Evaluation of the effectiveness of preventive nursing measures for pressure injuries in patients in the neurology intensive care unit

BACKGROUND Patients in neurology intensive care units(ICU)are prone to pressure injuries(PU)due to factors such as severe illness,long-term bed rest,and physiological dysfunction.PU not only causes pain and complications to patients,but also increases medical burden,prolongs hospitalization time,and affects the recovery process.AIM To evaluate and optimize the effectiveness of pressure injury prevention nursing measures in neurology ICU patients.METHODS A retrospective study was conducted,and 60 patients who were admitted to the ICU of the Department of Neurology were selected and divided into an observation group and a control group according to the order of admission,with 30 people in each group.The observation group implemented pressure injury prevention and nursing measures,while the control group adopted routine care.RESULTS Comparison between observation and control groups following pressure injury prevention nursing intervention revealed significantly lower incidence rates in the observation group compared to the control group at 48 h(8.3%vs 26.7%),7 d(16.7%vs 43.3%),and 14 d(20.0%vs 50.0%).This suggests a substantial reduction in pressure injury incidence in the observation group,with the gap widening over time.Additionally,patients in the observation group exhibited quicker recovery,with a shorter average time to get out of bed(48 h vs 72 h)and a shorter average length of stay(12 d vs 15 d)compared to the control group.Furthermore,post-intervention,patients in the observation group reported significantly improved quality of life scores,including higher scores in body satisfaction,feeling and function,and comfort(both psychological and physiological),indicating enhanced overall well-being and comfort following the implementation of pressure injury prevention nursing measures.CONCLUSION Implementing pressure injury preventive care measures for neurology ICU patients will have better results. 展开更多

The technology of expansion fracturing with liquid CO_(2)(EFLCO_(2))has attracted increasing attention due to reduced vibration and damage.The disposable fracturing tube has been developed and is gradually replacing the Cardox tube.However,there is a lack of impact pressure testing of disposable tubes under real working conditions,selection of gas explosion design parameters,and systematic analysis of blasting vibration.This limitation has constrained the widespread application of disposable fracturing tubes in engineering.A joint monitoring of the pressure-time curves in the disposable tubes and boreholes was conducted.The rock-breaking effect of varying hole spacing parameters in the EFLCO_(2)design was analyzed,and a systematic study was carried out on the vibration peak value,frequency,and energy characteristics.The results show that(1)the pressure distribution characteristics,stress peak value,and duration in the disposable tubes are different from those of Cardox tubes,which show multi-peak distribution,low-pressure peak value,and short duration.The correlation between the pressure in the disposable tube,filling pressure,and liquid CO_(2)weight is established,and a theoretical calculation method for the borehole wall pressure is proposed;(2)The hole spacing in rocks of different hardness is suggested;and(3)At the same scale distance,the peak particle velocity(PPV)caused by EFLCO_(2)(PPVCO_(2))is significantly smaller than that caused by blasting(PPVexplosive).The ratio of PPVexplosive to PPVCO_(2)is a power function related to scale distance,and a distance-related zonality exist in this relationship.The frequency composition of the vibration signal caused by EFLCO_(2)is relatively simple with a narrow frequency band.Its PPV and energy are mainly concentrated in the low-frequency band.This research contributes to the optimization of disposable fracturing tubes,gas explosion design,and vibration hazard control. 展开更多

To study the relative sensitivity of permeability to pore pressure Pp and confining pressure Pc for clay-rich rocks, permeability measurements were performed on samples of four clay-rich sandstones. A new method (hereafter denoted the "slide method") was developed and used for analyzing the permeability data obtained. The effective pressure coefficients for permeability nk were calculated. The values of nk were found to be greater than 1.0 and insensitive to changes in pressure. These results confirmed observations previously made on clay-rich rocks. Also, the coefficients nk obtained had different characteristics for different samples because of differences in the types of clay they contained. The effective pressure law (σeff=Pc-nkPp) determined using the slide method gave better results about k(oefr) than classic Terzaghi's law (σeff=Pc-nkPp). 展开更多

This paper analyzes the control mechanism of coal and gas outbursts and proposes the concept of an effective pressure relief protection range, based on the stress relief of the underlying coal-rock mass and the development of a plastic zone. Also this study developed a stress change and fracture development model of the underlying coal-rock mass. In addition, the stress and depth of fracture of any point in the floor were deduced with the application of Maple Calculation Software. The specific engineering parameters of the Pingdingshan No. 12 colliery were applied to determine the relationship between the depth of fracture in the floor and the mining height. The pressure-relief principle of the underlying coal-rock mass was analyzed while varying the mining height of the upper protective seam. The findings indicate that as the depth of fracture in the floor increases, the underlying coal-rock mass experiences a limited amount of pressure relief, and the pressure relief protection range becomes narrower.Additionally, the stress distribution evolves from a ‘‘U" shape into a ‘‘V" shape. A 2.0 m mining height of protective seam situates the outburst-prone seam, Ji_(15), within the effective pressure relief protection range. The fracture development and stress-relief ratio rises to 88%, ensuring the pressure-relief effect as well as economic benefits. The measurement data show that: after mining the upper protective seam, the gas pressure of Ji_(15) dropped from 1.78 to 0.35 MPa, demonstrating agreement between the engineering application and the theoretical calculation. 展开更多

A calculation model based on effective medium theory has been developed for predicting elastic properties of dry carbonates with complex pore structures by integrating the Kuster-Toksǒz model with a differential method.All types of pores are simultaneously introduced to the composite during the differential iteration process according to the ratio of their volume fractions.Based on this model,the effects of pore structures on predicted pore-pressure in carbonates were analyzed.Calculation results indicate that cracks with low pore aspect ratios lead to pore-pressure overestimation which results in lost circulation and reservoir damage.However,moldic pores and vugs with high pore aspect ratios lead to pore-pressure underestimation which results in well kick and even blowout.The pore-pressure deviation due to cracks and moldic pores increases with an increase in porosity.For carbonates with complex pore structures,adopting conventional pore-pressure prediction methods and casing program designs will expose the well drilling engineering to high uncertainties.Velocity prediction models considering the influence of pore structure need to be built to improve the reliability and accuracy of pore-pressure prediction in carbonates. 展开更多

It may cause errors when we analyze high pressure pneumatics under ideal gas assumption. However, the real gas effect on the performances of high pressure pneumatics is seldom investigated. In this paper, the real gas effects on air enthalpy and internal energy are estimated firstly to study the real gas effect on the energy conversion. Under ideal gas assumption, enthalpy and internal energy are solely related to air temperature. The estimation result indicates that the pressure enthalpy and pressure internal energy of real pneumatic air obviously decrease the values of enthalpy and internal energy for high pressure pneumatics, and the values of pressure enthalpy and pressure internal energy are close. Based on the relationship among pressure, enthalpy and internal energy, the real gas effects on charging and discharging processes of high pressure pneumatics are estimated, which indicates that the real gas effect accelerates the temperature and pressure decreasing rates during discharging process, and decelerates their increasing rates during charging process. According to the above analysis, and for the inconvenience in building the simulation model for real gas and the difficulty of measuring the detail thermal capacities of pneumatics, a method to compensate the real gas effect under ideal gas assumption is proposed by modulating the thermal capacity of the pneumatic container in simulation. The experiments of switching expansion reduction (SER) for high pressure pneumatics are used to verify this compensating method. SER includes the discharging process of supply tanks and the charging process of expansion tank. The simulated and experimental results of SER are highly consistent. The proposed compensation method provides a convenient way to obtain more realistic simulation results for high pressure pneumatics. 展开更多

In this paper, we evaluate the difference between the first and second measurements of blood pressure (BP) when BP is measured twice using the results of 17,775 medical checkups. The two measurements for both systolic BP (SBP) and the diastolic BP (DBP) fluctuated a large amount even though they were measured at a short interval. The first measurements were 6.7 and 2.4 mmHg higher than the second ones for SBP and DBP, suggesting a white coat effect. Then, the factors that might affect the differences between the two measurements were analyzed by the regression models. For both SBP and DBP, the difference increased as the first measurement increased. Age, gender, BMI and alcohol consumption were other important factors affecting the difference. In the case of a typical male individual, the typical criteria for hypertension of 140/90, 160/100 and 180/110 mmHg criteria in the first measurement would correspond to 135/86, 150/94 and 165/102 mmHg in the second measurement. The necessity of developing accurate and cost-efficient BP measurement methods is strongly suggested. 展开更多

This paper reports the current-voltage characteristics of [001]-oriented AlAs/InxGa1-xAs/GaAs resonant tunnelling diodes (RTDs) as a function of uniaxial external stress applied parallel to the [110] and the [1^-10] orientations, and the output characteristics of the GaAs pressure sensor based on the pressure effect on the RTDs. Under [110] stress, the resonance peak voltages of the RTDs shift to more positive voltages. For [1^-10] stress, the peaks shift toward more negative voltages. The resonance peak voltage is linearly dependent on the [110] and [1^-0] stresses and the linear sensitivities are up to 0.69 mV/MPa, -0.69 mV/MPa respectively. For the pressure sensor, the linear sensitivity is up to 0.37 mV/kPa. 展开更多

A variational method is adopted to investigate the properties of shallow impurity states near the interface in a free strained wurtzite GaN/AlxGa1-xN heterojunction under hydrostatic pressure and external electric field by using a simplified coherent potential approximation. Considering the biaxial strain due to lattice mismatch or epitaxial growth and the uniaxial strains effects, we investigated the Stark energy shift led by an external electric field for impurity states as functions of pressure as well as the impurity position, A1 component and areal electron density. The numerical result shows that the binding energy near linearly increases with pressure from 0 to 10 GPa. It is also found that the binding energy as a function of the electric field perpendicular to the interface shows an un-linear red shift or a blue shift for different impurity positions. The effect of increasing x on blue shift is more significant than that on the red shift for the impurity in the channel near the interface. The pressure influence on the Stark shift is more obvious with increase of electric field and the distance between an impurity and the interface. The increase of pressure decreases the blue shift but increases the red shift. 展开更多

Laser-induced breakdown spectroscopy(LIBS) has attracted extensive attention as a new technique for in-situ marine application. In this work, the influence of deep-sea high pressure environment on LIBS signals was investigated by using a compact LIBS-sea system developed by Ocean University of China for the in-situ chemical analysis of seawater. The results from the field measurements show that the liquid pressure has a significant effect on the LIBS signals. Higher peak intensity and larger line broadening were obtained as the pressure increases. By comparing the variations of the temperature and salinity with the LIBS signals, a weak correlation between them can be observed. Under high pressure conditions, the optimal laser energy was higher than that in air environment. When the laser energy exceeded 17 mJ, the effect of laser energy on the signal intensity weakened. The signal intensity decreases gradually at larger delays. The obtained results verified the feasibility of the LIBS technique for the deep-sea in-situ detection, and we hope this technology can contribute to surveying more deep-sea environments such as the hydrothermal vent regions. 展开更多

The laser pulses with pulse width of 10ns and pulse energy of about 320mJ at 1.06μm wavelength is focused on the aluminium target mounted on a ballistic pendulum, and the air pressure in the chamber changes from 2.8 × 10^ 3 to 1.01 × 10^5pa. The experimental results show that the impulse coupling coefficient changes as the air pressure and the distance of the target from focus change. The mechanical effects of the plasma shock wave on the target are analysed at different distances from focus and the air pressure. 展开更多

A damage coefficient concerning modulus attenuation was defined. Response models of stress and displacement of surrounding rock of soft rock roadway and analytical expressions to calculate plastic zones under different interior pressures and non-uniform original rock stresses were derived based on damage theories and a triple linear elastic-plastic strain softening model. Influence laws of dilatancy gradient on damage development, distributions of stresses and displacement in plastic region were analyzed. Interior pressure conditions to develop plastic region under different origin rock stresses were established and their influences on plastic region distribution were also discussed. The results show that the order of maximum principle stress is exchanged between ~0 and trr with the increase of interior pressure P0, which causes distributions of plastic zone and stress shift. Dilatancy effect which has great influences on the damage propagation and displacements in plastic region has little effect on the size of plastic region and stress responses. The conclusions provide a theoretical basis for a reasonable evaluation of stability and effective supporting of weakly consolidated soft rock roadway. 展开更多

Pore pressure is essential data in drilling design,and its accurate prediction is necessary to ensure drilling safety and improve drilling efficiency.Traditional methods for predicting pore pressure are limited when forming particular structures and lithology.In this paper,a machine learning algorithm and effective stress theorem are used to establish the transformation model between rock physical parameters and pore pressure.This study collects data from three wells.Well 1 had 881 data sets for model training,and Wells 2 and 3 had 538 and 464 data sets for model testing.In this paper,support vector machine(SVM),random forest(RF),extreme gradient boosting(XGB),and multilayer perceptron(MLP)are selected as the machine learning algorithms for pore pressure modeling.In addition,this paper uses the grey wolf optimization(GWO)algorithm,particle swarm optimization(PSO)algorithm,sparrow search algorithm(SSA),and bat algorithm(BA)to establish a hybrid machine learning optimization algorithm,and proposes an improved grey wolf optimization(IGWO)algorithm.The IGWO-MLP model obtained the minimum root mean square error(RMSE)by using the 5-fold cross-validation method for the training data.For the pore pressure data in Well 2 and Well 3,the coefficients of determination(R^(2))of SVM,RF,XGB,and MLP are 0.9930 and 0.9446,0.9943 and 0.9472,0.9945 and 0.9488,0.9949 and 0.9574.MLP achieves optimal performance on both training and test data,and the MLP model shows a high degree of generalization.It indicates that the IGWO-MLP is an excellent predictor of pore pressure and can be used to predict pore pressure. 展开更多

Switching expansion reduction(SER)uses a switch valve instead of the throttle valve to realize electronically controlled pressure reduction for high pressure pneumatics.A comprehensive and interactive pneumatic simulation model according to the experimental setup of SER has been built.The mathematical model considers heat exchanges,source air pressure and temperature,environmental temperatures and heat transfer coefficients variations.In addition,the compensation for real gas effect is used in the model building.The comparison between experiments and simulations of SER indicates that,to compensate the real gas effect in high pressure discharging process,the thermal capacity of air supply container in simulation should be less than the actual value.The higher the pressure range,the greater the deviation.Simulated and experimental results are highly consistent within pressure reduction ratios ranging from 1.4 to 20 and output air mass flow rates ranging from 3.5 to 132 g/s,which verifies the high adaptability of SER and the validity of the mathematic model and the compensation method. 展开更多

The stable sliding surface in the model tests can be considered to be composed of two parts. Within the height range of 0.82 H-1.0 H, it is a plane at an angle of π/4+φ/2 to the horizontal plane. In the height range of 0-0.82 H, it is a curve between a logarithmic spiral and a plane at an angle of π/4+φ/2 to the horizontal. A new method applicable to any sliding surface is proposed for active earth pressure with the consideration of arching effect. The active earth pressure is computed with the actual shape of the slip surface and compared with model test data and with predictions obtained by existing methods. The comparison shows that predictions from the newly proposed method are more consistent with the measured data than the predictions from the other methods. 展开更多

Interface and scale effects are the two most important factors which strongly affect the structure and the properties of nano-/micro-crystals under pressure.We conduct an experiment under high pressure in situ alternating current impedance to elucidate the effects of interface on the structure and electrical transport behavior of two Zn Se samples with different sizes obtained by physical grinding.The results show that(i) two different-sized Zn Se samples undergo the same phase transitions from zinc blend to cinnabar-type phase and then to rock salt phase;(ii) the structural transition pressure of the859-nm Zn Se sample is higher than that of the sample of 478 nm,which indicates the strong scale effect.The pressure induced boundary resistance change is obtained by fitting the impedance spectrum,which shows that the boundary conduction dominates the electrical transport behavior of Zn Se in the whole experimental pressure range.By comparing the impedance spectra of two different-sized Zn Se samples at high pressure,we find that the resistance of the 478-nm Zn Se sample is lower than that of the 859-nm sample,which illustrates that the sample with smaller particle size has more defects which are due to physical grinding. 展开更多

The screening effect of the random-phase-approximation on the states of shallow donor impurities in free strained wurtzite GaN/AlxGa1-xN heterojunctions under hydrostatic pressure and an external electric field is investigated by using a variational method and a simplified coherent potential approximation. The variations of Stark energy shift with electric field, impurity position, A1 component and areal electron density are discussed. Our results show that the screening dramatically reduces both the blue and red shifts as well as the binding energies of impurity states. For a given impurity position, the change in binding energy is more sensitive to the increase in hydrostatic pressure in the presence of the screening effect than that in the absence of the screening effect. The weakening of the blue and red shifts, induced by the screening effect, strengthens gradually with the increase of electric field. Furthermore, the screening effect weakens the mixture crystal effect, thereby influencing the Stark effect. The screening effect strengthens the influence of energy band bending on binding energy due to the areal electron density. 展开更多

Following large-scale volume fracturing in shale oil reservoirs,well shut-in measures are generally employed.Laboratory tests and field trials have underscored the efficacy of fracturing fluid imbibition during the shut-in phase in augmenting shale oil productivity.Unlike conventional reservoirs,shale oil reservoirs exhibit characteristics such as low porosity,low permeability,and rich content of organic matter and clay minerals.Notably,the osmotic pressure effects occurring between high-salinity formation water and low-salinity fracturing fluids are significant.The current understanding of the mobilization patterns of crude oil in micro-pores during the imbibition process remains nebulous,and the mechanisms underpinning osmotic pressure effects are not fully understood.This study introduces a theoretical approach,by which a salt ion migration control equation is derived and a mathematical model for spontaneous imbibition in shale is introduced,which is able to account for both capillary and osmotic pressures.Results indicate that during the spontaneous imbibition of low-salinity fluids,osmotic effects facilitate the migration of external fluids into shale pores,thereby complementing capillary forces in displacing shale oil.When considering both capillary and osmotic pressures,the calculated imbibition depth increases by 12%compared to the case where only capillary forces are present.The salinity difference between the reservoir and the fracturing fluids significantly influences the imbibition depth.Calculations for the shutin phase reveal that the pressure between the matrix and fractures reaches a dynamic equilibrium after 28 days of shut-in.During the production phase,the maximum seepage distance in the target block is approximately 6.02 m. 展开更多

By taking the influence of optical phonon modes into account, this paper adopts the dielectric continuum phonon model and force balance equation to investigate the electronic mobility parallel to the interfaces for AlAs/GaAs semiconductor quantum wells (QWs) under hydrostatic pressure. The scattering from confined phonon modes, interface phonon modes and half-space phonon modes are analysed and the dominant scattering mechanisms in wide and narrow QWs are presented. The temperature dependence of the electronic mobility is also studied in the temperature range of optical phonon scattering being available. It is shown that the electronic mobility reduces obviously as pressure increases from 0 to 4GPa, the confined longitudinal optical (LO) phonon modes play an important role in wide QWs, whereas the interface optical phonon modes are dominant in narrow QWs, the half-space LO phonon modes hardly influence the electronic mobility expect for very narrow QWs. 展开更多
