in comparison groups.RESULTS PF comprised 4%of all trauma admissions.89 patients had CB.286(52%)patients had concomitant injuries including 93(17%)patients with abdominal co-injuries.Geriatric patients compared to non-geriatric had more falls as MOI,lower ISS and AIS pelvis,higher GCS,less abdominal co-injuries,similar CB and angio-embolization rates,less orthopedic pelvic surgeries,shorter lengths of stay and higher mortality.After propensity matching,orthopedic pelvic surgery rates remained lower(8%vs 19%,P<0.001),hospital length of stay shorter,and mortality higher(13%vs 4%,P<0.001)in geriatric patients.Out of 89 patients with CB,45(51%)were embolized.After propensity matching,patients with CB compared to without CB had more pelvic angiography(71%vs 12%,P<0.001),higher embolization rates(64%vs 22%,P=0.02)and comparable mortality.CONCLUSION Half of the patients with PF had concomitant co-injuries,including abdominal co-injuries in 17%.Similarly injured geriatric patients had higher mortality.Half of the patients with CB required an embolization.展开更多

METHODS: A total of 82 patients with colonic injuries caused by blunt trauma...AIM: To retrospectively evaluate the preoperative diagnostic approaches and management of colonic injuries following blunt abdominal trauma. METHODS: A total of 82 patients with colonic injuries caused by blunt trauma between January 1992 and December 2005 were enrolled. Data were collected on clinical presentation, investigations, diagnostic methods, associated injuries, and operative management. Colonic injury-related mortality and abdominal complications were analyzed. RESULTS: Colonic injuries were caused mainly by motor vehicle accidents. Of the 82 patients, 58 (70.3%) had other associated injuries. Laparotomy was performed within 6 h after injury in 69 cases (84.1%), laparoscopy in 3 because of haemodynamic instability. The most commonly injured site was located in the transverse colon. The mean colon injury scale score was 2.8. The degree of faecal contamination was classified as mild in 18 (22.0%), moderate in 42 (51.2%), severe in 14 (17.1%), and unknown in 8 (9.8%) cases. Sixty-seven patients (81.7%) were treated with primary repair or resection and anastomosis. Faecal stream diversion was performed in 15 cases (18.3%). The overall mortality rate was 6.1%. The incidence of colonic injury- related abdominal complications was 20.7%. The only independent predictor of complications was the degree of peritoneal faecal contamination (P = 0.02). CONCLUSION: Colonic injuries following blunt trauma are especially important because of the severity and complexity of associated injuries. A thorough physical examination and a combination of tests can be used to evaluate the indications for laparotomy. One stage management at the time of initial exploration is mostoften used for colonic injuries.展开更多
BACKGROUND:Point-of-care ultrasound(POCUS)has become increasingly integrated into the practice of emergency medicine.A common application is the extended focused assessment with sonography in trauma(eFAST)exam.The Ame...BACKGROUND:Point-of-care ultrasound(POCUS)has become increasingly integrated into the practice of emergency medicine.A common application is the extended focused assessment with sonography in trauma(eFAST)exam.The American College of Emergency Physicians has guidelines regarding the scope of ultrasound in the emergency department and the appropriate documentation.The objective of this study was to conduct a review of performed,documented and billed eFAST ultrasounds on trauma activation patients.METHODS:This was a retrospective review of all trauma activation patients during a 10-month period at an academic level-one trauma center.A list comparing all trauma activations was crossreferenced with a list of all billed eFAST scans.Medical records were reviewed to determine whether an eFAST was indicated,performed,and appropriately documented.RESULTS:We found that 1,507 of 1,597 trauma patients had indications for eFAST,but 396(27%)of these patients did not have a billed eFAST.Of these 396 patients,87(22%)had documentation in the provider note that an eFAST was performed but there was no separate procedure note.The remaining 309(78%)did not have any documentation of the eFAST in the patient’s chart although an eFAST was recorded and reviewed during ultrasound quality assurance.CONCLUSION:A significant proportion of trauma patients had eFAST exams performed but were not documented or billed.Lack of documentation was multifactorial.Emergency ultrasound programs require appropriate reimbursement to support training,credentialing,equipment,quality assurance,and device maintenance.Our study demonstrates a significant absence of adequate documentation leading to potential revenue loss for an emergency ultrasound program.展开更多

Chest X-ray has low sensitivity in detecting BDL and lesions can be reliably suspected only in case of intra-thoracic herniation of abdominal viscera. Thanks to its wide availability, time-effectiveness and spatial resolution, multi-detector computed tomography (CT) is the imaging modality of choice for diagnosing BDL; several direct and indirect CT signs are associated with BDL. Given its high tissue contrast resolution, magnetic resonance imaging can accurately depict BDL, but its use in an emergency setting is limited because of longer acquisition times and need for patient’s collaboration.展开更多

The plane acoustic wave interacts with the bow shock, and generates all types of disturbances behind the shock, which may take various routes to generate the boundarylayer unstable mode. In this paper, two routes of receptivity are investigated in detail.One is through the disturbance in the entropy layer. The other is through the slow acoustic wave transmitted downstream the bow shock, which can excite the boundary-layer mode due to the synchronization mechanism. The results show that, for a low frequency slow acoustic wave, the latter route plays a leading role. The entropy-layer instability wave is able to excite the first mode near the neutral point, but its receptivity efficiency is much lower.展开更多

Pressure wave plays an important role in the occurrence of behind armor blunt trauma(BABT),and ballistic gelatin is widely used as a surrogate of biological tissue in the research of BABT.Comparison of pressure wave i...Pressure wave plays an important role in the occurrence of behind armor blunt trauma(BABT),and ballistic gelatin is widely used as a surrogate of biological tissue in the research of BABT.Comparison of pressure wave in the gelatin behind armor for different rifle bullets is lacking.The aim of this study was to observe dynamic changes in pressure wave induced by ballistic blunt impact on the armored gelatin block and to compare the effects of bullet type on the parameters of the transient pressure wave.The gelatin blocks protected with National Institute of Justice(NIJ) class III bulletproof armor were shot by three types of rifle bullet with the same level of impact energy.The transient pressure signals at five locations were recorded with pressure sensors and three parameters(maximum pressure,maximum pressure impulse,and the duration of the first positive phase) were determined and discussed.The results indicated that the waveform and the twin peak of transient pressure wave were not related to the bullet type.However,the values of pressure wave's parameters were significantly affected by bullet type.Additionally,the attenuation of pressure amplitude followed the similar law for the three ammunitions.These findings may be helpful to get some insight in the BABT and improve the structure design of bullet.展开更多

In the present study, the genetic diversity of one selected strain (Pujiang No. 1), two domesticated populations (GA and HX) and four wild populations (LZ, YN, SS and JL) of blunt snout bream (Megalobrama ambly...In the present study, the genetic diversity of one selected strain (Pujiang No. 1), two domesticated populations (GA and HX) and four wild populations (LZ, YN, SS and JL) of blunt snout bream (Megalobrama amblycephala) was analyzed using 17 microsatellite markers. The results showed that an average of 4.88-7.65 number of alleles (A); an average of 3.20-5.33 effective alleles (Ne); average observed beterozygosity (Ho) of 0.6985-0.9044; average expected beterozygosity (He) of 0.6501--0.7805; and the average polymorphism information content (PIC) at 0.5706-0.7226. Pairwise FST value between populations ranged from 0.0307-0.1451, and Nei's standard genetic distance between populations was 0.0938-0.4524. The expected heterozygosities in the domesticated populations (GA and HX) were significantly lower than those found in three wild populations (LZ, SS and JL), but no difference was detected when compared with the wild YN population. Likewise, no difference was found between the four wild populations or two domesticated populations. The expected heterozygosity in Pujiang No. 1 was higher than the two domesticated populations and lower than the four wild populations. Regarding pairwise Fsr value between populations, permutation test P-values were significant between the GA, HX and PJ populations, but not between the four wild populations. These results showed that the expected beterozygosity in the selected strain of blunt snout bream, after seven generations of selective breeding, was lower than that of wild populations, but this strain retains higher levels of genetic diversity than domesticated populations. The genetic differences and differentiation amongst wild populations, domesticated populations and the genetically improved strain of blunt snout bream will provide important conservation criteria and guide the utilization of germplasm resources.展开更多

In a Mach 3.8 wind tunnel, both instantaneous and time-averaged flow structures of different scales around a blunt double-cone with or without supersonic film cooling were visualized via nano-tracer planar laser scatt...In a Mach 3.8 wind tunnel, both instantaneous and time-averaged flow structures of different scales around a blunt double-cone with or without supersonic film cooling were visualized via nano-tracer planar laser scattering (NPLS), which has a high spatiotemporal resolution. Three experimental cases with different injection mass flux rates were carried out. Many typical flow structures were clearly shown, such as shock waves, expansion fans, shear layers, mixing layers, and turbulent boundary layers. The analysis of two NPLS images with an interval of 5 us revealed the temporal evolution characteristics of flow structures. With matched pressures, the laminar length of the mixing layer was longer than that in the case with a larger mass flux rate, but the full covered region was shorter. Structures like K-H (Kelvin-Helmholtz) vortices were clearly seen in both flows. Without injection, the flow was similar to the supersonic flow over a backward- facing step, and the structures were relatively simpler, and there was a longer laminar region. Large scale structures such as hairpin vortices were visualized. In addition, the results were compared in part with the schlieren images captured by others under similar conditions.展开更多

文摘Stability and transition prediction of hypersonic boundary layer on a blunt cone with small nose bluntness at zero angle of attack was investigated. The nose radius of the cone is 0.5 mm; the cone half-angle is 5°, and the Mach number of the oncoming flow is 6. The base flow of the blunt cone was obtained by direct numerical simulation. The linear stability theory was applied for the analysis of the first mode and the second mode unstable waves under both isothermal and adiabatic wall condition, and e^N method was used for the prediction of transition location. The N factor was tentatively taken as 10, as no experimentally confirmed value was available. It is found that the wall temperature condition has a great effect on the transition location. For adiabatic wall, transition would take place more rearward than those for isothermal wall. And despite that for high Mach number flows, the maximum amplification rate of the second mode wave is far bigger than the maximum amplification rate of the first mode wave, the transition location of the boundary layer with adiabatic wall is controlled by the growth of first mode unstable waves. The methods employed in this paper are expected to be also applicable to the transition prediction for the three dimensional boundary layers on cones with angle of attack.

文摘A physical mechanism by which nose bluntness suppresses second-mode instability is proposed.Considered are 7 degree half-angle straight cones with nose bluntness radii of 0.15 mm, 3.556 mm,5 mm, 9.525 mm, 12.7 mm and 25.4 mm at tunnel conditions relevant to the AFOSR-Notre Dame Large Mach 6 Quiet Tunnel. It is shown that second-mode suppression is achieved via entropy layer modulation of the basic state density gradient. A weakening of the density gradient disrupts the acoustic resonance necessary to sustain second-mode growth. These results are consistent with the thermoacoustic interpretation which posits that second-mode instability can be modeled as thermoacoustic resonance of acoustic energy trapped within an acoustic impedance well.Furthermore, the generalized inflection point criterion of Lees and Lin is applied to develop a criterion for the existence of second-mode instability based on the strength of the basic state density gradient.

文摘In this work,we studied the bluntness effect on the hypersonic boundary-layer transition over a slender cone at Mach 6 with interchangeable tips in a noisy Ludwieg tube tunnel before the so-called“transition reversal”phenomenon occurs.The evolution of instability waves is characterized using surface flush-mounted pressure sensors deployed along the streamwise direction within unit Reynolds number from 4E+6/m≤Reunit≤10E+6/m,and the bluntness of the cone nose ranges from 0.1 mm to 5 mm.Power spectral density(PSD)of pressure fluctuation indicates that small nose bluntness(ReR≤2000)has little influence on the evolution of instability waves along the hypersonic boundary-layer,whereas with a moderate nose size(2000≤ReR≤5000),the hypersonic boundary layer transition is delayed monotonically as the nose radius increases before the boundary-layer turns into fully laminar without instability waves.The delaying effect can be attributed to the increased entropy-layer swallowing distance with a large tip radius.Instability wave characterization reveals that the second mode instability wave plays a dominant role before the transition reversal happens.The quadratic phase locking of second mode instabilities can be identified by bispectral analysis,and it attenuates as the nose tip radius increases.
文摘BACKGROUND Traumatic internal carotid artery(ICA)occlusion is a rare complication of skull base fractures,characterized by high mortality and disability rates,and poor prognosis.Therefore,timely discovery and correct management are crucial for saving the lives of such patients and improving their prognosis.This article retrospectively analyzed the imaging and clinical data of three patients,to explore the imaging characteristics and treatment strategies for carotid artery occlusion,combined with severe skull base fractures.CASE SUMMARY This case included three patients,all male,aged 21,63,and 16 years.They underwent plain film skull computed tomography(CT)examination at the onset of their illnesses,which revealed fractures at the bases of their skulls.Ultimately,these cases were definitively diagnosed through CT angiography(CTA)examinations.The first patient did not receive surgical treatment,only anticoagulation therapy,and recovered smoothly with no residual limb dysfunction(Case 1).The other two patients both developed intracranial hypertension and underwent decompressive craniectomy.One of these patients had high intracranial pressure and significant brain swelling postoperatively,leading the family to choose to take him home(Case 2).The other patient also underwent decompressive craniectomy and recovered well postoperatively with only mild limb motor dysfunction(Case 3).We retrieved literature from PubMed on skull base fractures causing ICA occlusion to determine the imaging characteristics and treatment strategies for this type of disease.CONCLUSION For patients with cranial trauma combined with skull base fractures,it is essential to complete a CTA examination as soon as possible,to screen for blunt cerebrovascular injury.
文摘To explore the response law of non-lethal large-size kinetic energy projectiles to blunt attack on skin tissue,and to evaluate the skin injury characteristics of the attacked personnel and the use safety of kinetic energy projectiles.Based on the LS-DYNA simulation software,a three-layer skin simulation model and a Flash-Ball rubber bullet model are established,and the force-time and deformation-time biomechanical corridors of the Flash-Ball rubber bullet impacting human skin tissue are obtained.The corridor curve and the energy transfer and diffusion are analyzed and compared.The safety evaluation of the damage caused by the rubber bullet shooting a human body at different distances is carried out using the empirical formula of the penetration limit.Finally,the safe shooting distance is obtained.The results show that the model used in the simulation has a good correlation with the experimental data,its biomechanical corridor characteristics are different from those of conventional vehicle impact and smallsize projectile response characteristics.The energy transfer and action time of medium and low-speed impact may cause greater damage.The fat layer is the largest energy absorption unit.The minimum safe shooting distance to ensure skin tissue from penetrating damage is 15.8 m,and the limit specific kinetic energy of skin damage is 7.88 J/cm^(2).This study can be extended to the study of biomechanical response law and safety evaluation under the impact of the same type of large kinetic energy projectile,which provides an important theoretical reference for the police to use large kinetic energy projectiles to conduct safe shooting in peacekeeping operations.
文摘BACKGROUND The spleen is the most commonly injured solid organ in blunt abdominal trauma,and splenic pseudoaneurysm rupture is associated with a high risk of mortality.Nonoperative management has become the standard treatment for hemodynam-ically stable patients with splenic injuries.On the other hand,delayed splenic pseudoaneurysms can develop in any patient,and at present,there are no known risk factors that may reliably predict their occurrence.Furthermore,there is a lack of consensus regarding the most appropriate strategies for monitoring and mana-ging splenic injuries,especially lower-grade(I-III).AIM To determine the predictors of pseudo-aneurysm formation following splenic injury and develop follow-up strategies for early detection of pseudoaneurysms.METHODS We retrospectively analyzed patients who visited the Level I Trauma Center bet-ween January 2013 and December 2022 and were diagnosed with spleen injuries after blunt abdominal trauma.RESULTS Using the American Association for the Surgery of Trauma spleen injury scale,the splenic injuries were categorized into the following order based on severity:Grade I(n=57,17.6%),grade II(n=114,35.3%),grade III(n=89,27.6%),grade IV(n=50,15.5%),and grade V(n=13,4.0%).Of a total of 323 patients,35 underwent splenectomy and 126 underwent angioembolization.19 underwent delayed angioembolization,and 5 under-went both initial and delayed angioem-bolization.In 14 patients who had undergone delayed angioembolization,no extravasation or pseudoaneurysm was observed on the initial computed tomography scan.There are no particular patient-related risk factors for the formation of a delayed splenic pseudoaneurysm,which can occur even in a grade I spleen injury or even 21 days after the injury.The mean detection time for a delayed pseudoaneurysm was 6.26±5.4(1-21,median:6,inter-quartile range:2-9)days.CONCLUSION We recommend regular follow-up computed tomography scans,including an arterial and portal venous phase,at least 1 week and 1 month after injury in any grade of blunt traumatic spleen injury for the timely detection of delayed pseudoaneurysms.

文摘The internal carotid artery occlusion caused by head and neck trauma,also known as traumatic intracranial artery occlusion,is relatively rare clinically.Traumatic skull base fracture is a common complication of traumatic brain injury.Traumatic skull base fracture is one of the causes of traumatic internal carotid artery occlusion.If not detected early and treated in time,the prognosis of patients is poor.This editorial makes a relevant analysis of this disease.
文摘BACKGROUND Pelvic fractures(PF)with concomitant injuries are on the rise due to an increase of high-energy trauma.Increase of the elderly population with age related comorbidities further complicates the management.Abdominal organ injuries are kindred with PF due to the proximity to pelvic bones.Presence of contrast blush(CB)on computed tomography in patients with PF is considered a sign of active bleeding,however,its clinical significance and association with outcomes is debatable.AIM To analyze polytrauma patients with PF with a focus on the geriatric population,co-injuries and the value of contrast blush.METHODS This retrospective cohort study included 558 patients with PF admitted to level 1 trauma center(01/2017-01/2023).Analyzed variables included:Age,sex,mechanism of injury(MOI),injury severity score(ISS),Glasgow coma scale(GCS),abbreviated injury scale(AIS),co-injuries,transfusion requirements,pelvic angiography,embolization,laparotomy,orthopedic pelvic surgery,intensive care unit and hospital lengths of stay,discharge disposition and mortality.The study compared geriatric and non-geriatric patients,patients with and without CB and abdominal co-injuries.Propensity score matching was implemented in comparison groups.RESULTS PF comprised 4%of all trauma admissions.89 patients had CB.286(52%)patients had concomitant injuries including 93(17%)patients with abdominal co-injuries.Geriatric patients compared to non-geriatric had more falls as MOI,lower ISS and AIS pelvis,higher GCS,less abdominal co-injuries,similar CB and angio-embolization rates,less orthopedic pelvic surgeries,shorter lengths of stay and higher mortality.After propensity matching,orthopedic pelvic surgery rates remained lower(8%vs 19%,P<0.001),hospital length of stay shorter,and mortality higher(13%vs 4%,P<0.001)in geriatric patients.Out of 89 patients with CB,45(51%)were embolized.After propensity matching,patients with CB compared to without CB had more pelvic angiography(71%vs 12%,P<0.001),higher embolization rates(64%vs 22%,P=0.02)and comparable mortality.CONCLUSION Half of the patients with PF had concomitant co-injuries,including abdominal co-injuries in 17%.Similarly injured geriatric patients had higher mortality.Half of the patients with CB required an embolization.

文摘A self-developed double-nozzle gas atomization technique was used to produce AlSi10Mg powder.Effects of delivery tube diameter,gas pressure,and melt superheat on powder characteristics were investigated.The concepts of bluntness and outgrowth were introduced to analyze powder sphericity and satellite index quantitatively.The results showed that the median diameters of all atomized powders ranged from 25 to 33μm.The highest yield rate(72.13%)of fine powder(<50μm)was obtained at a superheat of 350 K.The powder size decreased with increasing melt superheat but increased with increasing delivery tube diameter.Powders with bluntness values between 96%and 98%accounted for over 60%.The outgrowth values demonstrated that 70%-85%of all powders did not contain satellite particles,with few powders adhered two or three particles.Not only Al and Si phases were present but also a metastable Al9Si phase was detected.
文摘AIM: To retrospectively evaluate the preoperative diagnostic approaches and management of colonic injuries following blunt abdominal trauma. METHODS: A total of 82 patients with colonic injuries caused by blunt trauma between January 1992 and December 2005 were enrolled. Data were collected on clinical presentation, investigations, diagnostic methods, associated injuries, and operative management. Colonic injury-related mortality and abdominal complications were analyzed. RESULTS: Colonic injuries were caused mainly by motor vehicle accidents. Of the 82 patients, 58 (70.3%) had other associated injuries. Laparotomy was performed within 6 h after injury in 69 cases (84.1%), laparoscopy in 3 because of haemodynamic instability. The most commonly injured site was located in the transverse colon. The mean colon injury scale score was 2.8. The degree of faecal contamination was classified as mild in 18 (22.0%), moderate in 42 (51.2%), severe in 14 (17.1%), and unknown in 8 (9.8%) cases. Sixty-seven patients (81.7%) were treated with primary repair or resection and anastomosis. Faecal stream diversion was performed in 15 cases (18.3%). The overall mortality rate was 6.1%. The incidence of colonic injury- related abdominal complications was 20.7%. The only independent predictor of complications was the degree of peritoneal faecal contamination (P = 0.02). CONCLUSION: Colonic injuries following blunt trauma are especially important because of the severity and complexity of associated injuries. A thorough physical examination and a combination of tests can be used to evaluate the indications for laparotomy. One stage management at the time of initial exploration is mostoften used for colonic injuries.
文摘BACKGROUND:Point-of-care ultrasound(POCUS)has become increasingly integrated into the practice of emergency medicine.A common application is the extended focused assessment with sonography in trauma(eFAST)exam.The American College of Emergency Physicians has guidelines regarding the scope of ultrasound in the emergency department and the appropriate documentation.The objective of this study was to conduct a review of performed,documented and billed eFAST ultrasounds on trauma activation patients.METHODS:This was a retrospective review of all trauma activation patients during a 10-month period at an academic level-one trauma center.A list comparing all trauma activations was crossreferenced with a list of all billed eFAST scans.Medical records were reviewed to determine whether an eFAST was indicated,performed,and appropriately documented.RESULTS:We found that 1,507 of 1,597 trauma patients had indications for eFAST,but 396(27%)of these patients did not have a billed eFAST.Of these 396 patients,87(22%)had documentation in the provider note that an eFAST was performed but there was no separate procedure note.The remaining 309(78%)did not have any documentation of the eFAST in the patient’s chart although an eFAST was recorded and reviewed during ultrasound quality assurance.CONCLUSION:A significant proportion of trauma patients had eFAST exams performed but were not documented or billed.Lack of documentation was multifactorial.Emergency ultrasound programs require appropriate reimbursement to support training,credentialing,equipment,quality assurance,and device maintenance.Our study demonstrates a significant absence of adequate documentation leading to potential revenue loss for an emergency ultrasound program.
文摘Blunt diaphragmatic lesions (BDL) are uncommon in trauma patients, but they should be promptly recognized as a delayed diagnosis increases morbidity and mortality. It is well known that BDL are often overlooked at initial imaging, mainly because of distracting injuries to other organs. Sonography may directly depict BDL only in a minor number of cases. Chest X-ray has low sensitivity in detecting BDL and lesions can be reliably suspected only in case of intra-thoracic herniation of abdominal viscera. Thanks to its wide availability, time-effectiveness and spatial resolution, multi-detector computed tomography (CT) is the imaging modality of choice for diagnosing BDL; several direct and indirect CT signs are associated with BDL. Given its high tissue contrast resolution, magnetic resonance imaging can accurately depict BDL, but its use in an emergency setting is limited because of longer acquisition times and need for patient’s collaboration.

文摘The hypersonic boundary-layer receptivity to slow acoustic waves is investigated for the Mach 6 flow over a 5-degree half-angle blunt cone with the nose radius of 5.08 mm. The plane acoustic wave interacts with the bow shock, and generates all types of disturbances behind the shock, which may take various routes to generate the boundarylayer unstable mode. In this paper, two routes of receptivity are investigated in detail.One is through the disturbance in the entropy layer. The other is through the slow acoustic wave transmitted downstream the bow shock, which can excite the boundary-layer mode due to the synchronization mechanism. The results show that, for a low frequency slow acoustic wave, the latter route plays a leading role. The entropy-layer instability wave is able to excite the first mode near the neutral point, but its receptivity efficiency is much lower.
文摘Indirect traumatic optic neuropathy(ITON) refers to optic nerve injury resulting from impact remote to the optic nerve. The mechanism of injury is not understood, and there are no confirmed protocols for prevention, mitigation or treatment. Most data concerning this condition comes from case series of civilian patients suffering blunt injury, such as from sports- or motor vehicle-related concussion, rather than military-related ballistic or blast damage. Research in this field will likely require the development of robust databases to identify patients with ITON and follow related outcomes, in addition to both in-vivo animal and virtual human models to study the mechanisms of damage and potential therapies.

文摘Pressure wave plays an important role in the occurrence of behind armor blunt trauma(BABT),and ballistic gelatin is widely used as a surrogate of biological tissue in the research of BABT.Comparison of pressure wave in the gelatin behind armor for different rifle bullets is lacking.The aim of this study was to observe dynamic changes in pressure wave induced by ballistic blunt impact on the armored gelatin block and to compare the effects of bullet type on the parameters of the transient pressure wave.The gelatin blocks protected with National Institute of Justice(NIJ) class III bulletproof armor were shot by three types of rifle bullet with the same level of impact energy.The transient pressure signals at five locations were recorded with pressure sensors and three parameters(maximum pressure,maximum pressure impulse,and the duration of the first positive phase) were determined and discussed.The results indicated that the waveform and the twin peak of transient pressure wave were not related to the bullet type.However,the values of pressure wave's parameters were significantly affected by bullet type.Additionally,the attenuation of pressure amplitude followed the similar law for the three ammunitions.These findings may be helpful to get some insight in the BABT and improve the structure design of bullet.

文摘Hydraulic fracture is important in unconventional oil and gas exploration.During the propagation of the hydraulic fracture,the crack tip is blunted due to the development of the process zone in the near-tip area.In this study,the blunting of the hydraulic fracture in polymethyl methacrylate specimens due to multi-timescale stress concentration is investigated.The ratio of the initiation toughness to the arrest toughness of the blunted hydraulic fracture is measured using both the dynamic and the static methods.Results show that a hydraulic fracture can be blunted with the time span of stress concentration from 1 ms to 600 s.It is also shown that the blunting of hydraulic fracture is a highly localized process.The morphology of the blunted crack depends on the stress distribution in the vicinity of the crack tip.

文摘In the present study, the genetic diversity of one selected strain (Pujiang No. 1), two domesticated populations (GA and HX) and four wild populations (LZ, YN, SS and JL) of blunt snout bream (Megalobrama amblycephala) was analyzed using 17 microsatellite markers. The results showed that an average of 4.88-7.65 number of alleles (A); an average of 3.20-5.33 effective alleles (Ne); average observed beterozygosity (Ho) of 0.6985-0.9044; average expected beterozygosity (He) of 0.6501--0.7805; and the average polymorphism information content (PIC) at 0.5706-0.7226. Pairwise FST value between populations ranged from 0.0307-0.1451, and Nei's standard genetic distance between populations was 0.0938-0.4524. The expected heterozygosities in the domesticated populations (GA and HX) were significantly lower than those found in three wild populations (LZ, SS and JL), but no difference was detected when compared with the wild YN population. Likewise, no difference was found between the four wild populations or two domesticated populations. The expected heterozygosity in Pujiang No. 1 was higher than the two domesticated populations and lower than the four wild populations. Regarding pairwise Fsr value between populations, permutation test P-values were significant between the GA, HX and PJ populations, but not between the four wild populations. These results showed that the expected beterozygosity in the selected strain of blunt snout bream, after seven generations of selective breeding, was lower than that of wild populations, but this strain retains higher levels of genetic diversity than domesticated populations. The genetic differences and differentiation amongst wild populations, domesticated populations and the genetically improved strain of blunt snout bream will provide important conservation criteria and guide the utilization of germplasm resources.

文摘Under ultrasound guidance, a blunt suture needle was inserted around the Glissonian pedicle and then sutured. This technique significantly reduced the blood loss and facilitated the procedure of partial hepatectomy. We applied this technique in 182 patients who needed partial hepatectomy. We concluded that this method is simple and easy to occlude the vascular inflow and outflow, and allows an accurate delineation of the anatomic zone and therefore, simplifies the procedure of partial hepatectomy.
文摘Blunt isolated pancreatic trauma is uncommon,accounting for 1%-4% of high impact abdominal injuries.In addition,its diagnosis can be difficult;physical signs may be poor and laboratory findings nonspecific,resulting in delayed treatment.Preserving the spleen during distal pancreatectomy (DP) is controversial.One of the spleen's functions regards immunity;complications following splenectomy include leukocytosis,thrombocytosis,overwhelming post splenectomy sepsis and some degree of immunodeficiency.This is why many authors favor its preservation.We describe a case of a young man with an isolated pancreatic trauma due to a blunt abdominal trauma with a delayed presentation who was treated with spleen-preserving DP and we discuss the value of this procedure with reference to the literature.

文摘In a Mach 3.8 wind tunnel, both instantaneous and time-averaged flow structures of different scales around a blunt double-cone with or without supersonic film cooling were visualized via nano-tracer planar laser scattering (NPLS), which has a high spatiotemporal resolution. Three experimental cases with different injection mass flux rates were carried out. Many typical flow structures were clearly shown, such as shock waves, expansion fans, shear layers, mixing layers, and turbulent boundary layers. The analysis of two NPLS images with an interval of 5 us revealed the temporal evolution characteristics of flow structures. With matched pressures, the laminar length of the mixing layer was longer than that in the case with a larger mass flux rate, but the full covered region was shorter. Structures like K-H (Kelvin-Helmholtz) vortices were clearly seen in both flows. Without injection, the flow was similar to the supersonic flow over a backward- facing step, and the structures were relatively simpler, and there was a longer laminar region. Large scale structures such as hairpin vortices were visualized. In addition, the results were compared in part with the schlieren images captured by others under similar conditions.
