Shared decision making in rural general practices:a qualitative exploration of older rural South Australians'perceived involvement in clinical consultations with doctors

Background:Shared decision-making(SDM)implementation is a priority for Australian health systems,including general practices but it remains complex for specific groups like older rural Australians.We initiated a qualitative study with older rural Australians to explore barriers to and facilitators of SDM in local general practices.Methods:We conducted a patient-oriented research,partnering with older rural Australians,families,and health service providers in research design.Participants who visited general practices were purposively sampled from five small rural towns in South Australia.A semi-structured interview guide was used for interviews and reflexive thematic coding was conducted.Results:Telephone interviews were held with 27 participants.Four themes were identified around older rural adults’involvement in SDM:(1)Understanding of"patient involvement";(2)Positive and negative outcomes;(3)Barriers to SDM;and(4)Facilitators to SDM.Understanding of patient involvement in SDM considerably varied among participants,with some reporting their involvement was contingent on the“opportunity to ask questions”and the“treatment choices”offered to them.Alongside the opportunity for involvement,barriers such as avoidance of cultural care and a lack of continuity of care are new findings.Challenges encountered in SDM implementation also included resource constraints and time limitations in general practices.Rural knowledge of general practitioners and technology integration in consultations were viewed as potential enablers..Conclusion:Adequate resources and well-defined guidelines about the process should accompany the implementation of SDM in rural general practices of South Australia.Innovative strategies by general practitioners promoting health literacy and culturally-tailored communication approaches could increase older rural Australians'involvement in general.

Exploration of the Integration of Curriculum Ideology and Politics into the Educational Pathway of Standardized Training for General Practice Residents

In recent years,the national higher education reform has proposed the concept of curriculum ideological and political education as a major element.This integration into the standardized training of general practice residents(commonly referred to as general resident training)is an unavoidable trend in the teaching field.Based on the actual situation of our unit at The First Affiliated Hospital of Xi’an Medical University,this paper emphasizes the necessity of incorporating ideological and political education into the general resident training curriculum,despite the absence of pertinent practical experience in how to effectively incorporate such courses into comprehensive residential training.In our hospital,we must assess the ideological and political education in the curriculum of general resident training,proposing a scientific,reasonable ideological and political education concept system for general practice courses,integrating ideological and political education with general practice resident training,and strengthening the team of ideological and political education physicians to teach them to carry out such education,this would foster enthusiasm and initiative.Strengthening admission education and training for general practice resident training,as well as regularly hosting symposiums to share ideas and understand the mission of doctors can provide effective experience for ideological and political education in general resident training courses.

The course ROS Robot Programming Practice is designed to equip students with both the foundational principles and practical applications of the Robot Operating System(ROS)through structured instruction and hands-on exercises.This paper summarizes the course’s pedagogical approach and the insights gained from its implementation,focusing on key areas such as the practical teaching platform,instructional system,teaching methods,and evaluation mechanisms.The practical teaching platform incorporates robotic arms and mobile robots,along with commonly used sensors,to engage students and stimulate their interest in robotics.A well-structured teaching system is employed,guiding students through a series of experiments-ranging from basic tasks to modeling and simulation experiments,sensor experiments,mobile robot tasks,robotic arm exercises,and comprehensive project practices.Regarding teaching methods,a blended learning approach and progressive instructional model are utilized to ensure active student participation.This approach follows a logical progression,starting with fundamental ROS knowledge,advancing to robot applications,and culminating in comprehensive project-based practice.In terms of evaluation,a dual approach combining process and outcome assessments is employed,ensuring that students’performance is evaluated comprehensively through various metrics.

Exploration of Practice Teaching Reform Based on Outcome-Based Education Concept:Taking Environmental Design Major as an Example

The concept of OBE(outcome-based education)is more focused on designing and implementing teaching activities around expected learning outcomes than the traditional teaching model and enhancing the quality of education through clear and measurable learning outcomes,which has led to profound changes in the field of education.However,environmental design practice teaching faces a series of challenges in the implementation of the OBE concept,including the complexity of the assessment system,personalized learning and creative learning,and over-reliance on quantitative assessment.Therefore,in order to further deepen the professionalization of the OBE concept,we have conducted research on the design education practice models at home and abroad,explored the practice teaching models and their applications such as project system,studio model,practical internships,and technology-driven interdisciplinary integration,and took the practice innovative teaching models of Stanford d.school and Rhode Island School of Design(RISD)as examples to explore the characteristics of the curricula,cultivation modes,and the development of the environmental design professional practice teaching model.The characteristics of OBE and RISD are discussed to provide a global vision and local contextual reference for the practice teaching reform of environmental design majors.

Exploration and Practice of Nitrogen Addition Process for LF Refining Ladle Bottom Blowing Nitrogen Steel Liquid

This article discusses and analyzes the law of nitrogen increase in liquid steel and the main factors affect-ing the nitrogen increasing of molten steel,through the way of adding nitrogen to molten steel by bottom blowing nitrogen gas in LF refining process.It is considered that the main factors affecting the nitrogen increasing instability of molten steel are the initial temperature of LF refining,nitrogen relative element,surface active elements[O]and[S]of steel liquid,and bottom blowing rate of ladle.The large-scale production practice shows that T[O]not more than 50×10-6 and[S]is not more than 0.020 in LF refining at the initial temperature of not less than 1570.The liquid steel nitrogen enrichment test is carried out by ladle bottom blowing nitrogen gas after 20 min of refining,the flow rate is set as(6.0~7.0)NL/min per ton,and it is turned to 2 NL/min at 6 min before the end of refining,the nitrogen increasing rate of liquid steel is basically stable at(5~6)×10-6 per minute.

As an introductory course for the emerging major of big data management and application,“Introduction to Big Data”has not yet formed a curriculum standard and implementation plan that is widely accepted and used by everyone.To this end,we discuss some of our explorations and attempts in the construction and teaching process of big data courses for the major of big data management and application from the perspective of course planning,course implementation,and course summary.After interviews with students and feedback from questionnaires,students are highly satisfied with some of the teaching measures and programs currently adopted.

Practice patterns among ophthalmic surgeons in treating concomitant oculoplastic conditions in preoperative period:A questionnaire-based study

BACKGROUND Addressing oculoplastic conditions in the preoperative period ensures both the safety and functional success of any ophthalmic procedure.Some oculoplastic conditions,like nasolacrimal duct obstruction,have been extensively studied,whereas others,like eyelid malposition and thyroid eye disease,have received minimal or no research.AIM To investigate the current practice patterns among ophthalmologists while treating concomitant oculoplastic conditions before any subspecialty ophthalmic intervention.METHODS A cross-sectional survey was disseminated among ophthalmologists all over India.The survey included questions related to pre-operative evaluation,anaesthetic and surgical techniques preferred,post-operative care,the use of adjunctive therapies,and patient follow-up patterns.RESULTS A total of 180 ophthalmologists responded to the survey.Most practitioners(89%)felt that the ROPLAS test was sufficient during pre-operative evaluation before any subspecialty surgery was advised.The most common surgical techniques employed were lacrimal drainage procedures(Dacryocystorhinostomy)(63.3%),eyelid malposition repair(36.9%),and ptosis repair(58.7%).Post-operatively,47.7%of respondents emphasized that at least a 4-week gap should be maintained after lacrimal drainage procedures and eyelid surgeries.Sixty-seven percent of ophthalmologists felt that topical anaesthetic procedures should be preferred while performing ocular surgeries in thyroid eye disease patients.CONCLUSION Approximately 50%of ophthalmologists handle prevalent oculoplastic issues themselves,seeking the expertise of an oculoplastic surgeon under particular conditions.Many ophthalmologists still favor using ROPLAS as a preliminary screening method before proceeding with cataract surgery.Eyelid conditions and thyroid eye disease are not as commonly addressed before subspecialty procedures compared to issues like nasolacrimal duct obstruction and periocular infections.

Machine learning applications in healthcare clinical practice and research

Machine learning(ML)is a type of artificial intelligence that assists computers in the acquisition of knowledge through data analysis,thus creating machines that can complete tasks otherwise requiring human intelligence.Among its various applications,it has proven groundbreaking in healthcare as well,both in clinical practice and research.In this editorial,we succinctly introduce ML applications and present a study,featured in the latest issue of the World Journal of Clinical Cases.The authors of this study conducted an analysis using both multiple linear regression(MLR)and ML methods to investigate the significant factors that may impact the estimated glomerular filtration rate in healthy women with and without non-alcoholic fatty liver disease(NAFLD).Their results implicated age as the most important determining factor in both groups,followed by lactic dehydrogenase,uric acid,forced expiratory volume in one second,and albumin.In addition,for the NAFLD-group,the 5th and 6th most important impact factors were thyroid-stimulating hormone and systolic blood pressure,as compared to plasma calcium and body fat for the NAFLD+group.However,the study's distinctive contribution lies in its adoption of ML methodologies,showcasing their superiority over traditional statistical approaches(herein MLR),thereby highlighting the potential of ML to represent an invaluable advanced adjunct tool in clinical practice and research.

Exploration and Application of the Flipped Classroom Based on Outcome-Oriented Educational Principles in Vocational Nursing Practical Training

Purpose:To explore the practical effects of using the flipped classroom approach based on outcome-oriented educational concepts in nursing practical training at vocational colleges.Methods:From February 2023 to February 2024,a total of 62 nursing trainees from our school were sampled and randomly divided into a study group and a control group using a randomized controlled trial design,with 31 students in each group.The control group received conventional training,while the study group received practical training based on the outcome-oriented educational concept in flipped classrooms.Comparison was made between the two groups on assessment scores,teaching satisfaction,and self-directed learning ability ratings.Results:Statistical comparisons showed that the study group’s assessment scores,teaching satisfaction,and self-directed learning ability ratings were significantly higher than those of the control group(P<0.05).Conclusion:The flipped classroom model based on outcome-oriented educational concepts in nursing practical training at vocational colleges can improve students’assessment scores and self-directed learning abilities,and enhance teaching satisfaction,demonstrating potential for wider application and dissemination.

Exploration of Practical Teaching of Resource Recycling Science and Engineering Major Based on the Training of Applied Talents

Resource recycling science and engineering is a new interdisciplinary subject.In order to meet the needs of the development of the circular economy,it is highly necessary to improve students’innovative abilities and practical skills.This paper analyzes the problems existing in the practical teaching of this major and puts forward some approaches to the construction of practical teaching of resource-based major from the aspects of content system construction of practical teaching of resource recycling major,deep cooperation between schools and enterprises,construction of dual-teacher teams,and integration teaching of industry-university-research competition.It has effectively improved the training quality of applied talents in the resource recycling major.
