Chinese Medicine Syndromes in Congestive Heart Failure:A Literature Study and Retrospective Analysis of Clinical Cases

Objective： To discuss the characteristics of Chinese medicine （CM） syndrome factors and distribution of congestive heart failure （CHF）, and provide a basis for the diagnosis criteria of essential syndromes. Methods： Based on databases of China National Knowledge infrastructure （CNKI, 1980--2012） and Chinese Journal of Chongqing VlP Database （1989-2012）, the eligible studies in CHF and extracted factors associated with compound syndromes were analyzed. All the syndromes were classified into deficiency, excess, and deficiency-excess in complexity syndrome were classified. Compound syndromes were separated into syndrome factors including single, double, three or four factors, along with the frequency of occurrence. The relation of CHF syndromes with age, gender, primary disease, brain natriuretic peptide （BNP） and cardiac functional grade was studied in 1,451 CHF cases （between December 2010 and September 2012）, and the clinical distribution of common CHF syndromes was summarized. Results： The literature study involved 6,799 CHF cases in 66 literatures after screening. Of the different factors affecting CHF, qi deficiency was the most important one. In deficiency syndrome, Xin （Heart）-qi-deficiency was the most common single factor, and deficiency of both qi and yin was the most common double factor. The retrospective analysis involved 1,451 CHF cases （431 cases with test results of BNP）. The xin blood stasis and obstruction and deficiency of both qi and yin syndrome were mostly seen in female patients, and phlegm-blocking-Xin-vessel and qi-deficiency-blood-stasis syndrome mostly in males. Xin-qi-deficiency and qi-deficiency-blood-stasis syndrome were mostly seen in patients aged 50--60 years. Patients aged over 60 years likely manifest deficiency of both qi and yin and Xin blood stasis and obstruction syndrome. The severity of syndrome is aggravated with increased BNP and cardiac functional grade. Conclusions： The essential syndromes of CHF include qi-deficiency-blood-stasis and deficiency of both qi and yin. The clinical distribution is linked to patients＇ age and gender. BNP and cardiac functional grade is closely related to CHF syndromes, which may indicate the severity of CM syndromes of CHF.

Erxian decoction potentially prevents postmenopausal osteoporosis by modulating miR-335: a study based on bioinformatics analysis and preliminary clinical case validation

Background:miRNAs are closely related to bone metabolism.Studies have shown that Erxian decoction can improve bone metabolism,possibly achieving this regulatory effect through miRNA targets.Netinfer was used to predict the miRNA targets of Erxian decoction for the treatment of postmenopausal osteoporosis,and the results were validated by clinical trials.Methods:In this study,we identified possible targets of Erxian decoction in osteoporosis by means of network pharmacological analysis and bioinformatic prediction.Fifteen cases of postmenopausal osteoporosis with kidney Yin and Yang deficiency(In traditional Chinese medicine,kidney Yin nourishes and moistens the tissues of the internal organs of the body,while kidney Yang promotes and warms the tissues of the internal organs of the body.)were treated with Erxian decoction for four weeks,and serum bone metabolism indices(P1NP,osteocalcin,andβ-CTX)and miRNA-335-5p expression were measured before and after treatment.Results:The constructed miRNA postmenopausal osteoporosis related gene network of the effective compound of the Erxian decoction has 296 points and 981 edges.The 39 postmenopausal osteoporosis related genes regulated by miRNA-335-5p were enriched in ossification,while the signaling pathways were enriched in rheumatoid arthritis,the Toll signaling pathway,the HIF-1 signaling pathway,and the MAPK signaling pathway.After taking Erxian decoction,the expression of the serum bone formation index(P1NP,osteocalcin)and miRNA-335-5p gene expression levels increased significantly.The alterations in P1NP and osteocalcin were correlated with the changes in miRNA-335-5p.Conclusion:Circulating miRNA-335-5p may serve as an important target of Erxian decoction in the treatment of postmenopausal women.The effect of Erxian decoction on bone formation is significant,but the underlying mechanism requires further investigation.

FEMALE LARYNGOCARCINOMA (CLINICAL ANALYSIS OF 267 CASES)

Two hundred and sixty-seven cases of female laryngocarcinoma, treated in our hospital in the past 33 years, were analysed. It is suggested that a relating preponderance of male sex hormonal influence after menopause should be a contributing factor responsible for the high prevalemce of laryngeal cancer in women. The other etiological factor was cigarette smoking. The pathological types and patterns of metastasis to cervical lymphnodes in the female laryngocarcinoma were similar to those observed in male patients but the superglottic cancer was more common than the flottic type. However, no significant difference in prognosis was observed between superglottic and glottic cancers. Provided indications are strictly adhered to, partial laryngectomy that can give therapeutic resulrs similar or even superior to total laryngocarcinoma.

Clinical analysis of 251 cases of soft tissue sarcomas

Objective To explore the therapeutic principles and prognostic factors of soft tissue sarcoma. Methods Two hundred and fifty-one patients with soft tissue sarcoma (STS) treated at Shanghai Cancer Hospital during 1986 -1990 were reviewed retrospectively. Results The 1-, 3 - , 5 - , 10 - year tumor-free survival rates were 67. 74% , 57. 16% , 52, 41% , 38. 60% , respectively.The overall survival rates for 1,3,5 and 10 years were 81. 01 % ,67.75% ,60.79% ,and 49.23% respectively. Log-rank test showed that the patients with different pathological findings, histological grades,mass location and size, anatomical depth, and surgical margin showed different outcomes. Whether the sarcomas invaded the vessels or metastasized would influence the survival rates. The patients who underwent different interventions or operations also had different outcomes. The prognosis of STS was associated with age,histological type,histological grade, tumor size, surgical margin and metastasis according to the Cox regression

Clinical experience of 2 086 cases of kidney preservation

Objective To summarize the experience of 20 years in using hypertonic citrate adenine(HC-A) for clinical kidney preservation. Methods A total of 2 086 cases of kidneys for transplantation from Angust 1980 to December 2000 were reported. The donors’ age ranged from 18 to 49 years. The cadaveric kidneys were harvested with en-bloc method, and were perfused via renal arteries with 250 ml HC- A at 2 - 4 ℃ and 10 cm H2O, and then stored in HC- A at 2 - 4 ℃ throughout preservation. Serum creatinine was measured at 1 week and 1 month after transplantation. Graft survival rates at 1 year and 3,5,10 years and mean graft survival periods were followed up. According to the cold ischemic time, the cases were divided into 3 groups; Group 1, n = 573, with cold ischemic time less than 12 h; Group 2, n = 779, with 12 to 24 h; and Group 3, n = 734, with more than 24 h. Misdiagnosis Features of Ancient Clinical Records Based on Apriori Algorithm

Objective:To analyze misdiagnosis features in clinical cases of“Classified Medical Cases of Famous Physicians”and“Supplement to Classified Case Records of Celebrated Physicians.”Materials and Methods:Two hundred and five ancient misdiagnosed cases were analyzed in aspects of locations(exterior-interior type,qi-blood type and Zang‑Fu organs type)and patterns(heat-cold type and deficiency-excess type)by Apriori Algorithm Method.Results:The main types of misdiagnosis in those medical casesare as follows::Zang‑Fu location misjudgment,misjudging the interior as the exterior,misjudging deficiency pattern as excess pattern,and misjudging cold pattern as heat pattern.Among them,the most outstanding type is the misjudgment of deficiency–cold pattern as excess–heat pattern.Conclusions:(1)Accurate judgment of location and differentiation of deficiency and excess patterns are the key points in diagnosing the diseases correctly.The confusion of true deficiency–cold and pseudo‑excess–heat pattern should be taken seriously.(2)Data mining on ancient clinical cases offers a new methodology for assisting clinical diagnosis of traditional Chinese medicine.

Radiation and Differential Diagnosis of Computed Tomography (CT) Data for Coronavirus Infection “COVID-19” with Clinical Examples

This article is presenting data from a retrospective analysis of medical records and computed tomography (CT) scans of patients’ chests with coronavirus infection “COVID-19” who applied to the diagnostic center of URFA in Osh during the first wave of the pandemic in the Kyrgyz Republic, with a description of individual clinical cases and their differential diagnosis. Chest computed tomography is one of the main methods in visual diagnosis of pneumonia in COVID-19 in hospitalized patients, which allows determining signs, symptoms for effective treatment.

Influence of pontic design of anterior fixed dental prosthesis on speech: A clinical case study

BACKGROUND The pontic design of fixed dental prostheses(FDPs)is strongly associated with the phonetic function,and the phonetic function of anterior FDPs with different pontic designs remains understudied.AIM To investigate the immediate and short-term influence of pontic design of anterior FDPs on Chinese speech in a clinical case using objective acoustic analysis.METHODS Two FDPs with two types of pontic design(saddle pontic and modified ridge lap pontic)were fabricated for one patient with maxillary anterior teeth missing.The acoustic analysis of patient’s articulation was conducted immediately after wearing the FDPs and 1 wk after wearing these FDPs.RESULTS The effect of FDP on Chinese vowels(/a/,/o/,/e/,/i/,/u/,and/ü/)was insignificant,because the recovery of vowel distortion occurred within 1 wk for both FDPs.Three(/f/,/s/,and/sh/)of eight Chinese fricative consonants were found to have obvious distortions,and the/s/sound distortion last for more than 1 wk for the patient wearing FDP with modified ridge lap pontic design.CONCLUSION The influence of anterior FDP on articulation of Chinese vowels is insignificant,while the articulation of Chinese fricative consonants is more susceptible.When fabricating anterior FDPs for patients with speech related professions,saddle pontic design can be an alternative option compared with modified ridge lap pontic design.
