The increased frequency and intensity of drought and wet climate events resulting from climatechange pose a major threat to global ecological stability. Variations in stability among different ecosystemshave been confirmed, but it remains unclear whether there are differences in stability within the sameterrestrial vegetation ecosystem under the influence of climate events in different directions and intensities.China's grassland ecosystem includes most grassland types and is a good choice for studying this issue.This study used the Standardized Precipitation Evapotranspiration Index-12 (SPEI-12) to identify thedirections and intensities of different types of climate events, and based on Normalized DifferenceVegetation Index (NDVI), calculated the resistance and resilience of different grassland types for 30consecutive years from 1990 to 2019 (resistance and resilience are important indicators to measurestability). Based on a traditional regression model, standardized methods were integrated to analyze theimpacts of the intensity and duration of drought and wet events on vegetation stability. The resultsshowed that meadow steppe exhibited the highest stability, while alpine steppe and desert steppe had thelowest overall stability. The stability of typical steppe, alpine meadow, temperate meadow was at anintermediate level. Regarding the impact of the duration and intensity of climate events on vegetationecosystem stability for the same grassland type, the resilience of desert steppe during drought was mainlyaffected by the duration. In contrast, the impact of intensity was not significant. However, alpine steppewas mainly affected by intensity in wet environments, and duration had no significant impact. Ourconclusions can provide decision support for the future grassland ecosystem governance.展开更多
The Jenkyns Event,more widely known as the Toarcian Oceanic Anoxic Event(T-OAE),is marked by globally distributed negative carbon-isotope excursions,widespread oxygen depletion,and large-scale organic carbon burial,wh...The Jenkyns Event,more widely known as the Toarcian Oceanic Anoxic Event(T-OAE),is marked by globally distributed negative carbon-isotope excursions,widespread oxygen depletion,and large-scale organic carbon burial,which indicate major climate/environmental perturbations in Earth's surface systems during the Early Jurassic.Although extensive research has been conducted in European continental settings,particularly in the western peri-Tethys regions,the impacts of this event beyond Europe remains largely unexplored.Here,a multiapproach study including investigations into the sporepollen assemblages,pyrite framboids,clay minerals,total organic carbon(TOC)levels,and organic carbon isotope(δ13Corg)levels in a lacustrine borehole section(MED1)from the Yin'gen-Ejinaqi Basin,North China,provides evidence of the occurrence of the Jenkyns Event and its extensive sedimentary responses in the eastern Tethys terrestrial systems.Two distinct spore-pollen assemblages have been identified in MED1(drilling depth:982.4 m to 1267.5 m),with the Cycadopites-Protopinus-Osmundacidites assemblage in the lower part(1267.5 m to 1132.9 m)indicating a middle Early Jurassic age and the Classopollis assemblage in the upper part(1132.9 m to 985.7 m)suggesting a Toarcian age.Framboidal pyrite data suggest more anoxic conditions during the deposition of black mudstone and shale intercalations in the lower part of the Classopollis assemblage(1132.9 m to 1066.9 m),which combined with organic carbon enrichment and negativeδ13Corg excursions,are considered the paleoenvironmental response to the Jenkyns Event in the study area.Furthermore,the evolution of vegetation groups changed from plant groups characterized by bisaccate and cycad pollen,as well as fern spores,to vegetation groups represented by Cheirolepidiaceae pollen across the Jenkyns Event,as evidenced by sporepollen data,together with the clay mineral assemblage change characterized by a notable increase in illite at the expense of kaolinite,suggests that while a subtropical-temperate climate persisted,a change toward warmer and drier conditions most likely occurred in the early Toarcian in the study area.In contrast to the humidification evidenced in many coastal settings,this aridification trend in the Yin'gen-Ejinaqi Basin aligns with the conditions in many inland areas.It is hypothesized that the underlying cause of these divergent changes may be linked to certain patterns of spatially variable water availability on land,potentially driven by extremified hydrological conditions.展开更多

However, in aerospace, energetic particles, such as heavy ions, protons, and alpha particles, can induce single event effects(SEEs) that lead CNNs to malfunction and can significantly impact the reliability of a CNN system. In this paper, the MNIST CNN system was constructed based on a 28 nm systemon-chip(SoC), and then an alpha particle irradiation experiment and fault injection were applied to evaluate the SEE of the CNN system. Various types of soft errors in the CNN system have been detected, and the SEE cross sections have been calculated. Furthermore, the mechanisms behind some soft errors have been explained. This research will provide technical support for the design of radiation-resistant artificial intelligence chips.展开更多
BACKGROUND The incidence of chronic kidney disease among patients with diabetes mellitus(DM)remains a global concern.Long-term obesity is known to possibly influence the development of type 2 diabetes mellitus.However...BACKGROUND The incidence of chronic kidney disease among patients with diabetes mellitus(DM)remains a global concern.Long-term obesity is known to possibly influence the development of type 2 diabetes mellitus.However,no previous meta-analysis has assessed the effects of body mass index(BMI)on adverse kidney events in patients with DM.AIM To determine the impact of BMI on adverse kidney events in patients with DM.METHODS A systematic literature search was performed on the PubMed,ISI Web of Science,Scopus,Ovid,Google Scholar,EMBASE,and BMJ databases.We included trials with the following characteristics:(1)Type of study:Prospective,retrospective,randomized,and non-randomized in design;(2)participants:Restricted to patients with DM aged≥18 years;(3)intervention:No intervention;and(4)kidney adverse events:Onset of diabetic kidney disease[estimated glomerular filtration rate(eGFR)of<60 mL/min/1.73 m2 and/or microalbuminuria value of≥30 mg/g Cr],serum creatinine increase of more than double the baseline or end-stage renal disease(eGFR<15 mL/min/1.73 m2 or dialysis),or death.RESULTS Overall,11 studies involving 801 patients with DM were included.High BMI(≥25 kg/m2)was significantly associated with higher blood pressure(BP)[systolic BP by 0.20,95%confidence interval(CI):0.15–0.25,P<0.00001;diastolic BP by 0.21 mmHg,95%CI:0.04–0.37,P=0.010],serum albumin,triglycerides[standard mean difference(SMD)=0.35,95%CI:0.29–0.41,P<0.00001],low-density lipoprotein(SMD=0.12,95%CI:0.04–0.20,P=0.030),and lower high-density lipoprotein(SMD=–0.36,95%CI:–0.51 to–0.21,P<0.00001)in patients with DM compared with those with low BMIs(<25 kg/m2).Our analysis showed that high BMI was associated with a higher risk ratio of adverse kidney events than low BMI(RR:1.22,95%CI:1.01–1.43,P=0.036).CONCLUSION The present analysis suggested that high BMI was a risk factor for adverse kidney events in patients with DM.展开更多
BACKGROUND Coagulopathy and thromboembolic events are associated with poor outcomes in coronavirus disease 2019(COVID-19)patients.There is conflicting evidence on the effects of chronic anticoagulation on mortality an...BACKGROUND Coagulopathy and thromboembolic events are associated with poor outcomes in coronavirus disease 2019(COVID-19)patients.There is conflicting evidence on the effects of chronic anticoagulation on mortality and severity of COVID-19 disease.AIM To summarize the body of evidence on the effects of pre-hospital anticoagulation on outcomes in COVID-19 patients.METHODS A Literature search was performed on LitCovid PubMed,WHO,and Scopus databases from inception(December 2019)till June 2023 for original studies reporting an association between prior use of anticoagulants and patient outcomes in adults with COVID-19.The primary outcome was the risk of thromboembolic events in COVID-19 patients taking anticoagulants.Secondary outcomes included COVID-19 disease severity,in terms of intensive care unit admission or invasive mechanical ventilation/intubation requirement in patients hospitalized with COVID-19 infection,and mortality.The random effects models were used to calculate crude and adjusted odds ratios(aORs)with 95%confidence intervals(95%CIs).RESULTS Forty-six observational studies met our inclusion criteria.The unadjusted analysis found no association between prior anticoagulation and thromboembolic event risk[n=43851,9 studies,odds ratio(OR)=0.67(0.22,2.07);P=0.49;I2=95%].The association between prior anticoagulation and disease severity was non-significant[n=186782;22 studies,OR=1.08(0.78,1.49);P=0.64;I2=89%].However,pre-hospital anticoagulation significantly increased all-cause mortality risk[n=207292;35 studies,OR=1.72(1.37,2.17);P<0.00001;I2=93%].Pooling adjusted estimates revealed a statistically non-significant association between pre-hospital anticoagulation and thromboembolic event risk[aOR=0.87(0.42,1.80);P=0.71],mortality[aOR=0.94(0.84,1.05);P=0.31],and disease severity[aOR=0.96(0.72,1.26);P=0.76].CONCLUSION Prehospital anticoagulation was not significantly associated with reduced risk of thromboembolic events,improved survival,and lower disease severity in COVID-19 patients.展开更多

METHODS: We conducted a systematic review and meta-analysis. MEDLINE and EMBASE were searched (up to October 2007) to identify randomised controlled tri- als comparing bismuth with placebo or no treatment, or bismuth salts in combination with antibiotics as part of eradication therapy with the same dose and duration of antibiotics alone or, in combination, with acid suppresion. Total numbers of adverse events were recorded. Data were pooled and expressed as relative risks with 95% confidence intervals (CI).RESULTS: We identified 35 randomised controlled trials containing 4763 patients. There were no serious adverse events occurring with bismuth therapy. There was no statistically significant difference detected in total adverse events with bismuth rrelative risk (RR) = 1.01; 95% CI: 0.87-1.16], specific individual adverse events, with the exception of dark stools (RR = 5.06; 95% CI: 1.59-16.12), or adverse events leading to withdrawal of therapy (RR = 0.86; 95% CI: 0.54-1.37). CONCLUSION: Bismuth for the treatment of H py/ori is safe and well-tolerated. The only adverse event occurring significantly more commonly was dark stools.展开更多

Various approaches have been proposed for FDD like Analytical redundancy(AR), Principal component analysis(PCA), Discrete event system(DES) model etc., in the literature. Performance of FDD schemes greatly depends on accuracy of the sensors which measure the system parameters.Due to various reasons like faults, communication errors etc.,sensors may occasionally miss or report erroneous values of some system parameters to FDD engine, resulting in measurement inconsistency of these parameters. Schemes like AR, PCA etc.,have mechanisms to handle measurement inconsistency, however,they are computationally heavy. DES based FDD techniques are widely used because of computational simplicity, but they cannot handle measurement inconsistency efficiently. Existing DES based schemes do not use Measurement inconsistent(MI)parameters for FDD. These parameters are not permanently unmeasurable or erroneous, so ignoring them may lead to weak diagnosis. To address this issue, we propose a Measurement inconsistent discrete event system(MIDES) framework, which uses MI parameters for FDD at the instances they are measured by the sensors. Otherwise, when they are unmeasurable or erroneously reported, the MIDES invokes an estimator diagnoser that predicts the state(s) the system is expected to be in, using the subsequent parameters measured by the other sensors. The efficacy of the proposed method is illustrated using a pumpvalve system. In addition, an MIDES based intrusion detection system has been developed for detection of rogue dynamic host configuration protocol(DHCP) server attack by mapping the attack to a fault in the DES framework.展开更多

The 802.11 Wi-Fi standard provides power management feature that allows stations(STAs) to enter into sleep state to preserve energy without any frame losses. After the STA wakes up, it sends a null data or PS-Poll frame to retrieve frame(s) buffered by the access point(AP), if any during its sleep period. An attacker can launch a power save denial of service(PS-DoS) attack on the sleeping STA(s) by transmitting a spoofed null data or PS-Poll frame(s) to retrieve the buffered frame(s) of the sleeping STA(s) from the AP causing frame losses for the targeted STA(s). Current approaches to prevent or detect the PS-DoS attack require encryption,change in protocol or installation of proprietary hardware. These solutions suffer from expensive setup, maintenance, scalability and deployment issues. The PS-DoS attack does not differ in semantics or statistics under normal and attack circumstances.So signature and anomaly based intrusion detection system(IDS) are unfit to detect the PS-DoS attack. In this paper we propose a timed IDS based on real time discrete event system(RTDES) for detecting PS-DoS attack. The proposed DES based IDS overcomes the drawbacks of existing systems and detects the PS-DoS attack with high accuracy and detection rate. The correctness of the RTDES based IDS is proved by experimenting all possible attack scenarios.展开更多

METHODS: We identified all the articles pu...AIM: To identify severe adverse events (SAEs) leading to treatment discontinuation that occur during antiviral therapy in hepatitis C virus (HCV)-infected cirrhotic patients. METHODS: We identified all the articles published prior to December 2011 in the PubMed, Medline, Lilacs, Scopus, Ovid, EMBASE, Cochrane and Medscape databases that presented these data in cirrhotic patients. These studies evaluated the rate of SAEs leading to discontinuation of standard care treatment: Pegylated interferon (PegIFN) alpha 2a (135-180 μg/wk) or PegIFN alpha 2b (1 or 1.5 μg/kg per week) and ribavirin (800-1200 mg/d). Patients with genotype 1 + 4 underwent treatment for 48 wk, whereas those with genotypes 2 + 3 were treated for 24 wk.RESULTS: We included 17 papers in this review, comprising of 1133 patients. Treatment was discontinued due to SAEs in 14.5% of the patients. The most common SAEs were: severe thrombocytopenia and/or neutropenia (23.2%), psychiatric disorders (15.5%), decompensation of liver cirrhosis (12.1%) and severe anemia (11.2%). The proportion of patients who needed to discontinue their therapy due to SAEs was significantly higher in patients with Child-Pugh class B and Cvs those with Child-Pugh class A: 22%vs 11.4% (P = 0.003). A similar discontinuation rate was found in cirrhotic patients treated with PegIFN alpha 2a and those treated with PegIFN alpha 2b, in combination with ribavirin: 14.2%vs 13.7% (P = 0.96). The overall sustained virological response rate in cirrhotic patients was 37% (95%CI: 33.5-43.1) but was significantly lower in patients with genotype 1 + 4 than in those with genotype 2 + 3: 20.5% (95%CI: 17.9-24.8) vs 56.5% (95%CI: 51.5-63.2), (P < 0.0001). CONCLUSION: Fourteen point five percent of HCV cirrhotic patients treated with PegIFN and ribavirin needed early discontinuation of therapy due to SAEs, the most common cause being hematological disorders.展开更多

Patients with cirrhosis undergoing ERCP are believed to have increased risk...AIM To investigate indications and outcomes of endoscopic retrograde cholangiopancreatography(ERCP) in cirrhotics, especially adverse events. Patients with cirrhosis undergoing ERCP are believed to have increased risk. However, there is a paucity of literature describing the indications and outcomes of ERCP procedures in patients with cirrhosis, especially focusing on adverse events.METHODS We performed a systematic appraisal of major literature databases, including PubMed and EMBASE, with a manual search of literature from their inception until April 2017.RESULTS A total of 6,505 patients from 15 studies were analyzed(male ratio 59%, mean age 59 years), 11% with alcoholic and 89% with non-alcoholic cirrhosis, with 56.2% Child-Pugh class A, and 43.8% class B or C. Indications for ERCP included choledocholithiasis 60.9%, biliary strictures 26.2%, gallstone pancreatitis 21.1% and cholangitis 15.5%. Types of interventions included endoscopic sphincterotomy 52.7%, biliary stenting 16.7% and biliary dilation 4.6%. Individual adverse events included hemorrhage in 4.58%(95%CI: 2.77-6.75%, I^2 = 85.9%), post-ERCP pancreatitis(PEP) in 3.68%(95%CI: 1.83-6.00%, I^2 = 89.5%), cholangitis in 1.93%(95%CI: 0.63-3.71%, I^2 = 87.1%) and perforation in 0.00%(95%CI: 0.00-0.23%, I^2 = 37.8%). Six studies were used for comparison of ERCPrelated complications in cirrhosis vs non-cirrhosis, which showed higher overall rates of complications in cirrhosis patients with pooled OR of 1.63(95%CI: 1.27-2.09, I2 = 65%): higher rates of hemorrhage with OR of 2.05(95%CI: 1.62-2.58, I^2 = 2.1%) and PEP with OR of 1.33(95%CI: 1.04-1.70, I2=65%), but similar cholangitis rates with OR of 1.23(95%CI: 0.67-2.26, I^2 = 44.3%).CONCLUSION There is an overall higher rate of adverse events related to ERCP in patients with cirrhosis, especially hemorrhage and PEP. A thorough risk/benefit assessment should be performed prior to undertaking ERCP in patients with cirrhosis.展开更多

However, we still have little knowledge about t...Changes in the sizes of precipitation events in the context of global climate change may have profound impacts on ecosystem productivity in arid and semiarid grasslands. However, we still have little knowledge about to what extent grassland productivity will respond to an individual precipitation event. In this study, we quantified the duration, the maximum, and the time-integrated amount of the response of daily gross primary productivity (GPP) to an individual precipitation event and their variations with different sizes of precipitation events in a typical temperate steppe in Inner Mongolia, China. Results showed that the duration of GPP-response (τ<sub>R</sub>) and the maximum absolute GPP-response (GPP<sub>max</sub>) increased linearly with the sizes of precipitation events (P<sub>es</sub>), driving a corresponding increase in time-integrated amount of the GPP-response (GPP<sub>total</sub>) because variations of GPPtotal were largely explained by τ<sub>R</sub> and GPP<sub>max</sub>. The relative contributions of these two parameters to GPP<sub>total</sub> were strongly P<sub>es</sub>-dependent. The GPP<sub>max</sub> contributed more to the variations of GPP<sub>total</sub> when P<sub>es</sub> was relatively small (<20 mm), whereas τ<sub>R</sub> was the main driver to the variations of GPP<sub>total</sub> when P<sub>es</sub> was relatively large. In addition, a threshold size of at least 5 mm of precipitation was required to induce a GPP-response for the temperate steppe in this study. Our work has important implications for the modeling community to obtain an advanced understanding of productivity-response of grassland ecosystems to altered precipitation regimes.展开更多

文摘Ecological stability is a core issue in ecological research and holds significant implications forhumanity. The increased frequency and intensity of drought and wet climate events resulting from climatechange pose a major threat to global ecological stability. Variations in stability among different ecosystemshave been confirmed, but it remains unclear whether there are differences in stability within the sameterrestrial vegetation ecosystem under the influence of climate events in different directions and intensities.China's grassland ecosystem includes most grassland types and is a good choice for studying this issue.This study used the Standardized Precipitation Evapotranspiration Index-12 (SPEI-12) to identify thedirections and intensities of different types of climate events, and based on Normalized DifferenceVegetation Index (NDVI), calculated the resistance and resilience of different grassland types for 30consecutive years from 1990 to 2019 (resistance and resilience are important indicators to measurestability). Based on a traditional regression model, standardized methods were integrated to analyze theimpacts of the intensity and duration of drought and wet events on vegetation stability. The resultsshowed that meadow steppe exhibited the highest stability, while alpine steppe and desert steppe had thelowest overall stability. The stability of typical steppe, alpine meadow, temperate meadow was at anintermediate level. Regarding the impact of the duration and intensity of climate events on vegetationecosystem stability for the same grassland type, the resilience of desert steppe during drought was mainlyaffected by the duration. In contrast, the impact of intensity was not significant. However, alpine steppewas mainly affected by intensity in wet environments, and duration had no significant impact. Ourconclusions can provide decision support for the future grassland ecosystem governance.

文摘The Jenkyns Event,more widely known as the Toarcian Oceanic Anoxic Event(T-OAE),is marked by globally distributed negative carbon-isotope excursions,widespread oxygen depletion,and large-scale organic carbon burial,which indicate major climate/environmental perturbations in Earth's surface systems during the Early Jurassic.Although extensive research has been conducted in European continental settings,particularly in the western peri-Tethys regions,the impacts of this event beyond Europe remains largely unexplored.Here,a multiapproach study including investigations into the sporepollen assemblages,pyrite framboids,clay minerals,total organic carbon(TOC)levels,and organic carbon isotope(δ13Corg)levels in a lacustrine borehole section(MED1)from the Yin'gen-Ejinaqi Basin,North China,provides evidence of the occurrence of the Jenkyns Event and its extensive sedimentary responses in the eastern Tethys terrestrial systems.Two distinct spore-pollen assemblages have been identified in MED1(drilling depth:982.4 m to 1267.5 m),with the Cycadopites-Protopinus-Osmundacidites assemblage in the lower part(1267.5 m to 1132.9 m)indicating a middle Early Jurassic age and the Classopollis assemblage in the upper part(1132.9 m to 985.7 m)suggesting a Toarcian age.Framboidal pyrite data suggest more anoxic conditions during the deposition of black mudstone and shale intercalations in the lower part of the Classopollis assemblage(1132.9 m to 1066.9 m),which combined with organic carbon enrichment and negativeδ13Corg excursions,are considered the paleoenvironmental response to the Jenkyns Event in the study area.Furthermore,the evolution of vegetation groups changed from plant groups characterized by bisaccate and cycad pollen,as well as fern spores,to vegetation groups represented by Cheirolepidiaceae pollen across the Jenkyns Event,as evidenced by sporepollen data,together with the clay mineral assemblage change characterized by a notable increase in illite at the expense of kaolinite,suggests that while a subtropical-temperate climate persisted,a change toward warmer and drier conditions most likely occurred in the early Toarcian in the study area.In contrast to the humidification evidenced in many coastal settings,this aridification trend in the Yin'gen-Ejinaqi Basin aligns with the conditions in many inland areas.It is hypothesized that the underlying cause of these divergent changes may be linked to certain patterns of spatially variable water availability on land,potentially driven by extremified hydrological conditions.

文摘The High-energy Fragment Separator(HFRS),which is currently under construction,is a leading international radioactive beam device.Multiple sets of position-sensitive twin time projection chamber(TPC)detectors are distributed on HFRS for particle identification and beam monitoring.The twin TPCs'readout electronics system operates in a trigger-less mode due to its high counting rate,leading to a challenge of handling large amounts of data.To address this problem,we introduced an event-building algorithm.This algorithm employs a hierarchical processing strategy to compress data during transmission and aggregation.In addition,it reconstructs twin TPCs'events online and stores only the reconstructed particle information,which significantly reduces the burden on data transmission and storage resources.Simulation studies demonstrated that the algorithm accurately matches twin TPCs'events and reduces more than 98%of the data volume at a counting rate of 500 kHz/channel.

文摘Convolutional neural networks(CNNs) exhibit excellent performance in the areas of image recognition and object detection, which can enhance the intelligence level of spacecraft. However, in aerospace, energetic particles, such as heavy ions, protons, and alpha particles, can induce single event effects(SEEs) that lead CNNs to malfunction and can significantly impact the reliability of a CNN system. In this paper, the MNIST CNN system was constructed based on a 28 nm systemon-chip(SoC), and then an alpha particle irradiation experiment and fault injection were applied to evaluate the SEE of the CNN system. Various types of soft errors in the CNN system have been detected, and the SEE cross sections have been calculated. Furthermore, the mechanisms behind some soft errors have been explained. This research will provide technical support for the design of radiation-resistant artificial intelligence chips.

文摘BACKGROUND The incidence of chronic kidney disease among patients with diabetes mellitus(DM)remains a global concern.Long-term obesity is known to possibly influence the development of type 2 diabetes mellitus.However,no previous meta-analysis has assessed the effects of body mass index(BMI)on adverse kidney events in patients with DM.AIM To determine the impact of BMI on adverse kidney events in patients with DM.METHODS A systematic literature search was performed on the PubMed,ISI Web of Science,Scopus,Ovid,Google Scholar,EMBASE,and BMJ databases.We included trials with the following characteristics:(1)Type of study:Prospective,retrospective,randomized,and non-randomized in design;(2)participants:Restricted to patients with DM aged≥18 years;(3)intervention:No intervention;and(4)kidney adverse events:Onset of diabetic kidney disease[estimated glomerular filtration rate(eGFR)of<60 mL/min/1.73 m2 and/or microalbuminuria value of≥30 mg/g Cr],serum creatinine increase of more than double the baseline or end-stage renal disease(eGFR<15 mL/min/1.73 m2 or dialysis),or death.RESULTS Overall,11 studies involving 801 patients with DM were included.High BMI(≥25 kg/m2)was significantly associated with higher blood pressure(BP)[systolic BP by 0.20,95%confidence interval(CI):0.15–0.25,P<0.00001;diastolic BP by 0.21 mmHg,95%CI:0.04–0.37,P=0.010],serum albumin,triglycerides[standard mean difference(SMD)=0.35,95%CI:0.29–0.41,P<0.00001],low-density lipoprotein(SMD=0.12,95%CI:0.04–0.20,P=0.030),and lower high-density lipoprotein(SMD=–0.36,95%CI:–0.51 to–0.21,P<0.00001)in patients with DM compared with those with low BMIs(<25 kg/m2).Our analysis showed that high BMI was associated with a higher risk ratio of adverse kidney events than low BMI(RR:1.22,95%CI:1.01–1.43,P=0.036).CONCLUSION The present analysis suggested that high BMI was a risk factor for adverse kidney events in patients with DM.
文摘BACKGROUND Coagulopathy and thromboembolic events are associated with poor outcomes in coronavirus disease 2019(COVID-19)patients.There is conflicting evidence on the effects of chronic anticoagulation on mortality and severity of COVID-19 disease.AIM To summarize the body of evidence on the effects of pre-hospital anticoagulation on outcomes in COVID-19 patients.METHODS A Literature search was performed on LitCovid PubMed,WHO,and Scopus databases from inception(December 2019)till June 2023 for original studies reporting an association between prior use of anticoagulants and patient outcomes in adults with COVID-19.The primary outcome was the risk of thromboembolic events in COVID-19 patients taking anticoagulants.Secondary outcomes included COVID-19 disease severity,in terms of intensive care unit admission or invasive mechanical ventilation/intubation requirement in patients hospitalized with COVID-19 infection,and mortality.The random effects models were used to calculate crude and adjusted odds ratios(aORs)with 95%confidence intervals(95%CIs).RESULTS Forty-six observational studies met our inclusion criteria.The unadjusted analysis found no association between prior anticoagulation and thromboembolic event risk[n=43851,9 studies,odds ratio(OR)=0.67(0.22,2.07);P=0.49;I2=95%].The association between prior anticoagulation and disease severity was non-significant[n=186782;22 studies,OR=1.08(0.78,1.49);P=0.64;I2=89%].However,pre-hospital anticoagulation significantly increased all-cause mortality risk[n=207292;35 studies,OR=1.72(1.37,2.17);P<0.00001;I2=93%].Pooling adjusted estimates revealed a statistically non-significant association between pre-hospital anticoagulation and thromboembolic event risk[aOR=0.87(0.42,1.80);P=0.71],mortality[aOR=0.94(0.84,1.05);P=0.31],and disease severity[aOR=0.96(0.72,1.26);P=0.76].CONCLUSION Prehospital anticoagulation was not significantly associated with reduced risk of thromboembolic events,improved survival,and lower disease severity in COVID-19 patients.
文摘AIM: To assess the safety of bismuth used in Helicobacter pylori (H pylorl) eradication therapy regimens. METHODS: We conducted a systematic review and meta-analysis. MEDLINE and EMBASE were searched (up to October 2007) to identify randomised controlled tri- als comparing bismuth with placebo or no treatment, or bismuth salts in combination with antibiotics as part of eradication therapy with the same dose and duration of antibiotics alone or, in combination, with acid suppresion. Total numbers of adverse events were recorded. Data were pooled and expressed as relative risks with 95% confidence intervals (CI).RESULTS: We identified 35 randomised controlled trials containing 4763 patients. There were no serious adverse events occurring with bismuth therapy. There was no statistically significant difference detected in total adverse events with bismuth rrelative risk (RR) = 1.01; 95% CI: 0.87-1.16], specific individual adverse events, with the exception of dark stools (RR = 5.06; 95% CI: 1.59-16.12), or adverse events leading to withdrawal of therapy (RR = 0.86; 95% CI: 0.54-1.37). CONCLUSION: Bismuth for the treatment of H py/ori is safe and well-tolerated. The only adverse event occurring significantly more commonly was dark stools.

文摘This paper focuses on studying the Poisson theory and the integration method of a Birkhoffian system in the event space. The Birkhoff's equations in the event space are given. The Poisson theory of the Birkhoffian system in the event space is established. The definition of the Jacobi last multiplier of the system is given, and the relation between the Jacobi last multiplier and the first integrals of the system is discussed. The researches show that for a Birkhoffian system in the event space, whose configuration is determined by (2n + 1) Birkhoff's variables, the solution of the system can be found by the Jacobi last multiplier if 2n first integrals are known. An example is given to illustrate the application of the results.

文摘Dear editor,Recently,researchers have obtained many new results about the multi-agent systems(MASs)[1]-[3].In[1],the fixed-time cooperative control(FTCC)algorithm of linear MASs with matched disturbances was proposed.The nonholonomic chained-form dynamics case was considered in[2].In[3],the output tracking problem with data packet dropout was solved for high-order MASs.Moreover,delay frequently occurs because of the non-ideal data transmission[4],and the corresponding FTCC algorithm of MASs with delay was given in[5].

文摘Fault detection and diagnosis(FDD) facilitates reliable operation of systems. Various approaches have been proposed for FDD like Analytical redundancy(AR), Principal component analysis(PCA), Discrete event system(DES) model etc., in the literature. Performance of FDD schemes greatly depends on accuracy of the sensors which measure the system parameters.Due to various reasons like faults, communication errors etc.,sensors may occasionally miss or report erroneous values of some system parameters to FDD engine, resulting in measurement inconsistency of these parameters. Schemes like AR, PCA etc.,have mechanisms to handle measurement inconsistency, however,they are computationally heavy. DES based FDD techniques are widely used because of computational simplicity, but they cannot handle measurement inconsistency efficiently. Existing DES based schemes do not use Measurement inconsistent(MI)parameters for FDD. These parameters are not permanently unmeasurable or erroneous, so ignoring them may lead to weak diagnosis. To address this issue, we propose a Measurement inconsistent discrete event system(MIDES) framework, which uses MI parameters for FDD at the instances they are measured by the sensors. Otherwise, when they are unmeasurable or erroneously reported, the MIDES invokes an estimator diagnoser that predicts the state(s) the system is expected to be in, using the subsequent parameters measured by the other sensors. The efficacy of the proposed method is illustrated using a pumpvalve system. In addition, an MIDES based intrusion detection system has been developed for detection of rogue dynamic host configuration protocol(DHCP) server attack by mapping the attack to a fault in the DES framework.

文摘Wi-Fi devices have limited battery life because of which conserving battery life is imperative. The 802.11 Wi-Fi standard provides power management feature that allows stations(STAs) to enter into sleep state to preserve energy without any frame losses. After the STA wakes up, it sends a null data or PS-Poll frame to retrieve frame(s) buffered by the access point(AP), if any during its sleep period. An attacker can launch a power save denial of service(PS-DoS) attack on the sleeping STA(s) by transmitting a spoofed null data or PS-Poll frame(s) to retrieve the buffered frame(s) of the sleeping STA(s) from the AP causing frame losses for the targeted STA(s). Current approaches to prevent or detect the PS-DoS attack require encryption,change in protocol or installation of proprietary hardware. These solutions suffer from expensive setup, maintenance, scalability and deployment issues. The PS-DoS attack does not differ in semantics or statistics under normal and attack circumstances.So signature and anomaly based intrusion detection system(IDS) are unfit to detect the PS-DoS attack. In this paper we propose a timed IDS based on real time discrete event system(RTDES) for detecting PS-DoS attack. The proposed DES based IDS overcomes the drawbacks of existing systems and detects the PS-DoS attack with high accuracy and detection rate. The correctness of the RTDES based IDS is proved by experimenting all possible attack scenarios.
文摘AIM: To identify severe adverse events (SAEs) leading to treatment discontinuation that occur during antiviral therapy in hepatitis C virus (HCV)-infected cirrhotic patients. METHODS: We identified all the articles published prior to December 2011 in the PubMed, Medline, Lilacs, Scopus, Ovid, EMBASE, Cochrane and Medscape databases that presented these data in cirrhotic patients. These studies evaluated the rate of SAEs leading to discontinuation of standard care treatment: Pegylated interferon (PegIFN) alpha 2a (135-180 μg/wk) or PegIFN alpha 2b (1 or 1.5 μg/kg per week) and ribavirin (800-1200 mg/d). Patients with genotype 1 + 4 underwent treatment for 48 wk, whereas those with genotypes 2 + 3 were treated for 24 wk.RESULTS: We included 17 papers in this review, comprising of 1133 patients. Treatment was discontinued due to SAEs in 14.5% of the patients. The most common SAEs were: severe thrombocytopenia and/or neutropenia (23.2%), psychiatric disorders (15.5%), decompensation of liver cirrhosis (12.1%) and severe anemia (11.2%). The proportion of patients who needed to discontinue their therapy due to SAEs was significantly higher in patients with Child-Pugh class B and Cvs those with Child-Pugh class A: 22%vs 11.4% (P = 0.003). A similar discontinuation rate was found in cirrhotic patients treated with PegIFN alpha 2a and those treated with PegIFN alpha 2b, in combination with ribavirin: 14.2%vs 13.7% (P = 0.96). The overall sustained virological response rate in cirrhotic patients was 37% (95%CI: 33.5-43.1) but was significantly lower in patients with genotype 1 + 4 than in those with genotype 2 + 3: 20.5% (95%CI: 17.9-24.8) vs 56.5% (95%CI: 51.5-63.2), (P < 0.0001). CONCLUSION: Fourteen point five percent of HCV cirrhotic patients treated with PegIFN and ribavirin needed early discontinuation of therapy due to SAEs, the most common cause being hematological disorders.

文摘This paper is devoted to studying the conformal invariance and Noether symmetry and Lie symmetry of a holonomic mechanical system in event space. The definition of the conformal invariance and the corresponding conformal factors of the holonomic system in event space are given. By investigating the relation between the conformal invariance and the Noether symmetry and the Lie symmetry, expressions of conformal factors of the system under these circumstances are obtained, and the Noether conserved quantity and the Hojman conserved quantity directly derived from the conformal invariance are given. Two examples are given to illustrate the application of the results.
文摘AIM To investigate indications and outcomes of endoscopic retrograde cholangiopancreatography(ERCP) in cirrhotics, especially adverse events. Patients with cirrhosis undergoing ERCP are believed to have increased risk. However, there is a paucity of literature describing the indications and outcomes of ERCP procedures in patients with cirrhosis, especially focusing on adverse events.METHODS We performed a systematic appraisal of major literature databases, including PubMed and EMBASE, with a manual search of literature from their inception until April 2017.RESULTS A total of 6,505 patients from 15 studies were analyzed(male ratio 59%, mean age 59 years), 11% with alcoholic and 89% with non-alcoholic cirrhosis, with 56.2% Child-Pugh class A, and 43.8% class B or C. Indications for ERCP included choledocholithiasis 60.9%, biliary strictures 26.2%, gallstone pancreatitis 21.1% and cholangitis 15.5%. Types of interventions included endoscopic sphincterotomy 52.7%, biliary stenting 16.7% and biliary dilation 4.6%. Individual adverse events included hemorrhage in 4.58%(95%CI: 2.77-6.75%, I^2 = 85.9%), post-ERCP pancreatitis(PEP) in 3.68%(95%CI: 1.83-6.00%, I^2 = 89.5%), cholangitis in 1.93%(95%CI: 0.63-3.71%, I^2 = 87.1%) and perforation in 0.00%(95%CI: 0.00-0.23%, I^2 = 37.8%). Six studies were used for comparison of ERCPrelated complications in cirrhosis vs non-cirrhosis, which showed higher overall rates of complications in cirrhosis patients with pooled OR of 1.63(95%CI: 1.27-2.09, I2 = 65%): higher rates of hemorrhage with OR of 2.05(95%CI: 1.62-2.58, I^2 = 2.1%) and PEP with OR of 1.33(95%CI: 1.04-1.70, I2=65%), but similar cholangitis rates with OR of 1.23(95%CI: 0.67-2.26, I^2 = 44.3%).CONCLUSION There is an overall higher rate of adverse events related to ERCP in patients with cirrhosis, especially hemorrhage and PEP. A thorough risk/benefit assessment should be performed prior to undertaking ERCP in patients with cirrhosis.

文摘In this paper, the conservation laws of generalized Birkhoff system in event space are studied by using the method of integrating factors. Firstly, the generalized Pfaff-Birkhoff principle and the generalized Birkhoff equations are established, and the definition of the integrating factors for the system is given. Secondly, based on the concept of integrating factors, the conservation theorems and their inverse for the generalized Birkhoff system in the event space are presented in detail, and the relation between the conservation laws and the integrating factors of the system is obtained and the generalized Killing equations for the determination of the integrating factors are given. Finally, an example is given to illustrate the application of the results.

文摘The propagation of single-event effects(SEEs)on a Xilinx Zynq-7000 system on chip(SoC)was inves-tigated using heavy-ion microbeam radiation.The irradia-tion results reveal several functional blocks’sensitivity locations and cross sections,for instance,the arithmetic logic unit,register,D-cache,and peripheral,while irradi-ating the on-chip memory(OCM)region.Moreover,event tree analysis was executed based on the obtained microbeam irradiation results.This study quantitatively assesses the probabilities of SEE propagation from the OCM to other blocks in the SoC.

文摘Changes in the sizes of precipitation events in the context of global climate change may have profound impacts on ecosystem productivity in arid and semiarid grasslands. However, we still have little knowledge about to what extent grassland productivity will respond to an individual precipitation event. In this study, we quantified the duration, the maximum, and the time-integrated amount of the response of daily gross primary productivity (GPP) to an individual precipitation event and their variations with different sizes of precipitation events in a typical temperate steppe in Inner Mongolia, China. Results showed that the duration of GPP-response (τ<sub>R</sub>) and the maximum absolute GPP-response (GPP<sub>max</sub>) increased linearly with the sizes of precipitation events (P<sub>es</sub>), driving a corresponding increase in time-integrated amount of the GPP-response (GPP<sub>total</sub>) because variations of GPPtotal were largely explained by τ<sub>R</sub> and GPP<sub>max</sub>. The relative contributions of these two parameters to GPP<sub>total</sub> were strongly P<sub>es</sub>-dependent. The GPP<sub>max</sub> contributed more to the variations of GPP<sub>total</sub> when P<sub>es</sub> was relatively small (<20 mm), whereas τ<sub>R</sub> was the main driver to the variations of GPP<sub>total</sub> when P<sub>es</sub> was relatively large.
