Lung ultrasound score evaluation of the effect of pressure-controlled ventilation volume-guaranteed on patients undergoing laparoscopicassisted radical gastrectomy

BACKGROUND Laparoscopic-assisted radical gastrectomy(LARG)is the standard treatment for early-stage gastric carcinoma(GC).However,the negative impact of this proce-dure on respiratory function requires the optimized intraoperative management of patients in terms of ventilation.AIM To investigate the influence of pressure-controlled ventilation volume-guaranteed(PCV-VG)and volume-controlled ventilation(VCV)on blood gas analysis and pulmonary ventilation in patients undergoing LARG for GC based on the lung ultrasound score(LUS).METHODS The study included 103 patients with GC undergoing LARG from May 2020 to May 2023,with 52 cases undergoing PCV-VG(research group)and 51 cases undergoing VCV(control group).LUS were recorded at the time of entering the operating room(T0),20 minutes after anesthesia with endotracheal intubation(T1),30 minutes after artificial pneumoperitoneum(PP)establishment(T2),and 15 minutes after endotracheal tube removal(T5).For blood gas analysis,arterial partial pressure of oxygen(PaO_(2))and partial pressure of carbon dioxide(PaCO_(2))were observed.Peak airway pressure(P_(peak)),plateau pressure(Pplat),mean airway pressure(P_(mean)),and dynamic pulmonary compliance(C_(dyn))were recorded at T1 and T2,1 hour after PP establishment(T3),and at the end of the operation(T4).Postoperative pulmonary complications(PPCs)were recorded.Pre-and postoperative serum interleukin(IL)-1β,IL-6,and tumor necrosis factor-α(TNF-α)were measured by enzyme-linked immunosorbent assay.RESULTS Compared with those at T0,the whole,anterior,lateral,posterior,upper,lower,left,and right lung LUS of the research group were significantly reduced at T1,T2,and T5;in the control group,the LUS of the whole and partial lung regions(posterior,lower,and right lung)decreased significantly at T2,while at T5,the LUS of the whole and some regions(lateral,lower,and left lung)increased significantly.In comparison with the control group,the whole and regional LUS of the research group were reduced at T1,T2,and T5,with an increase in PaO_(2),decrease in PaCO_(2),reduction in P_(peak) at T1 to T4,increase in P_(mean) and C_(dyn),and decrease in Pplat at T4,all significant.The research group showed a significantly lower incidence of PPCs than the control group within 3 days postoperatively.Postoperative IL-1β,IL-6,and TNF-αsignificantly increased in both groups,with even higher levels in the control group.CONCLUSION LUS can indicate intraoperative non-uniformity and postural changes in pulmonary ventilation under PCV-VG and VCV.Under the lung protective ventilation strategy,the PCV-VG mode more significantly improved intraop-erative lung ventilation in patients undergoing LARG for GC and reduced lung injury-related cytokine production,thereby alleviating lung injury. 展开更多

The study of the dynamic disaster mechanism of coal and gas outburst two-phase flow is crucial for improving disaster reduction and rescue ability of coal mine outburst accidents.An outburst test in a T-shaped roadway was conducted using a self-developed large-scale outburst dynamic disaster test system.We investigated the release characteristics of main energy sources in coal seam,and obtained the dynamic characteristics of outburst two-phase flow in a roadway.Additionally,we established a formation model for outburst impact flow and a model for its flow in a bifurcated structure.The results indicate that the outburst process exhibits pulse characteristics,and the rapid destruction process of coal seam and the blocking state of gas flow are the main causes of the pulse phenomenon.The outburst energy is released in stages,and the elastic potential energy is released in the vertical direction before the horizontal direction.In a straight roadway,the impact force oscillates along the roadway.With an increase in the solid–gas ratio,the two-phase flow impact force gradually increases,and the disaster range extends from the middle of the roadway to the coal seam.In the area near the coal seam,the disaster caused by the two-phase flow impact is characterized by intermittent recovery.In a bifurcated roadway,the effect of impact airflow on impact dynamic disaster is much higher than that of two-phase flow,and the impact force tends to weaken with increasing solid-gas ratio.The impact force is asymmetrically distributed;it is higher on the left of the bifurcated roadway.With an increase in the solid-gas ratio,the static pressure rapidly decreases,and the bifurcated structure accelerates the attenuation of static pressure.Moreover,secondary acceleration is observed when the shock wave moves along the T-shaped roadway,indicating that the bifurcated structure increases the shock wave velocity. 展开更多

In order to guarantee healthy development of the natural gas industrial chain, it is urgent to establish a risk alert system, which is based on a risk assessment index system. First of all, the risks of the natural gas industrial chain are defined in the paper; then the risk factors are analyzed according to the present status of the natural gas industrial chain, and five categories of risk factors are summarized: resource risk, transport risk, marketing risk, risk of unbalanced chain links, and environment risk. The paper presents the principles of the risk assessment index system. The natural gas industrial chain risk assessment index system is established with four levels and forty-six risk indices. 展开更多

The description of pores and fracture structures is a consistently important issue and certainly a difficult problem, especially for shale or tight rocks. However, the exploitation of so-called unconventional energy, such as shale methane and tight-oil, has become more and more dependent on an understanding of the inner structure of these unconventional reservoirs. The inner structure of porous rocks is very difficult to describe quantitatively using normal mathematics, but fractal geometry, which is a powerful mathematical tool for describing irregularly-shaped objects, can be applied to these rocks. To some degree, the cementation index and tortuosity can be used to describe the complexity of these structures. The cementation index can be acquired through electro-lithology experiments, but, until now, tortuosity could not be quantitatively depicted. This research used the well-logging curves of a gas shale formation to reflect the characteristics of the rock formations, and the changes in the curves to indicate the changes of the rock matrix, the pores, the connections among the pores, the permeability, and the fluid type. The curves that are affected most by the rock lithology, such as gamma ray, acoustic logging, and deep resistivity curves, can provide significant information about the micro-or nanostructure of the rocks. If the rock structures have fractal characteristics, the logging curves will also have fractal properties. Based on the definition of a fractal dimension and the Hausdorff dimension, this paper presents a new methodology for calculating the fractal dimensions of logging curves. This paper also reveals the deep meaning of the rock cementation index, m, through the Hausdorff dimension, and provides a new equation to calculate this parameter through the resistivity and porosity of the formation. Although it represents a very important relationship between the saturation of hydrocarbons with pores and resistivity, the Archie formula was not available for shale and tight rock. The major reason for this was an incorrect understanding of the cementation index, and the calculation of saturation used a single m value from the bottom to the top of the well. Unfortunately, this processing method is clearly inappropriate for the intensely heterogeneous material that is shale and tight rock. This paper proposes a method of calculating m through well-logging curves based on a fractal geometry that can change with different lithologies, so that it would have more agreement with in situ scenarios than traditional methods. 展开更多

OBJECTIVE: To observe the effects of Suhuang Zhike Capsule on pulmonary function, blood gas analysis index, serum PCT and CRP expression in patients with acute exacerbation of chronic obstructive pulmonary disease. METHODS: A total of 88 patients with acute exacerbation of chronic obstructive pulmonary disease admitted to the Department of Respiratory Medicine, Xuancheng People's Hospital of Anhui Province from December 2014 to December 2016 were randomly divided into the observation group and the control group, with 44 cases in each group. The control group was given routine clinical treatment, and the observation group was given Suhuang Zhike Capsule on the basis of the treatment method of the control group. After 7 days' treatment, the improvement of lung function indexes were observed and evaluated before and after treatment(forced expiratory volume in one second(FEV_1), forced expiratory volume occupancy in the 1^(st) second percentage of vital capacity(FEVl/FVC), peak expiatory flow(PEF)), blood gas analysis index(Arterial oxygen partial pressure(PaO_2) and arterial blood carbon dioxide partial pressure(PaCO_2), oxygenation index(OI)) and serum cytokine levels(procalcitonin(PCT) and C reactive protein(CRP). RESULTS: The total effective rate of the observation group was significantly higher than that of the control group(P < 0.05). The FEV_1, FEV_1/FVC and PEF in 2 groups were significantly increased after the treatment(P < 0.05), and the above lung function indexes in the observation group were significantly higher than the control group(P < 0.05). PaO_2 and OI were significantly increased after the treatment(P < 0.05), PaCO_2 was significantly decreased after the treatment(P < 0.05), and the improvement of above blood gas analysis indexes were significantly superior of the observation group than the control group(P < 0.05). The serum PCT and CRP levels in 2 groups were significantly decreased after the treatment(P < 0.05), and the improvement in the observation group were more significant than that in the control group(P < 0.05). CONCLUSION: Suhuang Zhike Capsule can inhibit serum inflammatory cytokine levels in patients with acute exacerbation of chronic obstructive pulmonary disease, improve blood gas analysis indicators, and improve lung functions. 展开更多

It was found out that the change of refractive index of ambient gas can lead to obvious change of the color of Morpho butterfly's wing. Such phenomenon has been employed as a sensing principle for detecting gas. In the present study, Rigorous Coupled-Wave Analysis (RCWA) was described briefly, and the partial derivative of optical reflection efficiency with respect to the refractive index of ambient gas, i.e., sensitivity of the sensor, was derived based on RCWA. A bioinspired grating model was constructed by mimicking the nanostructure on the ground scale of Morpho didius butterfly's wing. The analytical sensitivity was verified and the effect of the grating shape on the reflection spectra and its sensitivity were discussed. The results show that by tuning shape parameters of the grating, we can obtain desired reflection spectra and sensitivity, which can be applied to the design of the bioinspired refractive index based gas sensor. 展开更多

In this paper, firstly, the conception of cities ability reducing earthquake disasters is presented, and the ability could be evaluated with three basic elements the possible seis-mic casualty and economic loss during the future earthquakes that are likely to occur in the city and its surround-ings and time required for recovery after earthquake; based upon these three basic elements, a framework, which consists of six main components, for evaluating citys ability reducing earthquake disasters is proposed; then the statistical relations between the index system and the ratio of seismic casualty, the ratio of economic loss and re-covery time are gained utilizing the cities prediction results of earthquake disasters which were made during the ninth five-year plan; at last, the method defining the comprehensive index of cities ability reducing earthquake disasters is presented. Thus the relatively comprehensive theory frame is set up. The main method of casting coal spontaneous combustion is prediction of index gases, with carbon monoxide(CO) commonly used as an index gas. However, coal spontaneous combustion is not the sole source of CO evolution; primal CO is generated through coalification, which can lead to forecasting mistakes. Through theoretical analysis, primal CO generation and emission from coal seams was determined.In this study, six coal samples were analyzed under six different experimental conditions. The results demonstrated the change in coal seam primal gas and concentration as functions of time, different coal samples, occurrence, various gas types and composition concentration, which are in agreement with the previous study on primal CO generation. Air charging impacts on primal gas emission. Analysis of the experimental data with SPSS demonstrates that the relationship between primal CO concentration and time shows a power exponent distribution. 展开更多

Evaluation of Forest Damaged Area and Severity Caused by Ice-snow Frozen Disasters over Southern China with Remote Sensing 被引量:2

The accurate assessment of forest damage is important basis for the forest post-disaster recovery process and ecosystem management. This study evaluates the spatial distribution of damaged forest and its damaged severity caused by ice-snow disaster that occurred in southern China during January 10 to February 2 in 2008. The moderate-resolution imaging spectroradiometer(MODIS)13 Q1 products are used, which include two vegetation indices data of NDVI(Normalized Difference Vegetation Index) and EVI(Enhanced Vegetation Index). Furtherly, after Quality Screening(QS) and Savizky-Golay(S-G) filtering of MODIS 13 Q1 data, four evaluation indices are obtained, which are NDVI with QS(QSNDVI), EVI with QS(QSEVI), NDVI with S-G filtering(SGNDVI) and EVI with S-G filtering(SGEVI). The study provides a new way of firstly determining the threshold for each image pixel for damaged forest evaluation, by computing the pre-disaster reference value and change threshold with vegetation index from remote sensing data. Results show obvious improvement with the new way for forest damage evaluation, evaluation result of forest damage is much close to the field survey data with standard error of only 0.95 and 1/3 less than the result that evaluated from other threshold method. Comparatively, the QSNDVI shows better performance than other three indices on evaluating forest damages. The evaluated result with QSNDVI shows that the severe, moderate, mild damaged rates of Southern China forests are 47.33%, 34.15%, 18.52%, respectively. By analyzing the influence of topographic and meteorological factors on forest-vegetation damage, we found that the precipitation on freezing days has greater impact on forest-vegetation damage, which is regarded as the most important factor. This study could be a scientific and reliable reference for evaluating the forest damages from ice-snow frozen disasters. 展开更多

The seismic intensities, lithologic characteristics and terrain features from a 3000 km2-region near the epicenter of the Lushan earthquake are used to analyze earthquake-induced geological disaster. The preliminary results indicate that secondary effects of the earthquake will affect specific areas, including those with glutenite and carbonate bedrock, a seismic intensity of IX, slopes between 40° and 50°, elevations of less than 2500 m, slope change rates between 20° and 30°, slope curvatures from - 1 to -0.5 and 0. 5 to 1, and relief between 50 and 100 m. Regions with susceptibility indices greater than 0.71 are prone to landslides and collapses. The secondary features are mainly distributed on both sides of the ridges that extend from Baosheng to Shuangshi and from Baosheng to Longxing. Other features are scattered on both sides of the ridges that extend from Qishuping to Baosheng and from Masangping to Lingguan. The distribution of the earthquake-related features trends in the NE direction, and the area that was most affected by the Lushan earthquake covers approximately 52.4 km^2. 展开更多

[Result] The hail days exhibited significant interdecadal variability, and it had a significant negative correlation with annual mean minimum temperature. The hail usually occurred from March to August, most frequently in June but less in autumn and winter. It most likely took place from aftemoon to sunset. The spatial distribution showed that the hail occurred more in northeast of Huaibei, south of Dabie Mountain and some areas of Huangshan Mountain, but less in Jianghuai region. The index of hail intensity was defined by three factors, the diameter, duration and gust grade, and the percentile method was used to grade the intensity of the hail disaster. This work provided a possibility for quantitative assessment of the intensity of hail disaster. The results of hail disaster risk zoning in Anhui Province showed that high dsk zones were concentrated in northeast of Huaibei, and low risk zones were mainly in south of Huaihe River. Finally, vedfied by historic disaster, it showed that the results of risk zoning had a certain rationality and feasibility. Conclusion The research could provide reference basis for hail disaster defense and assessment. 展开更多

To efficiently develop deep shale gas in southern Sichuan Basin,under the guidance of“extreme utilization”theory,a basic idea and solutions for deep shale gas development are put forward and applied in practice.In view of multiple influencing factors of shale gas development,low single-well production and marginal profit of wells in this region,the basic idea is to establish“transparent geological body”of the block in concern,evaluate the factors affecting shale gas development through integrated geological-engineering research and optimize the shale gas development of wells in their whole life cycle to balance the relationship between production objectives and development costs.The solutions are as follows:(1)calculate the gold target index and pinpoint the location of horizontal well drilling target,and shale reservoirs are depicted accurately by geophysical and other means to build underground transparent geological body;(2)optimize the drilling and completion process,improve the adaptability of key tools by cooling,reducing density and optimizing the performance of drilling fluid,the“man-made gas reservoir”is built by comprehensively considering the characteristics of in-situ stress and fractures after the development well is drilled;(3)through efficient management,establishment of learning curve and optimization of drainage and production regime,the development quality and efficiency of the well are improved across its whole life cycle,to fulfil“extreme utilization”development of shale gas.The practice shows that the estimated ultimate recovery of single wells in southern Sichuan Basin increase by 10%-20%than last year. 展开更多

This paper establishes a model that would allow China's oil and gas enterprises to scientifically evaluate and measure their low-carbon level and status. It considers various characteristics of China's oil and gas enterprises and the implications of low-carbon development, and is based on an overall analysis of factors that influence the reduction of carbon emissions. In view of low-carbon economic theories and the general principles of an evaluation index system, a comprehensive system for measuring the low-carbon status of China's oil and gas enterprises has been developed. This measurement system is comprised of four main criteria (energy structure, energy utilization, carbon emissions and utilization, and low carbon management) as well as thirty indexes. By the Delphi method and the analytical hierarchy process (AHP), the weight of the rules hierarchy and indexes hierarchy were determined. The standardized indexes were then integrated using a linear weighted sum formula, and a comprehensive formula for index measurement was established. Taking into account the status of low- carbon development in the petroleum and petrochemical industry at home and abroad, an evaluation criterion is proposed comprising four levels: ideal low-carbon, economical low-carbon, medium-carbon and high-carbon, whose values were organized within the settings of [0, 1]. 展开更多

BACKGROUND Term pregnancy-induced labor refers to the use of artificial methods to induce uterine contractions and terminate pregnancy after 37 wk.It is a common method to prevent overdue pregnancy and to deal with high-risk pregnancies.In addition,it can alleviate maternal complications and cause the fetus to leave the adverse intrauterine environment early,which is beneficial to the outcome of pregnancy.AIM To explore the effect of a birthing ball on labor by inducing cervical ripening and its influence on labor and the neonatal blood gas index.METHODS Twenty-two women who were scheduled to undergo labor induction and delivery in the obstetrics department of our hospital were randomly divided into two groups:the delivery ball group(childbirth ball combined with COOK balloon induction)and the conventional group(COOK balloon induction alone).The cervical Bishop score before and after intervention,duration of labor at each stage,mode of delivery,neonatal umbilical venous blood pH,oxygen partial pressure(PO_(2)),carbon dioxide partial pressure(PCO_(2)),and the 1-min Apgar score were recorded.RESULTS After the intervention,the cervical Bishop score of the delivery ball group(7.84±1.52)was significantly higher than that of the conventional group(7.32±1.29)(P<0.05),and the cervical Bishop scores of the two groups after intervention were significantly higher than those before intervention(P<0.05).After the intervention,the first stage of labor(510.9±98.7 min),the second stage of labor(43.0±8.5 min),and the total duration of labor(560.0±120.9 min)in the delivery ball group were lower than those in the routine group,with a first stage of labor of 602.1±133.2 min,a second stage of labor of 48.4±9.1 min,and a total duration of labor of 656.8±148.5 min(P<0.05).There was no significant difference in the time of the third stage of labor between the two groups(P>0.05).There was no significant difference in the pH,PO_(2),and PCO_(2) values of newborns between the delivery ball group and the conventional group(P>0.05).The 1-min Apgar score of the delivery ball group was higher than that of the conventional group(9.10±0.38 points vs 8.94±0.31 points,P<0.05).The natural delivery rate of the delivery ball group was higher than that of the conventional group(91.00%vs 78.00%,P<0.05).CONCLUSION The use of a birthing ball combined with a COOK balloon for inducing labor has a better effect on promoting cervical ripening,shortening the time of labor,and improving the Apgar score of newborns. 展开更多

As mines become deeper,the potential for coal and gas outbursts in deep rock cross-cut coal uncovering is enhanced.The outburst precursors are unclear,which restricts the effectiveness and reliability of warning systems.To reveal the evolution characteristics of coal and gas outburst precursor information in deep rock cross-cut coal uncovering,briquette specimens are constructed and experiments are conducted using a self-developed true triaxial outburst test system.Using acoustic emission monitoring technology,the dynamic failure of coal is monitored,and variations in the root mean square(RMS)of the acoustic emissions allow the effective cracking time and effective cracking gas pressure to be defined.These characteristics are obviously different in deep and shallow coal.The characteristic parameters of gas outburst exhibit stepwise variations at different depths.The RMS and cumulative RMS have stepped failure characteristics with respect to changes in gas pressure.The characteristic parameters of coal failure are negatively correlated with the average in-situ stress and effective stress,but positively correlated with the lateral pressure coefficient of in-situ stress and the critical gas pressure.The transition characteristics are highly sensitive in all cases.The critical depth between deep and shallow coal and gas outbursts is 1700 m.The expansion multiple of acoustic emission intensity from the microfracture stage to the sharp-fracture stage of coal is defined as the outburst risk index,N1.For depths of 1100–1700 m,N1≥7 denotes a higher risk of outburst,whereas at depths of 1700–2500 m,N1≥3 indicates enhanced risk. 展开更多
