BACKGROUND Patients with diabetes mellitus are at higher risk of myocardial ischemia/reperfusion injury(MI/RI).Shuxin decoction(SXT)is a proven recipe modification from the classic herbal formula"Wu-tou-chi-shi-z...BACKGROUND Patients with diabetes mellitus are at higher risk of myocardial ischemia/reperfusion injury(MI/RI).Shuxin decoction(SXT)is a proven recipe modification from the classic herbal formula"Wu-tou-chi-shi-zhi-wan"according to the traditional Chinese medicine theory.It has been successfully used to alleviate secondary MI/RI in patients with diabetes mellitus in the clinical setting.However,the underlying mechanism is still unclear.AIM To further determine the mechanism of SXT in attenuating MI/RI associated with diabetes.METHODS This paper presents an ensemble model combining network pharmacology and biology.The Traditional Chinese Medicine System Pharmacology Database was accessed to select key components and potential targets of the SXT.In parallel,therapeutic targets associated with MI/RI in patients with diabetes were screened from various databases including Gene Expression Omnibus,DisGeNet,Genecards,Drugbank,OMIM,and PharmGKB.The potential targets of SXT and the therapeutic targets related to MI/RI in patients with diabetes were intersected and subjected to bioinformatics analysis using the Database for Annotation,Visualization and Integrated Discovery.The major results of bioinformatics analysis were subsequently validated by animal experiments.RESULTS According to the hypothesis derived from bioinformatics analysis,SXT could possibly ameliorate lipid metabolism disorders and exert anti-apoptotic effects in MI/RI associated with diabetes by reducing oxidized low density lipoprotein(LDL)and inhibiting the advanced glycation end products(AGE)-receptor for AGE(RAGE)signaling pathway.Subsequent animal experiments confirmed the hypothesis.The treatment with a dose of SXT(2.8 g/kg/d)resulted in a reduction in oxidized LDL,AGEs,and RAGE,and regulated the level of blood lipids.Besides,the expression of apoptosis-related proteins such as Bax and cleaved caspase 3 was down-regulated,whereas Bcl-2 expression was up-regulated.The findings indicated that SXT could inhibit myocardial apoptosis and improve cardiac function in MI/RI in diabetic rats.CONCLUSION This study indicated the active components and underlying molecular therapeutic mechanisms of SXT in MI/RI with diabetes.Moreover,animal experiments verified that SXT could regulate the level of blood lipids,alleviate cardiomyocyte apoptosis,and improve cardiac function through the AGE-RAGE signaling pathway.展开更多

OBJECTIVE:To assess the effect of activating blood drugs(AB drugs) on blood glucose and lipid levels of diabetic patients.METHODS:Systematic searches were conducted on 6 electronic databases and the search time was cu...OBJECTIVE:To assess the effect of activating blood drugs(AB drugs) on blood glucose and lipid levels of diabetic patients.METHODS:Systematic searches were conducted on 6 electronic databases and the search time was cut off in May 2016.Randomized controlled trials(RCTs) were included if they investigated diabetic patients who received Western medicine combined with AB drugs in the experimental groups and solitary usage of corresponding Western medicine or placebo in the control groups.6 outcome measures,which were considered as inclusion,were changes in fasting blood glucose(FBG),glycosylated haemoglobin A1 c(HbA1c),total triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-c) and high density lipoprotein cholesterol(HDL-c).The literature quality was assessed by Cochrane score for risk of bias in Review Manager 5.2,as well as meta-analysis was performed.RESULTS:7 RCTs comprising 999 patients met all inclusion criteria.Meta-analysis showed beneficial effects of Westernmedicine combined with AB drugs on modulating FBG,TC,LDL-c and HDL-c compared with Western medicine or placebo(P<0.05),while no difference was observed in HbA1c,and TG(P>0.05).To some extent,the therapeutic effects of Western medicine combined with AB drugs were better than that of solitary usage of Western medicine or placebo.Besides,AB drugs were safe and reliable with little toxic and side-effects.CONCLUSIONS:The meta-analysis suggests beneficial effects of Western medicine combined with AB drugs on modulating blood glucose and lipids in diabetic patients.展开更多
Background The effectiveness and safety of initiating biphasic insulin aspart 30 in patients who were poorly controlled on oral glucose-lowering drugs were studied in randomized controlled trials,while results from cl...Background The effectiveness and safety of initiating biphasic insulin aspart 30 in patients who were poorly controlled on oral glucose-lowering drugs were studied in randomized controlled trials,while results from clinical practice remain limited.This subgroup analysis was to provide such findings from a large-scale non-interventional study.Methods A1chieve was a multinational,prospective,open-label,non-interventional,24-week study in patients with type 2 diabetes initiating insulin analogues in 28 countries across Asia,Africa,Europe,and Latin America.After physician had taken the decision to use this insulin,any patient with type 2 diabetes who was not treated with or who had started the study insulin within 4 weeks before inclusion was eligible.Patients were treated with study insulin alone or in combination with oral glucose-lowering drugs.Data on adverse drug reactions,hypoglycemia and glycemic control were collected at baseline,week 12 and 24.This is a report of a Chinese subgroup analysis from the A1chieve study.Results Totally,4 100 patients constituted this subgroup.No serious adverse drug reactions were reported.Rates of total,major,nocturnal hypoglycemic events (events/patient per year) were 1.47,0.10,0.31 at baseline and 1.35,0.00,0.22 at week 24,respectively.Glycemic control was improved as measured by hemoglobin A1c (mean 9.3% to 7.0%,reduction -2.3%),fasting plasma glucose (mean 10.2 to 6.8 mmol/L,reduction-3.5 mmol/L) and postprandial plasma glucose (mean 14.4 to 8.8 mmol/L,reduction-5.6 mmol/L),all P <0.001.Change in mean body weight was +0.3 kg (P <0.001).Conclusion In this subgroup analysis of the A1chieve study,biphasic insulin aspart 30 improved glycemic control with low risk of hypoglycemia.展开更多

It is the view of the authors that accumulation of heat and dampness in the heart and stomach and deficiency of both spleen and kidney with blood stasis would be the main predisposing factors in pathogenesis of incipient diabetic nephropathy. A controlled clinical study of 32 patients treatment with modified recipe of Wang Kentang’s Bai Fu Ling Wan, a proved formula of clearing away the stomach-heat, purging the heart fire, strengthening the stomach and tonifying the kidney, and simultaneously resolving blood stasis and turbid qi was performed. The total effective rate was 93.8%, superior to that in the control group treated with Capoton (59.9%). A marked improvement in symptoms, control of blood glucose level and reduction of urinary protein obtained in the treatment group suggests that this treatment is effective in retarding the development of the disease.展开更多
In order to observe the therapeutic effects of Huatan Yigan Decoction (化痰益肝汤 Decoction for resolving phlegm and replenishing the liver) on antidiabetic-induced hepatopathy, 129 cases were divided into two groups,...In order to observe the therapeutic effects of Huatan Yigan Decoction (化痰益肝汤 Decoction for resolving phlegm and replenishing the liver) on antidiabetic-induced hepatopathy, 129 cases were divided into two groups, 72 cases in the treatment group were administrated with Huatan Yigan Decoction and 57 cases in the control group with the liver-protecting drugs. Their liver function, plasma prothrombin time (PT), blood urea nitrogen and symptoms of the patients before and after treatment were observed during the three therapeutic courses. Results: The total effective rate and cured rate of the treatment group manifested an obvious difference in comparison with those of the control group (P<0.05 or P<0.01). Even though the liver function, blood urea nitrogen (BUN) and symptoms in both groups were improved markedly, these indicators in the treatment group were improved more markedly than those in control group (P<0.01). No adverse effect was found during the treatment. Conclusion: Huatan Yigan Decoction shows a repair action on hepatic injury.展开更多
