Treatment of 140 cerebral palsied children with a combined method based on traditional Chinese medicine(TCM)and western medicine

Objective: To observe and evaluate a method that is effective and practical for treatment of cerebral palsied (CP)children in China. Method: The patient's age and disease type and individual specific conditions were considered in choosing therapy methods accordingly: Chinese herbs, acupuncture, auricular seed pressure, point finger pressing, massage, orthopedic hand manipulation, physiotherapy, occupational therapy, language therapy, etc. Meanwhile we created a new CP treatment model that combines hospitalized treatment with family therapy. Results: The majority of CP patients improved greatly in motor and social adaptation capacities after treatment. Wilcoxon paired rank sum test analysis showed that there were significant differences between the data before and after treatment (P＜0.01). Conclusion: This combined therapy method, based on traditional Chinese medicine and western medicine plus family supplemental therapy, is an effective and practical treatment strategy for CP children in China.

Analysis of 278 Cases of Clinical Application of Western Medicine with Traditional Chinese Medicine Syndrome Differentiation

Since the beginning of the Western Learning, modern medicine has developed rapidly in China, and the integration of Chinese and western medicine has become inevitable. Since Zhang Xichun's "Medicine in the West", using traditional Chinese medicine theory to guide the practice of western medicine, through long-term clinical diagnosis, we gradually realized the advantages of applying western medicine according to TCM syndrome differentiation. Therefore, 278 patients from the Second People's Hospital of Gansu Province from July 2015 to December 2017 were enrolled in the relevant departments of the Department of Internal Medicine. On the basis of routine treatment, TCM was obtained through inspection, inquiry, listening and smelling, palpation and pulse-taking. The syndrome type is based on the medicinal properties, menstruation and ef?cacy of more than ten kinds of western medicines, so that they can reach Chinese medicine[1]. Therefore, this paper aims to improve the accuracy of western medicine use, reduce the incidence of adverse reactions, discover the new ef?cacy of western medicine,and promote the integration of Chinese and western medicine.

The Eastern Cultural Signature of Traditional Chinese Medicine: Empirical Evidence and Theoretical Perspectives

Background: Holistic thinking, which is rooted in Eastern culture, is assumed to be the core of traditional Chinese medicine (TCM). Recently, such holistic thinking has been proposed to be applicable to Western medicine practices for alleviating serious side effects;however, the obscure and often ill-defined terms of TCM, such as qi, yin yang, and wuxing, pose considerable obstacles for further understanding TCM. In the present study, we explored whether and how TCM is actually related to the scientific construct of holistic thinking, to elucidate the particular cultural signature of TCM. Methods: A random sample of 101 college students majoring in TCM and 93 non-medical college students was recruited for the study. Two psychological scales—the Chinese Holistic Thinking Scale and the TCM Competence Scale were used respectively to measure the holistic thinking and participants’ ability to apply the TCM in practice. Results: We found that individuals who thought more holistically were better at applying TCM to modern medical problems. Interestingly, TCM was associated with holistic thinking in both TCM and non-medical students, suggesting that this association is intrinsic. Further exploration revealed that the association and variability facets of Eastern holistic thinking—which emphasize that the world is interconnected and ever-changing, respectively—significantly accounted for the individual differences in competence in utilizing TCM in practice. Conclusion: In short, our study provides the first empirical evidence linking TCM to the Eastern holistic thinking style, which not only deepens the understanding of TCM from a scientific perspective but also promotes dialogue between TCM and Western medicine for building safer and more effective health care systems.

The Origin of Traditional Chinese Medicine and Thinking on Chinese Medicine Development

Although the traditional Chinese medicine (TCM) has developed a profound theory system and a fame of outstanding advantage in curative effect, it seems that the TCM is not quite adaptable to the deveiopment and requirement of the modern society.A principle of “Adopting the advanced part of Western medicine to serve the TCM" should be adhered to for the traditional Chinese medicine. First, TCM should strive for a consensus of TCM theory which focuses on the guiding principles of “diagnosis and treatment based on an overall analysis of the illness and the patient’s condition"and “the opposite principle of five internal organs".Among these principles, “keeping relative balance of three internal systems"is a valuable guidance of the diagnosis and treatment theory. Secondly, macroscopic should be integrated with microscopic, totality with parts,phenomenology with materialism.A dialectical law should be applied to guide analysis. In treatment, adjustment and balance are basic principles.It is necessary not only prepare the medicine in advance accoding to the predicted disease of the year,but also adopt methods to restrict disease in advance.Basides, TCM dose type should be ameliorated in order to meet the needs of society.

孙桂芝病证结合辨证在肿瘤治疗中的应用

病证结合是孙桂芝教授治疗肿瘤病学术思想的重要组成部分,把辨证与辨病有机地结合起来,进行综合诊治,既照顾到整体,又注意到局部;辨证为先还是辨病为先在临床诊疗中要根据实际情况而定,二者互补。特别是现代科学技术的发展弥补了中医诊疗方面的不足,辨病治疗成为中医治疗学的重要部分。孙桂芝教授临证始终强调辨证与辨病紧密相连,中西手段并用,诊断疾病现代仪器和中医四诊联合,治疗上病证相参结合,攻补兼施,标本兼治,既要兼顾全局,也要重视细节。

基于关联规则的血脂异常手部特征与中医证候、西医分型相关性的临床研究

血脂异常属于中医“痰浊”“膏脂”范畴,具有隐匿性、进行性、全身性的特点,与多种心脑血管疾病的发生密切相关。其危险因素多、合并症多,但无特征性症状表现,这增加了血脂异常的治疗难度。中医手诊属于中医望诊范畴,是指通过观察手部形态、颜色、青筋(粗血管)、纹理等信息获得望诊资料以便于辨证论治,具有整体性、直观性的特点。课题组前期研究发现血脂异常患者手部大、小鱼际形态及颜色与正常人群存在差异。该研究进一步探索血脂异常患者的手部特征与中医证候、西医分型的相关性,将手诊应用于血脂异常的辨治,对丰富血脂异常的四诊信息、提高临床辨证准确性、发挥中医望诊及“治未病”优势具有重要临床应用价值。

心房颤动的中西医研究进展

西医认为,心房颤动(简称房颤)的发病机制与心房的电重构、结构重构、炎症因子、氧化应激及分子遗传机制等因素有一定的相关性,临床治疗房颤除了使用常规抗心律失常药物外,射频消融术、左心耳封堵术等非药物疗法也有较好的疗效。中医则认为,房颤的病因病机多为本虚标实,其中本虚多以气阴两虚为主,标实多以痰浊、瘀血、水饮为主。历代医家治疗房颤是在经方、时方、验方的基础上结合临证经验而自拟方剂,可获得较好疗效。目前,关于房颤的研究仍存在以下问题:(1)缺乏从不同机制的交叉关系去研究是否存在有共同的靶点或信号通路;(2)中医证型较为繁杂,尚无统一的诊疗规范及疗效判定标准;(3)缺乏大样本、多中心、随机的双盲试验。今后,应充分利用现代药理学研究,探究中药疗效背后存在的作用机制,从而开展严谨、有效的大样本的临床试验。

中西医治疗腹膜透析患者微炎症状态概况

腹膜透析(peritoneal dialysis, PD)是慢性肾脏病终末期的肾脏替代治疗方法之一,长期PD会导致患者微炎症状态等并发症发生。西医主要采用血管紧张素受体阻滞药(angiotensin receptor blockers, ARB)类、他汀类、左卡尼汀(肉碱)等药物治疗;中医药治疗主要包括中成药、中药复方、中西医结合以及其他治疗等。目前,研究存在的问题是:⑴主要为临床试验研究,缺乏高质量的基础研究;(2)临床研究样本量较小,缺乏多中心、大样本、随机的对照研究,治疗周期相对较短,缺乏对其远期疗效资料的统计分析;(3)缺乏公认的证型标准和诊疗规范,缺乏统一的结果指标。今后,应加强该领域的基础研究,充分利用现代科学技术来阐明疾病的发病机制,为中医药治疗PD微炎症状态提供更多的科学依据,完善临床研究相关设计,提供更加合理的研究成果,制定更加规范的诊疗标准,在临床治疗中,根据实际情况去选择合适的治疗手段,发挥出中西医结合治疗的优势,完善中西医结合诊疗体系,提高腹膜透析患者生存率及生活质量。

中医理论体系下西药的运用——西药中药化的方法探究

随着中西医结合的不断深入,医学工作者在临床工作中发现,西药因囿于西医体系,很难在中医药理论体系指导下的临床诊疗上发挥作用,极有必要走西药中药化之路,以弥补这一方面的不足。本文以阿司匹林为主要范例,从推测西药的中药性质入手,总结归纳几种研究方法及注意事项,并大致总结目前研究西药中药化上的诸多不足,同时收集汇总目前相关文献中提到的西药的中药性质,以抛砖引玉,供后续研究者参考。

功能性消化不良与内脏高敏感性相关性的研究进展

功能性消化不良是一种常见的消化系统疾病,具有反复发作、迁延难愈的特点,临床表现为慢性或反复的上腹部疼痛、腹胀、恶心、呕吐和腹泻等症状。其发病机制与内脏高敏感性、胃肠动力障碍、幽门螺杆菌感染、胃酸分泌增加、微生物失调、精神障碍等多种因素相关。近年来,由于生活节奏加快和饮食结构改变,该病发病率不断上升,引起了临床工作者的广泛关注。内脏高敏感性作为功能性消化不良的一个重要病理机制,发病原因极为复杂,主要由中枢敏感性增高、受体激活、递质释放等引起。西医治疗该病侧重于对神经递质及受体进行干预,同时也注重患者心理对疾病进展程度的影响,采用多种药物联合,可有效降低患者内脏高敏感性引起的一系列不良反应,并改善生活质量。中医药治疗功能性消化不良有独特的优势,能够多维度、多层面、多靶点的整体性调节。目前,对于功能性消化不良与内脏高敏感性之间的关系,以及中医药干预措施的研究,仍处于探索初期。探讨功能性消化不良与内脏高敏感性之间的关系,对于指导临床治疗以及新药研发有一定意义。文章从内脏高敏感性角度出发,全面搜集和整理了相关文献,系统梳理了当前关于内脏高敏感性与功能性消化不良之间关系的研究进展,为今后深入探索二者之间的关系提供理论依据,也为功能性消化不良的针对性治疗提供新思路。

养阴解郁疏肝汤联合常规西药治疗围绝经期综合征患者的效果

目的:观察养阴解郁疏肝汤联合常规西药治疗围绝经期综合征患者的效果。方法:选取2019年1月至2023年1月该院收治的85例围绝经期综合征患者进行前瞻性研究,按照随机数字表法分为研究组43例与对照组42例。对照组采用雌二醇片/雌二醇地屈孕酮片复合包装治疗,研究组在对照组基础上联合养阴解郁疏肝汤治疗。比较两组治疗前后性激素[促卵泡激素(FSH)、黄体生成素(LH)、雌二醇(E2)]水平、子宫内膜下血流参数指标[阻力指数(RI)、搏动指数(PI)]水平、中医证候积分、临床症状评分、临床疗效和不良反应发生率。结果:治疗后,两组FSH、LH水平均低于治疗前,且研究组低于对照组,两组E2水平均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05);治疗后,两组RI和PI水平均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);两组腰膝酸痛、潮热汗出、神疲乏力等中医证候积分均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);两组Kupperma评分均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);研究组治疗总有效率为95.35%(41/43),高于对照组的78.57%(33/42),差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:养阴解郁疏肝汤联合常规西药治疗围绝经期综合征患者可提高治疗总有效率,改善性激素水平,降低子宫内膜下血流参数指标水平、临床症状评分和中医证候积分,其效果优于常规西药治疗。

羚角钩藤汤辅助西医治疗对中风后偏瘫患者中医症候、肢体功能的影响研究

目的:观察羚角钩藤汤辅助西医治疗对改善中风后偏瘫患者中医症候、肢体功能的影响。方法:选取2020年7月—2022年2月平顶山市第二人民医院收治的129例中风后偏瘫患者作为研究对象,经随机数表法分为观察组(65例,予以中药羚角钩藤汤+西医治疗)和对照组(64例,仅予以西医治疗),比较两组患者治疗后的中医症候、肢体功能及脑组织血液流变学改善情况,对所有患者开展为期6个月的短期随访,观察患者脑神经功能恢复情况以及药物相关副反应发生情况。结果:治疗后,观察组的头痛眩晕、心烦易怒等主证以及面赤身热,气粗口臭等次证均低于对照组,差异有统计学意义(t=3.252、2.760、2.895、3.255,P<0.05);治疗后,观察者的Fugl-Meyer运动功能评分量表(FMA)高于对照组,差异有统计学意义(t=3.016、3.220,P<0.05);治疗后,观察组血浆黏度(PSV)、全血高切黏度(HSV)、全血低切黏度(LSV)均低于对照组,差异有统计学意义(t=3.021、3.000、3.498,P<0.05);随访期间,观察组的卒中量表(NIHSS)评分低于对照组,改良Barthel指数(MBI)评分高于对照组,差异有统计学意义(t=2.950、3.081,P<0.05);随访期间,观察组的不良反应发生率略高于对照组,差异无统计学意义(χ^(2)=0.160,P>0.05)。结论:在予以中风后偏瘫患者常规西医治疗的同时采用羚角钩藤汤配合治疗可促进患者症候恢复,还可以有效改善患者肢体功能、促进脑组织血液流变学及神经功能恢复,且此疗法安全性较高。

中西医治疗宫颈人乳头瘤病毒感染研究进展

中西医治疗宫颈人乳头瘤病毒感染的方法主要包括西药治疗、中药治疗、物理及手术治疗、联合治疗等,且联合治疗较单一治疗的临床疗效更为显著。但现有研究仍存在以下不足之处:(1)目前只有个别联合治疗方案研究表明了其显著的疗效,缺乏更多研究结果来证实;(2)中药内服联合外用的治疗药物较少,且治疗方案较单一,阴道纳药多使用现有中成药制剂,缺乏更多关于阴道纳药、宫颈涂药的药物研究;(3)许多中药方剂治疗HPV感染疗效确切,但未经现代药理学研究证实。今后,可着重从以下几方面进行研究:(1)对于只有个别研究证实其疗效的联合治疗方案,可采用随机对照的临床研究方法进行更多的研究证实;(2)进一步探索其他疗效确切的外用中药制剂及疗效更为显著、安全性更高的中药联合治疗方案,如中药汤剂内服联合中药洗剂阴道冲洗及中药膏剂宫颈涂药治疗,口服中药汤剂改善体质的同时,阴道给药直达病所,双管齐下提高疗效;(3)对于一些临床上确有其效,但尚未经过现代医学药理毒理研究证实的中药方剂,可以结合网络药理学研究,探索这些方剂中的药物是否确有抗宫颈HPV感染、抗宫颈癌的效果,以期为中药方剂治疗本病提供现代药理学依据。
