Aim:Traditional facelift techniques rely on pulling. They approach the superficial or intermediate layers where the facial nerves and muscles are located, increasing the risk of facial nerve injury. They approach the central oval from the periphery and produce unnatural vectors of pull and aesthetic results.Alternative techniques that work on the subperiosteal plane using endoscopic techniques are described.Modern concepts of volume augmentation, beautification and rejuvenation of the facial expression are an inherent part of such techniques,or can be easily integrated. Methods:The central oval is approached via four small scalp incisions and additional intraoral, upper gingivo-buccal incisions. The interconnected frontal subperiosteal, temporal subfascial and midface subperiosteal areas are lifted, imbricated and suspended sequentially. The brow/forehead is suspended to the skull using cortical screws. The midface and lower periorbita are suspended to the fascia of the temporal muscle. The buccal fat pad is used to enhance the ogee line of the midface.Other three-dimensional volumetric maneuvers can easily be applied. In this setting, upper and lower lid blepharoplasties become more straightforward, skin only procedures.Actinic or nicotine damaged skin can be treated with lasers, peels or fluidified fat grafting in the same setting. The excess skin on the lower face and neck can be redrapped with standard cervicofacial techniques.Deep subplatysmal cervicoplasty can be done concomitantly,or at another time to complete comprehensive rejuvenation. Results:The procedures described herein has been performed in 824 patients with excellent aesthetic results and low complication rate. The average rate of rejuvenation was 18 years. ;Conclusion:These combination techniques are called Biplanar Endoscopic Assisted Mask and Triplanar Endoscopic Assisted Mask facial rejuvenation. They are advanced techniques of facial rejuvenation that provide comprehensive, natural, long lasting results. 展开更多

Background:More and more consumers are paying attention to skin rejuvenation. However, there is a lack of a non-invasive and efficient solution. Objective:To evaluate the efficacy of a trinity permeation synergism(TPS),which consists of a firming essence,an atomizer and a photoelectric penetrator, for facial anti-aging efficacy.Material and methods:In this work,in vitro cell experiments and human efficacy study were used to evaluate the firming and anti-wrinkle effects.Cell experiments were used to verify the effect of the firming essence on the cell proliferation, migration, and anti-inflammation in keratinocytes(HaCaT),and on the gene expression levels of type I and type III collagen(Col-1 and Col-3)and type I matrix metalloproteinase(MMP-1)in human skin fibroblasts(HSF).After in vitro test,60 women aged 35–60 years were enrolled in the randomized test,of which 30 subjects were randomly selected to be the experimental group and treated with the TPS system, while the left 30 subjects were treated with the firming essence only considered as control. After 28 days, skin elasticity, skin redness value, and skin wrinkles were measured to evaluate the efficacy of the TPS system. Results:Cell experiments showed that the firming essence can significantly improve the proliferation and the migration of HaCaT cells. It also promoted the expression level of Col-1 and Col-3 gene, and inhibited the expression level of MMP-1 gene in HSF cells. After confirming the efficacy of firming essence, the efficacy benefit of the TPS was further studied. The 28-day tests show that combined use firming essence with atomizer and penetrator can significantly increase skin elasticity, reduce skin hemoglobin value and skin wrinkles on Day 28.Moreover,all the mentioned improvements are significantly better than that in the control group. Conclusion:Through efficient delivery in the whole process,TPS boosts the efficacy of active components in the firming essence.TPS offers an efficient,non-invasive,and convenient way for enhanced facial rejuvenation efficacy. 展开更多

Background Ears play an important role in facial profiles.Lying ears are not perceived favorably in Asian cultures;therefore, surgical procedures are implemented to adjust this defect. However, surgery has complications and requires recovery time.We introduced a hyaluronic acid(HA)injection technique to aesthetically improve ear prominence. Methods This retrospective study examined 25 Chinese patients aged 20-47 years with congenital lying ears who received HA injections in the auricular base at Shanghai Ninth People’s Hospital and Hangzhou Yijie Medical Cosmetology Clinic between November 2019 and December 2022.The average injection volume was 6.32±1.22 mL in the auricular base unilaterally, and the follow-up time ranged from 6 to 24 months.Morphological measurement data were recorded.Two-dimensional(2D)and three-dimensional(3D)images were obtained to evaluate the effects. Results All patients showed improvements in ear prominence. The cranioauricular angle(CAA)increased from 22.24°±4.53°to 65.90°±5.38°immediately post-injection and was maintained at 49.18°±4.74°6 months later. The ear projection(EP)increased from 14.75±2.72 mm to 26.26±2.94 mm immediately post-injection and was 22.51±2.68 mm after 6 months. No serious complications were noted. Conclusion HA injection in the auricular base effectively corrected the lying ears and aesthetically optimized the facial profile. It also had a face-lift effect that made the face look younger and more compact. The injection procedure was minimally invasive and required no recovery period. The effect was maintained for a long time and was satisfactory. 展开更多

Since the advent of facelift surgery, there has been a progressive evolution in tech-nique. Methods of dissection trended towards progressively aggressive surgery with deeper dissection for repositioning of ptotic facial tissues. In recent decades, the pendulum has swung towards more minimally invasive options. Likewise, there has been a shift in focus from repo-sitioning alone to the addition of volumization for facial rejuvenation. The techniques in this article are reviewed in a chronologic fashion with a focus on historical development as well as brief discussion on efficacy in relation to the other existing options. There is currently no gold standard technique with a plethora of options with comparable efficacy. There is controversy over which approach is optimal and future research is needed to better delineate optimal treatment options, which may vary based on the patient.
