Treatment of Unicameral and Aneurysmal Bone Cysts by Minimally Invasive Percutaneous Injection of Grafton DBF Putty Using the Kyphon Cement Delivery System

However, often size increas... <strong>Background: </strong>Simple Unicameral and Aneurysmal Bone Cysts are benign lesions that may heal spontaneously especially after fracture which may be the first symptom. However, often size increases causing pain, and complications of fractures can severely compromise the patient.<strong> Aim:</strong> The results in a series of cases treated minimally invasive using a new device for the application of allogenic bone material appear highly promising and shall be presented. <strong>Patients and Methods:</strong> Eight consecutive patients with symptomatic Unicameral Bone Cysts (UBC) were treated by percutaneous instillation of Grafton<span style="white-space:nowrap;">?</span> DBF Putty (demineralised allogenic bone containing fibers) mixed with autologous bone marrow using the Kyphon<span style="white-space:nowrap;">?</span> Cement Delivery System (Medtronic), which allows the injection of this high viscosity paste by controlled high pressure. Five patients with Aneurysmal Bone Cysts (ABC) were treated accordingly after inactivation by Aethoxysclerol 3% and lacking bone formation. Using this approach a high rate of bone regeneration was observed in these patients at 8 months to 5 years follow-up (f/u). <strong>Conclusion:</strong> The presented technique of a minimally invasive biologic treatment led to highly satisfying results using the Grafton<span style="white-space:nowrap;">?</span> DBF Putty with its higher potential for bone regeneration than demineralized bone matrix not containing fibres (DBM). 展开更多

The wavelength-tunable and switchable narrow bandwidth mode-locking operation is demonstrated in an all fiber laser based on semiconductor-saturable absorber mirror (SESAM). Two narrow-band fiber Bragg gratings centered at 1029.9nm and 1032nm respectively with a polarization controller inserted between them are used to realize the wavelength switchable between 1029.9nm and 1032nm. The laser delivers different pulse widths of 7.5ps for 1030nm and 20ps for 1032nm. The maximum output power for both could reach -6.5mW at single pulse operation. The output wavelength couM be tuned to about 0.gnm intervals ranging from 1030.2nm to 1031.1 nm and from 1032.15nm to 1033.7nm with the temperature change of the fiber Bragg grating, respectively. 展开更多

The Internet of thing(Io T) emerges as a possible solution to realize a smart life in the modern age. In this article, we design and realize a novel near field communication(NFC)-driven smart home system for Io T, which integrates the wireless sensor network(WSN), social networks, and the cloud computing. NFC technology provides a way for users to exchange information between them and the system by simply contacting. So, we propose to use NFC as the system drive in the architecture, such that users can intuitively interact with the system and deliver their intentions. Then, the corresponding service over the system will control or adjust the “things” at home to fit users' needs. Furthermore, the proposed system provides a platform for developers to easily and rapidly implement their smart home related services. In the system, WSN sensing and control, NFC communications and identification, user profile management and preference analysis, and social network integration are all provided as platform services. A discrete event heuristic feedback control system fo r a steel plate storage at Odense Steel Shipyard Ltd is developed and implemente d in a computer-based simulation model. The plant is subject to stochastic dist urbances. The control system is able to handle this stochastic behaviour bec ause of the feedback design. The present simulation results indicate that a bene fit in the range of 30%~40% is reachable by modifying the plant. Plant description The steel plate storage is located at Odense Steel Shipyard Ltd, Denmark. The st eel plate storage contains steel plates used to build ship sections. The size of the storage is 600x35 meters. It consists of 32x8 stacks of plates. Plates are delivered and removed dynamically. The plate storage contains 5.000 steel plates in average. Over time, new plates delivered to the storage replace plates removed from the storage. The maximum si ze of a plate is 17x4 meters with a weight of up to 22 tons. The plates are uniq ue and dedicated to a specific purpose but in average the 5 000 plates consist o f 2 500 types. Plates are transported around the plate storage by two portal cranes. The cranes drive on a common rail and hence they cannot overtake each other. The cranes us e magnetic force to lift the plates. The crane can deliver the lifted plate to a conveyor belt or to another stack depending on whether the plate has been order ed for the current day or later. The plant is subject to different kinds of disturbances. The processes performed at the plant are stochastic. Furthermore, the due dates of the plates are frequ ently changed and hence the effect of previously sorting might be lost. The control task is to dispatch movements of plates to the two cranes. The overa ll goal is to minimize the costs at the plate storage and hence the two cranes a re used for executing the movements as fast as possible without colliding and re spect other physical constraints. Problem solution Different approaches have been investigated to solve the problem. A discrete eve nt heuristic feedback controller is used as an online algorithm. The controller is combined with a local search heuristic for improving the initial plan deliver ed by the controller. The improved plan is used as a reference for another feedb ack controller. To indicate the size of the problem, consider an example where the online algori thm is used for executing 600 movements for one day. Through the day the algorit hm finds the locally optimal movement between 20 executable movements. Then the number of different solutions is 20 600! Notice that the possibility of cho osing another destination for the plates to be moved is not taken into account! Results Without time disturbances the controller using an optimised plan as a reference supply the best performance. Nevertheless, when time disturbances are introduces the discrete event heuristic feedback controller performs as well. The comparis on depends on the size of the disturbances. When the disturbances reach a certai n level the discrete event heuristic feedback controller supply the best perform ance and robustness. If the performance is compared with the working system of today, it seems realis tic that a 30%~40% reduction in time is reachable. 展开更多

It is necessary to verify the efficiency of this optimization before the real system adjustment, which is costly and dangerous to some distance. A Simulation method, which employs object oriented paradigm to construct simulation system and stochastic approach to calculate process parameters, was put forward. Delivering bad news regarding patient's health is an unavoidable task for physicians and medical students in their careers.This is especially difficult for beginners.Disclosing undesired medical information to patients is technically and mentally demanding.Only with proper communication skills and sufficient patient-contact practice can medical practitioners smoothly cope with this stressful mission.There are a number of common mistakes we fall into when delivering bad news,which might induce or exacerbate patients' anxiety.Sometimes we are prone to disclose medical information in a "straight-to-the-point" manner.This is often a futile attempt made on the physician's part to ease his or her own uncomfortableness when delivering bad news,but unfortunately it does no good to facilitate patients' acceptance of the actual condition.Instead of using medical jargons,physicians are obliged to use "layman's terms" that make better sense to their patients.Declaring a clinical diagnosis without corresponding explanation on the prognosis and the discussion of treatment can also bring much confusion and anxiety to patients.Another even worse situation might be that we finish the "lecture" in our own words without confirming the patient's comprehension and addressing their questions.Professionals have recommended a six-step protocol on delivering bad news to patients:"SPIKES":S-Setting up the interview;P-assessing the patient's Perception;I-obtaining the patient's Invitation;K-giving Knowledge and information to the patient;E-addressing the patient's Emotions with Empathic responses;S-Strategy and Summary. 展开更多

Recombinant bacteria secreting the transmembrane-truncated region of white spot syndrome virus (WSSV) envelope protein VP28 named DhpVB were constructed, using live Escherichia coli as a deliverer and a con- stitutive secretory expression plasmid as the expression vector. With the ability to deliver recombinant pro- tein products outside, DhpVB could make VP28 directly interact with the shrimp when injected into shrim- p. In order to test the protection potential, live DhpVB were injected into the marine shrimp Exopalamon carincauda Holthuis for three times and WSSV challenge was performed twice simultaneously at the last two injections of the bacteria. The results showed that DhpVB displayed statistically significant advantage over bacteria without VP28 after the second challenge with an average RI (Resistance index) of 0.67±0.08, compared with 0.41±0.09 of bacteria without VP28 (P 〈0.05). The data suggested that a quasi-immune response may exist in E. carincauda and live bacteria carrying VP28 could enhance the shrimp resistance against WSSV. 展开更多

BACKGROUND: The development of a harmless and effi- cient nonviral gene delivery system that can facilitate the penetration of nucleic acids through the plasma membrane is a key to successful gene therapy. The aim of this study was to test a nonviral gene transferring vector's function of delivering DNA into liver cells to provide an important clue for gene transfer in liver gene therapy. METHODS: The complex of DNA and DNA delivering protein was injected into mice through their tail veins. Then the mice were killed and their liver tissue was sec- tioned. The gene transferring results were detected using a confocal laser scanning microscope. RESULTS: Fluorescence analysis indicated that both DNA- membrane penetrating peptide (MPP) complex and DNA- hepatocyte specific receptor binding domain ( HSRBD) - MPP complex could go into liver cells. The fluorescence value of liver cells in the DNA-HSRBD-MPP group was higher than that in the DNA-MPP group. CONCLUSIONS; MPP can successfully deliver DNA and protein into cells, and MPP with a HSRBD can specifically deliver DNA into liver cells. These have laid a foundation for further study on the nonviral liver cell gene delivering system. 展开更多

BACKGROUND:The study aims to investigate an optimal indicator for changing the filter during the continuous renal replacement therapy(CRRT)in intensive care unit(ICU)patients with acute kidney injury(AKI).METHODS:Patients with AKI requiring CRRT in an ICU were randomly divided into two groups for crossover trial,i.e.,groups A and B.Patients in the group A were firstly treated with continuous veno-venous hemofiltration(CVVH),followed by continuous veno-venous hemodiafiltration(CVVHDF).Patients in the group B were firstly treated with CVVHDF followed by CVVH.Delivered doses of solutes with different molecular weights at the indicated time points between groups were compared.A correlation analysis between the delivered dose and pre-filter pressure(P_(PRE))and transmembrane pressure(P_(TM))was performed.Receiver operating characteristic(ROC)curves were constructed to evaluate the accuracy of P_(TM) as an indicator for filter replacement.RESULTS:A total of 50 cases were analyzed,27 in the group A and 23 in the group B.Delivered doses of different molecular-weight solutes significantly decreased before changing the filter in both modalities,compared with those at the initiation of treatment(all P<0.05).In the late stage of CRRT,the possible rebound of serum medium-molecular-weight solute concentration was observed.P_(TM) was negatively correlated with the delivered dose of medium-molecular-weight solute in both modalities.The threshold for predicting the rebound of serum concentration of medium-molecularweight solute by P_(TM) was 146.5 mm Hg(1 mm Hg=0.133 k Pa).CONCLUSIONS:The filter can be used as long as possible within the manufacturer’s safe use time limits to remove small-molecular-weight solutes.P_(TM) of 146.5 mm Hg may be an optimal indicator for changing the filter in CRRT therapies to remove medium-molecular-weight solutes. 展开更多
