Objective: The purpose of this study was to clarify the collaborative activities and mutual recognition between community comprehensive care unit nurses (Ns) and care managers (CM) in supporting the discharge of the elderly from the hospital. Methods: A total of 300 nurses working in community comprehensive care wards and 360 care managers working in B City in A Prefecture were surveyed using an anonymous self-administered questionnaire. Results: The highest percentage of responses regarding necessary collaborative activities with multiple professions in supporting hospital discharge were the same for Ns and CMs. The items regarding practice with the highest percentages were “relationship as a team” for Ns, and “user-centered awareness” for CM. While these professionals were willing to share information about their patients’ lives after discharge, the percentage of those explaining their expertise was low. It is thought that collaborative activities focusing on these aspects would lead to more appropriate discharge support. 展开更多

Objective: To present a model of extended home nursing support provided through WeChat for pediatric colostomy patients between the first and second surgery to support parents and to assist them in nursing possible complications at home, to reduce colostomy complications caused by improper care, to improve the quality of life of patients at home and to ensure the success of the recovery and second surgery. Methods: A combination of relevant clinical information, health care guidelines, and training as well as the establishment of a specific monitoring protocol was provided to the parents during the patient’ s first hospitalization. A support system for nursing the colostomy was established with regular follow-up via WeChat after the patient’ s first discharge from the hospital using a predetermined protocol. During the 3 to 6 months of home care in-between the colostomy operations, the charge nurse initiated the phone follow-up and provided specific, individual guidance and feedback. If necessary, extended serv-ice at predetermined intervals was provided in the 1st week, the 1st month, the 2nd month and the 3rd month. Results: Extended assistance for the home care of patients with pediatric colostomy complications im-proved the quality of life of the patients and caregivers and was accepted by all parties involved upon in-troduction. The assistance provided support for the parents at all hours, efficiently reduced the level of complications for this type of patient and facilitated fast referrals to hospital care in cases of emergen-cies. Furthermore it paved the way to successful second stage surgeries, provided direct feedback to the charge nurse and improved the work satisfaction and sense of achievement of the nurses involved. Conclusions: All 80 cases supported by WeChat successfully underwent two-stage pediatric colostomy operations. The response of the parents and the medical staff involved was very positive. This method is easy to use, economical to operate and could be applied generally to support home care. 展开更多

Background: Considering the development of international transportation in modern societies, nursing care needs to be sensitive to the cultures and customs of patients from other countries. The purpose of this study is to determine, from the perspective of culturally sensitive nursing support, the cultural practices that foreigners consider important when they visit a Japanese hospital for check-ups or hospitalization. Methods: A survey was completed by 110 foreign participants at a shrine in Kyoto City. The purpose of the survey was to evaluate the concerns of foreign visitors regarding Japanese medical providers, caused by cultural differences such as customs of daily life. The statistical analyses were performed using descriptive statistics as well as cross-tabulation, chi-square, and Fisher’s exact tests. Results: After grouping participants by continent, the top concerns observed in all groups about visiting a Japanese hospital were accessibility to and cost of medical care. First, as for medical facilities, participants from North and South America (North and South American) and Europe (Europeans) were significantly more concerned than those in Asia (Asians) (P P P P P Conclusion: Nurses need opportunities to learn and understand differences in cultures. If nurses have opportunities to learn cultural differences as well as to experience caring for foreign patients, their competencies in culturally sensitive nursing support will improve over time. It can be said that this is an issue not only in Japan but also in all other countries. 展开更多

Living with a congenital heart condition can require a daunting and uncertain patient journey during which the support of a specialist nursing service is vital. A descriptive, cross-sectional evaluation completed over two years, utilised a postal questionnaire to investigate patient’s satisfaction with aspects of service. Qualitative responses to open questions on satisfaction with emotional support (n = 103) and contributions to well-being (n = 90) were analysed using a framework approach which identified eight themes. Professional knowledge and expertise, service accessibility, caring attributes, enabling patients to cope with anxiety, depression and meeting changing needs contributed positively to both patient well-being and emotional support. Family support was influential on emotional support alone, whilst mediating medical liaison exerted an impact on well-being by enhancing feelings of security. In conclusion, the specialist nursing service contributed positively to well-being and emotional support of patients and thereby to selective aspects of continuity of care. 展开更多

The purpose of this study was to explore the process of family support provided by nurses to families with a borderline personality disorder (BPD) patient. Semi-structured interviews were conducted with 16 nurses who had provided care to BPD patients. Data obtained from the interviews were qualitatively analyzed using a modified grounded theory approach. As an overall core category of family support processes practiced by nurses for families with BPD patients, family support practiced without awareness that the nurses were supporting families was extracted. Through this process, nurses held perceptions that were premises for family support, which were formed through their individual nursing experiences and perspectives. Nurses also had diverse perceptions concerning the image of families. Through the integration of perceptions that were premises for family support and perceptions of an image of the family, nurses underwent a process of “determination and ambivalence about the need for family support.” Then, nurses provided “family support practice” when they acknowledged the need for family support. During the “family support practice,” nurses had difficulties in providing family support. When family support was not successfully provided, nurses provided “family support practice with seeking more effective ways through trial and error.” For cases in which nurses did not acknowledge the need for intervention, they intentionally chose “not to provide family support.” Furthermore, during the “family support practice,” nurses had contradictory perspectives of family support. Such family support processes ultimately led to an awareness of the same family support required for the future. Family support was provided with “family support practice” and “family support practice with seeking more effective ways through trial and error.” In some cases, however, the process ended in “not to provide family support intentionally.” Experiences and perspectives in providing family support are important factors in carrying out future family support. Developing the positive implications of these factors and reducing psychological strain on nurses may ensure smooth implementation of family support. Thus, nurses need to recognize that they are supporting the family, which is identified as a core category. 展开更多

This study investigated self-efficacy in breastfeeding support among public health nurses (PHNs) working at maternal and child health (MCH) services in Japanese municipalities, and its related factors such as education, working experience, breastfeeding experience, and postgraduate/continuous education. Directors of the MCH divisions were requested to select one PHN to answer the self-report questionnaires sent. The questionnaires were collected from October 2010 to January 2011. Of the 1750 questionnaires sent, 831 were returned (response rate: 47.5%);102 were excluded from analysis because of missing data (valid response rate: 41.7%). After adjusting the weights of PHN sampling and collection rates by prefecture and population size, Model 1 of the multiple regression analysis showed that the self-efficacy in breastfeeding support was significantly higher in large/urban municipalities (β = 0.13) and in the Kanto district. Model 2 revealed that self-efficacy was significantly associated with working experience (β = 0.13) and breastfeeding experience (β = 0.22) but not with educational background. Model 3 showed that self-efficacy was significantly associated with postgraduate education (β = 0.14) and continuous education (β = 0.12). This suggested that PHNs’ self-efficacy increased through self-enrichment. It is necessary to create training opportunities about breastfeeding support for PHNs—especially those working in remote and/or small municipalities— and to provide them with new and important information. 展开更多

Objective: The associations between social support and burnout were explored in ICU nurses of Shanghai. Methods: We performed a cross-sectional study of 356 ICU nurses by applying random cluster sampling. Data were collected using self-reported questionnaires under the instruction of trained investigators. Data on emotional exhaustion, depersonalisation and feelings of low personal accomplishment etc. were collected, calculated and analyzed. Results: The participants had a mean age of 26.96 years (SD 4.07). The mean value (M) and stand- ard deviation (SD) of emotional exhaustion was M=31.85, SD=8.38, those of depersonalisation was M= 11.69, SD= 5.54 and those of feelings of low personal accomplishment was M= 19. 79, SD = 7.02. The high degree of emotional exhaustion (EE), depersonalisation (DP), and lack of personal accomplishment (PA) were revealed to be 76.4%, 39.6%, and 94.9%, respectively. The major influencing factors of emotional exhaustion included support from co-workers(b=0. 343, t = 1.98, P=0. 049), taking leave(b=-1. 182, t=-3. 747, P=0. 001), requisition of work(b=-l. 41, t=-2. 369, P=0. 018), and supervisor support(b=-0. 524, t=-3. 926, P=0. 001). The major influencing factors of depersonalisation were support from the supervisor(b=-0. 333, t=-4. 146, P=0.001), age(b=-0. 89, t=-2. 272, P= 0. 024) and requisition of work(b=-0. 148, t=-2. 124, P=0. 034). There was a positive co-relationship between personal accomplishment and supervisor support. Conclusions: Supervisor support, age, and requisition of work were the major influencing factors of depersonalisation. In addition, supervisor support plays an important role in low personal accomplishment. Further research should focus on supervisor support, co-worker support, time on leave, and requisition of work associated with emotional exhaustion. 展开更多

During the height of the Coronavirus Disease (COVID-19) Pandemic, nurses all over the world, including members of the National Association of Nigerian Nurses in North America (NANNNA) experienced a heightened and unprecedented level of stress, anxiety, fear, uncertainty, concern for their safety and that of their loved ones. This prompted NANNNA leadership to initiate and conduct support programs for nurses especially those on the frontline of care. This project evaluates the effectiveness of the support programs conducted by NANNNA during the early surge of COVID-19. The Centers for Disease Control’s (CDC’s) Framework for Evaluation in Public Health guided this evaluation. Focus group discussion and online survey was utilized to gather data. While 19% of respondents reported being diagnosed with COVID-19, the majority (81%) were not. About 44% of respondents had family members who were diagnosed with COVID-19, while 18% reported loss of a close family member due to COVID-19 complications. Most respondents reported that the program was impactful in information sharing as well as significant in helping them feel connected with other nurses. Hence, this subsequently enhanced their support networks, provided emotional and spiritual support. Survey results revealed that NANNNA support programs had positive effects in helping members mitigate the fear, anxiety and uncertainty experienced during the surge of the COVID-19. 展开更多

This study aimed to explore breastfeeding support by public health nurses (PHNs) working in Japanese municipalities in order to promote a breastfeeding-friendly environment. We interviewed 20 participants including five PHNs, four midwives, four doctors (one obstetrician and three pediatricians), and seven mothers who experienced breastfeeding between June and August 2010 and analyzed the data using content analysis. Collected data were classified into four categories including “collecting information”, “assessment”, “direct and individual support”, and “support for group and community” with 15 subcategories: infant’s condition, mother’s condition, mother-infant relationship, child rearing environment, mother’s state-of-mind, identifying necessary support, psychological support, concrete support, breastfeeding on demand, necessary information, advice to supporters, introducing services, breastfeeding-friendly environment, breastfeeding support groups and cooperation with relevant organization. This study suggested that the PHNs’ responsibilities for breastfeeding support should be extended beyond the currently understood scope of their duties. The study demonstrated that PNH needed to create a breastfeeding-friendly environment that included networking with breast care support organizations and peer groups and to promote breastfeeding in public areas including the workplace. 展开更多
