The decreasing risk of CRC observed in patients undergoing colonoscopy is correlated with the adenoma detection rate(ADR). Due to the fact that screening programs usually start from the age of 50, very few data are available on the risk of adenoma between 40 and 49 years. However, the incidence of CRC is increasing in young populations and it is not uncommon in routine practice to detect adenomas or even advanced neoplasia during colonoscopy in patients under 50 years.AIM To compare the ADR and advanced neoplasia detection rate(ANDR) according to age in a large series of patients during routine colonoscopy.METHODS All consecutive patients who were scheduled for colonoscopy were included.Exclusion criteria were as follows: patients scheduled for partial colonoscopy or interventional colonoscopy(for stent insertion or stenosis dilation).Colonoscopies were performed in our unit by a team of 30 gastroenterologists in2016. We determined the ADR and ANDR in each age group in the whole population and in the population with an average risk of CRC(excluding patients with personal or family history of advanced adenoma or cancer).RESULTS6027 colonoscopies were performed in patients with a median age of 57 years(range, 15-96). The ADR and ANDR were 28.6% and 9.7%, respectively, in the whole population. When comparing patients aged 40-44(n = 382) and 45-49 years (n = 515), a strong increase in all parameters from 45 years was observed, with the ADR rising from 9.7% in patients aged 40-44 to 21.2% between 45 and 49(P <0.001) and the ANDR increasing from 3.1% in patients aged 40-44 to 6.4% in those aged 45-49 years(P < 0.03). With regard to patients aged 50-54(n = 849), a statistically significant increase in the ADR and ANDR was not observed between patients aged 45-49 and those aged 50-54 years. In the population with an average risk of CRC, the ADR and ANDR were still significantly higher in patients aged 45-49 compared with those aged 40-44 years.CONCLUSION This study shows a significant two-fold increase in the ADR and ANDR in patients aged 45 years and over.展开更多

Fellows' procedural logs were used to retrieve colonoscopy procedural volumes, and these were treated as the time variable. Findings from screening colonoscopies were used to calculate colonoscopy outcomes for each fellow for the prior 50 colonoscopies at each time point. ADR and PDR were plotted against colonoscopy procedural volumes to produce individual longitudinal graphs. Repeated measures linear mixed effects models were used to study the change of ADR and PDR with increasing procedural volume.RESULTS During the study period, 12 fellows completed full three years of training and were included in the analysis. The average ADR and PDR were, respectively, 31.5% and 41.9% for all fellows, and 28.9% and 38.2% for attendings alone. There was a statistically significant increase in ADR with increasing procedural volume(1.8%/100 colonoscopies, P = 0.002). Similarly, PDR increased 2.8%/100 colonoscopies(P = 0.0001), while there was no significant change in advanced ADR(0.04%/100 colonoscopies, P = 0.92). The ADR increase was limited to the right side of the colon, while the PDR increased in both the right and left colon. The adenoma per colon and polyp per colon also increased throughout training. Fellows reached the attendings' ADR and PDR after 265 and 292 colonoscopies, respectively.CONCLUSION We found that the ADR and PDR increase with increasing colonoscopy volume throughout fellowship. Our findings support recent recommendations of ≥ 275 colonoscopies for colonoscopy credentialing.展开更多

METHODSWe conducted a retrospective chart review of 2604 first-time average risk screening colonoscopies performed at MD Anderson Cancer Center from 2010-2013. ADR and CIR were calculated in each sedation group. Multivariable regression analysis was performed to adjust for potential confounders of age and body mass index (BMI). RESULTSOne-third of the exams were done with propofol (n = 874). Overall ADR in the propofol group was significantly higher than moderate sedation (46.3% vs 41.2%, P = 0.01). After adjustment for age and BMI differences, ADR was similar between the groups. CIR was 99% for all exams. The mean cecal insertion time was shorter among propofol patients (6.9 min vs 8.2 min; P < 0.0001). CONCLUSIONDeep sedation with propofol for screening colonoscopy did not significantly improve ADR or CIR in our population of average risk patients. While propofol may allow for safer sedation in certain patients (e.g., with sleep apnea), the overall effect on colonoscopy quality metrics is not significant. Given its increased cost, propofol should be used judiciously and without the implicit expectation of a higher quality screening exam.展开更多

METHODSWe performed a retrospective review of all patients who underwent a screening colonoscopy at Grady Memorial Hospital between July 1, 2009 and June 30, 2015. Patients with a history of colon polyps or cancer and those with poor colon preparation or failed cecal intubation were excluded from the analysis. Associations of fellowship training level with the ADR and PDR relative to attendings alone were assessed using unconditional multivariable logistic regression. Models were adjusted for sex, age, race, and colon preparation quality. RESULTSA total of 7503 colonoscopies met the inclusion criteria and were included in the analysis. The mean age of the study patients was 58.2 years; 63.1% were women and 88.2% were African American. The ADR was higher in the fellow participation group overall compared to that in the attending group: 34.5% vs 30.7% (P = 0.001), and for third year fellows it was 35.4% vs 30.7% (aOR = 1.23, 95%CI: 1.09-1.39). The higher ADR in the fellow participation group was evident for both the right and left side of the colon. For the PDR the corresponding figures were 44.5% vs 40.1% (P = 0.0003) and 45.7% vs 40.1% (aOR = 1.25, 95%CI: 1.12-1.41). The ADR and PDR increased with increasing fellow training level (P for trend < 0.05). CONCLUSIONThere is a stepwise increase in ADR and PDR across the years of gastroenterology training. Fellow participation is associated with higher adenoma and polyp detection.展开更多

However, many research works used local-scale case studies that focused on a specific earthquake characteristic using knowledgedriven techniques, such as crustal deformation analysis. In this study, a data-driven-based analysis is used to detect anomalies using displacement rates and deformation pattern features extracted from daily global navigation satellite system(GNSS) data using a machine learning algorithm. The GNSS data with188 and 1181 continuously operating reference stations from Indonesia and Japan, respectively, are used to identify the anomaly of recent major earthquakes in the last two decades. Feature displacement rates and deformation patterns are processed in several window times with 2560 experiment scenarios to produce the best detection using tree-based algorithms. Tree-based algorithms with a single estimator(decision tree), ensemble bagging(bagging, random forest and Extra Trees), and ensemble boosting(AdaBoost, gradient boosting, LGBM, and XGB) are applied in the study. The experiment test using realtime scenario GNSSdailydatareveals high F1-scores and accuracy for anomaly detection using slope windowing 365 and 730 days of 91-day displacement rates and then 7-day deformation pattern features in tree-based algorithms. The results show the potential for medium-term anomaly detection using GNSS data without the need for multiple vulnerability assessments.展开更多

Given its increased cost, propofol should be used judiciously and without the implicit expectation of a higher quality screening exam.

文摘To solve the problem of poor detection and limited application range of current intrusion detection methods, this paper attempts to use deep learning neural network technology to study a new type of intrusion detection method.Hence,we proposed an intrusion detection algorithm based on convolutional neural network(CNN)and AdaBoost algorithm. This algorithm uses CNN to extract the characteristics of network traffic data, which is particularly suitable for the analysis of continuous and classified attack data. The AdaBoost algorithm is used to classify network attack data that improved the detection effect of unbalanced data classification. We adopt the UNSW-NB15 dataset to test of this algorithm in the PyCharm environment. The results show that the detection rate of algorithm is99.27%and the false positive rate is lower than 0.98%.Comparative analysis shows that this algorithm has advantages over existing methods in terms of detection rate and false positive rate for small proportion of attack data.
文摘To investigate whether adenoma and polyp detection rates (ADR and PDR, respectively) in screening colonoscopies performed in the presence of fellows differ from those performed by attending physicians alone. Fellow participation is associated with higher adenoma and polyp detection.

文摘Research on strain anomalies and large earthquakes based on temporal and spatial crustal activities has been rapidly growing due to data availability, especially in Japan and Indonesia. The results show the potential for medium-term anomaly detection using GNSS data without the need for multiple vulnerability assessments.

文摘Defect detection is vital in the nonwoven material industry, ensuring surface quality before producing finished products.Recently, deep learning and computer vision advancements have revolutionized defect detection, making it a widely adopted approach in various industrial fields. This paper mainly studied the defect detection method for nonwoven materials based on the improved Nano Det-Plus model.Using the constructed samples of defects in nonwoven materials as the research objects, transfer learning experiments were conducted based on the Nano DetPlus object detection framework.Within this framework, the Backbone, path aggregation feature pyramid network(PAFPN)and Head network models were compared and trained through a process of freezing, with the ultimate aim of bolstering the model's feature extraction abilities and elevating detection accuracy. The half-precision quantization method was used to optimize the model after transfer learning experiments, reducing model weights and computational complexity to improve the detection speed.Performance comparisons were conducted between the improved model and the original Nano Det-Plus model,YOLO,SSD and other common industrial defect detection algorithms, validating that the improved methods based on transfer learning and semi-precision quantization enabled the model to meet the practical requirements of industrial production.
文摘A network intrusion detection system is critical for cyber security against llegitimate attacks. In terms of feature perspectives, network traffic may include a variety of elements such as attack reference, attack type,a subcategory of attack, host information, malicious scripts, etc. In terms of network perspectives, network traffic may contain an imbalanced number of harmful attacks when compared to normal traffic. It is challenging to identify a specific attack due to complex features and data imbalance issues. To address these issues, this paper proposes an Intrusion Detection System using transformer-based transfer learning for Imbalanced Network Traffic(IDS-INT).IDS-INT uses transformer-based transfer learning to learn feature interactions in both network feature representation and imbalanced data.First, detailed information about each type of attack is gathered from network interaction descriptions, which include network nodes, attack type, reference, host information, etc.Second, the transformer-based transfer learning approach is developed to learn detailed feature representation using their semantic anchors.Third, the Synthetic Minority Oversampling Technique(SMOTE)is implemented to balance abnormal traffic and detect minority attacks.Fourth, the Convolution Neural Network(CNN)model is designed to extract deep features from the balanced network traffic. Finally, the hybrid approach of the CNN-Long Short-Term Memory(CNN-LSTM)model is developed to detect different types of attacks from the deep features.Detailed experiments are conducted to test the proposed approach using three standard datasets,i.e.,UNsWNB15,CIC-IDS2017,and NSL-KDD.An explainable AI approach is implemented to interpret the proposed method and develop a trustable model.

文摘Interactive holography offers unmatched levels of immersion and user engagement in the field of future display.Despite of the substantial progress has been made in dynamic meta-holography,the realization of real-time,highly smooth interactive holography remains a significant challenge due to the computational and display frame rate limitations. In this study,we introduced a dynamic interactive bitwise meta-holography with ultra-high computational and display frame rates. To our knowledge, this is the first reported practical dynamic interactive metasurface holographic system. We spa-tially divided the metasurface device into multiple distinct channels, each projecting a reconstructed sub-pattern.The switching states of these channels were mapped to bitwise operations on a set of bit values, which avoids complex holo-gram computations, enabling an ultra-high computational frame rate. Our approach achieves a computational frame rate of 800 kHz and a display frame rate of 23 kHz on a low-power Raspberry Pi computational platform.According to this methodology,we demonstrated an interactive dynamic holographic Tetris game system that allows interactive gameplay, color display, and on-the-fly hologram creation. Our technology presents an inspiration for advanced dynamic meta-holography,which is promising for a broad range of applications including advanced human-computer interaction,real-time 3D visualization, and next-generation virtual and augmented reality systems.

文摘In this paper,we study the one-dimensional motion of viscous gas near a vacuum, with the gas connecting to a vacuum state with a jump in density. The interface behavior, the pointwise decay rates of the density function and the expanding rates of the interface are obtained with the viscosity coefficientμ(ρ)=ρ^(α)for any 0<α<1;this includes the timeweighted boundedness from below and above. The smoothness of the solution is discussed. Moreover,we construct a class of self-similar classical solutions which exhibit some interesting properties, such as optimal estimates. The present paper extends the results in[Luo T,Xin Z P,Yang T.SIAM J Math Anal,2000,31(6):1175-1191]to the jump boundary conditions case with density-dependent viscosity.

文摘The widespread adoption of blockchain technology has led to the exploration of its numerous applications in various fields.Cryptographic algorithms and smart contracts are critical components of blockchain security.Despite the benefits of virtual currency, vulnerabilities in smart contracts have resulted in substantial losses to users. While researchers have identified these vulnerabilities and developed tools for detecting them, the accuracy of these tools is still far from satisfactory, with high false positive and false negative rates. In this paper,we propose a new method for detecting vulnerabilities in smart contracts using the BERT pre-training model, which can quickly and effectively process and detect smart contracts.More specifically,we preprocess and make symbol substitution in the contract, which can make the pre-training model better obtain contract features. We evaluate our method on four datasets and compare its performance with other deep learning models and vulnerability detection tools, demonstrating its superior accuracy.

文摘Esophageal cancer ranks among the most prevalent malignant tumors globally, primarily due to its highly aggressive nature and poor survival rates.According to the 2020 global cancer statistics, there were approximately 604000 new cases of esophageal cancer, resulting in 544000 deaths. The 5-year survival rate hovers around a mere 15%-25%.Notably,distinct variations exist in the risk factors associated with the two primary histological types, influencing their worldwide incidence and distribution.Squamous cell carcinoma displays a high incidence in specific regions, such as certain areas in China, where it meets the cost-effect-iveness criteria for widespread endoscopy-based early diagnosis within the local population.Conversely, adenocarcinoma(EAC)represents the most common histological subtype of esophageal cancer in Europe and the United States. The role of early diagnosis in cases of EAC originating from Barrett's esophagus(BE)remains a subject of controversy. The effectiveness of early detection for EAC,particularly those arising from BE,continues to be a debated topic. The variations in how early-stage esophageal carcinoma is treated in different regions are largely due to the differing rates of early-stage cancer diagnoses. In areas with higher incidences, such as China and Japan, early diagnosis is more common, which has led to the advancement of endoscopic methods as definitive treatments. These techniques have demonstrated remarkable efficacy with minimal complications while preserving esophageal functionality.Early screening, prompt diagnosis, and timely treatment are key strategies that can significantly lower both the occurrence and death rates associated with esophageal cancer.

文摘BACKGROUND Improved adenoma detection rate(ADR)has been demonstrated with artificial intelligence(AI)-assisted colonoscopy. However, data on the real-world appli-cation of AI and its effect on colorectal cancer(CRC)screening outcomes is limited.AIM To analyze the long-term impact of AI on a diverse at-risk patient population undergoing diagnostic colonoscopy for positive CRC screening tests or sympt-oms.METHODS AI software(GI Genius,Medtronic)was implemented into the standard proced-ure protocol in November 2022.Data was collected on patient demographics, procedure indication, polyp size, location, and pathology.CRC screening outcomes were evaluated before and at different intervals after AI introduction with one year of follow-up.RESULTS We evaluated 1008 colonoscopies(278 pre-AI,255 early post-AI,285 established post-AI,and 190 late post-AI).The ADR was 38.1%pre-AI,42.0%early post-AI(P=0.77),40.0%established post-AI(P=0.44),and 39.5%late post-AI(P=0.77).There were no significant differences in polyp detection rate(PDR,baseline 59.7%),advanced ADR(baseline 16.2%),and non-neoplastic PDR(baseline 30.0%)before and after AI introduction.CONCLUSION In patients with an increased pre-test probability of having an abnormal colonoscopy, the current generation of AI did not yield enhanced CRC screening metrics over high-quality colonoscopy.Although the potential of AI in colonoscopy is undisputed, current AI technology may not universally elevate screening metrics across all situations and patient populations.Future studies that analyze different AI systems across various patient populations are needed to determine the most effective role of AI in optimizing CRC screening in clinical practice.
文摘A large number of network security breaches in IoT networks have demonstrated the unreliability of current Network Intrusion Detection Systems(NIDSs).Consequently, network interruptions and loss of sensitive data have occurred, which led to an active research area for improving NIDS technologies. In an analysis of related works,it was observed that most researchers aim to obtain better classification results by using a set of untried combinations of Feature Reduction(FR)and Machine Learning(ML)techniques on NIDS datasets. However, these datasets are different in feature sets, attack types, and network design. Therefore, this paper aims to discover whether these techniques can be generalised across various datasets.Six ML models are utilised:a Deep Feed Forward(DFF),Convolutional Neural Network(CNN),Recurrent Neural Network(RNN),Decision Tree(DT),Logistic Regression(LR),and Naive Bayes(NB).The accuracy of three Feature Extraction(FE)algorithms is detected;Principal Component Analysis(PCA),Auto-encoder(AE),and Linear Discriminant Analysis(LDA),are evaluated using three benchmark datasets:UNSW-NB15,ToN-IoT and CSE-CIC-IDS2018.Although PCA and AE algorithms have been widely used, the determination of their optimal number of extracted dimensions has been overlooked. The results indicate that no clear FE method or ML model can achieve the best scores for all datasets. The optimal number of extracted dimensions has been identified for each dataset, and LDA degrades the performance of the ML models on two datasets. The variance is used to analyse the extracted dimensions of LDA and PCA.Finally, this paper concludes that the choice of datasets significantly alters the performance of the applied techniques. We believe that a universal(benchmark)feature set is needed to facilitate further advancement and progress of research in this field.

文摘As computer technology continues to advance, factories have increasingly higher demands for detecting defects. However, detecting defects in a plant environment remains a challenging task due to the presence of complex backgrounds and defects of varying shapes and sizes. To address this issue, this paper proposes YOLO-DD,a defect detectionmodel based on YOLOv5 that is effective and robust. To improve the feature extraction process and better capture global information, the vanilla YOLOv5 is augmented with a new module called Relative-Distance-Aware Transformer(RDAT).Additionally,an Information Gap Filling Strategy(IGFS)is proposed to improve the fusion of features at different scales. The classic lightweight attention mechanism Squeeze-and-Excitation(SE)module is also incorporated into the neck section to enhance feature expression and improve the model’s performance.Experimental results on the NEU-DET dataset demonstrate that YOLO-DDachieves competitive results compared to state-of-the-art methods, with a 2.0% increase in accuracy compared to the original YOLOv5,achieving 82.41% accuracy and38.25FPS(framesper second).Themodel is also testedon a self-constructed fabric defect dataset, and the results show that YOLO-DD is more stable and has higher accuracy than the original YOLOv5,demonstrating its stability and generalization ability. The high efficiency of YOLO-DD enables it to meet the requirements of industrial high accuracy and real-time detection.

文摘As the risks associated with air turbulence are intensified by climate change and the growth of the aviation industry,it has become imperative to monitor and mitigate these threats to ensure civil aviation safety. The eddy dissipation rate(EDR)has been established as the standard metric for quantifying turbulence in civil aviation. This study aims to explore a universally applicable symbolic classification approach based on genetic programming to detect turbulence anomalies using quick access recorder(QAR)data. The detection of atmospheric turbulence is approached as an anomaly detection problem.Comparative evaluations demonstrate that this approach performs on par with direct EDR calculation methods in identifying turbulence events. Moreover, comparisons with alternative machine learning techniques indicate that the proposed technique is the optimal methodology currently available. In summary, the use of symbolic classification via genetic programming enables accurate turbulence detection from QAR data, comparable to that with established EDR approaches and surpassing that achieved with machine learning algorithms. This finding highlights the potential of integrating symbolic classifiers into turbulence monitoring systems to enhance civil aviation safety amidst rising environmental and operational hazards.

文摘Transformer-based models have facilitated significant advances in object detection. However, their extensive computational consumption and suboptimal detection of dense small objects curtail their applicability in unmanned aerial vehicle(UAV)imagery.Addressing these limitations,we propose a hybrid transformer-based detector,H-DETR,and enhance it for dense small objects, leading to an accurate and efficient model.Firstly,we introduce a hybrid transformer encoder, which integrates a convolutional neural network-based cross-scale fusion module with the original encoder to handle multi-scale feature sequences more efficiently.Furthermore,we propose two novel strategies to enhance detection performance without incurring additional inference computation.Query filter is designed to cope with the dense clustering inherent in drone-captured images by counteracting similar queries with a training-aware non-maximum suppression.Adversarial denoising learning is a novel enhancement method inspired by adversarial learning, which improves the detection of numerous small targets by counteracting the effects of artificial spatial and semantic noise.Extensive experiments on the VisDrone and UAVDT datasets substantiate the effectiveness of our approach, achieving a significant improvement in accuracy with a reduction in computational complexity. Our method achieves 31.9%and 21.1%AP on the VisDrone and UAVDT datasets, respectively, and has a faster inference speed, making it a competitive model in UAV image object detection.

文摘Time-series data provide important information in many fields, and their processing and analysis have been the focus of much research. However, detecting anomalies is very difficult due to data imbalance, temporal dependence, and noise. Therefore, methodologies for data augmentation and conversion of time series data into images for analysis have been studied. This paper proposes a fault detection model that uses time series data augmentation and transformation to address the problems of data imbalance, temporal dependence, and robustness to noise. The method of data augmentation is set as the addition of noise. It involves adding Gaussian noise, with the noise level set to 0.002,to maximize the generalization performance of the model. In addition,we use the Markov Transition Field(MTF)method to effectively visualize the dynamic transitions of the data while converting the time series data into images. It enables the identification of patterns in time series data and assists in capturing the sequential dependencies of the data. For anomaly detection, the PatchCore model is applied to show excellent performance, and the detected anomaly areas are represented as heat maps.It allows for the detection of anomalies, and by applying an anomaly map to the original image,it is possible to capture the areas where anomalies occur. The performance evaluation shows that both F1-score and Accuracy are high when time series data is converted to images.Additionally, when processed as images rather than as time series data, there was a significant reduction in both the size of the data and the training time. The proposed method can provide an important springboard for research in the field of anomaly detection using time series data.Besides,it helps solve problems such as analyzing complex patterns in data lightweight.
