Few experimental studies have evaluated the efficacy of continuing educational programs aimed at the improvement of nurses’ pain-management skills. This study assessed whether a standardized educational program aimed at nurses could increase the use of the Numeric Rating Scale-11 in both documenting and reducing postoperative pain-intensity levels in hospitalized surgical patients. The study had a quasi-experimental pre- and post-intervention design. Data were collected from records of surgical patients prior to and after the standardized educational program was completed. There were no significant differences between pre- and post-intervention groups in terms of either pain-documentation frequency or pain-intensity level. The study showed no increase in the frequency of postoperative pain documentation and no reduction of surgical patients’ postoperative pain-intensity level. This finding indicates that the standardized educational program on postoperative pain management was insufficient to bring about changes in clinical practice. 展开更多

Background/Purpose: Obesity and secondary conditions continue to disproportionally affect the health of children living in urban areas. Studies show that a lack of resources and physical activity-unfriendly communities discourage 60 minutes of daily activity, including strengthening exercises, as recommended by the Centers for Disease Control and Prevention. Using Social Ecological theory, the purpose of this study was to examine the multi-level influences of a yoga-based intervention on urban, inner city youth. Method: Using a mixed-methods design, ninety-three 3 - 5th grade students at five urban elementary schools participated in a ten-week yoga intervention. Analysis/Results: RM-ANOVA results revealed a significant reduction in stress and bullying behaviors among participants, and multiple regression analyses revealed that program attendance, change in stress, and change in yoga enjoyment significantly predicted change in yoga participation outside PE, when controlling for gender and age F(5, 87) = 5.36, p R2 = 0.19, but did not have a significant impact on physical activity participation outside of school. Student interviews and non-participant observations revealed strong enjoyment of yoga which led students to report substantial increases in yoga-related activities outside of school. Students also revealed that experience in yoga improved focus, attention, and reduced stress. Conclusions: Through convergence of qualitative and quantitative methods, this study showed a positive relationship between the number of yoga sessions attended (dose), enjoyment of yoga, and participation in yoga outside PE with friends and family. Findings suggest that urban PE should include more individual, non-competitive activities such as yoga, which students find to be stress-relieving, fun, inexpensive and easy to perform at home. 展开更多

Despite effective treatment options, more than 40% of cancer patients receive inadequate pain management. Our previous pilot study resulted in substantial adaptations of a cancer pain self-management intervention, the German PRO-Self? Plus Pain Control Program originally developed in the United States. This program will be implemented into clinical practice at the Medical Center-University of Freiburg. The purpose of this multiple methods pilot study is to test the implementation regarding feasibility and effects in clinical practice. In a randomized, wait-list controlled pilot study, adult oncology in-patients of a palliative care consultation service with pain >3/10 will be recruited. The intervention will be performed by a specialized advanced practice nurse with an in-hospital visit and, after discharge, via phone calls and visits. The follow-up will be personalized according to a clinical algorithm that factors in pain intensity, satisfaction with pain management, and patient adherence. The intervention includes structured and tailored components and is based on three key strategies: information, skill building and nurse coaching. The specific aims of this study are threefold: 1) to test the feasibility of the study and intervention procedures;2) to establish effect sizes of main outcome variables (e.g. decrease pain intensity, reduce the number of patients with pain as main symptom) for subsequent power calculation;3) to explore participants’ experiences with pain self-management support and their view of burden and benefit from study participation in a qualitative substudy. During the study period, which includes three data collection time points (T0 before, T1 one week and T2 six weeks after discharge), data will be collected via field notes of study nurses and questionnaires of patients. The results of this pilot study will build the basis for a larger comparative effectiveness study in which long term outcomes of a cancer pain self-management intervention in clinical practice will be evaluated. 展开更多

Objective: Falls are one of the most common direct causes of fractures which are major causes of morbidity and mortality in osteoporosis patient. There are many factors related to falls, by interfering of fall prevention education on the patients with osteoporosis, our study is to investigate whether fall prevention education can reduce the falling rate on the osteoporosis patients treated with zoledronic acid. Methods: A total of 178 eligible female patients who first visited our osteoporosis department during January 2016 to June 2017 were invited to participate in this study, and all participants were randomly divided into an observation group (92 cases) and an intervention group (86 cases). All patients were administrated zoledronic acid once and received a questionnaire survey about factors related to falls at the beginning and a year later. The patients in the observation group only received regular orders of adopting a healthy lifestyle while those in the intervention group received fall prevention assessment and education, and a telephone follow-up and reinforced fall prevention education a month after discharge. A year later, relevant data regarding the falls of each patient of both groups during the year and the data of the questionnaire survey were collected for intergroup comparison. Results: The difference of the improvement of fall risk factors between the two groups is statistically significant (P Conclusion: For an osteoporosis patient treated with zoledronic acid, fall prevention education is an effective method to reduce the risk of falling, which would result in a lower risk of fractures and a better prognosis. 展开更多

How to effectively implement the work of reducing the burden of basic education has been concerned by people.By using the knowledge of game theory, this paper analyzes the behavioral goals of the relevant subjects in reducing the burden of basic education, and puts forward some suggestions. We should change the"focus"to"characteristics"and guide the diversified development of primary and secondary education.By adjusting the matching degree of various levels of education, the pressure of entering school can be reduced.Through social reform, the interests of different classes were adjusted and the residual negative influence of imperial examination culture was eliminated,so as to successfully remove the stumbling block of"excessive academic burden"in the educational reform of the new era and create conditions for cultivating a large number of outstanding talents needed for national development and national rejuvenation. 展开更多

Since the founding of the People’s Republic of China in 1949,China’s per capita education level has risen from less than two years to 9.91 years. However,as the province with the lowest per capita education level in China,Tibet has only reached the level of 6.75 years per capita. This illustrates the worrying educational inequality that still exists between China’s poorer and more developed regions. This educational inequality can also be expressed by the Gini coefficient of education. In order to eliminate this educational inequality, the Chinese government has made great efforts. The Chinese government has expanded the demand for education through the right incentive system and the nine-year compulsory education law.On the other hand, the Chinese government directly or indirectly grants subsidies to education suppliers and demanders to achieve the purpose of increasing education output.Based on the positive externality of education itself, the Chinese government attaches great importance to the cultivation of human resources for offspring, and hopes to transform China’s quantity-based demographic dividend into quality-based demographic dividend, and finally realize sustained economic growth. We believe that some education-related policies promulgated by China have indeed reduced educational inequality in poor areas to some extent, but there is still potential for progress in further addressing educational inequality in poor areas by the Chinese government. 展开更多

In 2012, traditional Chinese medicine(TCM) practitioners in Australia became nationally-registered allied health professionals in three categories: acupuncturist, Chinese herbal medicine practitioner, and Chinese herbal medicine dispenser. Australia was the first Western country to introduce national registration for Chinese medicine, followed by Portugal and, recently, New Zealand. The practice of TCM in Australia can be traced back to the beginning of Chinese immigration to Australia during the Victorian Gold Rush which began in the 1850s. The process which led to national registration commenced in the early 1970s with the establishment of the first acupuncture courses. Decades of gradual development of courses from unaccredited part-time diplomas to accredited bachelor degrees, and the development, by the profession, of national consensus on educational standards were essential elements in the process which led to registration. Professional associations, such as the Australian Acupuncture and Chinese Medicine Association Ltd.(AACMA), also developed a framework of professional selfregulation, including Codes of Ethics, Codes of Conduct, ongoing professional development requirements and Infection Control Guidelines, and provided leadership in the development of the profession. After decades of tribalism and division within the TCM profession, the National Academic Standards Committee brought almost all stakeholders together to reach a consensus on the Australian Guidelines for Traditional Chinese Medicine Education which were published by AACMA in 2001. Professional associations also collaborated on joint submissions to the government in support of registration, which was introduced first in the state of Victoria in 2000, and subsequently became national in 2012. Despite national registration, some barriers still remain, and professional associations continue to lobby the federal government for inclusion in Medicare, chronic disease management scheme, and Veterans Affairs. 展开更多

BACKGROUND Global education in psychiatry is heavily influenced by knowledge from Western,high-income countries, which obscures local voices and expertise.AIM To adapt a human simulation model to psychiatric education in a context that is specific to local languages and cultures.METHODS We conducted an observational study consisting of six human simulation sessions with standardized patients from two host countries, speaking their native languages, and following an adaptation of the co-constructive patient simulation(CCPS)model. As local faculty became increasingly familiar with the CCPS approach, they took on the role of facilitators—in their country’s native language.RESULTS Fifty-three learners participated:19 child and adolescent psychiatry trainees and 3 faculty members in Türkiye(as a group that met online during 3 consecutive months);and 24 trainees and 7 faculty in Israel(divided into 3 groups,in parallel in-person sessions during a single training day). Each of the six cases reflected local realities and clinical challenges, and was associated with specific learning goals identified by each case-writing trainee.CONCLUSION Human simulation has not been fully incorporated into psychiatric education:The creation of immersive clinical experiences and the strengthening of reflective practice are two areas ripe for development. Our adaptations of CCPS can also strengthen local and regional networks and psychiatric communities of practice. Finally, the model can help question and press against hegemonies in psychiatric training that overshadow local expertise. 展开更多
