THE CARBUNCLE PHENOMENON IS INCURABLE

Numerical approximations of multi-dimensional shock waves sometimes ex- hibit an instability called the carbuncle phenomenon. Techniques for suppressing carbuncles are trial-and-error and lack in reliability and generality, partly because theoretical knowledge about carbuncles is equally unsatisfactory. It is not known which numerical schemes are affected in which circumstances, what causes carbuncles to appear and whether carbuncles are purely mimerical artifacts or rather features of a continuum equation or model. This article presents evidence towards the latter： we propose that carbuncles are a special class of entropy solutions which can be physically correct in some circumstances. Using ＂filaments＂, we trigger a single carbuncle in a new and more reliable way, and compute the structure in detail in similarity coordinates. We argue that carbuncles can, in some circumstances, be valid vanishing viscosity limits. Trying to suppress them is making a physical assumption that may be false.

OBSERVATION OF 14 CASES OF LARYNGEAL CARBUNCLE TREATED BY WEI JIA'S BLOOD-LETTING THERAPY

The author used the pricking-blood therapy to treat 14 cases of laryngeal car-buncle.The pricking-blood therapy included clumpy pricking(local area),spot pricking(prickingSanshang points on the thumb and three points of the helix).The therapy has role of clearing awaypathogentic heat and toxic materials and promoting subsidence of swelling.It could avoid forming py-opoiesis and free from operating treatment.The treatment was given once daily,result:13 cases cured(93%),1 case failed(7%).In 14 cases,antero-upper type has 13 cases which cured,postero-uppertype has only 1 case which treated failure.It is because its imflammation was located on posterior totonsillae,exposed bad and pricked difficulty.So the therapy only used antero-upper type of laryngealcarbuncle and had a good effect.

A carbuncle cure for the Harten-Lax-van Leer contact(HLLC)scheme using a novel velocity-based sensor

A hybrid numerical flux scheme is proposed by adapting the carbunclefree modified Harten-Lax-van Leer contact(HLLCM) scheme to smoothly revert to the Harten-Lax-van Leer contact(HLLC) scheme in regions of shear. This hybrid scheme, referred to as the HLLCT scheme, employs a novel, velocity-based shear sensor. In contrast to the non-local pressure-based shock sensors often used in carbuncle cures, the proposed shear sensor can be computed in a localized manner meaning that the HLLCT scheme can be easily introduced into existing codes without having to implement additional data structures. Through numerical experiments, it is shown that the HLLCT scheme is able to resolve shear layers accurately without succumbing to the shock instability.

Large renal carbuncle mimicking intra-abdominal neoplasm on Ga-67 scintigraphy: A case of fever of unknown origin

Fever of unknown origin is still a medical challenge. 67Ga single photon emission computed tomography/computed tomography images are commonly used to evaluate the final diagnosis of fever of unknown origin. We presented a case of fever of unknown origin undergone 67Ga scintigraphy and an intra-abdominal 67Ga avid tumor was detected which suspected to be a neoplasm. Further contrast enhanced computed tomography revealed that the lesion was a large renal carbuncle. We concluded that the contrast-enhanced CT or single photon emission computed tomography/computed tomography with contrast enhancement can be performed to further improve diagnostic performance.

A study on the medication law of oral administration of traditional Chinese medicine in treating breast carbuncle based on the Traditional Chinese Medicine Prescription Dictionary

Objective:Based on the database of Traditional Chinese Medicine Prescription Dictionary,the prescription medication rules of oral administration of traditional Chinese medicine in the treatment of breast carbuncle were explored by using the software of Traditional Chinese Medicine Inheritance Support System(V2.5).Methods:To screen and retrieve the prescriptions for treating breast carbuncle in the Traditional Chinese Medicine Prescription Dictionary,and data mining was carried out by using association rule algorithm,complex system entropy clustering and so on.Results:A total of 208 prescriptions which met the inclusion criteria were selected,involving 175 traditional Chinese medicines.Among them,23 medicines were found to be more than 21 times in the treatment of breast carbuncle.The nature of the medicine is mainly cold or warm,the flavour of the medicine is mainly bitter or sweet,most of them belong to the spleen,liver,stomach.15 commonly used drug combinations and 9 new prescriptions were obtained.Conclusion:The key pathogenesis of breast carbuncle is liver depression and Qi stagnation,stomach heat blocking collaterals,the treatment is mainly to clear heat and detoxify,regulate Qi to diffuse the knot,which provides reference for the choose of traditional Chinese medicine prescription in treating breast carbuncle.

Network pharmacology-based approach to investigate the mechanisms of Yiyi Fuzi Baijiang Powder in the treatment of malignant tumors

Objective:To systematically investigate the main active components,targets,target-related diseases and action mechanism of Yiyi Fuzi Baijiang powder(YFBP)of Zhang Zhongjing,a Chinese physician of Eastern Han Dynasty of China(3 century A.D.)using the method of network pharmacology.Methods:The active components of YFBP were identified from several databases(TCMSP,TCM Database@Taiwan and TCMID).The potential active compounds were screened based on oral bioavailability and drug-like index in the TCMSP database.DrugBank,TCMSP,and TCMID database searches were performed to predict target-related diseases.Biological functions and KEGG pathway outcomes of all potential targets were analyzed using the DAVID database.Results:A total of 24 active compounds of YFBP,133 related targets,and 305 potential related diseases were obtained.These results showed that YFBP may treat inflammation,pain,and cancer.The functional enrichment analysis indicated that YFBP probably exerts its therapeutic effects by regulating neuroactive ligand-receptor interactions,the TNF signaling pathway,and the PI3K-Akt signaling pathway.Conclusion:This study provides preliminary confirmation that YFBP has the great potentials to treat inflammation,pain,and tumors,which supports Zhang Zhongjing’s original intention to treat intestinal carbuncle with ancient prescription YFBP,but also provides a scientific basis for its treatment in the malignant tumors.

一种鲁棒的HLLC格式及其稳定性分析

精确捕捉接触波和剪切波的Godunov型数值方法,如流行的HLLC格式,在模拟高超声速流动问题时会出现激波异常现象。对HLLC格式进行稳定性分析发现,流体主流方向的扰动都能有效衰减,但是横向的密度与剪切速度的扰动不会衰减。具有特殊对称性的二维Sedov爆轰波问题证明了横向通量和不稳定现象之间的密切联系。利用压力比和马赫数来探测数值激波层亚声速区的横向网格界面,并且在该界面的数值通量上增加熵波粘性和剪切波粘性来构造一种激波稳定的HLLC格式。分析表明,在熵波粘性和剪切波粘性的作用下,横向的所有扰动都会衰减。一系列数值测试证明了新格式不仅可以成功地抑制各类激波异常现象,还保留了原HLLC格式低耗散性的优点。

Nursing care of a patient with acute mastitis after dystocia

Objective:To discuss the prevention of acute mastitis during lactation and the nursing after its formation.Methods:The diet is light and easy to digest,keep the mood at ease,massage and drain the milk.After the occurrence of acute mastitis during lactation,prompt symptomatic treatment and care.Results:As a result after careful treatment and care acute mastitis during lactation healed.Conclusion:Effective prevention and care are the key to the treatment of acute mastitis during lactation.

A Systematic Review and Meta-Analysis on Gualou Niubang Decoction for Acute Mastitis

Objective:To systematically evaluate the clinical efficacy of Gualou Niubang decoction in the treatment of acute mastitis.Methods:Randomized controlled trials of Gualou Niubang decoction in the treatment of acute mastitis included in PubMed,Springer,Embase,Cochrane Library,CNKI,Wanfang,Weipu,and China Biomedical Literature Database from January 2016 to present.The Cochrane risk bias assessment tool was used to evaluate the risk of bias.Used Revman 5.3 software for Meta analysis.Results:There are 8 articles that meet the inclusion criteria,comprising 919 subjects,the test group were 462,cases and control group were 457 cases.The results of statistical analysis showed that the clinical efficacy of the experimental group was significantly higher than which of the control group,[RR=1.19,95%CI(1.07,1.33),P=0.002],the difference was statistically significant.Among the secondary outcome indicators,compared to the control group,Gualou Niubang decoction can effectively relieve the main symptoms of acute mastitis[MD=-1.96,95%CI(-2.27,-1.65),P<0.0001].Gualou Niubang decoction combined with antibiotics can shorten the length of hospital stay[MD=-5.23,95%CI(-6.02,-4.44),P<0.00001]and reduce white blood cell count[MD=-2.40,95%CI(-4.71,-0.08),P=0.04]are better than antibiotics.Conclusion:The use of Gualou Niubang decoction to treat acute mastitis can not only improve clinical efficacy,but also effectively relieve clinical symptoms,shorten hospital stay and reduce white blood cell count.

A Hybrid Numerical Method to Cure Numerical Shock Instability

In this note,we propose a new method to cure numerical shock instability by hybriding different numerical fluxes in the two-dimensional Euler equations.The idea of this method is to combine a”full-wave”Riemann solver and a”less-wave”Riemann solver,which uses a special modified weight based on the difference in velocity vectors.It is also found that such blending does not need to be implemented in all equations of the Euler system.We point out that the proposed method is easily extended to other”full-wave”fluxes that suffer from shock instability.Some benchmark problems are presented to validate the proposed method.
