AIM To evaluate the results of the Ilizarov method in management of cases with bone loss, soft tissue loss and infection.METHODS Twenty eight patients with severe leg trauma complicated by bone loss, soft tissue loss ...AIM To evaluate the results of the Ilizarov method in management of cases with bone loss, soft tissue loss and infection.METHODS Twenty eight patients with severe leg trauma complicated by bone loss, soft tissue loss and infection were managed by distraction osteogenesis in our institution. After radical debridement of all the infected and dead tissues the Ilizarov frame was applied, corticotomy was done and bone transport started. The wounds were left open to drain. Partial limb shortening was done in seven cases to reduce the size of both the skeletal and soft tissue defects. The average follow up period was 39 mo(range 27-56 mo). RESULTS The infection was eradicated in all cases. All the soft tissue defects healed during bone transport and plastic surgery was only required in 2 cases. Skeletal defects were treated in all cases. All patients required another surgery at the docking site to fashion the soft tissue and to cover the bone ends. The external fixation time ranged from 9 to 17 mo with an average of 13 mo. The complications included pin tract infection in 16 cases, wire breakage in 2 cases, unstable scar in 4 cases and chronic edema in 3 cases. According to the association for study and application of methods of Ilizarov score the bone results were excellent in 10, good in 16 and fair in 2 cases while the functional results were excellent in 8, good in 17 and fair in 3 cases. CONCLUSION Distraction osteogenesis is a good method that can treat the three problems of this triad simultaneously.展开更多

The results of 20 fossil bones from 10 Chinese sites, 19 of which are determined by two U-series methods, are given.Judging from independent age controls, 8 out of the 11 concordant age sets are unacceptable.The results in this paper suggest that uranium may cycle into or out of fossil bones, such geochemical events may take place at any time and no known preserving condition may securely protect them from being affected. So for the sites we have studied, the U-series dating of fossil bones is of limited reliability.展开更多

The mea...Gap non-union of the tibia is frequently associated with deformity, infection and shortening. Ilizarov’s method of bone transport was used in the management of twenty-five cases of gap non-union of the tibia. The mean bone gap was 6.53 cm (range 4 to 12 cm). Union was achieved in all cases within a mean period of 11.12 months. The mean time taken for union, per centimeter of bone gap was found to be 1.7 months/cm. Pin tract infection was the commonest complication, seen in 9 cases (36%). The other complications encountered were neuropraxia (n = 3) deviation of the transported bone segment (n = 7), buckling of skin at the advancing side of bone (n = 4), traumatic corticotomy (n = 3), incomplete corticotomy (n = 1), equinus deformity (n = 4), knee stiffness (n = 4) and curling of toes (n = 4). The bone healing results were excellent in 92% of cases and good in 8% of cases. The functional results were excellent in 84% of cases, good in 12% and fair in 4% of cases. Thus bone transport by Ilizarov’s method manages the bone loss and the associated conditions with good bone healing and functional results.展开更多

F...The After cancer resection including bone, recently, bone resected with cancer has been considered to be reusable. We newly examined superheated steam treatment for bone reuse and compared it to existing treatments. Forty male C3H/HeN mice were used to establish a model of mandible invasion by oral squamous cell carcinoma (OSCC). The mice were sacrificed to harvest the tumor invading mandible bone. The resected tumor with mandible bone was treated by one of four kinds of treatment, no treatment for the control, the Pasteur method, liquid nitrogen treatment, micro-wave treatment and superheated steam treatment. After each treatment, the resected bone was transplanted into a syn-geneic mouse back. Eight weeks after transplantation, the mice were sacrificed and evaluated pathologically. Grafted tumors showed recurrence: 7/7 in the control, 6/8 in the liquid nitrogen treatment, 1/8 the microwave treatment and 2/8 the superheated steam treatment groups. No recurrence, on the other hand, was observed in the Pasteur method (0/8). The Pasteur method is a good treatment to remove malignant cells.展开更多
Human bone may be damaged by impact in the cases of traffic accidents and ship impact. The impact responses of cancellous bone were analyzed based on the two-phase media theory. A direct analytical method is introduce...Human bone may be damaged by impact in the cases of traffic accidents and ship impact. The impact responses of cancellous bone were analyzed based on the two-phase media theory. A direct analytical method is introduced for solving this type of problems. First, flow function and potential function were introduced to decouple the controlling equations. Then direction solving method was used to obtain the solution. The solution is determined by the parameters of a (related with wave speed) and b (related with damping), as well as the boundary conditions. These two parameters a and b determine the propagation speed of the responses along the bone and the attenuation rate. It is shown that the responses: deformation, stress and pressure of the corpus medullae caused by loading, propagate toward the other end when the impact is acted on one end of the bone. The responses are discontinuous during propagate. The discontinuous surface moves with a constant speed. The responses at the cross section increase gradually from the bottom to the top because of the distribution of the loading at the boundary. The solutions can be used as the basis for certification of numerical simulation as well as the design of impact prevention of bone.展开更多

Results showed that the average F on left and right side of minipigs’ femur decreased from 15.4±1.8 and 16 .9±2.0 ml/ (100 g.min) (before exposure) to 10±1.8 and 11.1±1.6 ml/(100mg.min) (after expposure for 1.5 h under the pressure of 0.5 MPa and then decompression to normal with the rate of 0.03~0.04MPa/min), respectively; the blood flow of hematopoietic marrow tissues (f1)on both sides also decreased from 19.3±2.0 and 22.1±1.9 ml/(100mg.min) to 13.9±1.4 and 13.8±1.0 ml/(100 mg.min). The exact same alteration also happened in the experiment on New Zealand rabbits. It indicates that inadequate decompression in hyperbaric exposure can give rise to decrease animals’ bone blood flow and insufficient bone blood supply serves as one of the reasons for causing decompressive osteonecrosis.展开更多

METHODS: The marker gene, pbLacZ, was transferred into cultured BMSCs and the expression of transduced gene by X-gal staining was examined. Then plasmid pcDNA3-rhBMP7 was delivered to cultured BMSCs. Through immunohistochemical staining and RT-PCR assay, the expression of rhBMP7 gene was detected. RESULTS: The exogenous gene could be expressed efficiently in transduced BMSCs. CONCLUSION: The present study provided a theoretical basis to gene therapy on the problems of bone and cartilage tissue.展开更多
