These morphological changes, consisting in dilatation and stenosis of the biliary tree, are due to extensive venous collaterals occurring in an attempt to decompress the portal venous blockage. It is usually asymptomatic until it progresses to more advanced stages with cholestasis, jaundice, biliary sludge, gallstones, cholangitis and finally biliary cirrhosis. Imaging mo-dalities of the biliary tree such as Doppler ultrasound, computed tomography, magnetic resonance cholangiopancreatography and endoscopic retrograde cholangiopancreatography are essential to establish the diagnosis and the need of therapeutical interventions. Once the diagnosis is established, treatment with ursodesoxycholic acid seems to be beneficial. Decompression of the biliary tree to dilate, remove stones or implant biliary prosthesis by endoscopic or surgical procedures (hepato-yeyunostomy) usually resolves the cholestatic picture and prevents septic complications. The ideal treatment is the decompression of the portal system, with transjugular intrahepatic porto-systemic shunt or a surgical porto-systemic shunt. Unfortunately, few patients will be candidates for these procedures due to the extension of the thrombotic process. The purpose of this paper is to report the first 3 cases of PHB seen in a Colombian center and to review the literature.展开更多

They were managed by a team of seven endoscopists on 24-h call whose experience was categorized into two levels (high and low) according to the number of endoscopic hemostatic procedures undertaken before the study. Endoscopic treatment was standardized according to Forrest classification of lesions as well as the subsequent medical therapy. Time of endoscopy was subdivided into two time periods: routine (8 a.m.-5 p.m.) and on-call (5 p.m.-8 a.m.). For each category of experience and time periods rebleeding rate, transfusion requirement, need for surgery, length of hospital stay and mortality we compared. Multivariate analysis was used to discriminate the impact of different variables on the outcomes that were considered.RESULTS: Study population consisted of 272 patients (mean age 67.3 years) with endoscopic stigmata of hemorrhage. The patients were equally distributed among the endoscopists, whereas only 19% of procedures were done out of working hours. Rockall score and Forrest classification at admission did not differ between time periods and degree of experience.Univariate analysis showed that higher endoscopist's experience was associated with significant reduction in rebleeding rate (14% vs 37%), transfusion requirements (1.8±0.6 vs 3.0±1.7 units) as well as surgery (4% vs 10%), but not associated with the length of hospital stay nor mortality. By contrast, outcomes did not significantly differ between the two time periods of endoscopy. On multivariate analysis, endoscopist's experience was independently associated with rebleeding rate and transfusion requirements. Odds ratios for low experienced endoscopist were 4.47 for rebleeding and 6.90 for need of transfusion after the endoscopy.CONCLUSION: Endoscopist's experience is an important independent prognostic factor for non-variceal acute upper GI bleeding. Urgent endoscopy should be undertaken preferentially by a skilled endoscopist as less expert staff tends to underestimate some risk lesions with a negative influence on hemostasis.展开更多

In order to remove this contradiction, this phenomenon was studied in depth, theoretically and experimentally, using an experimental setup similar to Faraday’s. Calculations of the induced electromotive force, based on relativity on the one hand and on Faraday’s view on the other were made with the help of measurements of the magnetic field components. Accurate magnetic field measurements are confirmed by analytical calculations. Precise-induced electromotive force measurements confirmed Faraday’s view and contradicted the theory of relativity.展开更多

文摘Portal hypertensive biliopathy (PHB) is characterized by anatomical and functional abnormalities of the intrahepatic, extrahepatic and pancreatic ducts, in patients with portal hypertension associated to extrahepatic portal vein obstruction and less frequently to cirrhosis. The purpose of this paper is to report the first 3 cases of PHB seen in a Colombian center and to review the literature.
文摘AIM: To prospectively assess the impact of time of endoscopy and endoscopist's experience on the outcome of non-variceal acute upper gastrointestinal (GI) bleeding patients in a large teaching hospital.METHODS: All patients admitted for non-variceal acute upper GI bleeding for over a 2-year period were potentially eligible for this study. Urgent endoscopy should be undertaken preferentially by a skilled endoscopist as less expert staff tends to underestimate some risk lesions with a negative influence on hemostasis.
文摘BACKGROUND Meckel’s diverticulum is a common congenital malformation of the small intestine, with the three most common complications being obstruction,per-foration,and inflammation. To date, only a few cases have been reported world-wide.In children, the clinical symptoms are similar to appendicitis. As most of the imaging features are nonspecific, the preoperative diagnosis is not precise. In addition, the clinical characteristics are highly similar to pediatric acute appendicitis, thus special attention is necessary to distinguish Meckel’s diver-ticulum from pediatric appendicitis.Patients with poor disease control should undergo laparoscopic exploration to avoid serious complications, including intestinal necrosis, intestinal perforation and gastrointestinal bleeding.CASE SUMMARY This report presents three cases of appendicitis in children combined with intestinal obstruction, which was caused by fibrous bands(ligaments)arising from the top part of Meckel's diverticulum, diverticular perforation, and diver-ticular inflammation. All three patients, aged 11-12 years, had acute appendicitis as their initial clinical presentation. All were treated by laparoscopic surgery with a favorable outcome.A complete dataset including clinical presentation,dia-gnostic imaging, surgical information, and histopathologic findings was also provided.CONCLUSION Preoperative diagnosis of Meckel’s diverticulum and its complications is challenging because its clinical signs and complications are similar to those of appendicitis in children.Laparoscopy combined with laparotomy is useful for diagnosis and treatment.

文摘An authoritative parenting style has been shown to promote children’s emotion regulation in European-American family studies. However, little is known about how sleep problems and the child’s sibling status in Chinese families affect this relationship.Based on family system theory, this study attempts to better understand the relationship between authoritative parenting style and emotion regulation.Mothers of preschool children in Chinese kindergartens completed questionnaires about their children’s sleep habits, their authoritative parenting styles, and children’s emotion regulation.A total of 531 children participated in this study. Results showed that authoritative parenting was positively associated with emotional regulation.Sleep problems mediated the effects of authoritative parenting style on emotion regulation. The child’s sibling status moderated the mediating effects of sleep problems in authoritative parenting and emotion regulation relationships.Specifically, the relationship between the authoritative parenting style and sleep problems was significant for only children, while birth order had no significant influence on the authoritative parenting style and sleep problems in two-child families. These findings suggest that a lowauthoritative parenting style predicts low emotion regulation through sleep problems, and this depends on the child’s sibling status, indicating that children without siblings may impair emotion regulation due to increased sleep problems.
文摘In recent years, embodied cognition has ushered in a new research upsurge in the academic field, and has become a hot topic in the field of cognitive psychology. In this paper, from the perspective of embodied cognition, the interaction ways of a landscape device for children were discussed to achieve a more real and harmonious interaction between children and scenes. The research data of embodied cognition used by children was analyzed, and the drawbacks and breakthrough points of current landscape devices for children were discussed. The core characteristics of children’s growth period were extracted to establish children’s interaction model and summarize the interactive design methods of landscape devices for children.Embodied cognition has become the most intuitive way for children to know and understand the environment, and plays a pivotal role in children’s growth.Based on embodied cognition principle and interactive behavior mode, the interactive design of a landscape device for children was studied, and three interactive design modes, including simple and convenient interaction mode,multi-sensory interaction mode and game natural interaction mode were summarized. On the basis of this research, relevant design practice and research were carried out to bring a new vision to the design of children’s landscape.

文摘When studying the phenomenon of the induced electromotive force, which originates from Faraday’s unipolar inductor, the contrast between Faraday’s view of the magnetic field dynamic lines and the theory of relativity is revealed. Precise-induced electromotive force measurements confirmed Faraday’s view and contradicted the theory of relativity.
