In infertile males, abnormal karyotypes are more frequent than in the general population. Furthermore, meiotic disorders affecting the germ cell-line have been observed in men with normal somatic karyotypes consulting for infertility. In both cases, the production of unbalanced spermatozoa has been demonstrated. Basically addressed to establish reproductive risks, fluorescence in situ hybridization (FISH) on decondensed sperm heads has become the most frequently used method to evaluate the chromosomal constitution of spermatozoa in carriers of numerical sex chromosome abnormalities, carriers of structural chromosome reorganizations and infertile males with normal karyotype. The aim of this review is to present updated figures of the information obtained through sperm FISH studies with an emphasis on its clinical significance. Furthermore, the incorporation of novel FISH-based techniques (Multiplex-FISH; Multi-FISH) in male infertility studies is also discussed. (Asian J Androl 2005 Sep; 7: 227-236)展开更多

Methods A repeat HIV testing within 12 months was recorded if individuals had their first test with negative results, during 2013-2014 and retested within 12 months. An incident HIV diagnosis was recorded if individuals had their first test with negative results, during 2013-2015 and had a subsequent positive result at any point by the end of 2015. Data on HIV testing and diagnosis among individuals attending 32 VCT clinics from 2013 to 2015 and HIV diagnosis from other clinical services in Wuxi, China, were retrieved. A multivariate logistic regression model was used to analyze factors associated with repeat HIV testing. Cox regression was used to evaluate factors associated with incident HIV diagnosis. Results From 2013 to 2014, 11,504 individuals tested HIV negative at their first recorded test, with 655 (5.7%) retesting within 12 months. Higher repeat HIV testing within 12 months was associated with male gender [adjusted odds ratio (a OR) = 1.7, 95% confidence interval (CI): 1.4-2.2], risk behaviors [commercial heterosexual behaviors (a OR = 1.4, CI: 1.1-1.6), male-male sexual behaviors (a OR = 3.7, CI: 2.7-4.9)], injection drug use (a OR = 9.9, CI: 6.5-15.1), and having taken HIV tests previously (a OR = 2.0, CI: 1.6-2.4). From 2013 to 2015, 1,088 individuals tested negative on HIV test at their visit and at ≥ 2 subsequent tests; of them 30 had incident HIV diagnosis. The overall rate of incident HIV diagnosis among all VCT individuals was 1.6 (95% CI: 1.1-2.1) per 100 person-years. Incident HIV diagnosis was associated with male gender [adjusted hazard ratio (a HR) = 8.5, 95% CI: 1.9-38.1], attending hospital-based VCT clinics (a HR = 7.8, 95% CI: 1.1-58.3), and male-male sexual behavior (a HR = 8.4, 95% CI: 1.5-46.7). Individuals diagnosed at VCT clinics had higher CD4+ T cell count compared with those diagnosed at other clinical services (median 407 vs. 326 copies/mm3, P = 0.003). Conclusion VCT individuals in Wuxi, China, had a low repeat HIV testing rate and high HIV incidence. VCT-clinic-based interventions aimed at increasing repeat HIV testing are needed to detect more cases at an earlier stage, especially among individuals at high risk for HIV infection such as men who have sex with men.展开更多

The purpose of this study was to compare the lifestyle counseling provided for clients with different levels of readiness based on self-evaluations and independent assessment. Methods: Participants were PHNs with 10 years’ experience or less. Lifestyle counseling skills were measured through a simulated counseling session in a primary setting lasting up to 30 minutes, followed by a review session, where the participant provided self-evaluations, and the simulated client provided others-evaluations, of the session. The simulated clients played the role of clients at either the pre-contemplation or contemplation stage of preparedness as per the stages of behavior change theory. Results: The self-evaluation results showed that the mean scores for five of the six skill categories, are lower in the precontemplation-stage client cases than in the contemplation-stage client cases, and significantly so for two skill categories. The others-evaluations showed significantly lower mean scores for all skill categories in the precontemplation cases than in the contemplation cases. Conclusion: The PHNs and simulated clients agreed that lifestyle counseling skills were inadequate for the precontemplation cases, as compared with the contemplation cases. The lifestyle counseling skills of PHNs with less experience may not be well-adjusted to the readiness of the client, indicating difficulties in supporting less prepared clients.展开更多

Methods Four hundred and five married women aged 20~39 and their husbands were recruited from 14 family planning stations at 7 administrative districts of Shanghai. They were randomly divided into two groups, the counseling intervention (CI) group with 199 couples and the pamphlet distribution (PD) group with 206 couples. For the PD group, only pamphlets containing information on STI/AIDS/ condom were distributed and condoms given free of charge, whereas for the CI group, they were provided with not only full counseling on STI/AIDS/condom and free condoms, but also followed by demonstration of correct condom use. The two modes of interventions were conducted following the baseline survey, and the follow up data were collected one month later. Results 1. There was no statistical difference between the two groups on STI/AIDS/condom knowledge level, attitudes towards STI/AIDS patients and condom use intention at the baseline. 2. The follow up findings indicated that: ① A significant enhancement in knowledge level was observed in the CI group, with 20% increase for husbands and 24% for wives. While for the PD group, only slight increase was observed: 2% for husbands and 3% for wives; ② 93% husbands and 76% wives of the CI group changed their discriminatory attitudes towards STI patients, and towards AIDS patients, the percentages were 73% and 78%, respectively; ③ 68% husbands and 64% wives of the CI group expressed their intention for condom use; ④ For the PD group, however, there was no difference in attitudes towards STI/AIDS patients and condom use intention at the baseline and follow up. 3. The knowledge scores and attitudes towards STI/AIDS patients were mainly influenced by the means of intervention. While the intention of condom use was influenced not only by means of intervention but also by condom availability. Conclusions 1. Good counseling can not only greatly enhance people's STI/AIDS/ condom knowledge level but also apparently change their discrminatory attitudes towards STI/AIDS patients; 2. Effective counseling and condom availability are two important factors that influence condom use intention; 3. It is effective and feasible to integrate counseling with the routine procedure of family planning services, hence is worth being scaled up in Shanghai or even nationwide.展开更多

Over ninety percent of HIV infection in children is acquired through mother-to-child transmission. Objectives: This study assessed pregnant women’s knowledge of HIV/AIDS, their awareness of HIV testing and counseling, and compared the uptake of HTC to the previously practiced voluntary counseling and testing (VCT) in a teaching hospital in northern Nigeria. Materials and Methods: A pre-tested, structured interview questionnaire was administered on a cross-section survey of 270 antenatal clients in Ahmadu Bello University Teaching Hospital (ABUTH), Nigeria. Information about knowledge of HIV/AIDS and attitude towards HTC was elicited among respondents. Results: Eighty-nine percent of respondents were aware of HIV/AIDS.?The knowledge of route of disease transmission was high, with 87.4% and 81.1% of studyparticipants mentioning sexual activity and mother-to-child transmission, respectively. Majority of respondents (98.9%) were aware of HTC and approved of it (91.1%) while 7.8% declined and the remaining 1.1% were undecided. Fourteen percent of respondents didn’t approve of retesting in labour. Sixty-eight percent of respondents were aware of interventions to prevent mother-to-child transmission of HIV. Switching from VCT to HTC has dramatically improved the uptake of testing from 78.9% in 2006 to 91.1% in 2012. Conclusion: Routine HIV testing and counseling is more acceptable than voluntary counseling and testing among antenatal clients. Although the awareness and uptake of HTC were quite high among the antenatal clients, there is still a need to intensify health education and incorporate husbands into the counseling session when necessary to convince the remaining minority who are still ignorant of the benefits of HTC.展开更多

文摘Counseling people, particularly those in the military engaged in group living, who are in quarantine during the COVID-19 pandemic is a challenge. Therefore, supporting the people in quarantine who are experiencing psy-chological and interpersonal problems has become a new challenge in military mental health. This study’s primary concern was how to overcome the problems caused by physical quarantine. The study subject was a virtual coun-seling network and its operating experience during the quarantine period in Taiwan amidst the global COVID-19 pandemic. For soldiers who mainly live in groups, this study discussed how the virtual counseling network combined with the existing military support group to determine what influence the network had on peo-ple in quarantine. This study found that this group exhibited four types of experiences:togetherness, empathy, confidence, and belonging and cohesion.Such experiences are beneficial for group healing through mutual sup-port and companionship.Collective cohesion and psychological healing can be achieved through a virtual com-munity.This is worthy of attention, particularly in the pandemic or post-pandemic era.Physical isolation has become a fact of life, and such isolation is not just isolation from disease but also between regional boundaries.Counseling and support systems in virtual space or the operation of virtual teams must be considered for the future.
文摘Chromosome abnormalities are one of the major causes of human infertility. (Asian J Androl 2005 Sep; 7: 227-236)

文摘Objective We aimed to elucidate the rates of repeat HIV testing and incident HIV diagnosis, and baseline CD4^+ T cell count among individuals attending HIV voluntary counseling and testing (VCT) clinics in Wuxi, China. VCT-clinic-based interventions aimed at increasing repeat HIV testing are needed to detect more cases at an earlier stage, especially among individuals at high risk for HIV infection such as men who have sex with men.
文摘Objectives: Non-adherence to tuberculosis (TB) treatment causes development of multi-drug resistance (MDR). In Armenia, about 47% of previously-treated TB patients develop MDR-TB. This pilot intervention intended to explore the feasibility and effectiveness of a family-based-counseling (FBC) that included a psychological component in terms of improved adherence of TB patients to treatment and reduced stigma of TB. Methods: Overall, 136 regular TB patients (55) and their family members (81) participated in a single 90-minute interactive counseling session conducted in each household by the team of professional psychologist and TB nurse. To evaluate FBC effectiveness, we administered baseline and follow-up surveys to 52 TB patients and their 57 family members in 2012 and compared treatment outcomes of the study participants with the national data for 2011 and 2013. Results: We found that the intervention substantially improved the mean knowledge score of TB patients (from 19.2 to 21.6, p Conclusion: The evaluation showed that a low-cost one-time family–based educational intervention with a psychological component can be effective in improving treatment outcomes of TB patients.
文摘Objective:To assess the proportion of caregivers of children and patients who received malaria prevention counseling from their healthcare provider at the time of outpatient malaria treatment. Methods:This was a descriptive cross-sectional study involving 353 randomly selected adult patients and caregivers of children attending the general and paediatric outpatient clinics of a Nigerian hospital. An interviewer-administered questionnaire was used to collect data on participants'sociodemographics and last malaria-episode characteristics.Chi square test and logistic regression analysis were used to determine factors associated with and predictors of receiving malaria-prevention counseling, respectively. Results:The mean age of the respondents was(33.6±9.6)years;they were predominantly females(257,72.8%).Most had at least secondary-level education(304,86.1%);the family size was mostly 1-6 persons in 202(57.2%)respondents.They were treated mostly by doctors 256(72.5%),but only 132(37.4%)received malaria prevention counseling.Educational-level(x2=5.42,P=0.02),family size(Fisher's exact,P=0.01),place of treatment(Fisher's exact,P=0.0002),pretreatment malaria test(x2=9.48,P=0.002),who prescribed treatment(Fisher's exact P=0.001),consultation time(x2=22.09,P<0.001),and previous demand for malaria prevention counseling(x2=36.59,P<0.001)were associated with receiving malaria prevention counseling. However, consultation time[Odd ratio(OR)=2.38,95%confidence interval(CI):1.28,4.40,P=0.006]and previous demand for malaria prevention counseling(OR=3.62,95%CI:2.15,6.10,P<0.001)were independent predictors of receiving counseling. Conclusions:The proportion of respondents that received malaria prevention counseling was surprisingly low. This offers opportunity for policy review towards improving the quality of outpatient malaria care through alternative strategies for giving malaria prevention information, especially in busy clinics where consultation time is constrained.
文摘Purpose: Public health nurses (PHNs) are required to assess the readiness of the clients and provide lifestyle counseling accordingly. The lifestyle counseling skills of PHNs with less experience may not be well-adjusted to the readiness of the client, indicating difficulties in supporting less prepared clients.
文摘This paper addresses the topic of an interdisciplinary approach of chronic pain management from a biopsychosocial perspective. The first section provides an introduction to the definitions and theories of chronic pain and the various contributing factors (psychological, interpersonal/environmental and social support, and vocational). The second section presents the role of various health care professions (medical doctors, nurses, physical therapists, occupational therapists, psychologists and rehabilitation counselors) and the evidence of their treatment effectiveness. The third section discusses the concept of an interdisciplinary pain rehabilitation program (IPRP) and its evidence to support its effectiveness. Finally, the clinical implications of rehabilitation counseling and psychology as part of the inter-disciplinary program in treating individuals with chronic pain will be highlighted.

文摘BACKGROUND CYP21A2 gene mutations may all cause reduction or loss of 21-hydroxylase activity, leading to development of congenital adrenal hyperplasia(CAH)with different clinical phenotypes. For families with CAH children, genetic testing of the parents and genetic counseling are recommended to assess the risk of recurrence.CASE SUMMARY We report a case of CAH with a high suspicion before delivery. The risk of the child suffering from CAH during the pregnancy had been underestimated due to the deviation of genetic counseling and genetic testing results. Our report confirmed a CYP21A2 homozygous deletion in this case,CYP21A2 heterozygous deletion in the mother, and a rare 2+0 CYP21A2 deletion in the father.CONCLUSION It is important to analyze the distribution of CYP21A2 gene in the two alleles of parents of children with CAH.
文摘Objective To assess the impact of pamphlet distribution and counseling on STI/HIV/condom knowledge, attitudes towards STI/AIDS patients and condom use intention of married couples in Shanghai and explore the effective way of integrating STI/HIV prevention with family planning services. It is effective and feasible to integrate counseling with the routine procedure of family planning services, hence is worth being scaled up in Shanghai or even nationwide.

文摘Objective To evaluate the training course effects on integrated sexual and reproductive health (SRH) counseling, so as to improve family planning providers' capacities of SRH counseling. Methods The SRH training course was conducted by using new curriculum and participatory training methods. The purpose was meeting the SRH needs of clients. Evaluations were done by pre- and post-training questionnaires to trainees. Results The average total scores of trainees were 54.9 before trained and 83.4 after trained respectively. As to trainees' self evaluation of the capacities, the capacity for comfortably discussing sexuality and related issues with clients increased from 9.9% to 90.7%, the capacity for helping clients to understand their sexual risk factors increased from 12.5% to 92.2%, the capacity for helping clients assessing their own needs for SRH services, information and emotional support increased from 16.0% to 87.8%, and the capacity for helping clients to develop negotiation and communication skills increased from 12.0% to 86. 0%. Conclusion The training course of integrated SRH counseling changed the traditional training types and services concept o f family planning service providers, and the trainees' capacities of implementing integrated SRH counseling also improved very much.
文摘The researcher argues that KH Muhammad Ali Shodiqin or Gus Ali Gondrong,a chairperson of Roudotun Ni’mah Islamic boarding school in Semarang, can be used as an inspiration source to give counseling to Muslim clients with sholawat as sufistic counseling. His Islamic boarding school becomes famous not due to sholawat mafia, but because he is preaching in a way appropriate for youth. The figure of Gus Ali is loved by young people and most of them are street children,“naughty”children, and hoodlums.Gus Ali’s pupils or Santri is also not ordinary children born to ordinary families, but they are“extraordinary”kids, since they can get out of the dark society covering them. Most santris are hoodlums,thugs,ex-gambler,ex-drunkards,and even ex-prostitutes.Everyone receives guidance from Allah SWT so that they can be educated at Roudlotun Ni’mah Islamic boarding school. This may sound quite weird for common people, since the word“Mafia”stands for unifying mind and heart into Sholawat. It is expected that the followers will love to do sholawat, and do sholawat continuously and full of self-consciousness.Members of this community come from various life backgrounds;among them are the youth and ex-thugs.It is hoped that this youngster style sholawat group can attract many people, especially the youth. Thus, young people have a good habit by joining shalawat.

文摘Objective: The objective was to examine practices of primary care physicians and its determinants towards prostate cancer screening. Methods: Data was obtained from 204 primary care physicians using self-reports of prostate cancer screening practices, knowledge, attitudes towards prostate cancer screening. Results: nearly half of the respondent (54.7%) were practicing counseling and referring prostate cancer patients. The mean correct knowledge score was 54.3%, their attitude was not strong;the only statement that nearly seventy percent of physicians agreed upon was about the value of screening, however, the reliability and evidence to support DRE & PSA were in question. Knowledge and attitude were found to be the most significant predictors that determine physicians’ self practice. Conclusion: Background information and attitudes are important determinants of physicians’ practice behavior towards prostate cancer counseling and referral in our study.
文摘Background: HIV testing and counseling (HTC) plays a central role in specific prevention of mother-to-child transmission (PMTCT) interventions, as these interventions depend on the woman knowing her HIV status. Although the awareness and uptake of HTC were quite high among the antenatal clients, there is still a need to intensify health education and incorporate husbands into the counseling session when necessary to convince the remaining minority who are still ignorant of the benefits of HTC.
