This paper will introduce the clinical experiences of Professor Zheng-pingBai in treating gastric cancer.In his diagnosis and treatment of this disease,Professor Bai advocates the integration of disease,symptom,and syndrome differentiation,pointing out that gastric cancer is caused by the accumulation of phlegm-dampness and blood stasis in the stomach due to qi stagnation.Therefore,the basic pathogenic mechanism involves the loss of harmonious descent in the stomach.This implies that treatment should prioritize qi activity by calming the stomach and suppressing the abnormal ascent of qi,as well as emphasizing the elimination of phlegm and stasis,and this has achieved significant therapeutic efficacy.

Experience inheritance from famous specialists based on real- world clinical research paradigm of traditional Chinese medicine

The current modes of experience inheritance from famous specialists in traditional Chinese medicine （TCM） include master and disciple, literature review, clinical-epidemiology-based clinical research observation, and analysis and data mining via computer and database technologies. Each mode has its advantages and disadvantages. However, a scientific and instructive experience inheritance mode has not been developed. The advent of the big data era as well as the formation and practice accumulation of the TCM clinical research paradigm in the real world have provided new perspectives, techniques, and methods for inheriting experience from famous TCM specialists. Through continuous exploration and practice, the research group proposes the innovation research mode based on the real-world TCM clinical research paradigm, which involves the inheritance and innovation of the existing modes. This mode is formulated in line with its own development regularity of TCM and is expected to become the main mode of experience inheritance in the clinical field.

李素云从“聚于胃,关于肺”辨治胃食管反流性咳嗽思想探析

胃食管反流性咳嗽(gastroesophageal reflux cough,GERC)属于呼吸系统疾病,是慢性咳嗽的三大类型之一,多呈间断发作,缠绵难愈,影响生活质量。李素云教授临证多年,根据“五脏六腑皆令人咳”提出治病求本;病机方面,基于“聚于胃,关于肺”理论,提出肺气上逆为标,肝胃气逆为本。临证治疗该病主张不离于肺胃,亦不止于肺胃,以脏腑辨证为纲,审证求因,以“泄、清、补、疏”四字为法,每获佳效。该文就“聚于胃,关于肺”理论在该病中的运用及其辨治GERC的经验进行探讨,以期对该病的临床治疗思路有所补充。

马丽俐教授从脾论治瘾疹思路与用药经验

[目的]总结马丽俐教授从脾论治瘾疹的经验,为临床提供参考。[方法]通过临证收集和记录门诊病历,整理马教授从脾辨证论治瘾疹的经验,总结本病的病因病机及遣方用药规律,并举医案一则加以佐证。[结果]马教授认为,瘾疹的核心病机为脾虚失运,感邪致病,总属正虚邪中,本虚标实,临证常见脾虚营卫失和、脾虚湿困、脾虚肝郁。治疗上马教授灵活运用补运二法以健脾体,以调和营卫、运脾化湿、补土疏木分治,同时注重因人制宜,个体化论治。所举验案中患者年逾花甲,皮疹反复,诊断其为脾虚湿蕴之瘾疹,治宜运脾祛湿、祛邪止痒,后期兼扶正补虚,随症加减,用药灵活,疗效显著。[结论]马教授秉承以脾为本的辨治思想施治瘾疹,疗效明显,其经验值得借鉴和推广。

王友仁按动疗法治疗肩关节周围炎经验总结

按动疗法是北京按摩医院的特色疗法,操作上重视“整体观念、筋骨并重、按动结合、医患配合、平衡阴阳”等,对肩关节周围炎等筋伤类疾病有很好的治疗效果,于2021年被评为第五批北京市级非物质文化遗产。北京按摩医院王友仁主任医师是第六批全国老中医药专家学术经验继承工作指导老师,第四届首都国医名师,从事推拿临床工作60余年,在按动疗法治疗肩关节周围炎方面积累了丰富经验,重视正骨与按动相结合,并以“颈部按动”“肩部按动”“髋部按动”为治疗特色,提出“以按治痛,以动治动”“整体观念,颈肩同治”“对应取穴,以髋治肩”等治疗理念,讲求“动静结合,医患配合”“先求远,后求高”“先解决疼痛,后改善功能”等治疗原则。在解决肩关节周围炎的疼痛问题上,王老提出“不通则痛、不正则痛、不顺则痛”的治疗思路,并通过纠正偏歪的颈椎、理顺肩关节周围肌肉和韧带以及疏通肩部和髋部相对应的经络穴位,从而达到“通、正、顺”的效果。按动疗法中的“按”为阴,“动”为阳,按动结合就是阴阳结合的一种方法,医者的按压与患者主动或者被动的运动形成合力作用于肌肉和骨骼上,可达到筋松骨正、缓解疼痛的治疗效果。按动疗法治疗肩关节周围炎安全性高、患者配合度好、效果显著,值得临床推广。

民国新安医家程六如经方变通运用经验探析

经方是汉代以前经典医药著作中记载的方剂,以张仲景的方剂为代表。历代医家对于经方的研究颇多。程六如是民国时期较有影响力的新安医家,医术高超,擅于变通运用经方治疗内科诸病。他以乌梅丸刚柔并济治肝胃病;黄芩汤变化血热同疗痢疾;旋覆代赭汤加减专主降气,治疗咳嗽、呃逆等诸气上逆证;建中法建运中阳以治咳喘、泄泻;小柴胡汤变通和解半表半里,用治疟疾、湿温等证;白虎汤专清肺胃热,疗用于牙科、喉科等效果极佳,既拓展经方运用范围,亦提升疗效。文章总结其经方临证运用经验以冀对临床经方的使用有所启发。

国医大师严世芸运用补阳还五汤异病同治验案举隅

介绍国医大师严世芸教授运用补阳还五汤异病同治的临床验案。验案分别为脑梗死、慢性硬膜下血肿、冠心病经皮冠状动脉介入(PCI)术后合并心力衰竭、心脏神经官能症、周围性面瘫、白癜风。认为6则验案疾病、症状虽不尽相同,但主要病机均为气虚血瘀,临证选用补阳还五汤加减治之,疗效颇佳;体现了严教授异病同治、谨守病机的诊治原则,为拓宽补阳还五汤临床主治范围提供了借鉴。

由国家中医药管理局承办的全国中药特色技术传承人才培训项目旨在全国范围内培养一批能够较全面掌握中药各方面知识的中药特色技术传承人才。笔者参加了该项目线下、线上20余个基地的培训学习,其中培训内容以中药临床药学为主。带教老师学验俱丰,从典籍解读到中药应用多角度传授知识,笔者受益匪浅,特此将期间所学习的中药临床药学方面收获及心得进行总结,以期与同道共同学习,进一步传承中药临床药学。

Professor Wei-hua ZHANG’s clinical experience in treatment of primary dysmenorrhea with warming-unblocking needling method

The professor Wei-hua ZHANG’s experiences in treatment of primary dysmenorrhea with"warmingunblocking needling method"was introduced in this paper.Prof.Zhang believes that obstruction is the crucial in pathogenesis of primary dysmenorrhea no matter of the excess or the deficiency syndrome.Warming and unblocking principle is the essential in treatment.Hence,an unique therapeutic method is innovated,named warming-unblocking needling method,including three-layer and two-direction warming needling technique at Sanyinjiao(三阴交SP6),two-layer rotating warming-unblocking technique at Zhongji(中极CV3)and one-layer and two-direction warming needling technique at Ciliao(次髎BL32).In this set of needling procedure,acupoint is viewed as a three-dimensional hierarchical structure.A layered and multidirectional needling technique is adopted at main acupoints to enhance the sensations of diqi and to generate heat effect in the local.Additionally,moxibustion is combined to intensify warming and unblocking effect.Simultaneously,the supplementary acupoints are used in response to symptoms.All of the acupoints coordinately warm up the uterus,promote meridian circulation and stop pain.Hence,this therapeutic method achieves relatively excellent effect and deserves to be promoted in practice.
