AIM:To evaluate the feasibility of a preoperative colonoscopy through a self-expendable metallic stent(SEMS)and to identify the factors that affect complete colonoscopy.METHODS:A total of 48 patients who had SEMS plac...AIM:To evaluate the feasibility of a preoperative colonoscopy through a self-expendable metallic stent(SEMS)and to identify the factors that affect complete colonoscopy.METHODS:A total of 48 patients who had SEMS placement because of acute malignant colonic obstruction underwent preoperative colonoscopy.After effective SEMS placement,patients who showed complete resolution of radiological findings and clinical signs of acute colon obstruction underwent a standard bowel preparation.Preoperative colonoscopy was then performed using a standard colonoscope.If the passage of colonoscope was not feasible gastroscope was used.After colonoscopy,cecal intubation time,grade of bowel preparation,tumor location,stent location,presence of synchronous polyps or cancer,damage to colonoscopy and bleeding,and stent migration after colonoscopy were recorded.RESULTS:Complete evaluation with colonoscope was possible in 30 patients(62.5%).In this group,adenoma was detected in 13 patients(43.3%).The factors that affected complete colonoscopy were also analyzed:Tumor location at an angle;stent placement at an angle;and stent expansion diameter,which affected complete colonoscopy significantly.However in multivariate analysis,stent expansion diameter was the only significant factor that affected complete colonoscopy.Complete evaluation using additional gastroscope was feasible in 42 patients(87.5%).CONCLUSION:Preoperative colonoscopy through the colonic stent using only conventional colonoscope was unfavorable.The narrow expansion diameter of the stent may predict unfavorable outcome.In such a case,using small caliber scope should be considered and may expect successful outcome.展开更多
Quantum dynamical maps are defined and studied for quantum statistical physics based on Orlicz spaces.This complements earlier work[26]where we made a strong case for the assertion that statistical physics of regular ...Quantum dynamical maps are defined and studied for quantum statistical physics based on Orlicz spaces.This complements earlier work[26]where we made a strong case for the assertion that statistical physics of regular systems should properly be based on the pair of Orlicz spaces(Lcosh-1,L log(L+1)),since this framework gives a better description of regular observables,and also allows for a well-defined entropy function.In the present paper we"complete"the picture by addressing the issue of the dynamics of such a system,as described by a Markov semigroup corresponding to some Dirichlet form(see[4,13,14]).Specifically,we show that even in the most general non-commutative contexts,completely positive Markov maps satisfying a natural Det ailed Balance condition canonically admit an action on a large class of quantum Orlicz spaces.This is achieved by the development of a new interpolation strategy for extending the action of such maps to the appropriate intermediate spaces of the pair(L∞,L1).As a consequence,we obtain that completely positive quantum Markov dynamics naturally extends to the context proposed in[26].展开更多
AIM:To compare the morbidity and mortality in young and elderly hepatocellular carcinoma(HCC)patients undergoing liver resection.METHODS:We retrospectively enrolled 1543 consecutive hepatitis B(HBV)-related HCC patien...AIM:To compare the morbidity and mortality in young and elderly hepatocellular carcinoma(HCC)patients undergoing liver resection.METHODS:We retrospectively enrolled 1543 consecutive hepatitis B(HBV)-related HCC patients undergoing elective hepatic resection in our cohort,including 207elderly patients(≥65 years)and 1336 younger patients(<65 years).Patient characteristics and clinical outcomes after liver resection were compared between the two groups.RESULTS:Elderly patients had more preoperative comorbidities and lower alanine aminotransferase and aspartate aminotransferase levels.Positive rates for hepatitis B surface antigen(P<0.001),hepatitis B e antigen(P<0.001)and HBV DNA(P=0.017)were more common in younger patients.Overall complications and their severity classified using the Clavien system were similarin the two groups(33.3%vs 29.6%,P=0.271).Elderly patients had a higher rate of postoperative cardiovascular complications(3.9%vs 0.6%,P=0.001),neurological complications(2.9%vs 0.4%,P<0.001)and mortality(3.4%vs 1.2%,P=0.035),and had more hospital stay requirement(13 d vs 12 d,P<0.001)and more intensive care unit stay(36.7%vs 27.8%,P=0.008)compared with younger patients.However,postoperative hepatic insufficiency was more common in the younger group(7.7%vs 3.4%,P=0.024).CONCLUSION:Hepatectomy can be safely performed in elderly patients.Age should not be regarded as a contraindication to liver resection with expected higher complication and mortality rates.展开更多

The East Kunlun terrain is located on the northern Qinghai—Tibet plateau, composed of the East Kunlun Mountain and the Qaidam Basin and bounded by the Qilian terrain on the north and Bayanhar—Songpan Ganze terrain o...The East Kunlun terrain is located on the northern Qinghai—Tibet plateau, composed of the East Kunlun Mountain and the Qaidam Basin and bounded by the Qilian terrain on the north and Bayanhar—Songpan Ganze terrain on the south. It is regarded as a composite orogenic belt characterized by having developed superimposed ductile tectonic regimes reflecting the collision orogeny during Early Paleozoic and Triassic periods. It has also experienced transformation from ductile to brittle deformation caused by the post orogeny, since Jurassic after the formation of the East Kunlun Mountain and the Qaidam Basin. A Paleozoic subduction complex zone was recently recognized along the north border of the East Kunlun terrain from Da Qaidam to Dulan (Xu,et al, 1 999). It is composed of ophiolite (of Early Paleozoic age?), tectonic melange and very high\|pressure metamorphic rocks with eclogite (\%p\%=2 2GPa, \%t\%=720℃) (Yang,et al.,1998) and garnet\|peridotite (\%p\%=2 5GPa, t =837℃). The Anyemaqin Triassic subduction complex zone trending in NWW\|SEE was developed along the eastern segment of the south border of the East Kunlun terrain. It is mainly composed of ultramafic and mafic rocks, pillow basalt, radiolaria\|bearing clastic rocks, tectonic melange and mylonite. The subduction complex zone contains a series of the southward overthrusting imbricated slices. Instead of this subduction complex zone, a 2 km\|wide sinistral strike\|slip ductile shear zone trending in E\|W was developed along the western segment. On the basis of macroscopic and microscopic studies on a series of structural sections, we divide the East Kunlun Mountain into four tectonic units as follows: (1) North Proterozoic Metamorphic Basement Zone; (2) South Early Paleozoic—Triassic Superimposed Fold Zone; (3) South Triassic Transpression Zone; (4) Anyemaqin Back\|Arc Decollement\|Thrust Zone.展开更多
