It can cause a terrible pain. The author has treated 66 cases by massotherapy, who were confirmatively diagnosed as having protrusion of the lumbar intervertebral disc by X-ray film and computer-aided tomography, with satisfactory results as reported in the following.Clinical DataAmong the 66 cases in this series, 49 were male and 17 female, ranging in age from 26 to 59 years, averaging 36.2 years. All the patients had got lumbago and unilateral ischialgia, with the left side affected in 37 cases and the right side in 29 cases. The leg pain radiating to lateral malleolus was found in 28 cases, to dorsum of the foot in 24 cases, and to the toes in 14 cases. Intermittent claudication was present in 22 cases, numbness of the affected foot in 16 cases, pain exacerbated when coughing in 35 cases, scoliosis in 21 cases, and positive Lasegue's sign in 48 cases.展开更多

Objective:To investigate the inhibition effect of siRNA interference on NGF induced by inflammatory factor IL-6,and JUL—1 so as to provide novel targets for clinical treatment of discogenic low back pain.Methods:The ...Objective:To investigate the inhibition effect of siRNA interference on NGF induced by inflammatory factor IL-6,and JUL—1 so as to provide novel targets for clinical treatment of discogenic low back pain.Methods:The intervertebral disc nucleus and annulus fibrosus cells of rats were separated-The cells were co-cultured with different concentrations(10 nmol/L,20nmol/L,50 nmol/L,100 nmol/L)of IL-6 and IL-1β.The NGF-siRNA was leaded into the cocultured cells with its import ability assessed by flow cytometry instrument tests,hefore and after which the NCF mRNA expression was detected by real-time Q-PCR and the NGF content was detected by ELISA.Results:Flow cytometry instrument test results showed that the NGFsiRNA cell conversion rate was 99.8%.Real-time Q-PCR detection results showed that compared with negative control group,the NGF mRNA expression of co-cultured cells treated by 10 nmol/L,20 nmol/L,50 nmol/L,100 nmol/L IL-6 and IL-1βwere respectively raised 3.4,3.7,4.7,3.7 times which were all significantly down-regulated after the import of NGF-siRNA.EILSA detection results showed that compared with negative control group,the NGF content of cocultured medium treated by 10 nmol/L,20 nmol/L,50 nmol/L,100 nmol/L I-L6 and IL-1βwere respectively raised 2.9,3.3,4.5,7.4 times which were all significantly decreased after the import of NGF-siRNA.Conclusions:These molecular biological results suggest that inflammatory factor IL-6 and IL-1βcould stimulate NCF on intervertebral disc cells in vitro culture model and its efficiency is concentration dependent,while siRNA interference can inhibit the stimulation effect of IL-6 and IL-1βon intervertebral disc cell,which provides a new targets for the clinical treatment of discogenic low back pain.展开更多

Three hundred cases...Objective: To observe the clinical efficacy of electroacupuncture and waist-building exercise in treating lumbar disk herniation. Methods: A multi-center, randomized controlled trial was adopted. Three hundred cases of lumbar disk herniations were divided into two groups, an observation group in which 149 cases were treated by electroacupuncture and waist-building exercise, and a control group in which 151 cases were treated by electroacupuncture alone. The clinical efficacy and long-term relapse rate were observed and compared between the two groups. Results: Among 149 cases in the observation group, 80 cases were cured, 57 cases improved, 12 cases failed and 4 cases relapsed; the cure rate was 53.7%, the total effective rate was 91.9% and the relapse rate was 5.0%. Among 151 cases in the control group, 74 cases were cured, 51 cases improved, 26 cases failed and 12 cases relapsed; the cure rate was 49.0%, the total effective rate was 82.8% and the relapse rate was 16.2%. There were significant differences in the cure rate and the total effective rate between the two groups (P〈0.05). Conclusion: Combined electroacupuncture and waist-building exercise had better effects than electroacupuncture in treating lumbar disk herniation.展开更多

Methods: A comprehensive analysis was made by a method of metrological analysis according to the data from "China modem acupuncture-moxibustion information databank". Results: Points Huantiao (GB 30), Weizhong (BL 40), Yanglingquan (GB 34), and Jiaji (Ex-B 2) were most frequently used. Acupuncture was the most frequently used therapy and electroacupuncture, the next one. Conclusion: Electroacupuncture is a common and effective way to treat lumbar intervertebral disk herniation. Points Huantiao (GB 30), Weizhong (BL 40), Yanglingquan (GB 34), and Jiaji (Ex-B 2) can be used as a basic prescription for acupuncture-moxibustion treatment of lumbar intervertebral disk herniation. The therapeutic results were evaluated at the end of the third treatment session. Results: The total effective rate of the treatment group was 95.5%, and that of the control group was 57.8%, and the difference was statistically significant (P〈0.01). Methods: Seventy-five Sprauge-Dawley rats were used to establish the models of lumbar intervertebral disk displacement by operation, and divided into five groups including electro-acupuncture group, western medicine group, acupuncture plus medicine group, model group and normal control group, then they were treated with corresponding therapeutic method. The expression of inducible nitric oxide synthase (iNOS) mRNA in the lumbar intervertebral disk tissues of rats was detected by RTPCR. Results: The expression of iNOS mRNA in the lumbar intervertebral disk tissues of rats in model group was compared with that in normal control group, the significant difference was found (P〈 0.01); While the expressions of iNOS mRNA in the lumbar intervertebral disk tissues of rats in electroacupuncture group, western drug group and acupuncture plus medicine group were all inhibited, and compared with the model group, the significant differences were also found (P 〈 0.01). Conclusion: Electro-acupuncture could inhibit the expression of iNOS mRNA in the lumbar intervertebral disk tissue.展开更多

OBJECTIVE:To assess the efficacy and safety of heat-sensitive moxibustion in the treatment of lumbar disc herniation(LDH).METHODS:Randomized controlled trials(RCTs) involving heat-sensitive moxibustion in the treatmen...OBJECTIVE:To assess the efficacy and safety of heat-sensitive moxibustion in the treatment of lumbar disc herniation(LDH).METHODS:Randomized controlled trials(RCTs) involving heat-sensitive moxibustion in the treatment of LDH were retrieved from the Chinese Biological Medical Literature database(1978-20011),Weipu database(1989-2011),Wanfang digital journal(1998-2011),China National Knowledge Internet(1979-2011),PubMed(1966-2011),EMBASE(1980-2011),and Cochrane Library(Issue 1,2011).Hand-search of the relevant journals from the Library of Jiangxi University of Traditional Chinese Medicine was also adopted for the collection of data.Data were extracted and evaluated by two reviewers independently with a specially designed extraction form.The Cochrane Collaboration's RevMan 5.0.20 software was used for data analyses.RESULTS:A total of 6 trials involving 580 patients were included.Meta-analysis showed that the total effectiveness rate in the heat-sensitive moxibustion group was significantly different when compared with conventional moxibustion [RR=1.19,95% CI [1.06,1.33)] and diclofenac sodium [RR=1.47,95% CI [1.17,1.85)],but similar to that of acupuncture.The cure rate in the heat-sensitive moxibustion group was significantly different when compared with conventional moxibustion [RR=1.58,95% CI(1.04,2.40)] and diclofenac sodium [RR=1.91,95% CI(1.01,3.60)],but similar with that of acupuncture.In terms of the Japanese Orthopaedic Association scores,significant differences were noted in subjective indices,objective indices,and daily life subscales.Two trials reported that there were no adverse events over the duration of treatment.CONCLUSION:Compared with conventional moxibustion,acupuncture,and diclofenac sodium,heat-sensitive moxibustion in the treatment of LDH is superior in efficacy.Further large-scale trials are required to define the role of heat-sensitive moxibustion in the treatment of this disease.展开更多

Methods: From January 2000 to Octobe...Objective: To investigate the surgical procedures, options and surgical indications for far-lateral lumbar disc herniation between three different minimally invasive procedures. Methods: From January 2000 to October 2006, 52 patients with far-lateral lumbar disc herniation (29 males and 23 females, with the average age of 41.5 years) were treated with minimally invasive procedures. All the patients were assessed by X-ray and CT. Some were given additional myeography, discography, Computerized tomography myelography (CTM) and MRI examination. Yeung Endoscopy Spine System (YESS), METRx and X-tube procedures were performed in 25, 13 and 14 cases, respectively. All patients were followed up for a mean period of 13.5 months. Clinical outcomes were assessed by visual analog score (VAS) and Nakai criteria. Results: The results indicated that the three procedures could significantly improve the radiating leg symptoms (P〈0.05). The postoperative overall excellent and goodrates of YESS, METRx and X-tube procedures were 84.0%, 84.6% and 92.8% respectively, with no statistical difference among three groups (P〉0.05). The YESS procedure had several advantages including shortest operation time, simplest anesthesia and least trauma as compared with the other two procedures, especially for simple type I far-lateral lum- bar disc herniation. METRx procedure was specially suitable for simple type II. And the procedure of posterior endoscopic facetectomy, posterior lumbar interbody fusion and unilateral pedicle screw instrumentation with X-tube was designed for far-lateral disc herniation combined with degenerative lumbar instability. Conclusion: Minimally invasive strategies and options should be determined by different types of far-lateral lumbar disc herniation.展开更多
OBJECTIVE:To evaluate the clinical effect of traditional Chinese spinal orthopedic manipulation(TCSOM) in treating irritable bowel syndrome(IBS) compared with pinaverium bromide Dicetel(PBD),and to assess a possible c...OBJECTIVE:To evaluate the clinical effect of traditional Chinese spinal orthopedic manipulation(TCSOM) in treating irritable bowel syndrome(IBS) compared with pinaverium bromide Dicetel(PBD),and to assess a possible cause for IBS.METHODS:Two hundred ten IBS patients were randomly divided into the TCSOM group and the PBD group.All patients in the TCSOM group were treated with spinal manipulations 5 times.Pain intensity was assessed on a visual analogue scale and symptoms were evaluated based on bowel symptom scale(BSS) scores before and after treatment.A symptom improvement rating(SIR) was implemented to evaluate the effects of treatment,and to statistically compare theTCSOM and PBD groups.RESULTS:Post-treatment subjective assessment for the TCSOM treatment group showed that 92 cases had excellent results,10 cases had good results,3 cases had fair results,and 0 cases had poor results.In the PBD group,30 cases had excellent results,41 cases had good results,12 cases had fair results,and 22 cases had poor results.According to the SIR,which was based on the BSS,the TCSOM group included 92 cases with excellent results,10 cases with good results and 3 cases with poor results.There were no adverse side effects from treatment.Based on the BSS,the PBD group had 30 cases with excellent results,41 cases with good results,and 34 cases with poor results.We found that the symptom rating in the BSS in the TCSOM group showed a more significant improvement,such as a reduction in the severity and frequency of symptoms,compared with that in the PBD group(P<0.001).CONCLUSION:Displacement of intervertebral discs and/or vertebra in the thoracic or lumbar region appears to be a contributing factor in the symptoms of IBS.Correcting this displacement of intervertebral discs and/or vertebra to resolve the stimuli caused by pressure exerted on the nerves and vessels around the spine is an effective treatment for IBS.展开更多

文摘Objective: To review year 1956-2007 literature on acupuncture and moxibustion and explore the regularity of point selection and methods for acupuncture-moxibustion treatment of lumbar intervertebral disk herniation. Methods: A comprehensive analysis was made by a method of metrological analysis according to the data from "China modem acupuncture-moxibustion information databank". Results: Points Huantiao (GB 30), Weizhong (BL 40), Yanglingquan (GB 34), and Jiaji (Ex-B 2) were most frequently used. Acupuncture was the most frequently used therapy and electroacupuncture, the next one. Conclusion: Electroacupuncture is a common and effective way to treat lumbar intervertebral disk herniation. Points Huantiao (GB 30), Weizhong (BL 40), Yanglingquan (GB 34), and Jiaji (Ex-B 2) can be used as a basic prescription for acupuncture-moxibustion treatment of lumbar intervertebral disk herniation. Other acupoints are selected according to the syndrome.
文摘Objective: To observe the therapeutic effect of the treatment of lumbar disk herniation with hydro-acupuncture in combination with tuina. Methods: 380 patients were randomized into two groups, 290 in the treatment group receiving hydro-acupuncture together with tuina therapy, and 60 in the control group receiving acupuncture therapy plus pharmacotherapy. The therapeutic results were evaluated at the end of the third treatment session. Results: The total effective rate of the treatment group was 95.5%, and that of the control group was 57.8%, and the difference was statistically significant (P〈0.01). Conclusion: Lumbar disk herniation responds well to the treatment of combined hydro-acupuncture and tuina therapy.
文摘Lumbar intervertebral disk herniation is a commonly and frequently encountered disease in the orthopedic department, manifested by a series of symptoms such as low back pain, numbness and pain in one or two lower limbs, due to compression to the lumbosacral nerves by protrusion of the pulpiform nucleus after rupture of the fibrous rings. The author has treated 52 cases of this disease by acupuncture plus herbal medicine. Now, the report is given as follows.
文摘Objective: To make objective evaluation on treatment of lumbar intervertebral disc herniation with electric acupuncture. Method: All 200 cases were randomly allocated into an observation group (100 cases) and a control group (100 cases). Cases in the observation group were treated with electric acupuncture. Cases in the control group were treated with oral administration of Mobic. Result: The total effective rate in the observation group was 86.5%, and the total effective rate of the control group was 75.0%. There was a significant difference between the two groups (P〈0.01). Conclusion: Electric acupuncture is a safe and remarkably effective way for lumbar intervertebral disc herniation.
文摘Objective: The present study is to investigate the mechanism of dectrtyacupuncture in preventing and treating lumbar intervertebral disk displacement. Methods: Seventy-five Sprauge-Dawley rats were used to establish the models of lumbar intervertebral disk displacement by operation, and divided into five groups including electro-acupuncture group, western medicine group, acupuncture plus medicine group, model group and normal control group, then they were treated with corresponding therapeutic method. The expression of inducible nitric oxide synthase (iNOS) mRNA in the lumbar intervertebral disk tissues of rats was detected by RTPCR. Results: The expression of iNOS mRNA in the lumbar intervertebral disk tissues of rats in model group was compared with that in normal control group, the significant difference was found (P〈 0.01); While the expressions of iNOS mRNA in the lumbar intervertebral disk tissues of rats in electroacupuncture group, western drug group and acupuncture plus medicine group were all inhibited, and compared with the model group, the significant differences were also found (P 〈 0.01). Conclusion: Electro-acupuncture could inhibit the expression of iNOS mRNA in the lumbar intervertebral disk tissue.
文摘Objective: To seek out an effective treatment method for cervical disc extrusion and a significant laboratory examination.Methods: Randomized controlled trial was established into two groups: control group in which 50 subjects were given simple cervical traction and treatment group in which another 50 cases were given electro-acupuncture plus cervical traction. Results. The total effective rate was 92% in treatment group, obviously higher than 80% in control group; electromyogram examination showed decreased spontaneous potential. Conclusion: Electroacupuncture plus cervical traction is an effective therapy for cervical disc extrusion and electromyogram can help clinical practice.

文摘OBJECTIVE:To assess the efficacy and safety of heat-sensitive moxibustion in the treatment of lumbar disc herniation(LDH).METHODS:Randomized controlled trials(RCTs) involving heat-sensitive moxibustion in the treatment of LDH were retrieved from the Chinese Biological Medical Literature database(1978-20011),Weipu database(1989-2011),Wanfang digital journal(1998-2011),China National Knowledge Internet(1979-2011),PubMed(1966-2011),EMBASE(1980-2011),and Cochrane Library(Issue 1,2011).Hand-search of the relevant journals from the Library of Jiangxi University of Traditional Chinese Medicine was also adopted for the collection of data.Data were extracted and evaluated by two reviewers independently with a specially designed extraction form.The Cochrane Collaboration's RevMan 5.0.20 software was used for data analyses.RESULTS:A total of 6 trials involving 580 patients were included.Meta-analysis showed that the total effectiveness rate in the heat-sensitive moxibustion group was significantly different when compared with conventional moxibustion [RR=1.19,95% CI [1.06,1.33)] and diclofenac sodium [RR=1.47,95% CI [1.17,1.85)],but similar to that of acupuncture.The cure rate in the heat-sensitive moxibustion group was significantly different when compared with conventional moxibustion [RR=1.58,95% CI(1.04,2.40)] and diclofenac sodium [RR=1.91,95% CI(1.01,3.60)],but similar with that of acupuncture.In terms of the Japanese Orthopaedic Association scores,significant differences were noted in subjective indices,objective indices,and daily life subscales.Two trials reported that there were no adverse events over the duration of treatment.CONCLUSION:Compared with conventional moxibustion,acupuncture,and diclofenac sodium,heat-sensitive moxibustion in the treatment of LDH is superior in efficacy.Further large-scale trials are required to define the role of heat-sensitive moxibustion in the treatment of this disease.
文摘Objective: To investigate the surgical procedures, options and surgical indications for far-lateral lumbar disc herniation between three different minimally invasive procedures. Methods: From January 2000 to October 2006, 52 patients with far-lateral lumbar disc herniation (29 males and 23 females, with the average age of 41.5 years) were treated with minimally invasive procedures. All the patients were assessed by X-ray and CT. Some were given additional myeography, discography, Computerized tomography myelography (CTM) and MRI examination. Yeung Endoscopy Spine System (YESS), METRx and X-tube procedures were performed in 25, 13 and 14 cases, respectively. All patients were followed up for a mean period of 13.5 months. Clinical outcomes were assessed by visual analog score (VAS) and Nakai criteria. Results: The results indicated that the three procedures could significantly improve the radiating leg symptoms (P〈0.05). The postoperative overall excellent and goodrates of YESS, METRx and X-tube procedures were 84.0%, 84.6% and 92.8% respectively, with no statistical difference among three groups (P〉0.05). The YESS procedure had several advantages including shortest operation time, simplest anesthesia and least trauma as compared with the other two procedures, especially for simple type I far-lateral lum- bar disc herniation. METRx procedure was specially suitable for simple type II. And the procedure of posterior endoscopic facetectomy, posterior lumbar interbody fusion and unilateral pedicle screw instrumentation with X-tube was designed for far-lateral disc herniation combined with degenerative lumbar instability. Conclusion: Minimally invasive strategies and options should be determined by different types of far-lateral lumbar disc herniation.

文摘OBJECTIVE:To evaluate the clinical effect of traditional Chinese spinal orthopedic manipulation(TCSOM) in treating irritable bowel syndrome(IBS) compared with pinaverium bromide Dicetel(PBD),and to assess a possible cause for IBS.METHODS:Two hundred ten IBS patients were randomly divided into the TCSOM group and the PBD group.All patients in the TCSOM group were treated with spinal manipulations 5 times.Pain intensity was assessed on a visual analogue scale and symptoms were evaluated based on bowel symptom scale(BSS) scores before and after treatment.A symptom improvement rating(SIR) was implemented to evaluate the effects of treatment,and to statistically compare theTCSOM and PBD groups.RESULTS:Post-treatment subjective assessment for the TCSOM treatment group showed that 92 cases had excellent results,10 cases had good results,3 cases had fair results,and 0 cases had poor results.In the PBD group,30 cases had excellent results,41 cases had good results,12 cases had fair results,and 22 cases had poor results.According to the SIR,which was based on the BSS,the TCSOM group included 92 cases with excellent results,10 cases with good results and 3 cases with poor results.There were no adverse side effects from treatment.Based on the BSS,the PBD group had 30 cases with excellent results,41 cases with good results,and 34 cases with poor results.We found that the symptom rating in the BSS in the TCSOM group showed a more significant improvement,such as a reduction in the severity and frequency of symptoms,compared with that in the PBD group(P<0.001).CONCLUSION:Displacement of intervertebral discs and/or vertebra in the thoracic or lumbar region appears to be a contributing factor in the symptoms of IBS.Correcting this displacement of intervertebral discs and/or vertebra to resolve the stimuli caused by pressure exerted on the nerves and vessels around the spine is an effective treatment for IBS.
