Ethics of Human Resources Management in the Cameroonian Health System, Medical Nomadism and the Ineffectiveness of the Fight against High Blood Pressure

Context/objectives: The fight against Chronic Non-Communicable Diseases (NCDs) is a long-term undertaking, which requires available, motivated and well-managed human resources (HR). The administrative management of skills on both qualitative and quantitative levels is one of the essential functions of a health system. To better implement policies of fight against High Blood Pressure (HBP) and other chronic diseases, it is important to establish strategies to retain health personnel. This loyalty requires favorable working conditions and consideration of the contribution-reward couple. Good working conditions are likely to reduce the phenomenon of medical nomadism;conversely, poor HR management can contribute to their exodus towards exotic “green pastures”, thus leading to an additional crisis in the Cameroonian health system. The fight against HBP is a complex, multifaceted and multifactorial reality that requires appropriate management model for all types of resources mainly HR. The main objective of this research is to show the impact of poor management of human resources in Cameroon health system on medical nomadism and the ineffectiveness of the fight against High Blood Pressure. Method: A cross-sectional descriptive survey among five hundred (500) health facilities in the center region of Cameroon has been conducted. A stratified probabilistic technique has been used, and the number of health facilities to be surveyed has been determined using the “sample size estimation table” of Depelteau. The physical questionnaires have been printed and then distributed to data collectors. After data collection, the latter were grouped during processing in Excel sheets. The Chi-square test was used for data with a qualitative value and that of Kolmogorov-Sminorf for data with a quantitative value to assess the normality and reliability of data. The Crochach’s Alpha reliability test allowed us to have a summary of the means and variances and then to search for intragroup correlations between variables. Descriptive analysis was possible with the XLSTAT 2016 software. Results: 43.60% of Health Facilities (HF) managers were unqualified. 82.20% of HF managers have staff in a situation of professional insecurity. They are mainly contractual (49.00), decision-making agents (24.40%), casual agents (08.80). The proportion of unstable personnel is average of 22.00% and very unstable, 12.00%. 展开更多

The relationship between the Heaven and human beings is a central issue in the transformation of Confucianism in the Mid-Tang Dynasty.The discourses on the Heaven by Han Yu,Liu Zongyuan,and Liu Yuxi helped to promote the transformation and greatly influenced the transformation in literature.The differences in the poetry among the three authors may also be interpreted from the perspective of their perceptions of the Heaven—Han Yu’s discourse on the Heaven focused on the enemy-relationship between the Heaven and human beings,so his poems always showed tension and loss.Liu Zongyuan was interested in the alienation between the heaven and human beings,so his poems were more rational and placid than Han Yu’s.However,Liu Yuxi’s discourse on the Heaven paid attention to the positive influence of the Heaven on human beings,and most of his poems were filled with reason.The changes in the relationship between the Heaven and human beings lead to the broadened themes,innovated vocabulary,diversified styles,and varied fresh imageries.Likewise,the relationship between the Heaven and human beings established in the Song Dynasty led accordingly to a new type of literature different from the Tang literature. 展开更多

Thus, the Lifeboat ethics utilizes both reason and science. In a similar vein, Humanist ethics is also based on human reason and scientific methods. Notwithstanding these common grounds in the sources of the aforementioned ethics, there are dissimilarities in their respective ethical values that outnumber the similarities. This paper will attempt to clarify the clear tendency, in our contemporary world, to look at ethics as something that "cures" all wrongs, all unbalance, and all evils from the political, environmental, to the business ones. For this reason, there is nothing better than to turn to ethics in order to "moralise" the system and the world, when the numerous news of fraud, injustice, and scandals are a part of everyday life in the business world and in our society. But, is this the role of ethics? Can we reduce the aim of ethics to a mere technique? In this paper, it is argued that ethics needs to be replaced in the epistemological scale of knowledge, where until now it has occupied a place at the top. Ethics needs to be repositioned in a different level of knowledge where we may find the roots of its foundation. It is also discussed that it is at the core of being that the experience of the action takes place and develops and in it, ethics takes root. Furthermore, it is suggested that understanding ethics from this point of view presupposes a new ethos around a relational poietics which privileges inter-subjectivity, self-creation, and an active and dynamic construction of the human and of the world. 展开更多

Here we prefer continuous sedation until death (CSD). CSD differs from physician-assisted death (PAD) in many aspects. Firstly, CSD is not used to shorten life or cause death, but instead its intention is to relieve suffering. Secondly, once CSD is associated with a reduction in or suspension of food intake, the duration of sedation is too short to have an impact on survival rate. Thirdly, CSD is titrated while PAD is often overdosed. Fourthly, the sanctity of life could be maintained since sedation is used merely as a means of quelling suffering. Last but no least, for unconscious patients, the same medicines are frequently used for both palliative sedation and standard surgical procedures. 展开更多

This paper presents an ex-ante assessment of the potential implications of the 2011 Human Rights Watch Report on the South African fruit industry. The report explicitly mentions the lack of compliance with ethical codes within the fruit industry, the prevalence of exploitative conditions for farm workers and diverse human rights abuses in farms. The report recommends import ban on culprits as well as engaging third party auditing to ensure compliance. The high vulnerability of the smallholder farmers justified the special interest in the implications on their reputation and hence their ability to access profitable export markets. Although large-scale commercial farmers are potentially at risk of reputational damage due to the Human Rights Watch Report, they are better able to cope owing to their stronger capital base. The opinions of knowledgeable industry insiders were therefore elicited through qualitative research that used a five-point Likert scale to assess perceptions about market access. On the basis of the results, it is probable that the report will lead to improved working conditions for farm workers, improved concern for consumers' health, and enforcement of legislation by the govemment. The negative implications involve increased competition, possibility of retailers' rationalising their supply base and increased evaluation that ends at the farm gate. There is also high probability of increased marginalisation of the already disadvantaged smallholder suppliers, and possible increase in costs of auditing and accreditation for the entire fruit industry. Thus, active collaboration among all stakeholders to ensure the competitiveness of the fruit industry is inevitable. 展开更多

One of the aims of the Universal Declaration on B ioethics and Human Rights (UNESCO) is to "promote respect for human dignity and protect human rights",l Here are two overarching principles at work, ensuring that the biomedical sciences fulfill their task within an ethical framework. The principle of respect for human dignity is a universal moral concept, meant to be applied in human encounters. Protecting human rights underscores the legal principle of not only affirming the fundamental equality of all human beings, but equally safeguarding it. These two principles are universally defined, but are ordinarily specified by the particular value system of individual cultures in which they are employed. It is within such particular cultural application that their relevance stands out. The thrust of this paper is that, since principles are general action guides, they actually constitute a universal language for the analysis and evaluation of all human conduct. However, there is also recognition of the fact that moral contexts vary from culture to culture, and that while the scope of the two principles above is not restricted by any particular culture, it is indeed those cultural specifics of each moral context that constitute the framework within which the principles become operational. As general action guides, I will argue that these principles lack moral relevance outside of those particular cultural settings wherein they are contextualized. Without such relevance, these principles become meaningless mantras. I will further show that such principles do not merely uphold values informed by particular cultures, but they are an embodiment of values inherent to human nature in general. Consequently, these principles do not just serve as instruments for addressing issues peculiar to "Western bioethics" or any other particular cultural setting in an exclusive sense, but are also used for moderating bioethics discourse that transcend particular cultural boundaries. I will further explain that such universal discourse is potentially instructive with regards to how cultural universals are viewed in relation to the cultural particulars, and that this discourse essentially becomes a lingua franca for cross-cultural dialogue in bioethics. 展开更多

Personal ethics are strongly influenced by emotions, particularly secondary emotions, because these emotions expand ethical reasoning and development as the child matures. A well-developed consciousness profoundly influences a person's actions and conduct when solving problems of what is thought, or taught to be, right or wrong Compelling neurological evidence supports the claim that children begin to develop enduring ethical standards at an early age and that these standards are largely based on the experiences of early childhood. Essentially, the innate sense of ethics requires nurturing during infancy before it can be cognitively understood and practiced in maturity. In biological terms, the development of neural networks that regulate emotional growth, and subsequently, the capacity for ethical discrimination, depends on the infant's early social environment. Thus, the toddler's early epigenetic experiences enhance, or impede, its innate still dormant genetic potential. Importantly, personal character development and ethical discrimination begins long before the child's formal educational years. As a consequence, early learning has to discover ways of conserving adaptive thinking which can be applied to the choices that may confront future generations. Early ethics education, including accurate access to scientific, medical, and technological knowledge, is thus critical. Future generations will increasingly require education from a global perspective when making major ethical decisions in areas, such as nuclear technology, disposal of wastes, preservation of biodiversity, global warming, and unregulated human population growth. As long as our culture continues to reflect advances in science and technology, there is an obligation to make science education overlap with crucial periods in the advancement of ethical consciousness. Significantly, when considering the human capacity for excess at times of conflict, it is incumbent on the scientific community to integrate research-based knowledge with wide-ranging learning and problem-solving skills. Bioscience ethics, the established interface bridging applied science and applied bioethics, can assist in this process of integration. To become fully responsible adults, we must share our extraordinary cognitive talents and respect life on earth in all its rich diversity. In biological terms, human uniqueness resides primarily in our brains with its products being co-operation in family and ancestral units, long education, sophisticated language and culture, and importantly, ethical consciousness-all attributes held in trust by knowledge and wisdom for future generations. 展开更多
